Is Caries in the Early Mixed Dentition Associated with Caries Development during Orthodontic Treatment?

Objective: To investigate if caries in the pre-treatment early mixed dentition is associated with caries development in the permanent dentition during orthodontic treatment. Material and Methods: We included 41 consecutive patients (29 girls, 12 boys) with a pre-treatment documentation in the mixed (mean age 9.4 years) and a post treatment documentation in the permanent dentition (age 14.9 years) (two-phased treatment time 4.5 (±1.6) years). The DMFT/S indices were calculated. High-risk and low-risk groups were defined according to dmfs + DMFS score before treatment. Results: Initial dmft/s + DMFT/S (SD) was 5.15 (3.60) and 8.32 (6.64);final DMFT/S was 2.76 (2.84) and 3.01 (3.20). The missing (because of decay) second deciduous molars were most powerfully associated with caries increment during treatment, showing significant correlations to second premolars (r = 0.47, p = 0.003), while fillings on second deciduous molars seem to influence the prevalence of fillings on permanent molars (r = 0.44, p = 0.001). The increment at surface level was 2.01 (2.61) in the whole sample and 2.60 (3.81) in the high-risk group (4 boys, 6 girls). Compared to the low-risk group (10 girls), post-treatment caries experience was significantly higher in the high-risk group (p = 0.029). Boys were more at risk than girls (p = 0.005). Conclusions: Children with elevated caries experience in the early mixed dentition are exposed to higher caries risk during orthodontic treatment. Thus, in prevision of treatment, caries should already be assessed in the mixed dentition, so that an extended prophylaxis program can be initiated.

One-year rotational relapse frequency following conventional circumferential supracrestal fiberotomy

BACKGROUND Relapse following orthodontic treatment has been a common problem that can occur due to several factors. It was suggested that surgical circumferential supracrestal fiberotomy(CSF) is an effective measure to reduce this ***, very few studies have reported the amount of relapse that occurs *** To assess the frequency of rotational relapse on anterior teeth 1 year following *** Eleven adults(six male and five female) with a mean age of 23 years(standard deviation = 5.2), who had a total of 90 rotated anterior teeth, were included in this study. CSF was performed after comprehensive orthodontic treatment involving the use of full-fixed preadjusted edgewise appliances(Victory Series APC, 3 M,United States) with a 0.022-inch slot and Roth prescription brackets(Ovation;DENTSPLY GAC, Bohemia, New York, United States) and placement of a fixed lingual retainer from canine to canine in both arches using a 0.016 Australian wire(AJ Wilcock, Australia). Degrees of rotational correction and relapse were measured on three sets of casts [pretreatment, post-treatment(at the debond visit), and 1-year post-treatment]. Rotational relapse was categorized as follows:Unnoticeable relapse(0°), barely noticeable relapse(1°-3°), noticeable relapse(4°-9°), and clearly noticeable relapse(≥ 10°). The percent relapse that had occurred 1 year after teeth were aligned to their ideal position was calculated. Data were analyzed by dental arch type and tooth *** Mean rotational correction was 14.05° during posttreatment. Mean relapse at 1-year follow-up was 1.1°(10.8%). More than half(n = 52, 57.8%) of teeth were categorized as having unnoticeable relapse(0°). Of the remaining teeth, 31(34.5%) had barely noticeable relapse(1°-3°), 6(6.6%) had noticeable relapse(4°-9°), and only one(1.1%) had clearly noticeable relapse(> 10°). Machine Learning Models for Genetic Risk Assessment of Infants with Non-syndromic Orofacial Cleft

The isolated type of orofacial cleft, termed non-syndromic cleft lip with or without cleft palate(NSCL/P), is the second most common birth defect in China, with Asians having the highest incidence in the world. NSCL/P involves multiple genes and complex interactions between genetic and environmental factors, imposing difficulty for the genetic assessment of the unborn fetus carrying multiple NSCL/P-susceptible variants. Although genome-wide association studies(GWAS)have uncovered dozens of single nucleotide polymorphism(SNP) loci in different ethnic populations, the genetic diagnostic effectiveness of these SNPs requires further experimental validation in Chinese populations before a diagnostic panel or a predictive model covering multiple SNPs can be built. In this study, we collected blood samples from control and NSCL/P infants inHan and Uyghur Chinese populations to validate the diagnostic effectiveness of 43 candidate SNPs previously detected using GWAS. We then built predictive models with the validated SNPs using different machine learning algorithms and evaluated their prediction performance. Our results showed that logistic regression had the best performance for risk assessment according to the area under curve. Notably, defective variants in MTHFR and RBP4, two genes involved in folic acid and vitamin A biosynthesis, were found to have high contributions to NSCL/P incidence based on feature importance evaluation with logistic regression. This is consistent with the notion that folic acid and vitamin A are both essential nutritional supplements for pregnant women to reduce the risk of conceiving an NSCL/P baby. Moreover, we observed a lower predictive power in Uyghur than in Han cases, likely due to differences in genetic background between these two ethnic ***, our study highlights the urgency to generate the HapMap for Uyghur population and perform resequencing-based screening of Uyghur-specific NSCL/P markers.

Biofilm three-dimensional architecture influences in situ pH distribution pattern on the human enamel surface

To investigate how the biofilm three-dimensional(3D) architecture influences in situ pH distribution patterns on the enamel surface. Biofilms were formed on human tooth enamel in the presence of 1% sucrose or 0.5% glucose plus 0.5% fructose. At specific time points, biofilms were exposed to a neutral pH buffer to mimic the buffering of saliva and subsequently pulsed with 1% glucose to induce re-acidification. Simultaneous 3D pH mapping and architecture of intact biofilms was performed using two-photon confocal microscopy. The enamel surface and mineral content characteristics were examined successively via optical profilometry and microradiography analyses. Sucrose-mediated biofilm formation created spatial heterogeneities manifested by complex networks of bacterial clusters(microcolonies). Acidic regions(pH<5.5) were found only in the interior of microcolonies,which impedes rapid neutralization(taking more than 120 min for neutralization). Glucose exposure rapidly re-created the acidic niches, indicating formation of diffusion barriers associated with microcolonies structure. Enamel demineralization(white spots),rougher surface, deeper lesion and more mineral loss appeared to be associated with the localization of these bacterial clusters at the biofilm-enamel interface. Similar 3D architecture was observed in plaque-biofilms formed in vivo in the presence of sucrose. The formation of complex 3D architectures creates spatially heterogeneous acidic microenvironments in close proximity of enamel surface, which might correlate with the localized pattern of the onset of carious lesions(white spot like) on teeth.

One-year water-ageing of calcium phosphate composite containing nano-silver and quaternary ammonium to inhibit biofilms

Dental composites are commonly used restorative materials; however, secondary caries due to biofilm acids remains a major problem. The objectives of this study were （1） to develop a composite containing quaternary ammonium dimethacrylate （QADM）, nanoparticles of silver （NAg）, and nanoparticles of amorphous calcium phosphate （NACP）, and （2） to conduct the first investigation of the mechanical properties, biofilm response and acid production vs water-ageing time from 1 day to 12 months. A 4 x 5 design was utilized, with four composites （NACP-QADM composite, NACP-NAg composite, NACP-QADM-NAg composite, and a commercial control composite）, and five water-ageing time periods （1 day, and 3, 6, 9, and 12 months）. After each water- ageing period, the mechanical properties of the resins were measured in a three-point flexure, and antibacterial properties were tested via a dental plaque biofilm model using human saliva as an inoculum. After 12 months of water-ageing, NACP-QADM- NAg had a flexural strength and elastic modulus matching those of the commercial control （P〉 0.1）. Incorporation of QADM or NAg into the NACP composite greatly reduced biofilm viability, metabolic activity and acid production. A composite containing both QADM and NAg possessed a stronger antibacterial capability than one with QADM or NAg alone （P〈0.05）. The anti-biofilm activity was maintained after 12 months of water-ageing and showed no significant decrease with increasing time （P〉0.1）. In conclusion, the NACP-QADM-NAg composite decreased biofilm viability and lactic acid production, while matching the load- bearing capability of a commercial composite. There was no decrease in its antibacterial properties after 1 year of water-ageing. The durable antibacterial and mechanical properties indicate that NACP-QADM-NAg composites may be useful in dental restorations to combat caries.
