Malnutrition Assessed Using the Geriatric Nutritional Risk Index Is Associated with Preoperative Incidence of Deep Vein Thrombosis in japanese Patients Undergoing Total Knee Arthroplasty

Purpose: Few studies have evaluated the association between malnutrition and the risk of preoperative deep vein thrombosis (DVT) in patients undergoing primary total joint arthroplasty. This study aimed to investigate the prevalence of preoperative DVT in japanese patients undergoing total knee arthroplasty (TKA) and the importance of malnutrition in the risk of preoperative DVT. Methods: We retrospectively analyzed 394 patients admitted for primary TKA at our institution between January 2019 and December 2023. All patients scheduled for TKA at our institution had serum D-dimer levels measured preoperatively. Lower-limb ultrasonography was examined to confirm the presence of DVT in patients with D-dimer levels ≥ 1.0 µg/mL or who were considered to be at high risk of DVT by the treating physician. Based on the results of lower-limb ultrasonography, all patients were divided into the non-DVT and DVT groups. The incidence of and risk factors for preoperative DVT were investigated, as well as the correlation of DVT with the patient’s nutritional parameters. We used two representative tools for nutritional assessment: the Geriatric Nutritional Risk Index (GNRI) and Controlling Nutritional Status Score. Results: The mean age was 77.8 ± 6.9 years. Preoperative DVT was diagnosed in 57 of the 394 (14.5%) patients. Multivariate logistic regression analysis showed that advanced age and malnutrition status, assessed using the GNRI, were independent risk factors for preoperative DVT. Conclusion: A high incidence of preoperative DVT was observed in patients who underwent TKA. Malnutrition status, as assessed using the GNRI, increased the risk of preoperative DVT. Our findings suggest that clinicians should consider these factors when tailoring preventive strategies to mitigate DVT risk in patients undergoing TKA.

On the beginning of the japanese Upper Paleolithic:A review of recent archaeological and anthropological evidence

The beginning of the japanese Upper Paleolithic has mainly been examined using two major models:the Middle Paleolithic evolutionary model within the archipelago and the continental Upper Paleolithic diffusion/migration ***,recent archeological data from japan and nearby countries are challenging such simple *** paper critically reviews previous chronology of the japanese Paleolithic,including possible Lower and Middle Paleolithic(LP/MP),and attempts to show an alternative model of the beginning of the japanese Upper *** paper suggests several possible specimens of LP/MP and recommends further geoarchaeological investigation to understand the reliability and cultural relationship between possible LP/MP specimens and the Early Upper Paleolithic(EUP).The start of the japanese EUP is presently characterized by a flake industry with trapezoids and denticulates around 39-37 kaBP cal on Paleo-Honshu Island,which has partial resemblance with contemporary assemblages in China and the Korean Peninsula,although trapezoids are endemic only to the japanese EUP and may have derived from the ancestral lithic *** technology appeared earliest on Central Paleo-Honshu Island,about 1000 years later than the earliest flake *** blade technology may have originated from the elongated flake technology of the previous period,the sudden simultaneous emergence implies that it diffused from the Korean *** paper proposes that blade technology from the Korean Peninsula arrived on the northeastern Paleo-Honshu Island,including the japan Sea coastal region of western Honshu,rather than the southwest,where flake technology long prospered,due to differences in ecological settings and adaptation strategies between the two regions.

Malnutrition and Its Association with the Mortality of Patients with Femoral Intertrochanteric Fractures: A Retrospective Analysis of japanese Patients

Background: The incidence of femoral intertrochanteric fractures in older adults is higher than that of femoral neck fractures;however, both conditions are often analyzed together as proximal femoral fractures. Considering the difference in treatment, postoperative complication, and mortality risk, these two fractures should be analyzed separately. This study aimed to analyze 1-year mortality and its risk factors in patients with surgically treated femoral intertrochanteric fractures. Methods: Consecutive patients with intertrochanteric fractures who underwent surgical interventions at our institution between January 2017 and December 2021 were retrospectively reviewed. A total of 238 patients were eligible for inclusion in this study. Patients’ demographic and clinical information were retrospectively collected. Patients were divided into the 1-year mortality (n = 16) and survival (n = 222) groups. The incidence of 1-year mortality and its independent risk factors were investigated using univ..iate and multivariate logistic regression analyses. Results: The mean age of patients was 85.6 ± 8.5 years. The 1-year mortality rate was 6.7% (16/238). Preoperative albumin level, the Geriatric Nutritional Risk Index (GNRI), and malnutrition status (GNRI p = 0.02, p = 0.02, and p = 0.0011, respectively). Multivariate analysis showed that malnutrition status (GNRI p = 0.035) was an independent risk factor for 1-year mortality. Conclusion: Malnutrition status assessed using GNRI (GNRI < 92) was an independent risk factor for 1-year mortality. Our findings suggest that GNRI may be an effective screening tool for predicting postoperative 1-year mortality of patients with surgically treated femoral intertrochanteric fractures.

Basic Research on the Burden of Dropping off and Picking up Children: Differences by Employment Type and Gender in the tokyo.Metropolitan Area

There is a need to reduce the burden of child drop-off and pick-up for child-rearing generations, but most studies on the actual situation in japan are based on survey results. In this study, we analyzed differences in child drop-off and pick-up by employment type and gender, utilizing the “Metropolitan Area Person Trip Survey,” which is a statistical data set. The study targeted households in which both spouses were between 30 and 49 years old, had children under the age of 6, and included the following three groups. 1) Dual-income Group 1 (both spouses employed/on contract/temporary);2) Dual-income Group 2 (husband employed/on contract/temporary, wife part-time);3) Full-time housewife group (husband employed, wife unemployed). The analysis revealed that a) wives are almost always responsible for dropping off and picking up their children;b) husbands drop off and pick up their children less frequently in dual-income households;and c) households with children raising within 10 to 30 km of tokyo.Station have longer commuting times and need to reduce the burden of dropping off and picking up their children.

Background:Combined knee valgus and tibial internal rotation(VL+IR)moments have been shown to stress the anterior cruciate ligament(ACL)in several in vitro cadaveric *** utilize this knowledge for non-contact ACL injury prevention in sports,it is necessary to elucidate how the ground reaction force(GRF)acting point(center of pressure(CoP))in the stance foot produces combined knee VL+IR moments in risky maneuvers,such as ***,the effects of the GRF acting point on the development of the combined knee VL+IR moment in cutting are still ***:We first established the deterministic mechanical condition that the CoP position relative to the tibial rotational axis differentiates the GRF vector’s directional probability for developing the combined knee VL+IR moment,and theoretically predicted that when the CoP is posterior to the tibial rotational axis,the GRF vector is more likely to produce the combined knee VL+IR moment than when the CoP is anterior to the tibial rotational ***,we tested a stochastic aspect of our theory in a lab-controlled in vivo *** females performed 60˚cutting under forefoot/rearfoot strike conditions(10 trials each).The positions of lower limb markers and GRF data were measured,and the knee moment due to GRF vector was *** trials were divided into anterior-and posterior-CoP groups depending on the CoP position relative to the tibial rotational axis at each 10 ms interval from 0 to 100 ms after foot strike,and the occurrence rate of the combined knee VL+IR moment was compared between trial ***:The posterior-CoP group showed significantly higher occurrence rates of the combined knee VL+IR moment(maximum of 82.8%)at every time point than those of the anterior-CoP trials,as theoretically predicted by the deterministic mechanical ***:The rearfoot strikes inducing the posterior CoP should be avoided to reduce the risk of non-contact ACL injury associated with the com

Nitrogen is an indispensable element for life because it is a constituent of amino acids and a key constituent for various materials,as well as fine *** molecules occupy~80%of the atmosphere but are chemically inert due to the strength of the NN triple bond and absence of *** nonpolar nature makes N_(2)totally different from CO with an isoelectronic diatomic structure in ***,it was a challenge in chemistry to activate N_(2)to transform it into chemically usable molecules,such as NH_(3)and NO_(x).The Haber-Bosch(HB)process for the synthesis of NH_(3)from N_(2)and H_(2),which was established in 1913,marked the first practical chemical process of nitrogen fixation through extensive fundamental chemical research and much engineering ingenuity[1,2].This success of the industrial process saves human civilization from a hunger *** then,the HB process has been the predominant method for industrial NH_(3)*** annual NH_(3)production has reached180 million tons globally,which makes it the most mass-produced chemical comparable to methanol.
