1 Introduction Over one million cardiac pacemakers are implanted every year worldwide,[11 of which approximately 200,000 are implanted in the United States alone.121 Combined with an aging population and increasing pacing indications, these numbers are expected to grow. Since the first pacemaker implantation in 1950s, cardiac pacemaker technology has rapidly advanced. Reduction in generator size, increased battery longevity, quality of pacemaker leads, algorithmic and rate responsive programming-all have revolutionized and transformed the implantation and management of transvenous cardiac pacemaker （TV-PPM）.

Physical inactivity is very common in ***,association between physical inactivity and mortality in octogenarians with acute coronary syndrome(ACS)remains *** this study,we aimed to investigate association between physical inactivity and allcause mortality in octogenarian patients with *** this study,we included a total of 353 hospitalized patients,aged≥80 years,with ACS during the period of 5-year *** association between physical inactivity and all-cause mortality was analyzed by multivariable Cox *** the enrolled patients,132(37.4%)were defined as physically inactive,and 221(62.6%)as physically *** with physical inactivity tended to have lower survival rate(21.2%vs.56.5%,P〈0.001)and higher mortality rate(78.8%vs.43.5%,P〈0.001),and had a worse long-term outcome than those with physical activity(chi-square=27.52,and log rank P〈0.001).The physical inactivity was still an independent predictor for long-term allcause mortality independent of confounders including age,prior heart failure,stroke,ejection fraction,beta-blocker,clopidogrel and percutaneous coronary intervention(HR:2.35,95%CI:1.26–4.37,P=0.007).Our study demonstrates that physical inactivity is independently related to increased all-cause mortality in octogenarians with ACS.

To explore mechanism and protective effect of rosiglitazone on myocardial ischemia reperfusion(I/R) ***:A total of 48 male Japanese white big-ear rabbits were randomly divided into control group(A),I/R group(B),low dose of rosiglitazone group(C),high dose of rosiglitazone group(D).Plasma concentration of and also reduced the concentration of plasma serum creatine kinase(CK),***-sensitivity C-reactive protein(hsCRP).ultrasuperoxide dismutase(SOD),malondialdehyde(MD.A).lactic acid glutathione skin peroxidase (C-SH-PX).nitric oxide(NO)and endothelin(ET) were measured 1 h later after I/***-four hours after I/R the hearts were harvested for pathological and ultrastructural *** of myocardial infarction were ***:Plasma concentration of CK,***,NO. MDA and ET were decreased in C,D group compared with group *** concentration of T-SOD and GSH-Px were increased significantly in C.D group compared with group *** with group *** and ullraslructural changes in C and D group were *** was significant difference in myocardial infarction area between group C.D and group B(P<0.05). Myocardial infarction area and arrhythmia rate were lower in group C,D compare with group B. Rosiglitazone may protect myocardium from I/R injury by enhancing T-SOD and GSH-Px concentration,inhibit inflammatory reaction,and improve endothelial function.

The subcutaneous implantable cardioverter defibrillators （SICD） is an altemative to the transvenous ICD for the prevention of sudden cardiac death （SCD）. Multiple studies have shown that the SICD is safe and effective in treating ventricular arrhythmias. While earlier stud- ies mainly enrolled younger patients with channelopathies, more recent reports included patients with ＂typical＂ indications for ICD therapy for the prevention of SCD. In this review we summarize the data available to date on the SICD while highlighting its pros and cons.

Over the years, the use of transradial approach （TRA） for carotid artery stenting （CAS） with distal embolic protection has been recognized as a valid alternative to the conventional femoral approach, improving the outcomes compared to carotid endoatherectomy.0-33 Indeed, despite the femoral artery remains the conventional access site for the endovascular treatment of supraoartic vessels, concomitant anatomical variations and/or peripheral vascular disease could complicate the cannulation of such arteries. Moreover, the TRA has been related with a lower incidence of bleeding complications and a shorter bed rest after the interventional procedure.

Myocarditis is associated with an increased risk of sudden cardiac death (SCD) in the ***,Information on nationwide burden of SCD caused by myocarditis (SCD-myocarditis) is *** this study all deaths among persons in Denmark aged 1-35years in 2000-2009 and 36-49years in 2007-2009 (27.1 million person-years) were *** reports,death certificates,discharge summaries,and nationwide registries were used to identify all cases of *** the 10-year study period,there were 14294 deaths,of which we identified 1 363 (10%) *** autopsied SCD (n=753,55%),cause of death was myocarditis in 42 (6%) cases corresponding to an SCD-myocarditis incidence of 0.16 (95%CI: 0.11-0.21) per 100000 *** had significantly higher incidence rates of SCD-myocarditis compared to females with an incidence rate ratio of 2.2 (95%CI:1.1-4.1).Myocarditis was not registered as cause of death In any of the non-autopsied SCD (n=610,45%).In conclusion,after nationwide unselected Inclusion of 14294 deaths,we found that 6% of all autopsied SCD was caused by *** cases of SCD-myocarditis were reported in the non-autopsied SCD,which could reflect underdiagnosing of myocarditis in non-autopsied ***,our data suggest a female protection towards SCDmyocarditis.
