Orbital reconstruction:From simple materials to bioengineered solutions

Orbital fractures are a frequent and serious problem for practicing ophthalmologists. The complexity of the pathology is explained by the combined nature of the injuries(often associated with craniofacial injuries), multistage treatments, results that are often unsatisfactory, and a wide range of complaints about functional and cosmetic limitations. Over the years, significant progress has been made in the field of orbital reconstruction,allowing the transition from traditional methods using simple materials to innovative bioengineering *** evolution has been driven by advances in surgical technologies, imaging techniques, and biomaterials aimed at optimizing the restoration of the shape and function of the orbital region. Traditional approaches are based on the use of autologous tissues such as bone grafts and muscle flaps, which provide biocompatibility and natural integration, but have limitations in terms of customization and accessibility. The advent of patient-specific implants and 3D printing technology has revolutionized the reconstruction of the orbit, allowing implants to be precisely adapted to a patient's anatomy. Biocompatible materials, such as porous polyethylene, titanium, and silicone, have become the basis for orbital reconstruction, ensuring durability and compatibility while minimizing long-term complications. Bioengineered solutions hold promise for further advancements in orbital reconstruction. We searched Pub Med, Cyberleninka, and other verified databases for published articles on orbital reconstruction reported in the literature between 1960 and January 2024. In this article, we consider the advantages and disadvantages of each category of reconstruction materials and provide up-to-date information on the methods for modifying their properties using modern processing technologies.

Porous Shish-Kebab Structure Prepared from Oriented UHMWPE Films by Processing in Supercritical CO_(2)

For the first time, a highly crystalline porous shish-kebab structure with a high degree of crystallinity was obtained by using a combination of two methods for the formation of porous polymeric materials. A treatment procedure using supercritical carbon dioxide(sc CO_(2))was carried out for oriented ultrahigh molecular weight polyethylene(UHMWPE) films, which provided special conditions for the crystallization of dissolved UHMWPE macromolecules on the surface of oriented UHMWPE crystals. The prepared porous materials were investigated by scanning electron microscopy(SEM) and differential scanning calorimetry(DSC). The particularity of the obtained porous shish-kebab is the absence of the amorphous phase between lamellar crystals(kebabs). The obtained pores had an oval shape, and they were oriented in the orientation direction of the UHMWPE macromolecules. The pore size ranged from 0.05 μm to 4 μm. Controlling the conditions for the crystallization of the UHMWPE macromolecules using supercritical CO_(2) gives the possibility to control the size of both lamellar disks and pores formed.

Changes in renal function and morphological variations of kidney diseases in rheumatoid arthritis patients

A systematic review of cytoreductive prostatectomy outcomes and complications in oligometastatic disease

Objective: To analyze outcomes and complications of cytoreductive prostatectomy (CRP) for oligometastatic prostate cancer (PCa) in order to elucidate its role in this ***: We performed a systematic literature search using three databases (Medline, Scopus, and Web of Science). The primary endpoints were oncologic outcomes. The secondary endpoints were complication rates and functional ***: In all studies, overall survival was better or at least comparable variable in the groups with CRP compared to no local treatment. The greatest benefit from CRP in 5-year overall survival in one study was 67.4% for CRP versus 22.5% for no local treatment. Cancer-specific survival (CSS) showed the same trend. Several authors found significant benefits from CSS in the CRP group: from 79% vs. 46% to 100% vs. 61%. CRP was a predictor of better CSS (hazard ratio 0.264, p=0.004). Positive surgical margin rates differed widely from 28.6% to 100.0%. Urinary continence in CRP versus RP for localized PCa was significantly lower (57.4% vs. 90.8%, p<0.0001). Severe incontinence occurred seldom (2.5%-18.6%). Total complication rates after CRP differed widely, from 7.0% to 43.6%. Rates of grades 1 and 2 events prevailed. Patients on ADT alone also showed a considerable number of complications varying from 5.9% to 57.7%.Conclusion: CRP improves medium-term cancer control in patients with oligometastatic PCa. The morbidity and complication rates of this surgery are comparable with other approaches, but postoperative incontinence rate is higher compared with RP for localized disease.

Challenges in differential diagnosis between obstructive and non-obstructive azoospermia

Background:Some cases of non-obstructive azoospermia(NOA)are characterized by normal clinical parameters,including testicular volume and levels of reproductive hormones,mimicking obstructive azoospermia(OA).Methods:We performed a retrospective review of a consecutive series of 1417 patients undergoing primary surgical sperm retrieval between 2014 and ***-stimulating hormone(FSH)level below 7.6 IU/l and normal testicular size with a long axis measurement>4.6 cm were used as criteria to suspect ***:Four hundred and eighteen patients with normal testicular volume and FSH levels had an initial diagnosis of *** them,243(58.1%)had histological signs of spermatogenic dysfunction,and 175(41.9%)had true *** hundred eleven patients had long-standing obstruction(median:16.5 years)with a median Bergmann-Kliesch score(BKS)of 5(interquartile range[IQR]:4-6)and 100%sperm retrieval rate(SRR),though some required microdissection testicular sperm extraction(microTESE).Fifty-eight patients with a history of epididymo-orchitis had a median BKS of 4(IQR:2-6)and 100%*** patients with a history of unjustified medical treatment for male infertility had a median BKS of 3(IQR:1-4)and 80%***-four patients had uniform maturation arrest with a 5.5%SRR on ***:Men with normal testicular volume and FSH level may have evidence of spermatogenic failure on *** with complicated seminal tract obstruction commonly have hypospermatogenesis,but true NOA caused by uniform maturation arrest may also be *** counseling for suspected OA should not be overly optimistic,and couples should be warned about possibility of conversion to microTESE and risks of negative sperm retrieval.

Characteristics of gut microbiota in anastomotic leakage patients in cold zones post-colorectal cancer surgery: A high-throughput sequencing and propensity-score matching study
