Objectives: More than a year after its introduction, coVID-19 vaccination coverage was low in the Togolese population and little data were available on its benefits for hospitalized patients. This study aimed to describe the impact of coVID-19 vaccination on the prognosis of hospitalized patients. Methods: This was a retrospective cohort study of patients admitted to the Centre Hospitalier Régional Lomé commune (Togo) between June 1, 2021 and May 31, 2022. Primary outcomes (admission to the intensive care unit and death) were presented with frequency and proportion. Mortality rates were presented by sociodemographic and clinical characteristics and compared by appropriate statistical tests. Factors associated with inpatient death were described by performing a cox proportional hazard regression. Results: A total of 604 patients were hospitalized (50.0% women). The mean age was 54.03 ±17.1 years. Only 55 patients were fully vaccinated (9.1%). ICU admission was significantly more frequent in unvaccinated patients than in vaccinated ones (63.0% vs. 38.2%;p conclusion: coVID-19 vaccination had a significant benefit for patients with coVID-19 infection in terms of reducing the risk of death. Based on real-world data from sub-Saharan Africa, this information can help optimize the benefit of coVID-19 vaccination by raising community awareness and increasing vaccine coverage while reducing hesitancy.

The protective effect of Limosilactobacillus reuteri against gestational diabetes mellitus through restoring intestinal microbiota homeostasis and attenuating inflammation in mice

Limosilactobacillus reuteri QS01 is potential probiotic isolated from the intestinal microbiota of healthy women in early pregnancy. In the current study， we examined whether QS01 can prevent gestational diabetes mellitus (GDM) in an enhanced mouse model of the condition. Female C57BL/6J mouse offspring (F1 generation) born to dams fed a control (coN) or low-protein diet (GDM group) during gestation and lactation were used. Pregnant F1 mice fed a standard diet were randomly assigned to 5 groups: coN， GDM， and GDM mice treated with metformin， QS01 or Lacticaseibacillus rhamnosus HN001. An oral glucose tolerance test was performed before sacrifice at gestational day 17. Glucose tolerance was significantly ameliorated by all three treatments. QS01 supplementation fortified the intestinal mucosal barrier and inhibited the escalation of plasma inflammatory cytokines. QS01 treatment altered the cecal microbiota composition and function. Plasma and cecal metabolite profiles were modulated by QS01， prominently demonstrating significant upregulation of indole lactate and L-tryptophan in plasma and 5-hydroxy-tryptophan in the cecum， positively correlated with gut Lactobacillus abundance. In summary， QS01 plays a role in preventing GDM by remodeling the intestinal microbiota， reinforcing the intestinal mucosal barrier and alleviating chronic inflammation.

Introduction: Diabetes mellitus and hypertension are chronic diseases that are on the rise and pose a major public health problem among the priorities of healthcare systems. Objectives: determine the prevalence of diabetes and hypertension and describe the sociodemographic characteristics of those screened. Materials and Methods: This was a cross-sectional study organized during a diabetes and hypertension screening campaign that took place in conakry and five (05) inland regions on November 14, 2022. Results: 2050 people were screened, of whom 33.12% were housewives, the average age was 44.78 ± 16.23 years, and 55.27% were women. The screening sites were the city of conakry 741 (36.15%), the Labé region 424 (20.68%), the N’Zérékoré region 298 (14.54%), the Faranah region 241 (11.75%), the Mamou region 210 (10.24%) and the Boké region 136 (6.63%). The prevalence of diabetes and hypertension were respectively 12.5% and 43.38% for the Boké region;6.19% and 16.19% for the Mamou region;3.02% and 21.81% for the N’Zérékoré region;31.95% and 13.69% for the Faranah region;9.67% and 20.28% for the Labé region;2.83% and 20.28% for the city of conakry. conclusion: despite the limitations of the study duration, our series highlighted the high prevalence of diabetes and hypertension among people screened on World Diabetes Day 2022. Raising public awareness of a healthy lifestyle is essential for the prevention and control of diabetes and hypertension.

Minimally Invasive Surgery for Necrotizing Pancreatitis: A Case Report

Introduction: Necrotizing pancreatitis management is complex and varies significantly among clinicians. Minimally invasive approaches like transgastric necrosectomy via laparoscopy are emerging as effective treatment options. This case report underscores the technique’s efficacy, clinical outcomes, and role in reducing complications. Clinical Observation: A 59-year-old male with a history of smoking and alcoholism presented with severe abdominal pain, nausea, and vomiting. Over the following weeks, he developed symptoms including asthenia, weight loss, and melena. Diagnostic workup revealed severe anemia and Balthazar E necrotizing pancreatitis, with significant intra-abdominal fluid collections and signs of infection. After initial conservative management, the patient underwent transgastric necrosectomy via laparoscopy due to deteriorating clinical status. The procedure involved removing necrotic tissue and performing a cystogastroanastomosis and jejunostomy. Postoperative care included fasting, parenteral nutrition, broad-spectrum antibiotics, and enzymatic replacement. The patient recovered well, with reduced necrotic tissue on follow-up imaging, and was discharged twelve days post-surgery [1]. conclusion: Transgastric necrosectomy by laparoscopy is a valuable first-line surgical option for patients with symptomatic necrotizing pancreatitis, particularly in cases without prior interventions. This minimally invasive technique helps reduce major complications and mortality, offering a less invasive alternative to traditional open necrosectomy. The multidisciplinary approach and careful postoperative management were crucial to the patient’s favorable outcome. The case highlights the potential of transgastric necrosectomy as an effective treatment strategy in managing complex pancreatitis cases, including those with associated duodenal perforation [2].

The corrosion behavior of high-strength low-alloy 921A steel in a simulated marine atmospheric environment was studied using a high-throughput experimental *** corrosion behavior,corrosion morphology,and corrosion products of 921A steels were analyzed using various techniques,including corrosion mass loss method,polarization curve,white-light interferometry,scanning electron microscopy,energy-dispersive spectrometry,microbeam X-ray fluorescence spectrometry,X-ray diffraction technique,and X-ray photoelectron *** test results indicated that 921A steel exhibits better corrosion resistance than Q450NQR1 steel in simulated harsh atmospheric environments,as evidenced by a lower corrosion mass loss rate throughout the corrosion *** corrosion products of both steels consisted ofα-FeOOH,Fe_(3)O_(4),andγ-FeOOH,withα-FeOOH being more prevalent in the rust layer of 921A steel than in Q450NQR1 *** inner rust layer of 921A steel also exhibited an appositional enrichment region of Cr,Ni,Mo,and V,leading to its superior corrosion resistance compared to that of Q450NQR1 *** efficacy of high-throughput accelerated corrosion experimental methods was highlighted for evaluating the corrosion resistance of steel materials in harsh environmental *** findings suggest that 921A steel exhibits better corrosion resistance compared to Q450NQR1 steel and has the potential to be more suitable in harsh marine atmospheric *** characterization of the rust layer structures and composition reveals the parallel enrichment of certain elements in the inner rust layer of 921A steel,which enhances its corrosion resistance.

Anomalous response to creep deformation from dislocation density during electrically assisted creep aging

Adding numerous dislocations into metallic materials before the forming stage significantly enhances their deformability. However, this beneficial effect of dislocation defects may not have a simple monotonic relationship with increased dislocation density during electroplastic deformation. This is due to the complex interactions among the drifting electrons, dislocations and solute atoms. This study explores the effect of diverse initial dislocation densities on creep deformation during electrically aided creep aging of an aluminum–lithium alloy. Surprisingly, we discovered a threshold value for the dislocation density that affects electroplastic creep, i.e., an enhanced effect from dislocations weakens when exceeding this density threshold(an anomalous response to creep). Microstructural data also reveal that such an anomalous response originates mainly from differences in various dislocation density-tailored configurations, which can influence the dislocation motions and precipitation kinetics of the strengthening T1precipitates under the same action of pulsed currents. This study provides important insights into the dislocation density-mediated electroplastic creep of an aluminum–lithium alloy.

Climate Change-Related Disaster Risk Events in Togo: A Systematic Review

Togo is facing significant climate challenges that have profound consequences for its environment, economy, and population. This study provides an overview of various climate phenomena affecting Togo and highlights potential adaptation strategies. We used the inclusion and exclusion criteria (PRISMA) to search both French and English articles on climate change-related disaster risk events in Togo through Google Scholar, Directory of Open Access Journals (DOAJ), and PubMed databases using the keywords “Climate Change”, “Floods”, “Drought”, “coastal erosion”, “High winds”, “Epidemy”, Heatwaves”, and “Air pollution”. Twenty-five articles from 2000-2023 were included in this study after applying different criteria. Droughts, floods, coastal erosion, food and crop productivity loss, heatwaves, spread of vector-borne diseases, air pollution, and high winds are among the climate phenomena discussed. These challenges are driven by climate change, altering precipitation patterns, increasing temperatures, and rising sea levels. Drought, floods, coastal erosion, loss of food and crop productivity, spread of vector-borne diseases, air pollution and heatwaves are the most climate risks experienced by Togo. Drought contributes to decreased plant cover, water scarcity, and changes in the water and energy balance. Floods cause property damage, health risks, and disruptions to livelihoods. coastal erosion threatens coastal communities, infrastructure, and ecosystems. Adaptation strategies include early warning systems, improved water management, sustainable agriculture, urban and health planning, and greenhouse gas emissions reduction. Drought-resistant crops, mosquito control, and clean energy adoption are essential.

Hybrid Gene Selection Methods for High-Dimensional Lung Cancer Data Using Improved Arithmetic Optimization Algorithm

Lung cancer is among the most frequent cancers in the world,with over one million deaths per *** is required for lung cancer diagnosis and therapy to be effective,accurate,and *** expression microarrays have made it possible to find genetic biomarkers for cancer diagnosis and prediction in a high-throughput *** Learning(ML)has been widely used to diagnose and classify lung cancer where the performance of ML methods is evaluated to identify the appropriate *** and selecting the gene expression patterns can help in lung cancer diagnoses and ***,microarrays include several genes and may cause confusion or false ***,the Arithmetic Optimization Algorithm(AOA)is used to identify the optimal gene subset to reduce the number of selected *** can allow the classifiers to yield the best performance for lung cancer *** addition,we proposed a modified version of AOA which can work effectively on the high dimensional *** the modified AOA,the features are ranked by their weights and are used to initialize the AOA *** exploitation process of AOA is then enhanced by developing a local search algorithm based on two neighborhood ***,the efficiency of the proposed methods was evaluated on gene expression datasets related to Lung cancer using stratified 4-fold *** method’s efficacy in selecting the optimal gene subset is underscored by its ability to maintain feature proportions between 10%to 25%.Moreover,the approach significantly enhances lung cancer prediction *** instance,Lung_Harvard1 achieved an accuracy of 97.5%,Lung_Harvard2 and Lung_Michigan datasets both achieved 100%,Lung_Adenocarcinoma obtained an accuracy of 88.2%,and Lung_Ontario achieved an accuracy of 87.5%.In conclusion,the results indicate the potential promise of the proposed modified AOA approach in classifying microarray cancer data.

BACKGROUND A new nomenclature consensus has emerged for liver diseases that were previously known as non-alcoholic fatty liver disease(NAFLD)and metabolic dysfunction-associated fatty liver disease(MAFLD).They are now defined as metabolic dysfunction-associated steatotic liver disease(MASLD),which includes cardiometabolic criteria in *** condition,extensively studied in obese or overweight patients,constitutes around 30%of the population,with a steady increase *** patients account for approximately 10%-15%of the MASLD ***,the pathogenesis is complex and is not well *** To systematically review the literature on the diagnosis,pathogenesis,characteristics,and prognosis in lean MASLD patients and provide an interpretation of these new *** We conducted a comprehensive database search on PubMed and Google Scholar between January 2012 and September 2023,specifically focusing on lean NAFLD,MAFLD,or MASLD *** include original articles with patients aged 18 years or older,with a lean body mass index categorized according to the World Health Organization criteria,using a cutoff of 25 kg/m2 for the general population and 23 kg/m2 for the Asian *** We include 85 studies in our *** findings revealed that,for lean NAFLD patients,the prevalence rate varied widely,ranging from 3.8%to 34.1%.The precise pathogenesis mechanism remained elusive,with associations found in genetic variants,epigenetic modifications,and adaptative metabolic *** risk factors included metabolic syndrome,hypertension,and type 2 diabetes mellitus,but their prevalence varied based on the comparison group involving lean *** non-invasive tools,Fibrosis-4 index outperformed the NAFLD fibrosis score in lean *** modifications aided in reducing hepatic steatosis and improving cardiometabolic profiles,with some medications showing efficacy to a lesser ***,lean NAFLD patients ex
