Neutrophil-to-lymphocyte ratio associated with renal function in type 2 diabetic patients

BACKGROUNd Type 2 diabetes mellitus(T2dM)is a leading risk factor for the development and progression of chronic kidney disease(CKd).However,an accurate and con-venient marker for early detection and appropriate management of CKd in in-dividuals with T2dM is limited.Recent studies have demonstrated a strong correlation between the neutrophil-to-lymphocyte ratio(NLR)and CKd.None-theless,the predictive value of NLR for renal damage in type 2 diabetic patients remains understudied.This study included 1040 adults aged 65 or older with T2dM from Shanghai's Community Health service Center.The total number of neutrophils and lym-phocytes was detected,and NLR levels were calculated.CKd was defined as an estimated glomerular filtration rate≤60 mL/min/1.73 m².Participants were di-vided into four groups based on NLR levels.The clinical data and biochemical characteristics were compared among groups.A multivariate logistic regression model was used to analyze the association between NLR levels and CKd.RESULTS Significant differences were found in terms of sex,serum creatinine,blood urea nitrogen,total cholesterol,and low-density lipoprotein cholesterol among patients with T2dM in different NLR groups(PdM patients in the highest NLR quartile had a higher prevalence of CKd(P for trend=0.0011).Multivariate logistic regression analysis indicated that a high NLR was an independent risk factor for CKd in T2dM patients even after adjustment for important clinical and pathological parameters(P=0.0001,odds ratio=1.41,95%confidence intervals:1.18-1.68).CONCLUSION An elevated NLR in patients with T2dM is associated with higher prevalence of CKd,suggesting that it could be a marker for the detection and evaluation of diabetic kidney disease.

Bacteriological Profile of Infections Encountered in a Pneumology service in a Moderate-Income Country [Pneumology department of CHU Cocody (Côte d’Ivoire)]

Introduction: Infections represent a real public health problem aggravating the morbidity and mortality of hospitalized patients. Methods: This was a retrospective study with descriptive purposes over a period of 05 years, in the Pneumology department of the University Hospital of Cocody. Results: The average age in our population was 42 years. We observed a male predo- minance of 64.5%, with a sex ratio of 1.8. Medical history was dominated by HIV infection (23.5%), followed by tuberculosis (15.6%). Concerning lifestyle, smoking was found in 38% of cases. Symptoms progressed chronically in 80% of cases. An infectious syndrome was found in 75% of cases. Microbial culture was positive in 42% of cases. Pseudomonas aeruginosa and Klebsiella pneumoniae were encountered in 26.4% of cases each, followed by Echerichia coli in 10.4% of cases. Klebsiella pneumoniae in the first three years topped the list, but gradually. Pseudomonas aeruginosa maintained its leadership over the last three years. Pseudomonas aeruginosa isolates expressed a resistance rate of 9.8% to ceftazidime and 8.1% to imipenem;to aztreonam (36%), ticarcillin (33.3%) and levofloxacin. These strains were susceptible to fosfomycin (100%), mero- penem (96.6%) and amikacin (96%). For isolated strains of Entero- bacteria- ceae, resistance was observed about ticarcillin (83.3%) and amoxicillin clavula- nic acid (71.2%). Streptococcaceae showed resistance to tetracycline (69.2%) and erythromycin (50%). Over the years there has been an increase in re- sistance to amoxicillin-clavulanic acid ceftriaxone. The death rate was 14%. Conclusion: The bacterial profile of infections is dominated by germs respon- sible for nosocomial infection with significant mortality.

Tiered clustering-based management architecture in mega-satellite networks

Hybrid orbital networking and the proliferation of satellites raise explosive management complexity in mega-satellite networks(MSNs) and prompt the management architecture to the hierarchy. In this regard, we propose a tiered clustering-based management architecture(TCMA) to realize the high-efficient and low-cost management performance. Specifically, based on the constrained satellite coverage, we derive an expression of the mapping relationship between the required control overhead of the controller interaction,the average path length of the proposed TCMA, and the corresponding number of management tiers and cluster size. The expression can determine the optimal number of tiers of the TCMA by minimizing the required flow table size between controllers, while ensuring high transmission efficiency. We then exhibit the elasticity of the hierarchical TCMA by adding the peer connections at the second-highest management tier to reduce maximum redundant paths. Simulation results verify the effectiveness of the proposed TCMA and illustrate that a 3-tier TCMA is sufficient for thousands of satellites.

Factors associated with decreased compliance after on-site extracorporeal membrane oxygenation cannulation for acute respiratory distress syndrome:A retrospective,observational cohort study

Background Extracorporeal membrane oxygenation(ECMO)for acute respiratory distress syndrome(ARdS)is systematically associated with decreased respiratory system compliance(CRS).It remains unclear whether transportation to the referral ECMO center,changes in ventilatory mode or settings to achieve ultra-protective ventilation,or the natural evolution of ARdS drives this change in respiratory mechanics.Herein,we assessed the precise moment when CRS decreases after ECMO cannulation and identified factors associated with decreased CRS.Methods To rule out the effect of transportation and the different modes of ventilation on CRS,we conducted a retrospective,single-center,observational cohort study from January 2013 to May 2020,on 22 patients with severe ARdS requiring on-site ECMO and ventilated in pressure-controlled mode to achieve ultra-protective ventilation.CRS was assessed at different time points ranging from 12 h before ECMO cannulation to 72 h after ECMO cannulation.The primary outcome was the relative change in CRS between 3 h before and 3 h after ECMO cannulation.The secondary outcomes included variables associated with the relative changes in CRS within the first 3 h after ECMO cannulation and the relative changes in CRS at each time point.Results CRS decreased within the first 3 h after ECMO cannulation(−28.3%,95%confidence interval[CI]:−38.8 to−17.9,Pdecrease was mild before and after these first 3 h after ECMO cannulation.To achieve ultra-protective ventilation,respiratory rate decreased in the mean by–13 breaths/min(95%CI:−15 to−11)and driving pressure by−8.3 cmH2O(95%CI:−11.2 to−5.3),resulting in decreased tidal volume by−3.3 mL/kg of predicted body weight(95%CI:−3.9 to−2.6)as compared to before ECMO cannulation(Pduction,driving pressure reduction,and tidal volume reduction were significantly associated with decreased CRS after ECMO cannulation,whereas neither respiratory rate,positive end-expiratory pressure,inspired fraction of oxygen,fluid balance

Soluble p75 neurotrophic receptor as a reliable biomarker in neurodegenerative diseases: what is the evidence?

Neurodegenerative diseases are often misdiagnosed,especially when the diagnosis is based solely on clinical symptoms.The p75 neurotrophic receptor(p75^(NTR))has been studied as an index of sensory and motor nerve development and maturation.Its cleavable extracellular domain(ECd)is readily detectable in various biological fluids including plasma,serum and urine.There is evidence for increased p75NTR ECd levels in neurodegenerative diseases such as Alzheimer’s disease,amyotrophic lateral sclerosis,age-related dementia,schizophrenia,and diabetic neuropathy.Whether p75^(NTR) ECd could be used as a biomarker for diagnosis and/or prognosis in these disorders,and whether it could potentially lead to the development of targeted therapies,remains an open question.In this review,we present and discuss published studies that have evaluated the relevance of this emerging biomarker in the context of various neurodegenerative diseases.We also highlight areas that require further investigation to better understand the role of p75^(NTR) ECd in the clinical diagnosis and management of neurodegenerative disorders.

A new 3d plastoelastohydrodynamic lubrication model for rough surfaces

Plastoelastohydrodynamic lubrication of rough surfaces(R-PEHL)is a cutting-edge area of research in interface fluid-structure coupling analysis.The existing R-PEHL model calculates the elastic-plastic deformation of rough surface by the Love equation in a semi-infinite space smooth surface,which deviates from the actual surface.Therefore,it is an innovative work to study the exact solution of elastic-plastic deformation of rough surface and its influence on the solution results of R-PEHL model.In this paper,a new contact calculation model of plastoelastohydrodynamic lubrication(PEHL)with three-dimensional(3d)rough surface is proposed by integrating numerical method of EHL and finite element method.The new model eliminates an original error introduced by the assumption of semi-infinite space in contact calculation,providing wide applicability and high accuracy.Under the given rough surfaces and working conditions,the study reveals that:(1)the oil film pressure calculated by the new model is lower than that of the smooth surface in semi-infinite space by 200–800 MPa;(2)the Mises stress of the new model is 2.5%–26.6%higher than that of the smooth surface in semi-infinite space;(3)compared with the semi-infinite space assumption,the rough surface plastic deformation of the new model is increased by 71%–173%,and the local plastic deformation singularity may appear under the semi-infinite space assumption;(4)the plastic deformation caused by the first contact cycle on the rough surface of the new model accounts for 66.7%–92.9%of the total plastic deformation,and the plastic deformation of the semi-infinite space accounts for 50%–83.3%.This study resolves the contradiction between the smooth surface assumption and the rough surface in the existing R-PEHL model,establishing a solid logic foundation for the accurate solution of R-PEHL model.

Analysis of a Typical Case of Meteorological service in Tangpu Reservoir

The continuous rainy precipitation process from February to March in 2019 was selected to analyze the effect of meteorological service in Tangpu Reservoir basin,so as to sum up service experience and then lay a better foundation for subsequent services.In response to the rainy weather from december 2018 to early 2019,three rounds of flood discharge were carried out in Tangpu Reservoir.during February-March in 2019,the hit rate of short-term area rainfall forecast for Tangpu Reservoir was 80.0%.Compared with the median of forecast interval,the average absolute error was 7.6 mm,and the relative error was 32.7%.The large deviation in the forecast from March 27 to 28 was deeply analyzed,and it is found that the main reasons were excessive reliance on and trust in a single model,insufficient correction of the actual situation,and insufficient judgment of the nature of precipitation.For the future reservoir meteorological service,three aspects of thinking were put forward,such as further strengthening the sharing of hydrological and meteorological information,improving the forecasting ability,and deepening the research of runoff forecast models.

A Cloud-Fog Enabled and Privacy-Preserving IoT data Market Platform Based on Blockchain

The dynamic landscape of the Internet of Things(IoT)is set to revolutionize the pace of interaction among entities,ushering in a proliferation of applications characterized by heightened quality and diversity.Among the pivotal applications within the realm of IoT,as a significant example,the Smart Grid(SG)evolves into intricate networks of energy deployment marked by data integration.This evolution concurrently entails data interchange with other IoT entities.However,there are also several challenges including data-sharing overheads and the intricate establishment of trusted centers in the IoT ecosystem.In this paper,we introduce a hierarchical secure data-sharing platform empowered by cloud-fog integration.Furthermore,we propose a novel non-interactive zero-knowledge proof-based group authentication and key agreement protocol that supports one-to-many sharing sets of IoT data,especially SG data.The security formal verification tool shows that the proposed scheme can achieve mutual authentication and secure data sharing while protecting the privacy of data providers.Compared with previous IoT data sharing schemes,the proposed scheme has advantages in both computational and transmission efficiency,and has more superiority with the increasing volume of shared data or increasing number of participants.

Introduction Maintaining the functional integrity of respiratory muscles is crucial for critically ill patients to be weaned from mechanical ventilation and decannulated if tracheotomized.dysfunction of the diaphragm is frequent in patients under mechanical ventilation and has been associated with a longer duration of weaning.[1,2]Studies on expiratory muscles are scarce.Two recent studies demonstrated the feasibility and reproducibility of ultrasound in measuring the thickness of abdominal muscles,[3,4]with one study showing abdominal muscle atrophy in 22%of mechanically ventilated critically ill patients during the first week of mechanical ventilation.[4]

Case Retrieval Strategy of Turning Process Based on Grey Relational Analysis

To solve the problem of long response time when users obtain suitable cutting parameters through the Internet based platform,a case-based reasoning framework is proposed.Specifically,a Hamming distance and Euclidean distance combined method is designed to measure the similarity of case features which have both numeric and category properties.In addition,AHP(Analytic Hierarchy Process)and entropy weight method are integrated to provide features weight,where both user preferences and comprehensive impact of the index have been concerned.Grey relation analysis is used to obtain the similarity of a new problem and alternative cases.Finally,a platform is also developed on Visual Studio 2015,and a case study is demonstrated to verify the practicality and efficiency of the proposed method.This method can obtain cutting parameters which is suitable without iterative calculation.Compared with the traditional PSO(Particle swarm optimization algorithm)and GA(Genetic algorithm),it can obtain faster response speed.This method can provide ideas for selecting processing parameters in industrial production.While guaranteeing the characteristic information is similar,this approach can select processing parameters which is the most appropriate for the production process and a lot of time can be saved.
