Comparison between Emergency Severity Index and Heart Failure Triage Scale in heart failure patients: A randomized clinical trial

BACKGROUND:It is not clear whether Emergency Severity Index(ESI)is valid to triage heart failure(HF)patients and if HF patients benefi t more from a customized triage scale or not.The aim of study is to compare the effect of Heart Failure Triage Scale(HFTS)and ESI on mistriage among patients with HF who present to the emergency department(ED).METHODS:A randomized clinical trial was conducted from April to June 2017.HF patients with dyspnea were randomly assigned to HFTS or ESI groups.Triage level,used resources and time to electrocardiogram(ECG)were compared between both groups among HF patients who were admitted to coronary care unit(CCU),cardiac unit(CU)and discharged patients from the ED.Content validity was examined using Kappa designating agreement on relevance(K*).Reliability of both scale was evaluated using inter-observer agreement(Kappa).RESULTS:Seventy-three and 74 HF patients were assigned to HFTS and ESI groups respectively.Time to ECG in HFTS group was signifi cantly shorter than that of ESI group(2.05 vs.16.82 minutes).Triage level between HFTS and ESI groups was signifi cantly different among patients admitted to CCU(1.0 vs.2.8),cardiac unit(2.26 vs.3.06)and discharged patients from the ED(3.53 vs.2.86).Used resources in HFTS group was significantly different among triage levels(H=25.89;df=3;P<0.001).CONCLUSION:HFTS is associated with less mistriage than ESI for triaging HF patients.It is recommended to make use of HFTS to triage HF patients in the ED.

Comparative efficacy of honey 12.5% and chlorhexidine 0.2% mouthwashes on the oropharyngeal bacterial colonization in mechanically-ventilated patients: a randomized controlled trial

OBJECTIVE: To compare the efficacy of honey mouthwash 12.5% and chlorhexidine solution 0.2%to reduce the rate of oropharyngeal bacterial colonization in mechanically-ventilated patients.METHODS: This study was a randomized, single blind, phase Ⅲ controlled clinical trial. Sixty patients newly admitted to internal and trauma Intensive Care Units of the two educational hospitals of Sanandaj city affiliated with Kurdistan University of Medical Sciences were selected by convenience sampling and allocated to two groups of 30 patients using random blocks design. In each group,the mouthwash was applied twice a day for four consecutive days. Swab samples were taken from the mouth and throat of all patients three times a day(pre-intervention, two days, and four days after the intervention) and then the samples were transferred onto the blood agar and eosin methylene blue(EMB) culture plates and investigated for bacterial growth and colonization after 24-48 h.RESULTS: The findings showed that oropharyngeal colonization was not significantly different between the two groups, pre-intervention, two days,and four days after the intervention(P > 0.05). Rinsing with honey mouthwash 12.5% led to the inhibition of Staphylococcus aureus and Pseudomonas aeruginosa on the fourth day of the intervention in all samples.CONCLUSION: None of the studied solutions contributed to the reduction of oropharyngeal bacterial colonization. It seems that the growth inhibition of Staphylococcus aureus and Pseudomonas aeruginosa by the honey 12.5% mouthwash in mechanically-ventilated patients need further investigation.

Esophageal cancer is one of the most unknown and deadliest cancers worldwide,mainly because of its extremely aggressive nature and poor survival rate.Esophageal cancer is the 6th leading cause of death from cancer and the 8th most common cancer in the world.The 5-year survival is around 15%-25%.There are clear differences between the risk factors of both histological types that affect their incidence and distribution worldwide.There are areas of high incidence of squamous cell carcinoma(some areas in China) that meet the requirements for cost-effectiveness of endoscopy for early diagnosis in the general population of those areas.In Europe and United States the predominant histologic subtype is adenocarcinoma.The role of early diagnosis of adenocarcinoma in Barrett's esophagus remains controversial.The differences in the therapeutic management of early esophageal carcinoma(high-grade dysplasia,T1 a,T1 b,N0) between different parts of the world may be explained by the number of cancers diagnosed at an early stage.In areas where the incidence is high(China and Japan among others) early diagnoses is more frequent and has led to the development of endoscopic techniques for definitive treatment that achieve very effective results with a minimum number of complications and preserving the functionality of the esophagus.

Epidemiology of COVID-19 in patients referred to health centers in the Nahavand city,western Iran

Background:Considering the need and importance of a comprehensive study of the disease of COVID-19,this research investigated the epidemiology of COVID-19 in Nahavand,a western city of Iran,from March 2020 to the end of the 20th.March 2021.Methods:This study is a retrospective study.Information on the epidemiology of COVID-19 was collected by the Infectious Diseases Department of the Nahavand Health Center.A logistic regression model was used to estimate the disease.All data in this study were analyzed using SPSS version 16 software.The presented statistics related to referrals to health centers with the result of a positive RT-PCR test.Results:The RT-PCR test was positive in 3544 patients and the average age of the patients was 47 years,also 49.4%of patients(n=1753)were men.Seventy eight percent of adult deaths occurred in people over 60 years old and 56.29%in men.Out of all the patients,27.12 percent had an underlying disease that 6.7%died.No deaths of children under 12 and pregnant women were reported.The association between travel history,patient contacts,underlying disease and pregnancy for COVID-19 was significant(P<0.001).Conclusion:The most important point to mention in this study is the effect of increasing age,underlying disease and male gender on increasing hospitalization and death due to COVID-19.Approximately 1.77%of Nahavand residents fell ill with COVID-19 in a defined period of time.More than 95%of patients were older than 20 years.

A Comparative Study of Fabrication of Long Wavelength Diode Lasers Using CCl2F2/O2 and H2/CH4

We report comparatively on fabrication of two-section ridge-waveguide tapered 3 quantum well (QW) InGaAsP/InP (1300 nm) and 5 QW AlGaInAs/InP (1550 nm) diode lasers. Gas mixtures of CCl2F2/O2 and H2/CH4 were used to form ridge-waveguide on the lasers with InP-based material structures. As known, chlorine- and hydro-carbon based gases are used to fabricate ridge-waveguide structures. Here, we show the difference between the structures obtained by using the both gas mixtures in which surface and sidewall structures as well as performance of the lasers were analysed using scanning electron microscopy. It is demonstrated that gas mixtures of CCl2F2/O2 highly deteriorated the etched structures although different flow rates, rf powers and base pressures were tried. We also show that the structures etched with H2/CH4 gas mixtures produced much better results that led to the successful fabrication of two-section devices with ridge-waveguide. The lasers fabricated using H2/CH4 were characterized using output power-current (P-I) and spectral results.

AIM:To investigate the prevalence of gallstone dis-ease(GSD) and to evaluate the risk of symptomatic GSD among diabetic patients.METHODS:The study was conducted by analyzing the National Health Research Institutes(NHRI) dataset of ambulatory care patients,inpatient claims,and the updated registry of beneficiaries from 2000 to 2008.A total of 615 532 diabetic patients without a prior history of hospital treatment or ambulatory care visits for symptomatic GSD were identified in the year 2000.Age-and gender-matched control individuals free from both GSD and diabetes from 1997 to 1999 were randomly selected from the NHIR database(n = 614 871).The incidence densities of symptomatic GSD were estimated according to the subjects' diabetic status.The distributions of age,gender,occupation,income,and residential area urbanization were compared between diabetic patients and control subjects using Cox proportion hazards models.Differences between the rates of selected comorbidities were also assessed in the two groups.RESULTS:Overall,60 734 diabetic patients and 48 116 control patients developed symptomatic GSD and underwent operations,resulting in cumulative operation rates of 9.87% and 7.83%,respectively.The age and gender distributions of both groups were similar,with a mean age of 60 years and a predominance of females.The diabetic group had a significantly higher prevalence of all comorbidities of interest.A higher incidence of symptomatic GSD was observed in females than in males in both groups.In the control group,females under the age of 64 had a significantly higher incidence of GSD than the corresponding males,but this difference was reduced with increasing age.The cumulative incidences of operations for symptomatic GSD in the diabetic and control groups were 13.06 and 9.52 cases per 1000 person-years,respectively.Diabetic men exhibited a higher incidence of operations for symptomatic GSD than did their counterparts in the control group(12.35 vs 8.75 cases per 1000 person-years).CONCLUSION:The association of diabet

Efficacy of topical chamomile on the incidence of phlebitis due to an amiodarone infusion in coronary care patients:a double-blind,randomized controlled trial

BACKGROUND：Amiodarone is a useful antiarrhythmic drug.Phlebitis,caused by intravenous amiodarone,is common in patients in coronary care units（CCUs）.OBJECTIVE：The aim of this study was to evaluate the effect of topical chamomile on the incidence of phlebitis due to the administration of an amiodarone infusion into the peripheral vein.DESIGN,SETTING,PARTICIPANTS AND INTERVENTIONS：This was a randomized,double-blind clinical trial,conducted on 40 patients（n=20 per group）in two groups—an intervention group（chamomile ointment）and a control group（lanoline,as a placebo）,hospitalized in the CCUs and undergoing an amiodarone infusion into the peripheral vein over 24 h.Following the cannulation and commencement of the infusion,placebo or chamomile ointment was rubbed in,up to 10 cm superior to the catheter and repeated every eight hours for three days.The cannula site was then assessed based on the phlebitis checklist.MAIN OUTCOME MEASURES：The incidence and time of occurrence of phlebitis,relative risk,severity of phlebitis were the main outcome measures.RESULTS：Nineteen patients（19/20）in the control group had phlebitis on the first day of the study and one patient（20/20）on the second day.In the intervention group,phlebitis occurred in 13 cases（13/20）on the first day and another two（2/7）was found on the second day.The incidence of phlebitis was significantly different between two groups（P=0.023）.The cumulative incidence of phlebitis in the intervention group（15/20）is significantly later and lower than that in the control group（20/20）during two days（P=0.008）.Two patients in the intervention group did not develop phlebitis at all during the 3-day study.Also,the relative risk of phlebitis in the two groups was 0.68（P=0.008 5）.A significant difference was not observed with regard to phlebitis severity in both groups.CONCLUSION：It seems that phlebitis occurred to a lesser extent and at a later time frame in the intervention group compared to control group.Topical chamomile may be effective in decreasing the incidence of phlebiti
