Long-term clinical outcomes following total knee arthroplasty in patients with hemophilic arthropathy:a single-surgeon cohort after a 10-to 17-year follow-up

Background:Total knee arthroplasty(TKA)can reduce severe joint pain and improve functional disability in ***,the long-term outcomes have rarely been reported in ***,this study aimed to evaluate the long-term outcomes and complications of TKA in Chinese patients with hemophilic ***:We retrospectively reviewed patients with hemophilia who underwent TKA between 2003 and 2020,with at least 10 years of *** clinical results,patellar scores,patients’overall satisfaction ratings,and radiological findings were *** surgery for implants during the follow-up period was ***:Twenty-six patients with 36 TKAs were successfully followed up for an average of 12.4 *** Hospital for Special Surgery Knee Score improved from an average of 45.8 to *** average flexion contracture statistically significantly decreased from 18.1°to 4.2°.The range of motion(ROM)improved from 60.6°to 84.8°.All the patients accepted patelloplasty,and the patients’patellar score improved from 7.8 preoperatively to 24.9 at the last *** was no statistically significant difference in clinical outcomes between the unilateral and bilateral procedures,except for a better ROM at follow-up in the unilateral *** and enduring anterior knee pain was reported in seven knees(19%).The annual bleeding event was 2.7 times/year at the last follow-up.A total of 25 patients with 35 TKAs were satisfied with the procedure(97%).Revision surgery was performed in seven knees,with 10-and 15-year prosthesis survival rates of 85.8%and 75.7%,***:TKA is an effective procedure for patients with end-stage hemophilic arthropathy,which relieves pain,improves knee functions,decreases flexion contracture,and provides a high rate of satisfaction after more than ten years of follow-up.

Anatomical and Clinical Implication of Corona Mortis in the Anterior Approaches of Pelvis in Sudanese Patient

Background: Corona Mortis is an abnormal blood vessels connecting Between obturator and external iliac systems of vessels and inferior epigastric vessels. It is located behind the superior public ramus at a variable distance from the symphysis pubis. Orthopaedic surgeons and obestitrtion found these vessels in the field of surgery. So displaced fractures or iatrogenic injury, can cause life threatening bleeding and hence these vascular anomalies are referred to us crown of death. The bulk of the studies about the corona mortise were conducted in the cadavers or 3D CT angiography and there was no studies were conducted about corona mortise in Sudan. The purpose of this study was to study the corona mortis anatomical and clinical implication in the anterior approaches to the pelvis and acetabulum in Sudanese patient. Methodology: Hospital based descriptive cross-sectional study was conducted among patients sustaining anterior pelvic anterior acetabulum fracture and underwent open reduction internal fixation by reconstruction plates. This study was conducted in Bahri Teaching Hospital from June 2017 to June 2018. Through the most common anterior approaches to the pelvis and acetabulum (Stoppa-modified stoppa-ilioinguinal), done by single surgeon and fulfil data sheet intra-operatively. Results: In a total of 30 pelvic halves corona mortis was detected in 13 patients (43%).The Majority in the *** 7 (53.8%) specimens: arterial 4/13 (30.8%), venous 1/13 (7.7%) and both arterial and venous in 8/13 (61.5%) specimens respectively. Patient-specific instruments as a standard procedure in total knee arthroplasty: Logistics and postoperative radiological results in 70 patients

Serous Purpose: There is limited information in the literature surrounding the use of patient-specific instrumentation (PSI) by a large number of surgeons. This prospective observational study was therefore designed to evaluate the logistics of using PSI for total knee arthroplasty (TKA) in a multi-surgeon environment, as well as its accuracy in positioning components. Methods: Of 73 patients enrolled in this study, 3 were excluded after the surgeon decided intraoperatively to switch to conventional instrumentation. Results: Mean operative time was 77.6 minutes. The component size had to be changed in 19 patients and bone cuts corrected in 12. In 65 of the 70 cases (92.9%), mechanical alignment was within the optimal range from 3° varus to 3° valgus, with the remaining 5 cases considered outliers (7.1%). Mean overall mechanical alignment was 0.3° varus (standard deviation, ±2°). There was a distinct variance with regard to the position of the tibial component in the sagittal plane. Conclusions: PSI can be effectively incorporated in larger, multisurgeon practices. Although high accuracy was observed for overall mechanical alignment and component positioning in the frontal and sagittal planes, further attention must be paid to the tibial slope. We highly recommend the use of the extramedullary alignment rod to the proper position of the tibial block, as well as double-checking the slope before performing bone cuts.

The Clinical Outcomes of Transforaminal Percutaneous Endoscopic Discectomy in Treating Lumbar Disc Herniation: A Review

Percutaneous endoscopic lumbar discectomy (PELD) is a minimally invasive technique started during the late 20th century. This process is done through microscopic view under local anesthesia. There is a growing but still insufficient evidence that lumbar EDS shows slightly better results in terms of minor tissue damage, shorter hospital stay, faster return to ordinary daily activities, and patient satisfaction. Recurrence rate still remains a matter of debate, and is related with the surgical skills of the surgeon. The complication rate seems to be similar in both of the techniques i.e., open and endoscopic. More randomized controlled trials, systematic reviews and meta-analysis are needed to clarify whether lumbar EDS can be considered comparable if not superior to standard open discectomy. In spite of lacking defined clinical evidence, lumbar EDS is without doubt a rapidly expanding PELD and its future developments are incredibly promising. Due to less complication rate this technique can be considered as a gold standard compared to the open discectomy. The surgeons still require more cadaveric practices for learning the curve and to approach the herniated disc area. The main objective of this review article is to show the clinical outcomes of the Transforaminal Percutaneous Endoscopic Discectomy in treating the lumbar disc herniation.

Prediction of Intraoperative Fracture by Hammering Sound Frequency Analysis and Stress Estimation during Total Hip Arthroplasty

When a stem is inserted into the femur during total hip arthroplasty, sufficient fixation depends on the surgeon’s experience. An objective method of evaluating whether the stem has been correctly fixed may aid clinicians in their decision. We examined the relationship between the sound frequency caused by hammering the stem and the internal stress in artificial femurs, and evaluated the utility of sound frequency analysis to prevent intraoperative fracture. surgeons inserted one of two types of cementless stems (SL-PLUS and modified CLS) using routine operational procedures into 13 artificial femurs. These are the standard Zweymüllers used in Europe. The difference is the lateral shape;SL-PLUS has holes for removal and the modified CLS has fins to prevent rotation. We estimated stress in the femur via finite element analysis, measured the hammering force, and recorded the sound of hammering for frequency analysis. Finite element analysis revealed that the hammering sound frequency decreased as the maximum stress increased. A decrease in frequency suggested that fixation was sufficient and that continued hammering would increase the risk of fracture. Thus, evaluation of the change in sound frequency during stem insertion may indicate when the hammering force should be reduced, thereby preventing intraoperative periprosthetic fractures. Further frequency change may also predict fractures prior to visual confirmation. We concluded that sound frequency analysis has potential as an objective evaluation method to help prevent intraoperative periprosthetic fractures during stem insertion.
