Regulatory role of electroacupuncture on satellite glial cell activity in the colon and dorsal root ganglion of rats with irritable bowel syndrome

Objective: To investigate the role of satellite glial cells in irritable bowel syndrome (IBS) and the effect of electroacupuncture (EA) at the Tianshu (ST25) and Shangjuxu (ST37) combination. Methods: A model for visceral hypersensitivity in IBS was induced through colorectal distension (CRD) stimulation. Clean-grade male Sprague-Dawley (SD) rats were randomly divided into four groups: a normal group (NG)， a model group (MG)， an electroacupuncture group (EA)， and a glial cell inhibitor group (FCA). Bilateral EA (2/100 Hz， 1 mA， 30 min) was administered at the Tianshu (ST25) and Shangjuxu (ST37) in week 6. Abdominal withdrawal reflex (AWR) scores were used to assess the behavioral response associated with visceral hyperalgesia， while hematoxylin-eosin staining was employed to evaluate pathological changes in the colon. The protein and mRNA levels of glial fibrillary acidic protein (GFAP) in the colon and colon-related dorsal root ganglion (DRG) were analyzed using immunofluorescence， immunohistochemistry， Western blotting， real-time polymerase chain reaction. The impact of EA on electrophysiological properties of colon-related DRG neurons was observed through whole-cell patch clamp analysis. Results: EA significantly reduced the visceral pain behavior scores in rats with IBS in response to graded (20， 40， 60， 80 mm Hg) CRD stimulation. Additionally， EA downregulated the protein and mRNA expression levels of GFAP in the colon and colon-related DRG of rats with IBS. EA also regulated the resting membrane potential， rheobase and action potential of colon-related DRG neurons in rats with IBS. Conclusions: EA can regulate the excitatory properties of colon-related DRG neurons by downregulating the protein and mRNA expression of GFAP in the colon and colon-related DRG， indicating a potential neurobiological mechanism by which EA relieves visceral hypersensitivity in rats with IBS.

目的:评价针刺治疗中风后中枢性疼痛(CPSP)的有效性和安全性。方法:计算机检索PubMed、EMBASE、Cochrane Library、中国知网(CNKI)、万方数据知识服务平台(Wanfang)、维普资讯中文期刊服务平台(VIP)、中国生物医学文献数据库(CBM)中针刺治疗CPSP的随机对照试验文献。检索时间从建库至2023年7月。采用RevMan5.3软件进行meta分析;GRADEprofiler3.6.1软件进行证据质量评价。二分类资料采用相对危险度(RR)进行分析。连续性资料采用均数差(MD)进行分析,置信区间(CI)取95%进行计算。结果:纳入14篇文献,共1045例患者。Meta分析结果表明,与西药治疗CPSP相比,针刺治疗的临床有效率更高[RR=1.09,95%CI(1.01,1.19),Z=2.08,P0.05]。与西药治疗CPSP比较,针刺+西药治疗的临床有效率更高[RR=1.18,95%CI(1.05,1.34),Z=2.75,P<0.05]、VAS评分更低[MD=-1.04,95%CI(-1.26,-0.82),Z=9.25,P<0.001]、匹兹堡睡眠质量指数(PSQI)评分更低[MD=-2.67,95%CI(-4.80,-0.54),Z=2.46,P<0.05]。证据质量等级评价结果显示针刺或针刺+西药与西药对比治疗CPSP均无中、高质量的证据。结论:针刺治疗CPSP较西药治疗有一定的疗效优势,能够有效缓解疼痛,改善睡眠,且出现不良反应较少,安全性较高,但仍需高质量的随机对照试验进一步研究与验证。

Tuina alleviates neuropathic pain through regulate the activation of microglia and the secretion of inflammatory cytokine in spinal cord

OBJECTIVE: To observe the analgesic effects of Tuina on neuropathic pain(NPP) and the underlying mechanisms. METHODS: Forty-eight Sprague-Dawley(SD) rats were assigned by random into three treatment groups: sham, chronic constriction injury(CCI), and Tuina. Each group contained sixteen rats. CCI model was generated by ligating the right sciatic nerve. Behavioral changes of CCI were assessed by the paw withdrawal threshold(PWT) and paw withdrawal latency(PWL). In addition, biochemical techniques such as immunofluorescence staining, enzyme-linked immunosorbent assay(ELISA) and Western blotting were used to profile levels of microglia activation and inflammatory factors in the spinal dorsal horn(SDH) of rats. Tuina(clockwise pressing and rubbing) was performed at Chengshan(BL57) to observe the analgesic effects on CCI rats and the underlying mechanisms. RESULTS: Rats with CCI experienced significant reduction in the PWT and PWL of the right hind paw relative to CCI group at day 3. Tuina treatment rescued this situation significantly on days 10 and 14. Besides, Iba-1, microglia M1 receptor CD68, tumor necrosis factor-α(TNF-α), and interleukin-1β(IL-1β) were higher in the right SDH for CCI group compared to the sham group on day 14. As expected, Tuina partially downregulated the CCI-induced overexpressed Iba-1, CD68, TNF-α, and IL-1β in the SDH of CCI model. CONCLUSION: Tuina induces a time-dependent cumulative analgesic effect in CCI rats by inhibiting the activation of microglia and the secretion of IL-1β and TNF-α in SDH.

Objective: Rheumatoid arthritis(RA) is a chronic inflammatory and destructive arthritis, characterized by inflammatory infiltration and bone destruction. Huangqi Guizhi Wuwu Decoction(HGWD) is traditional Chinese medicine, which has been applied in the treatment of RA in clinical. The aim of this study was to investigate the therapeutic effect of HGWD on collagen-induced arthritis(CIA) mouse ***: DBA/1J female mice were used to establish the collagen-induced arthritis(CIA) model. HGWD was administered intragastrically once a day for four weeks starting on the 22nd day after the first immunization. The body weight, hind paw thickness and clinical score were measured every five days. Gait analysis, histopathological staining, enzyme-linked immunosorbent assay(ELISA), ultrasound imaging and micro-computed tomography imaging were performed to determine the effects of HGWD treatment on inflammation and bone structure in this model. Moreover, Real-time PCR and Western blot analysis were used to detect inflammatory factors m RNA and protein levels after HGWD intervention in RAW264.7 ***: HGWD attenuated symptoms of arthritis, suppressed inflammatory synovium area and the serum levels of inflammatory factors, inhibited joint space enlargement in the knee and ankle joints,reduced numbers of osteoclasts, protected bone destruction, as well as improved motor *** decreased the expression of m RNA for inflammatory factors and the protein expression levels of p-NF-ΚB and ***: These results suggested that HGWD suppresses inflammation, attenuates bone erosion and maintains motor function in collagen-induced arthritis mice.

The hypothalamic–pituitary–adrenal(HPA) axis is a critical component of the neuroendocrine system,playing a central role in regulating the body's stress response and modulating various physiological processes. Dysregulation of HPA axis function disrupts the neuroendocrine equilibrium, resulting in impaired physiological functions. acupuncture is recognized as a non-pharmacological type of therapy which has been confirmed to play an important role in modulating the HPA axis and thus favorably targets diseases with abnormal activation of the HPA axis. With numerous studies reporting the promising efficacy of acupuncture for neuroendocrine disorders, a comprehensive review in terms of the underlying molecular mechanism for acupuncture, especially in regulating the HPA axis, is currently in need. This review fills the need and summarizes recent breakthroughs, from the basic principles and the pathological changes of HPA axis dysfunction, to the molecular mechanisms by which acupuncture regulates the HPA axis. These mechanisms include the modulation of multiple neurotransmitters and their receptors, neuropeptides and their receptors, and microRNAs in the paraventricular nucleus,hippocampus, amygdala and pituitary gland, which alleviate the hyperfunctioning of the HPA *** review comprehensively summarizes the mechanism of acupuncture in regulating HPA axis dysfunction for the first time, providing new targets and prospects for further exploration of acupuncture.

目的:观察运动针法治疗缺血性中风的临床疗效,以及对患者运动功能和日常生活能力的改善作用。方法:将106例脑梗死导致的肢体运动功能障碍患者随机分为试验组和对照组,每组53例。两组患者均给予常规康复治疗,对照组加用头穴透刺治疗,试验组加用运动针法治疗。每周治疗3次,持续治疗4周。于治疗前、治疗2周和4周后,以及随访2个月时,采用Fugl-Meyer评估(FMA)量表、Lovett肌力评定量表、改良Ashworth量表(MAS)、日常生活活动(ADL)量表评分对患者运动功能、肌力、肌张力和日常生活能力进行评定。结果:治疗后两组运动功能和日常生活能力均提高;治疗4周后及随访2个月时试验组FMA及ADL评分高于对照组(P<0.05)。治疗后两组MAS评分均下降;治疗2周和4周后以及随访2个月时,试验组MAS评分低于对照组(P<0.05)。治疗后两组Lovett评分均提高;治疗4周后试验组Lovett评分高于对照组(P<0.05)。结论:在常规康复治疗基础上,运动针法与头穴透刺均能有效改善缺血性中风患者的运动功能,提高其日常生活能力;运动针法作用优于头穴透刺治疗。
