Facial nerve monitoring in parotid gland surgery:design and feasibility assessment of a potential standardized technique

Background:Even though the use of nerve monitoring during parotid gland surgery is not the gold standard to prevent damage to the nerve,it surely offers some advantages over the traditional *** from thyroid surgery,where a series of steps in intraoperative nerve monitoring have been described to confirm not only the integrity but—most importantly—the function of the recurrent laryngeal nerve,in parotid gland surgery,a formal guideline to follow while dissecting the facial nerve has yet to be ***:A five‐year retrospective study was done reviewing the intraoperative records of patients who underwent parotid gland surgery under neural *** operative findings regarding the neuromonitoring process,particularly in regard to the amplitude of two main branches,were revised.A literature search was done to search for guidelines to follow when a facial nerve loss of signal is ***:Fifty‐five patients were operated on using the nim 3 nerve Monitoring System(Medtronic);31 were female patients,and 47 patients had benign *** changes were observed in the amplitude records after a comparison was made between the first and the last *** were only three articles discussing the term loss of signal during parotid gland ***:Today,no sufficient attention has been given to the facial nerve monitoring process during *** study proposes a formal guideline to follow during this procedure as well as an instruction to consider when a loss of signal is observed to develop a uniform technique of facial nerve stimulation.

Introduction: neisseria is a large genus of bacteria that colonize mucosal surfaces. Of the 11 species that colonize humans, only two are pathogens, n. meningitidis and n. gonorrhoeae. Both are obligate human pathogens;the last is causal agent of gonorrhea disease. Although curable with timely antibiotic treatment, an annual incidence of more than 62 million cases is estimated by the World Health Organization and there are no successful vaccines available. In contrast, several prophylactic vaccine options for neisseria meningitidis meningitis do exist. Of note, there is trace of cross parenteral response induced between n. meningitidis and n. gonorrhoeae, and Proteoliposome (PL, also named outer membrane vesicles, OMV) vaccine has shown high impact on this response. Objective: To determine effect of VAMEnGOC-BC? and its derivates (AFPL1 and AFCo1) at mucosal and systemic level and possible cross response against neisseria gonhorroeae in Balb/c and C57Bl/6 mice. Methods: We evaluated cross response against n. gonorrhoeae in mouse serum IgG and saliva IgA after mucosal immunization with VA-MEnGOC-BC or its derivatives (AF, Adjuvant Finlay PL1 or AFCo (cochleate) 1). Results: Immunizations with AFPL1 or AFCo1 induce anti n. gonorrhoeae at saliva IgA and serum IgG responses similar to VA-MEnGOC-BC? vaccine. Conclusions: Such data confirms a new possible window of prime-boost vaccination strategy against gonorrhea and extends our knowledge regarding the effect of PL vaccines on cross responses.

First Characterization of Escherichia coliStrains Isolated from Wildlife Griffon Vulture (Gyps fulvus) in the Southeast of Spain

The aim of the present study was to characterize a collection of Escherichia coli strains isolated from asymptomatic griffon vulture (Gyps fulvus) during a reintroduction program in the southeast of Spain, in order to establish if griffon vulture could play a role in the spread of resistant or potentially pathogenic E. coli strains. For this purpose, 14 E. coli strains obtained from 10 griffon vulture were studied to establish their serotypes, phylogroups, virulencegene profiles and antimicrobial resistances. High heterogeneity was observed within the 14 strains isolated which belonged to three phylogroups (A, B1 and D), 8 serogroups (O2, O21, O29, O60, 073, O78, O103 and O141) and 13 different serotypes. Out of 34 genes screened, we have detected eight virulence genes that are typical of extraintestinal pathogenic E. coli (ExPEC) (fimH, fimAvMT78, iron, iucD, cvaC, iss, traT and tsh);however, none of the studied strains showed the ExPEC status. The 14 strains were also analyzed for the production of extended-spectrum beta-lactamases (ESBLs) and for antimicrobial resistances. none of the 14 strains were ESBL-producing E. coli, but high resistance-prevalences to ampicillin and cotrimoxazole were detected. To our knowledge, this is the first characterization of E. coli strains isolated from griffon vulture and although they did not show high virulencegene scores, they showed cotrimoxazole resistance.

dear Editor,Rauvolfia tetraphylla(aka the devil pepper)(Supplemental Figure 1)is a well-known medicinal plant that produces monoterpenoid indole alkaloids(MIAs).This MIA biosynthesis occurs in several organs,including leaves,stems,fruit,and roots,which accumulate the famous antiarrhythmic ajmaline(Kumar et al.2016a,2016b;Kumara et al.,2019).MIAs are natural products notably involved in plant adaptation to the environment and defense against *** mainly results from their high biological activities,which also explain their pharmacological properties.

neonatal necrotizing Fasciitis after Circumcision: A Case Report and Review of Literature

Background: necrotizing fasciitis is a soft tissue infection that occurs predominantly in adults. Although rare, only 70 cases have been reported in neonates. This entity, most commonly caused by multiple organisms is often fatal, with 50% mortality. The combination of a low incidence and high mortality in this subgroup strengthens the need for early diagnosis, prompt recognition and immediate surgical treatment in order to improve survival. Aim: The aim of this case report is to bring awareness of a clinical condition that relies mostly on physical examination and where early diagnosis and immediate surgical treatment are needed in order to improve survival. Case presentation: We report the case of an 8-day-old boy presenting neonatal necrotizing fasciitis after circumcision. Patient presented with lower back erythema and areas of fluctuation. He was started on antibiotic therapy and 24 hours after admission, he was taken to the operating room for multiple fasciotomies, debridement and cleansing of affected areas. Findings were consistent with necrotizing fasciitis. After surgical intervention, local care was given until complete healing was achieved, approximately one month after admission. Conclusion: The surgeon must be attentive for the possible occurrence of this lethal infection that may develop post operatively after clean or clean-contaminated procedures. Preparation for treating these patients immediately with adequate fluid resuscitation, appropriate antibiotics and aggressive surgical debridement of non-viable tissue will provide the maximal potential for recovery. These prompt measures will offer the best chance for survival and eventually decrease the 50% mortality rate associated to the neonatal population.
