Mastoid obliteration and reconstruction techniques:A review of the literature

Objective To review the published literature related to the different obliteration and reconstruction techniques in the management of the canal wall down *** A PubMed(Medline)and LILACS databases as well as crossed references search was performed with the following Mesh terms:“cholesteatoma”,“cholesteatoma-middle ear”,“otitis media”,“otitis media,suppurative”,“mastoiditis”,“mastoidectomy”,“canal wall down mastoidectomy”,“radical mastoidectomy”,“mastoid obliteration”and crossed *** criteria were adult patients subject to mastoid cavity obliteration and posterior canal wall *** technique and materials used,anatomic and functional results,complications,recurrence rates,and changes in quality of life,were analyzed.A total of 94 articles were screened,38 were included for full-text detailed *** Twenty-one articles fulfilled the inclusion *** and materials used for canal wall reconstruction,tympanoplasty,and ossiculoplasty were varied and included autologous,biosynthetic,or *** results were reported in 16 studies and were *** studies reported improvement in the quality of life using the GBI ***-up time ranged from 1 to 83 *** articles used imaging studies to evaluate postoperative disease *** highest recurrence rate reported for cholesteatoma after obliteration was 19%.The most frequently reported complications were retraction pockets and transient *** Plenty of techniques combining grafts and other materials have been used to overcome mastoidectomy cavity *** far,it is still not possible to standardize an ideal *** available level of evidence for this topic is low and limited.

Functional physiotherapy method results for the treatment of idiopathic clubfoot

BACKGROUND Idiopathic clubfoot is a congenital deformity of multifactorial *** initial treatment is eminently conservative;one of the methods applied is the Functional physiotherapy method(FPM),which includes different approaches:Robert debré(RD)and Saint-Vincent-de-Paul(SVP)among *** method is based on manipulations of the foot,bandages,splints and exercises adapted to the motor development of the child aimed to achieve a plantigrade and functional *** hypothesis was that the SVP method could be more efficient than the RD method in correcting deformities,and would decrease the rate of *** To compare the RD and SVP methods,specifically regarding the improvement accomplished and the frequency of surgery needed to achieve a plantigrade *** Retrospective study of 71 idiopathic clubfeet of 46 children born between February 2004 and January 2012,who were evaluated and classified in our hospital according to severity by the Dimeglio-Bensahel *** included moderate,severe and very severe ***-four feet were treated with the RD method and 37 feet with the SVP *** outcomes at a minimum of two years were considered as very good(by physiotherapy),good(by percutaneous heel-cord tenotomy),fair(by limited surgery),and poor(by complete surgery).RESULTS Complete release was not required in any case;limited posterior release was done in 23 cases(74%)with the RD method and 9(25%)with the SVP method(Pdes evidence of the superiority of the SVP method over the RD method,as a variation of the FPM,for the treatment of idiopathic clubfoot.

Home range variability and philopatry in Cinereous vultures(Aegypius monachus)breeding in Iberia

Large scavengers are strongly dependent on environmental conditions and carrion distribution and abundance,so season and breeding-related factors may influence the spatial ecology of species such as the Cinereous Vulture(Aegypius monachus),the largest European *** holds one of the biggest populations worldwide,but some aspects of the spatial ecology of the species in this region remain *** this study,17 adult Cinereous Vultures were GPS-tracked in order to study their spatial ecology during the adult *** average monthly home ranges(95%Kernel density Estimation,Kde)and core areas(50%Kde)were 6543±19,935 km^(2)and 1174±4004 km^(2),*** average monthly home range fidelity ranged between 50 and 73%.Differences in movement-related variables between the seasonal periods(incubation,chick-rearing and non-breeding)were *** the chick-rearing period,the monthly accumulated distance was higher than during the other periods:3316±1108(chick-rearing)vs.1621±622(incubation)vs.1726±1159 km per month(nonbreeding).Additionally,large home range sizes were more frequent during the chick-rearing *** are two likely causes for these seasonal ***,chick-rearing entails a higher energetic expenditure by the parental individuals in foraging activities,so larger movements and foraging areas are expected during this ***,the flight is favoured during spring and summer due to environmental *** chick-rearing and warm months is a great evolutionary advantage for soaring-gliding raptors,as it allows them to cover larger areas with low energy ***,six individuals tagged as nestlings highlight the philopatric behaviour of the species:vultures settle their breeding areas 54±51 km from their natal nest(range=9–138 km).

Estrogen receptor modulators are compounds of interest because of their estrogenic agonistic/antagonistic effects and tissue specificity. These compounds have many clinical applications, particularly for breast cancer treatment and osteoporosis in postmenopausal women, as well as for the treatment of climacteric symptoms. Similar to estrogens, neuroprotective effects of estrogen receptor modulators have been described in different models. However, the mechanisms of action of these compounds in the central nervous system have not been fully described. We conducted a systematic search to investigate the effects of estrogen receptor modulators in the central nervous system, focusing on the modulation of cytoskeletal proteins. We found that raloxifene, tamoxifen, and tibolone modulate some cytoskeletal proteins such as tau, microtuble-associated protein 1（MAP1）, MAP2, neurofilament 38（NF38） by different mechanisms of action and at different levels： neuronal microfilaments, intermediate filaments, and microtubule-associated proteins. Finally, we emphasize the importance of the study of these compounds in the treatment of neurodegenerative diseases since they present the benefits of estrogens without their side effects.

Chronic neck pain(CNP)is a worldwide health problem with several risk *** of the most widely used treatments for managing this condition is therapeutic exercise,which could generate a response called exerciseinduced hypoalgesia(EIH).There is no consensus on the best exercise modality to induce ***,this review aims to analyze and synthesize the state-of-the-art about the hypoalgesic effect of exercise in subjects with *** included articles on EIH and CNP in patients older than 18 years,with pain for more than three months,where the EIH response was *** that studied CNP associated with comorbidities or measured the response to treatments other than exercise were *** studies reviewed reported variable *** in healthy subjects has been shown to reduce indicators of pain sensitivity;however,in people with chronic pain,the response is *** investigations reported adverse effects with increased pain intensity and decreased pain sensitivity,others found no clinical response,and some even reported EIH with decreased pain and increased *** is an identifiable,stimulable,and helpful therapeutic response in people with *** research is still needed on subjects with CNP to clarify the protocols and therapeutic variables that facilitate the EIH *** addition,it is necessary to deepen the knowledge of the intrinsic and extrinsic factors that influence EIH in people with CNP.

Comparison between Classical and Intelligent Identification Systems for Classification of Gait Events

Gait event detection is important for diagnosis and evaluation. This is a challenging endeavor due to subjectivity, high amount of data, among other problems. ANFIS （Artificial Neural Fuzzy Inference Systems）, ARX （Autoregressive Models with Exogenous Variables）, OE （Output Error models）, NARX （Nonlinear Autoregressive Models with Exogenous Variables） and models based on NN （neural networks） were developed in order to detect gait events without the problems mentioned. The objective was to compare developed models＇ performance and determinate the most suitable model for gait events detection. Knee joint angle, heel foot switch and toe foot switch during normal walking in a treadmill were collected from a healthy volunteer. Gait events were classified by three experts in human motion. Experts＇ mean classification was obtained and all models were trained and tested with the collected data and experts＇ mean classification. Fit percentage was obtained to evaluate models performance. Fit percentages were： ANFIS： 79.49%, ARX： 68.8%, OE： 71.39%, NARX： 88.59%, NNARX： 67.66%, NNRARX： 68.25% and NNARMAX： 54.71%. NARX had the best performance for gait events classification. For ARX and OE, previous filtering is needed. NN＇s models showed the best performance for high frequency components, ANFIS and NARX were able to integrate criteria from three experts for gait analysis. NARX and ANFIS are suitable for gait event identification. Test with additional subjects is needed.
