Probiotic Medilac-S^(█) for the induction of clinical remission in a Chinese population with ulcerative colitis: A systematic review and meta-analysis

AIM To assess the effects of probiotic Medilac-S~ as adjunctive therapy for the induction of remission of ulcerative colitis(UC) in a Chinese population through a systematic review and meta-analysis. METHODS A systematic literature search was conducted to find randomized, controlled trials in a Chinese population with at least two study arms-a control arm which receives a conventional, oral aminosalicylate drug, and a treatment arm, which administers the same conventional drug in conjunction with the probiotic Medilac-S~ per os. Both English and Chinese databases were searched, inc.uding Pub Med, EMBASE, Google Scholar, Chinese National Knowledge Infrastructure, Wanfang Data, and VIP Search, and study data was extracted onto standardized abstraction sheets. Meta-analyses were conducted for primary and secondary outcomes of interest using a fixed or random effects model. The primary outcome was the induction of clinical remission and the secondary outcomes inc.uded changes in Sutherland index, endoscopic and histological scores, proportion of reported clinical symptoms and adverse events(AEs). For outcomes with sufficient data, the type of conventional drug therapy was also assessed to determine if the effects of combination therapy with Medilac-S~ was influenced by drug type. All tests were conducted using a type Ⅰ error rate of 0.05 and all confidence intervals(CI) were based on a 95% confidence level. Review protocol was uploaded to PROSPERO(CRD42018085658 upon completion).RESULTS Fifty-three clinical trials with a total of 3984 participants were identified and inc.uded in the review. Medilac-S~ adjunctive therapy significantly improved induction of clinical remission(RR = 1.21; 95%CI: 1.18-1.24; P < 0.0001) with the estimated likelihood of effective treatment, on average, 21% higher for those consuming the probiotic. A novel compound, Delta-9-Tetrahydrocannabinol Nitrogen Mustard (THC-mustard), waschemically synthesized and characterized. The rationale was to target a known anti-tumoragent, nitrogen mustard, to tumor cells with 'THC Receptors' and/or 'Estrogen Recep-tots', A microtest by vitro bioassay was designed and developed to compare the ID50of the drug in cell culture against various tumor cell types. The ID50’s of the THC-mustardwere determined against several tumor cell types in culture, compared with MechlorethamineHCI, Delta-9-Tetrahydrocannabinol (THC), and an equimolar mixture of THC and nitrogennmstard. A preliminary toxicity study by the NCI of the THC-mustard (in vivo) in Swissmale mice bearing the P388 tumor (1 dose × 1day) by the I. P. route was also carried out.A deseription of the rationale, chemical synthesis and characterization, bioassay, and resultsis herewith presented.

In this paper, the effect of four sequential cuts in side milling of Ti6Al4V on chip formation and residual stresses （RS） are investigated using finite element method （FEM）. While the open literature is limited mainly to the studies of orthogonal sequential cutting with the constant uncut chip thickness greater than 0.01 mm, it is suggested herein to investigate not only the variable uncut chip thickness which characterises the down milling process, but also the uncut chip thickness in the sub-micron range using a finite cutting edge radius. For the resulting ductile machining regime, the characteristics of the chip mor- phology, the force profiles, the plastic deformation and temperature distributions have been analyzed. Furthermore, this study revealed that the RS should be extracted toward the area where the insert exits the workpiece in the FE simulation of the down-milling process. The simulation of a number of sequential cuts due to the consecutive engagements of the insert is required in order to capture the gradual accumulation of the RS before reaching an asymptotic convergence of the RS profile. The predicted RS are in reasonable agreement with the experimental results.

A continuous wave carbon dioxide (CO2) laser (λ=10.6μm) was employed to treat the surface of Si3N4 MOR (modulus of rupture) bars. The effects of the CO2 laser process on physical and mechanical properties of ground Si3N4 samples were investigated. Scanning electron microscopy (SEM) analysis shows that the area occupied by cavities and fracture is decreased by about 49.4% after laser treatments. Cross-sectional metallography results indicate that the secondary YSiAlON phase in the Si3N4 ceramic is softened/melted and flowed into the defects. Four-point bending tests show that the flexural strength of the treated samples is improved to 10.9%. Fractographic analysis show that the fracture origins move from the surface to subsurface. It is concluded that laser surface processing have significant effects on fracture behavior of flexure Si3N4 ceramic.

This paper reviews and discusses the design of a low-power single-full-band (3.1-10.6 GHz) non- carrier impulse-radio ultra wideband (UWB) transmitter featuring 5th-order Gaussian derivative pulse shaping, integrated BPSK modulation, and 2.5 kV whole-chip ESD (electrostatic discharge) protection. The UWB transmitter design has been implemented in a commercial 0.18 μm CMOS technology with a very small die size of 0.25 mm2. The fabricated chips have demonstrated full functionality, extremely low power consumption of 0.14 pJ/p-mV, and an ultra short pulse width of 394 ps. This ESD-protected UWB transmitter has the potential to support wireless streaming to gigabit per second (Gbps).

Performance analysis of the Pangu Plaza under earthquake and wind loads is described in this paper. The plaza is a 39-story steel high-rise building, 191 m high, located in Beijing close to the 2008 Olympic main stadium. It has both fluid viscous dampers （FVDs） and buckling restrained braces or unbonded brace （BRB or UBB） installed. A repeated iteration procedure in its design and analysis was adopted for optimization. Results from the seismic response analysis in the horizontal and vertical directions show that the FVDs are highly effective in reducing the response of both the main structure and the secondary system. A comparative analysis of structural seismic performance and economic impact was conducted using traditional methods, i.e., inc.eased size of steel columns and beams and/or use of an inc.eased number of seismic braces versus using FVD. Both the structural response and economic analysis show that using FVD to absorb seismic energy not only satisfies the Chinese seismic design code for a ＂rare＂ earthquake, but is also the most economical way to improve seismic performance both for one-time direct investment and long term maintenance.

AIM: To determine the efficacy of probiotic supplementation on intestinal transit time(ITT) in adults and to identify factors that influence these outcomes. METHODS: We conducted a systematic review of randomized controlled trials of probiotic supplementation that measured ITT in adults. Study quality was assessed using the Jadad scale. A random effects meta-analysis was performed with standardized mean difference(SMD) of ITT between probiotic and control groups as the primary outcome. Meta-regression and subgroup analyses examined the impact of moderator variables on SMD of ***: A total of 15 clinical trials with 17 treatment effects representing 675 subjects were inc.uded in this analysis. Probiotic supplementation was moderately efficacious in decreasing ITT compared to control, with an SMD of 0.38(95%CI: 0.23-0.53, P < 0.001). Subgroup analyses demonstrated statistically greater reductions in ITT with probiotics in subjects with vs without constipation(SMD: 0.57 vs 0.22, P < 0.01) and in studies with high vs low study quality(SMD: 0.45 vs 0.00, P = 0.01). Constipation(R^2 = 38%, P < 0.01), higher study quality(R^2 = 31%, P = 0.01), older age(R^2 = 27%, P = 0.02), higher percentage of female subjects(R^2 = 26%, P = 0.02), and fewer probiotic strains(R^2 = 20%, P < 0.05) were predictive of decreased ITT with probiotics in meta-regression. Medium to large treatment effects were identified with B. lactis HN019(SMD: 0.67, P < 0.001) and B. lactis DN-173 010(SMD: 0.54, P < 0.01) while other probiotic strains yielded negligible reductions in ITT relative to ***: Probiotic supplementation is moderately efficacious for reducing ITT in adults. Probiotics were most efficacious in constipated subjects, when evaluated in high-quality studies, and with certain probiotic strains.
