An approach for optimization of the wall thickness (weight) of a thick-walled cylinder under axially non-uniform internal service pressure distribution

Today, improving the weight/load carrying capacity ratio of a part is the matter of studies in most of the scientific and industrial *** dimensions, the amount of material removed from outer and inner radius while manufacturing and the service pressure applied affect the residual stress distribution throughout the wall thickness and hence the load-bearing capacity of a thick-walled cylinder. Calculation of residual stresses after autofrettage process and optimization of autofrettage outline dimensions by using the amount of service pressures applied are common issues in *** this study, mandrel-cylinder tube interference dimensions were renovated by using traditional methods for swage autofrettage process of a gun barrel. Also, the residual stresses in the cylinder after autofrettage process, inside and outside material removal process and the variable service pressure throughout the cylinder applied were taken into consideration and incorporated into the design. By using the constrained optimization method, wall thickness(thus the weight) was optimized(minimized)to achieve the specified safety factor along the length of the cylinder. For the same cylinder, the results of the suggested analytical/with residual stress calculation approach were compared to analytical/without residual stress calculation results and numerical topology optimization method calculation results. Since the experimental measurement results are not yet available, it was not possible to compare them with the calculation *** suggested approach enabled 22.9% extra weight reduction in proportion to numerical topology optimization and enabled 4.2% extra weight reduction in proportion to analytical/without residual stress *** this approach, the gain from residual stresses after autofrettage operation, the loss of residual stresses after material removal, and the effects of service pressures can be taken into account for each stage of design.

Pancreatic head cancer in patients with chronicpancreatitis

BACKGROUNd: Chronic pancreatitis (CP) is a risk factor of pancreatic adenocarcinoma (PA). The discovery of a pancreatic head lesion in CP frequently leads to a pancreaticoduo denectomy (Pd) which preceded by a multidisciplinary meeting(mm). The aim of this study was to evaluate the relevance between this indication of Pd and the definitive pathological ***: Between 2000 and 2010, all patients with CP who underwent Pd for suspicion of PA without any histological proof were retrospectively analyzed. The operative decision has always been made at an mm. The definitive pathological finding was retrospectively confronted with the decision made at an mm, and patients were classified in two groups according to this concordance (group 1) or not (group 2). Clinical and biological parameters were analyzed, preoperative imaging were reread and confronted to pathological findings in order to identify predictive factors of malignant ***: during the study period, five of 18 (group 1) patients with CP had Pd were histologically confirmed to have PA, and the other 13 (group 2) did not have PA. The median age was52.5 ±8.2 years (gender ratio 3.5). The main symptoms were pain (94.4%) and weight loss (72.2%). There was no patient’s death. Six (33.3%) patients had a major complication (Claviendindo classification ≥3). There was no statistical difference in clinical and biological parameters between the two groups. The rereading of imaging data could not detect efficiently all patients with ***: Our results confirmed the difficulty in detecting malignant transformation in patients with CP before surgery and therefore an elevated rate of unnecessary Pd was found. A uniform imaging protocol is necessary to avoid Pd as a less invasive treatment could be proposed.

AIm: To evaluate the efficacy and safety of the FOLFIRI regimen in patients with metastatic pancreatic adenocarcinoma (PAC) after the failure of gemcitabine and platinum salts. mETHOdS: All consecutive patients with histologically confirmed, metastatic PAC and World Health Organiza-tion performance status (PS) ≤ 2 received FOLFIRI-1 [irinotecan 180 mg/m2 on day 1 and leucovorin 400 mg/m2 followed by 5-fluorouracil (5-FU) 400 mg/m2 bolus, then 5-FU 2400 mg/m2 as a 46-h infusion, biweekly] or FOLFIRI-3 (irinotecan 100 mg/m2 on day 1 and leucovorin 400 mg/m2, then 5-FU 2400 mg/m2 as a 46-h infusion and irinotecan 100 mg/m2 repeated on day 3, biweekly) after failure of gemcitabine and platinum-based chemotherapies as a systematic policy in two institutions between January 2005 and may 2010. Tumor response, time to progression (TTP), overall survival rate (OS) and grade 3-4 toxicities were retrospectively studied. Subgroup analyses were performed to search for prognostic factors. RESULTS: Sixty-three patients (52.4% male, median age 59 years) were analyzed. Among them, 42.9% were PS 0, 38.1% were PS 1 and 19.0% were PS 2. Fifty one patients (81.0%) had liver metastases. Before the FOLFIRI regimen, patients had received 1 line (n = 19), 2 lines (n = 39) or 3 lines (n = 5) of chemotherapy. median TTP obtained with the line before FOLFIRI was 3.9 mo (95% CI: 3.4-5.3 mo). A total of 480 cycles was completed (median: 6 cycles, range: 1-51 cycles). The main reason for discontinuing FOLFIRI was tumor progression (90.3%). Tumor control was achieved in 25 patients (39.7%) (partial response: n = 5, stable disease: n = 20) with FOLFIRI. median TTP was 3.0 mo (95% CI: 2.1-3.9 mo) and median OS was 6.6 mo (95% CI: 5.3-8.1 mo). dose adaptation was required in 36 patients (57.1%). Fifteen patients (23.8%) had grade 3-4 toxicities, mainly hematological (n = 11) or digestive (n = 4). Febrile neutropenia occurred in 3 patients. There was no toxic death. A de novo germline mLH1 mutation in a Lynch syndrome patient with discordant immunohistochemical and molecular biology test results

We describe a patient with a Homo sapiens mutL homolog 1 (mLH1)-associated Lynch syndrome with previous diagnoses of two distinct primary cancers:a sigmoid colon cancer at the age of 39 years, and a right colon cancer at the age of 50 years. The mutation identified in his blood and buccal cells, c.1771delG, ***591Ilefs*25, appears to be a de novo event, as it was not transmitted by either of his parents. This type of de novo event is rare in mLH1 as only three cases have been reported in the literature so far. Further-more, the discordant results observed between repli-cation error phenotyping and immunohistochemistry highlight the importance of the systematic use of both pre-screening tests in the molecular diagnosis of Lynch syndrome.

Occluded Gait Emotion Recognition Based on multi-Scale Suppression Graph Convolutional Network

In recent years,gait-based emotion recognition has been widely applied in the field of computer ***,existing gait emotion recognition methods typically rely on complete human skeleton data,and their accuracy significantly declines when the data is *** enhance the accuracy of gait emotion recognition under occlusion,this paper proposes a multi-scale Suppression Graph ConvolutionalNetwork(mS-GCN).ThemS-GCN consists of three main components:Joint Interpolation module(JI moudle),multi-scale Temporal Convolution Network(mS-TCN),and Suppression Graph Convolutional Network(SGCN).The JI module completes the spatially occluded skeletal joints using the(K-Nearest Neighbors)KNN interpolation *** mS-TCN employs convolutional kernels of various sizes to comprehensively capture the emotional information embedded in the gait,compensating for the temporal occlusion of gait *** SGCN extracts more non-prominent human gait features by suppressing the extraction of key body part features,thereby reducing the negative impact of occlusion on emotion recognition *** proposed method is evaluated on two comprehensive datasets:Emotion-Gait,containing 4227 real gaits from sources like BmL,ICT-Pollick,and ELmd,and 1000 synthetic gaits generated using STEP-Gen technology,and ELmB,consisting of 3924 gaits,with 1835 labeled with emotions such as“Happy,”“Sad,”“Angry,”and“Neutral.”On the standard datasets Emotion-Gait and ELmB,the proposed method achieved accuracies of 0.900 and 0.896,respectively,attaining performance comparable to other ***,on occlusion datasets,the proposedmethod significantly mitigates the performance degradation caused by occlusion compared to other methods,the accuracy is significantly higher than that of other methods.

Acute liver failure is frequent during heat stroke

Acute liver failure （ALF） is relatively frequent during heat stroke （HS）. This risk must be emphasized, because its inddence is higher than is usually thought. In a recent study by Weigand et al, two cases were reported in which liver failure was the leading symptom. We have confirmed their conclusion in a study of 25 cases of HS with ALF, compared with 25 other cases without ALE moreover, we observed that hypophosphatemia on admission could predict occurrence of ALF during HS. As for clinical and other biological parameters, phosphatemia should be monitored for at least 3 d in all cases of HS, even when it is thought to be mild.
