The main aim of this editorial is to comment on the recent article published by Garg et al in the World Journal of Gastroenterology 2023;29:4593-4603.This original research presents a new scoring system for fecal incontinence.Fecal incontinence is a chronic disease with a severe impact on the quality of life of the patients.Substantial social stigmatization often leads to significant underreporting of the condition even during visits to a specialist and could lead to further misman-agement or non-existent management of the disease.An important fact is that patients are often unable to describe their condition when not asked precisely defined questions.This problem is partially resolved by scoring questionnaires.Several scoring systems are commonly used;however,each of them has their shortcomings.For example,the absence of different kinds of leakage besides flatus and stool could further lead to underscoring the incontinence severity.Therefore,there has long been a call for a more precise scoring system.The correct identification of the presence and severity of fecal incontinence is paramount for further diagnostic approach and for choosing the appropriate therapy option.This editorial describes fecal incontinence,its effect on quality of life in general and further evaluates the diagnostic approach with a particular focus on symptom scoring systems and their implications for clinical practice.

OBJECTIVE:To evaluate the efficacy of bee venom acupuncture in humeroscapularis(PHS)patients.METHODS:One hundred and twenty patients diagnosed with PHS were assigned into four groups:BV1(0.01 mg/kg),BV2(0.005 mg/kg),BV3(0.0025 mg/kg),and control group(vitamin B1 plus novocain 3%injection)with 15 d of treatment.The outcomes of the study including visual analogue scale(VAS)score andβ-endorphin,inflammatory cytokines including interleukin-10(IL-10),IL-1βand tumor necrosis factorα(TNF-α)and shoulder function score were assessed at baseline,after 10 and 15 d of treatment.RESULTS:All four groups reported statistically significant improvement in VAS score,motion range,and shoulder function score(P<0.01),only the BV3 group showed significant increase of anti-inflammatory(IL-10)and decrease of pro-inflammatory(IL-1β,TNF-α)cytokines after treatment(P<0.05).The BV3 group presented a significant difference between all outcomes compared to the control and other groups.CONCLUSION:BV3 groups showed better recovery including reduced pain,improved motor function and normalized inflammatory cytokines than current therapy used in Vietnam and other groups.

OBJECTIVE:To evaluate the effectiveness of cycling in combination with electroacupuncture in treatment of poststroke hemiplegia patients at National Hospital of Acupuncture,Vietnam.METHODS:The study was designed as a single-centre,outcome-assessor-blinded parallel randomised controlled trial with 120 post-stroke hemiplegia patients randomly assigned into two groups:electroacupuncture plus cycling(CT group) and electroacupuncture(AT group).Patients were assessed before and after the treatment(using muscle grading,modified Rankin,Barthel,Orgorozo scores and electromyography).Statistical Man–Whitney U test,and Fisher’s exact tests were used to compare between CT and AT groups.RESULTS:The results reported statistically significant improvement in motor function in patients suffering from hemiplegia following ischemic stroke in both CT and AT groups.Patients in CT group experienced a greater improvement compared to those in AT group including better muscle contraction(increased frequency and amplitude of electromyography and increased muscle grading scale);increased recovery(Orgogozo scale),increased independency(Barthel scale) and decreased disability(Modified Rankin scale)(P < 0.01).CONCLUSIONS:Combination with cycling training significantly improves the recovery of post-stroke patients treated with electroacupuncture.

Human saliva is an indispensable fluid that maintains a healthy oral cavity which otherwise can lead to oral diseases(dental caries and periodontitis).In addition,salivary metabolites and microbiome profile provide early detection of systemic diseases such as cancer and obesity.Salivary diagnostic has gained popularity due to its non-invasive sampling technique.Fasting(abstinence from food or drink or both)research for weight loss and improve health is common,but studies using fasting saliva are scarce.Some metabolites in fasting saliva have been reported with interesting results,which can be enhanced by considering different confounding factors.For example,fasting saliva contains higher salivary nitrite,which is related to nitric oxide(NO).NO is a vasodilator supporting the healthy function of endothelial cells and its deficiency is connected to many diseases.The timely supply of NO through exogenous and endogenous means is highlighted and the potential advantage of fasting salivary composition changes in relation to COVID-19 infection is speculated.This review aims to provide a general discussion on the salivary composition,properties,and functions of the whole saliva,including the health benefits of fasting.

Objective: To evaluate transperineal laser ablation (TPLA) with Echolaser® (Echolaser® TPLA, Elesta S.p.A., Calenzano, Italy) as a treatment for benign prostatic hyperplasia (BPH) and prostate cancer (PCa) using the Delphi consensus method.Methods: Italian and international experts on BPH and PCa participated in a collaborative consensus project. During two rounds, they expressed their opinions on Echolaser® TPLA for the treatment of BPH and PCa answering online questionnaires on indications, methodology, and potential complications of this technology. Level of agreement or disagreement to reach consensus was set at 75%. If the consensus was not achieved, questions were modified after each round. A final round was performed during an online meeting, in which results were discussed and finalized.Results: Thirty-two out of forty invited experts participated and consensus was reached on all topics. Agreement was achieved on recommending Echolaser® TPLA as a treatment of BPH in patients with ample range of prostate volume, from 80 mL (80%), comorbidities (100%), antiplatelet or anticoagulant treatment (96%), indwelling catheter (77%), and strong will of preserving ejaculatory function (100%). Majority of respondents agreed that Echolaser® TPLA is a potential option for the treatment of localized PCa (78%) and recommended it for low-risk PCa (90%). During the final round, experts concluded that it can be used for intermediate-risk PCa and it should be proposed as an effective alternative to radical prostatectomy for patients with strong will of avoiding urinary incontinence and sexual dysfunction. Almost all participants agreed that the transperineal approach of this organ-sparing technique is safer than transrectal and transurethral approaches typical of other techniques (97% of agreement among experts). Pre-procedural assessment, technical aspects, post-procedural catheterization, pharmacological therapy, and expected outcomes were discussed, leading to statements and recommendations.Conclusion: Ech

Background:Enhanced depth-of-focus intraocular lenses(EDOF IOL)have filled the gap between monofocal and multifocal intraocular implants with optical qualities of monofocal lenses and usually minor dysphotopsias typical for multifocal lenses.The purpose of this study was to evaluate visual outcomes after bilateral implantation of a new EDOF IOL in patients with requirements for perfect near and intermediate vision.Methods:The study included 15 patients(29 eyes as one was amblyopic)with bilateral implantation of LUXSMART EDOF IOL(Bausch&Lomb)with a targeted myopia(between-0.25 and-0.50D)in both eyes.Monocular corrected and uncorrected visual acuity for far,intermediate and near as well as refractive outcomes were evaluated at 1,3,6 and 12 months after the surgery.Additionally,binocular visual acuity,contrast sensitivity and defocus curve were measured at the final follow-up visit.At 12 months’visit patients completed a questionnaire evaluating patient satisfaction,spectacle independence and presence of dysphotopsias.Results:Binocular uncorrected visual acuities at 12 month’s visit were 0.13±0.16,0.06±0.08,0.07±0.09 and 0.15±0.09 logMAR for far distance,80 cm,66 cm and 40 cm respectively.Corrected binocular visual acuities at 12 months were 0.00±0.00,0.05±0.07,0.05±0.06,0.13±0.16 respectively for distance,80 cm,66 cm and 40 cm.Automated refraction spherical equivalent at 12 months’visit stood at-0.70±0.48D,which was 0.46D less than calculated biometric target,however spherical equivalent of subjective refraction at 12 months equaled–0.49±0.46D,which was closer to preoperative biometric target.Defocus curve had gentle shape without peaks typical for monofocal IOLs.Binocular contrast sensitivity results were superior to average results for that age group and equaled 1.78±0.16 logMAR without correction and 1.81±0.13 logMAR with correction.Spectacle independence for near and intermediate distances was achieved in all patients and for far distance in 73.3%of patients.Burdensome dysphotopsias were not reported in any cas

目的建立护士主导的跨学科中心静脉导管维护管理团队(vascular access specialist team,VAST)模式,并评价其应用效果。方法为提高中心静脉导管维护质量,2021年,在某医院实施了护土主导的VAST管理模式。管理团队由1名资深静脉治疗专业护士主导,联合超声/放射技师、进行中心静脉穿刺的医生、静脉通路维护护士以及行政管理人员等组成。管理模式主要包括完善以护士为主要对象的在岗培训、建立跨学科合作中心静脉穿刺“绿色通道”、推行护土主导的规范静脉治疗查房、指导中心静脉治疗患者进行标准化的自我护理。在管理模式实施前(2020年7月至2021年4月)及实施后(2021年5月至2022年5月)分别调查首次置管成功率、患者的自我护理能力、导管维护依从性及患者综合满意度情况,并进行比较。结果实施护士主导的跨学科VAST管理模式后,中心静脉导管首次置管成功率从92.9%(209/225)提升至100%(225/225);中心静脉导管置入患者自我护理能力总分由实施前(74.7518.4)提升至(99.10±23.65);患者导管维护依从性从93.3%(210/225)提升至99.6%(224/225);患者对导管维护的满意度也优于实施前(P<0.05)。结论护士主导的VAST能有效提高中心静脉置管临床管理质量。

Hydatid disease or echinococcosis is a zoonotic parasitic disease.The lung is the second most commonly affected organ after the liver.Intra-thoracic and extrapulmonary hydatid disease is uncommon and may involve the pleura,mediastinum,heart,diaphragm,and chest wall.Unusual locations or complications of thoracic hydatid disease may pose a diagnostic challenge.We present imaging findings of cases with unusual location and presentations of thoracic hydatid disease with emphasis on their clinical implications.

One of the reasons for patients attending ENT clinic with a persistent feeling of Nasal obstruction is the presence of a narrow nasal valve. Currently, there are many surgical methods for widening narrow nasal valves. Yet, most of these methods are difficult to perform and with unpredictable results. The main purpose of this study is to describe and evalu- ate our technique of minimally invasive nasal valve surgery. Thirty three patients, who attended our clinic complaining from of nasal obstruction, were evaluated for indication for nasal valve surgery, complications, and postoperative re- sults. Thirty one out of the 33 patients (94%) who underwent the surgery reported subjective improvement. Visual ana- logue score (VAS score) was used to estimate the degree of pre and postoperative nasal obstruction, the mean preopera- tive score was 8.891 and the mean postoperative was 3.241 and the improvement was statistically significant (P = 3.174). In conclusion, it can be said that the technique of Minimal Invasive Nasal Valve Surgery was found to be safe and extremely effective for most patients having narrow nasal valve.
