BACKGROUND Rehabilitation of elderly patients with a high body mass index(BMI)after cholecystectomy carries risks and requires the adoption of effective perioperative management *** enhanced recovery after surgery(ERAs)protocol is a comprehensive treatment approach that facilitates early patient recovery and reduces postoperative *** To compare the effectiveness of traditional perioperative management methods with the ERAs protocol in elderly patients with gallbladder stones and a high *** This retrospective cohort study examined data from 198 elderly patients with a high BMI who underwent cholecystectomy at the shanghai fourth people's hospital from August 2019 to August *** them,99 patients were managed using the traditional perioperative care approach(non-ERAs protocol),while the remaining 99 patients were managed using the ERAs *** indicator data were collected for patients preoperatively,intraoperatively,and postoperatively,and surgical outcomes were compared between the two *** The comparison results between the two groups of patients in terms of age,sex,BMI,underlying diseases,surgical type,and preoperative hospital stay showed no statistically significant ***,the ERAs group had a significantly shorter preoperative fasting time than the non-ERAs group(4.0±0.9 h vs 7.6±0.9 h).Regarding intraoperative indicators,there were no significant differences between the two groups of ***,in terms of postoperative recovery,the ERAs protocol group exhibited significant advantages over the non-ERAs group,including a shorter hospital stay,lower postoperative pain scores and postoperative hunger scores,and higher satisfaction *** readmission rate was lower in the ERAs protocol group than in the non-ERAs group(3.0%vs 8.1%),although the difference was not ***,there were significant differences between the two groups in terms of postoperative nausea and vomit

Imatinib has been widely used as a selective kinase inhibitor for treating a variety of cancers,and this molecule is very hydrophobic so it is usually modified with mesylate salt in clinic to increase ***,pH-dependent aqueous solubility and relatively high dosage of imatinib mesylate greatly reduce the clinical *** solve this problem,we developed an intestine enzyme-responsive hydrogel to efficiently encapsulate hydrophobic imatinib with long-term controlled release and enhanced intestinal permeability through oral *** anhydride-modified carboxymethyl chitosan(MA-CMCs)was synthesized via amidation reaction and then MA-CMCs was crosslinked with photoinitator under UV-irradation to form a three-dimensional hydrophilic polymer *** intestine enzyme responsiveness was endowed with imatinib-loaded hydrogel through hydrolyzation of glucosidic bond,which could achieve enzyme-triggered long-term drug release of up to 2 ***,sodium deoxycholate was embedded into the hydrogel to synchronously open epithelial tight junctions with improved intestinal *** vitro studies revealed similar lethality against colon cancer cell for both imatinib mesylate and imatinib-loaded ***,significantly enhanced in vivo tumor inhibition(6-fold higher compared to imatinib mesylate)was achieved after oral administration with imatinib-loaded ***,this enzyme-responsive hydrogel could achieve long-term synchronous release of kinase inhibitor(imatinib)and tight junction permeation enhancer(sodium deoxycholate)at intestine with enhanced therapeutic efficiency,which could provide an effective approach to improve the bioavailability of hydrophobic anticancer chemodrugs with oral administration.

Early surgical resection and chemotherapy of bone cancer are commonly used in the treatment of bone tumor,but it is still highly challenging to prevent recurrence and fill the bone defect caused by the resection *** this work,we report a rational integration of photonic-responsive two-dimensional(2D)ultrathin niobium carbide(Nb2C)MXene nanosheets(Nss)into the 3D-printed bone-mimetic scaffolds(NBGs)for osteosarcoma *** integrated 2D Nb2C-MXene Nss feature specific photonic response in the second near-infrared(NIR-II)biowindow with high tissue-penetrating depth,making it highly efficient in killing bone cancer ***,Nb-based species released by the biodegradation of Nb2C MXene can obviously promote the neogenesis and migration of blood vessels in the defect site,which can transport more oxygen,vitamins and energy around the bone defect for the reparative process,and gather more immune cells around the defect site to accelerate the degradation of *** degradation of NBGs provides sufficient space for the bone ***,calcium and phosphate released during the degradation of the scaffold can promote the mineralization of new bone *** intrinsic multifunctionality of killing bone tumor cell and promoting angiogenesis and bone regeneration makes the engineered Nb2C MXeneintegrated composite scaffolds a distinctive implanting biomaterial on the efficient treatment of bone tumor.

The low immunogenicity of tumors remains one of the major limitations of cancer ***,we report a bacterial metabolisminitiated and photothermal-enhanced nanocatalytic therapy strategy to completely eradicate primary tumor by triggering highly effective antitumor immune ***,a microbiotic nanomedicine,designated as Cu_(2)O@Δst,has been constructed by conjugating PEGylated Cu_(2)O nanoparticles on the surface of an engineered salmonella typhimurium strain(Δst).Owing to the natural hypoxia tropism ofΔst,Cu_(2)O@Δst could selectively colonize hypoxic solid tumors,thus minimizing the adverse effects of the bacteria on normal *** bacterial metabolism within the tumor,Cu_(2)O@Δst generates H_(2)s gas and other acidic substances in the tumor microenvironment(TME),which will in situ trigger the sulfidation of Cu_(2)O to form Cus facilitating tumor-specific photothermal therapy(PTT)under local NIR laser irradiation on the one ***,the dissolved Cu+ions from Cu_(2)O into the acidified TME enables the nanocatalytic tumor therapy by catalyzing the Fenton-like reaction of decom-posing endogenous H_(2)O_(2) into cytotoxic hydroxyl radicals(·OH)on the other *** a bacterial metabolism-triggered PTT-enhanced nanocatalytic treatment could effectively destroy tumor cells and induce a massive release of tumor antigens and damage-associated molecular patterns,thereby sensitizing tumors to checkpoint blockade(ICB)*** combined nanocatalytic and ICB therapy results in the much-inhibited growth of distant and metastatic tumors,and more importantly,induces a powerful immunological memory effect after the primary tumor ablation.
