Sperm quality improvement after natural anti-oxidant treatment of asthenoteratospermic men with leukocytospermia

Aim： To study the immune-modulating and anti-oxidant effects of beta-glucan, papaya, lactoferrin, and vitamins C and E on sperm characteristics of patients with asthenoteratozoospermia associated with leucocytosis. Methods： Fifty-one patients referred to our Sterility Center for semen analysis were selected. Sperm parameters were assessed before and after patient＇s treatment with beta-glucan, lactoferrin, papaya, and vitamins C and E. DNA damage was assessed by the acridine orange test and sperm structural characteristics were evaluated by transmission electron microscopy. Results： After 90 days of treatment, an increase in the percentage of morphologically normal sperm （17.0 ± 5.2 vs. 29.8 ± 6.5） and total progressive motility （19.0± 7.8 vs. 34.8 ± 6.8） were detected. Structural sperm characteristics as well as chromatin integrity were also improved after treatment. In terms of leukocyte concentration in seminal fluid, a significant reduction was recorded （2.2 ± 0.9 vs. 0.9± 0.2）. Conclusion： The treatment of an inflammatory process by the synergic action of immune modulators and anti-oxidants could protect sperm during maturation and migration, leading to improved sperm function. （Asian JAndrol 2008 Mar;. 10： 201-206）

交感性眼炎是一眼穿透性外伤后发生的双侧肉芽肿性葡萄膜炎。其主要病理特征为葡萄膜的弥散性非坏死性肉芽肿性炎症反应。免疫病理学研究证实 :T淋巴细胞在发病机制中起着重要的作用。疾病早期的T淋巴细胞主要为CD4 + 细胞 ,而晚期则以CD8+ 细胞为主。B细胞占脉络膜浸润细胞的 2 0 %以下 ,可能与疾病的慢性过程相关。另外 ,局部还存在黏附分子、MHC和FasL等免疫相关分子的表达增加。交感性眼炎患者具有一定的遗传倾向 ,与宿主的HLA状态存在某种关联性。其治疗原则是使用免疫抑制剂尽快控制眼内炎症 ,但应密切观察其相应的毒副作用。另外 ,还讨论了有关眼球摘除的问题。

棘阿米巴角膜炎是棘阿米巴属引起的角膜表面的感染。通常由粘附于软性接触镜的棘阿米巴滋养体所传递。当寄生虫粘附于角膜上皮细胞表面时可触发感染。后来的过程包括:寄生虫介导的细胞病理效应、诱导凋亡和通过产生各种蛋白酶入侵角膜基质层。滋养体也可浸润角膜神经,造成放射状角膜神经炎。棘阿米巴包囊即具有免疫原性又具有抗原性。然而,诱导针对棘阿米巴抗原的细胞介导的免疫应答对棘阿米巴角膜炎即无减轻作用又无预防效应。但是,普通粘膜免疫系统对于棘阿米巴角膜炎却具有显著的预防作用。其机制是通过IgA抗体阻断感染性滋养体与上皮层的结合。局部使用糖皮质激素对于缓解疼痛和减轻炎症反应具有显著效果,但可通过诱导静止包囊的脱包囊、增加滋养体的细胞病理效应以及麻痹初始免疫系统使疾病复燃。因此,糖皮质激素的使用是一把“双刃剑”。

Transrectal ultrasonography in infertile patients with persistently elevated bacteriospermia

Aim： To identify and define prostate and seminal vesicle abnormalities in patients with chronic male accessory gland infection （MAGI） who failed to respond to antibacterial treatment. Methods： We selected 67 consecutive patients with MAGI and persistently elevated bacteriospermia （≥ 10^6 colony forming units [CFU]/mL） after three antibiotic courses. Fourteen infertile patients with initial chronic microbial （≥ 10^6 CFU/mL） MAGI who responded to antibacterial treatment （〈 10^3 CFU/mL） served as a control group. All patients and controls underwent transrectal ultrasonography （TRUS） scans and semen analysis. Patients with low seminal plasma volume （〈 1.5 mL） underwent both preejaculatory and post-ejaculatory TRUS examination. Results： TRUS revealed multiple abnormalities indicative of：（i） bilaterally extended prostato-vesiculitis （group A： 52 cases, 77.6%）（nine of these patients also had micro-emphysematous prostate abscess）; and （ii） prostato-vesiculitis with unilateral or bilateral sub-obstruction of the ejaculatory ducts （group B： 15 cases, 22.4%）. Mean sperm concentration, total sperm number, ejaculate volume and pH value were significantly higher in group A than in group B. In addition, sperm forward motility and the percentage of normal forms were significantly worse than in controls, whereas leukocyte concentration was significantly higher in group A. Group B patients had all sperm parameters, but their pH values, significantly different from those of controls. Conclusion： Although antibiotic therapy is considered suitable when microbial MAGI is suspected, it is impossible to account for a poor response to antibiotics merely on the basis of conventional criteria （clinical history, physical and ejaculate signs）. Thus, TRUS may be helpful in the follow-up of these patients.

Impact of HLA-G analysis in prevention, diagnosis and treatment of pathological conditions

Human leukocyte antigen-G(HLA-G) is a non-classical HLA class Ⅰ molecule that differs from classical HLA class Ⅰ molecules by low polymorphism and tissue distribution. HLA-G is a tolerogenic molecule with an immune-modulatory and anti-inflammatory function on both innate and adaptative immunity. This peculiar characteristic of HLA-G has led to investigations of its role in pathological conditions in order to define possible uses in diagnosis, prevention and treatment. In recent years, HLA-G has been shown to have an important implication in different inflammatory and autoimmune diseases, pregnancy complications, tumor development and aggressiveness, and susceptibility to viral infections. In fact, HLA-G molecules have been reported to alternate at both genetic and protein level in different disease situations, supporting its crucial role in pathological conditions. Specific pathologies show altered levels of soluble(s)HLA-G and different HLA-G gene polymorphisms seem to correlate with disease. This review aims to update scientific knowledge on the contribution of HLA-G in managing pathological conditions.

CD4+T cells play fundamental roles in orchestrating immune responses and tissue ***,our inability to associate peptide human leukocyte antigen class-II(HLA-II)complexes with their cognate T cell receptors(TCRs)in an unbiased manner has hampered our understanding of CD4^(+)T cell function and role in ***,we introduce TScan-II,a highly sensitive genome-scale CD4^(+) antigen discovery *** platform seamlessly integrates the endogenous HLA-II antigen-processing machinery in synthetic antigen-presenting cells and TCR signaling in T cells,enabling the simultaneous screening of multiple HLAs and *** genome-scale human,virome,and epitope mutagenesis libraries,TScan-II facilitates de novo antigen discovery and deep exploration of TCR *** demonstrate TScan-II's potential for basic and translational research by identifying a non-canonical antigen for a cancer-reactive CD4^(+)T cell ***,we identified two antigens for clonally expanded CD4^(+)T cells in Sjögren's disease,which bind distinct HLAs and are expressed in HLA-II-positive ductal cells within affected salivary glands.

Essential concept of transplant immunology for clinical practice

Our understanding of transplant immunology has advanced from gross allograft rejection to cellular response and to current molecular level. More sensitive assays have been developed to characterize patient sensitization and to detect pre-existing donor-specific antibodies(DSA) in pre-transplant crossmatch. After a transplant, pre-existing or de novo DSA are increasingly monitored to guide clinical management. Therefore, it is important for clinicians to understand the basic concepts and key components of transplant immunology as well as be familiarized with the modern immunological techniques used in kidney transplantation.

Among multiple sclerosis(MS)susceptibility genes,the strongest non-human leukocyte antigen(HLA)signal in the Italian population maps to the TNFSF14 gene encoding LIGHT,a glycoprotein involved in dendritic cell(DC)*** fine-mapping in a large Italian dataset(4,198 patients with MS and3,903 controls),we show that the TNFSF14 intronic SNP rs1077667 is the primarily MS-associated variant in the *** quantitative trait locus(e QTL)analysis indicates that the MS risk allele is significantly associated with reduced TNFSF14 messenger RNA levels in blood cells,which is consistent with the allelic imbalance in RNA-Seq reads(P<0.0001).The MS risk allele is associated with reduced levels of TNFSF14 gene expression(P<0.01)in blood cells from 84 Italian patients with MS and 80 healthy controls(HCs).Interestingly,patients with MS are lower expressors of TNFSF14 compared to HC(P<0.007).Individuals homozygous for the MS risk allele display an increased percentage of LIGHT-positive peripheral blood myeloid DCs(CD11 c+,P=0.035)in 37 HCs,as well as in in vitro monocyte-derived DCs from 22 HCs(P=0.04).Our findings suggest that the intronic variant rs1077667 alters the expression of TNFSF14 in immune cells,which may play a role in MS pathogenesis.
