Pharmaceutical delivery systems are developed to improve the physicochemical properties of therapeutic compounds. Emulsions are one of these drug delivering systems formulated using water, oils and lipids as main ingredients. Extensive data are usually generated on the physical and chemical characteristics of these oil-in-water and lipid emulsions. However, the oxidative tendency of emulsions is often overlooked. Oxidation impacts the overall quality and safety of these pharmaceutical emulsions. Additionally, introducing oxidatively unstable emulsions into biological systems further promotes oxidation in situ. Products of these reactions then continue to pose serious harm to cells and fuel other physiological oxidation reactions. Consequently, the increase of oxidation products leads to oxidative damage to biological systems. Thus, emulsions with lower lipid peroxidation are more stable and will reduce the negative effects of oxidation in situ. Preventive measures during the formulation of emulsions are important. Many naturally occurring and cost effective substances possess low oxidation tendencies and confer oxidative protection when used in emulsions. Additionally,certain preparatory methods should be employed to reduce or better control lipid peroxidation.Finally, emulsions must be evaluated for their oxidation susceptibility using the various techniques available. Careful attention to the preparation of emulsions and assessment of their oxidative stability will help produce safer emulsions without compromising efficacy.

Background There are different definitions to identify/classify children with medical complexity(CMC).We aimed to investigate and describe the definitions used to classify CMC in epidemiological studies.Methods PubMed,SciELO,LILACS,and EMBASE were searched from 2015 to 2020(last updated September 15th,2020)for original studies that presented the definition used to classify/identify CMC in the scientific research method.We applied the Preferred Reporting Items for Systematic Reviews and Meta-Analyses methodology.From the included studies,the following were identified:first author,year of publication,design,population,study period,the definition of CMC used,limitations,and strengths.Results Nine hundred and sixty-seven records were identified in the searched databases,and 42 met the inclusion criteria.Of the 42 studies included,the four most frequent definitions used in the articles included in this review were classification of CMC into nine diagnostic categories based on the International Classification of Diseases,Ninth Revision(ICD-9)(35.7%,15 articles);update of the previous classification for ICD-10 codes with the inclusion of other conditions in the definition(21.4%,nine articles);definition based on a medical complexity algorithm for classification(16.7%,seven articles);and a risk rating system(7.1%,three articles).Conclusions CMC definitions using diagnostic codes were more frequent.However,several limitations were found in its uses.Our research highlighted the need to improve health information systems to accurately characterize the CMC population and promote the provision of comprehensive care.

Sand wave migration near the southeastern corner of Martha's Vineyard, Massachusetts, USA

Sand waves of approximately 2 m in height were observed to migrate nearly 40 m with counterclockwise rotation between two bathymetric surveys performed three months apart near the southeastern corner of Martha’s Vineyard, Massachusetts. The region is characterized by strong tidal currents, intermittent energetic surface wave events, and shallow water with local depth ranging from 2 to 7 m. This study uses the process-based model, Delft3D, with a three-dimensional approach to examine the sand wave dynamics by incorporating surface waves, winds, currents, and bathymetric observations. The model successfully simulates sand wave migration in comparisons to observations. Model sensitivity analyses show that the sand wave migration reduces by 65% with the absence of the surface waves. The modeled sand wave migration speed is correlated with the tidal current Shields parameter, and sharp increases in migration speed occur when the wave-driven Shields parameter increases in response to energetic surface wave events. The combined effect of tides, surface waves, and bathymetry is the origin of the rotational aspect of the sand wave, using the Shields parameter as an indicator of tidal currents and surface wave influence on sand wave dynamics.

We examine a hierarchy of minimal conceptual models for tropical cyclone intensification.These models are framed mostly in terms of axisymmetric balance dynamics.In the first set of models,the heating rate is prescribed in such a way to mimic a deep overturning circulation with convergence in the lower troposphere and divergence in the upper troposphere,characteristic of a region of deep moist convection.In the second set,the heating rate is related explicitly to the latent heat release of ascending air parcels.The release of latent heat markedly reduces the local static stability of ascending air,raising two possibilities in the balance framework.The first possibility is that the effective static stability and the related discriminant in the Eliassen equation for the overturning circulation in saturated air,although small,remains positive so the Eliassen equation is globally elliptic.The second possibility,the more likely one during vortex intensification,is that the effective static stability in saturated air is negative and the Eliassen equation becomes locally hyperbolic.These models help to understand the differences between the early Ooyama models of 1968 and 1969,the Emanuel,1989 model,and the later Emanuel models of 1995,1997 and 2012.They provide insight also into the popular explanation of the WISHE feedback mechanism for tropical cyclone intensification.Some implications for recent work are discussed.

Knowledge of the plasticity of language pathways neurosurgeons to achieve maximum resection wh n patients with low-grade glioma is important for e preserving neurological function. The current study sought to investigate changes in the ventral language pathways in patients with low-grade glioma located in regions likely to affect the dorsal language pathways. The results revealed no significant difference in fractional anisotropy values in the arcuate fasciculus between groups or between hemispheres. However, fractional anisotropy and lateralization index values in the left inferior longitudinal fasciculus and lateralization index values in the left inferior fronto-occpital fasciculus were higher in patients than in healthy subjects. These results indicate plasticity of language pathways in patients with low-grade glioma. The ventral language pathways may perform more functions in patients than in healthy subjects. As such, it is important to protect the ventral language pathways intraoperatively.

Synoptic features in/around thermal fronts and cross-frontal heat fluxes in the southern Huanghai./Yellow Sea and East China Sea （HES） were examined using the data collected from four airborne expendable bathythermograph surveys with horizontal approxmately 35 km and vertical 1 m（from the surface to 400 m deep） spacings. Since the fronts are strongly affected by HES current system, the synoptic thermal features in/around them represent the interaction of currents with surrounding water masses. These features can not be obtained from climatological data. The identified thermal features are listed as follows ：（ 1 ） multiple boundaries of cold water, asymmetric thermocline intrusion, locally-split front by homogeneous water of approxmately 18 ℃, and mergence of the front by the Taiwan Warm Current in/around summertime southern Cheju - Changjiang/Yangtze front and Tsushima front; （2） springtime frontal eddy-like feature around Tsushima front; （3） year-round cyclonic meandering and summertime temperature-inversion at the bottom of the surface mixed layer in Cheju - Tsushima front; and （4） multistructure of Kuroshio front. In the Kuroshio front the mean variance of vertical temperature gradient is an order of degree smaller than that in other HES fronts. The southern Cheju- Changjiang front and Cheju -Tsushima front are connected with each other in the summer with comparable cross-frontal temperature gradient. However, cross-frontal heat flux and lateral eddy diffusivity are stronger in the southern Cheju - Changjiang front. The cross-frontal heat exchange is the largest in the mixing zone between the modified Huanghai Sea bottom cold water and the Tsushima Warm Current, which is attributable to enhanced thermocline intrusions.

As a non-destructive local gas breakdown phenomenon within a surface or bulk cavity surrounded by insulation,partial discharge(PD)contains important information which can be used to evaluate and diagnose electrical insulation systems.In this paper,distinctive characteristics of PD for DC versus AC are reviewed.A summary of experimental studies on DC PD in different insulation coordinations is presented,including PD in gaseous insulating media,PD in solid insulating media and PD in liquid insulating media.Conclusions and some thoughts based on existing studies are provided,and challenges and suggestions for future studies are given.This paper can serve as a guide reference for readers to learn the background and state-of-the-art of DC PD studies.More importantly,it is hopeful that this study can inspire novel ideas for further advancing fundamental research of DC PD mechanisms and PD mitigation methods.

Polysplenia syndrome, defined as the presence of multiple spleens of almost equal volume, is a rare condition involving congenital anomalies in multiple organ systems. We report this anomaly in a 41-year-old female who underwent a left lateral sectionectomy due to recurrent cholangitis and impacted left lateral duct stones. Polysplenia syndrome with preduodenal vein was diagnosed preoperatively by computed to-mography (CT) and surgery was done safely. Although the polysplenia syndrome with preduodenal portal vein (PDPV) in adult is rarely encountered, surgeons need to understand the course of the portal vein and exercise caution in approaching the biliary tract.
