Functional and structural changes in internal pudendal arteries underlie erectile dysfunction induced by androgen deprivation

Androgen deficiency is strongly associated with erectile dysfunction （ED）. Inadequate penile arterial blood flow is one of the major causes of ED. The blood flow to the corpus cavernosum is mainly derived from the internal pudendal arteries （IPAs）; however, no study has evaluated the effects of androgen deprivation on IPA＇s function. We hypothesized that castration impairs IPAs reactivity and structure, contributing to ED. In our study, Wistar male rats, 8-week-old, were castrated and studied 30 days after orchiectomy. Functional and structural properties of rat IPAs were determined using wire and pressure myograph systems, respectively. Protein expression was determined by Western blot and immunohistochemistry. Plasma testosterone levels were determined using the IMMULITE 1000 Immunoassay System. Castrated rats exhibited impaired erectile function, represented by decreased intracavernosal pressure/mean arterial pressure ratio. IPAs from castrated rats exhibited decreased phenylephrine- and electrical field stimulation （EFS）-induced contraction and decreased acetylcholine- and EFS-induced vasodilatation. IPAs from castrated rats exhibited decreased internal diameter, external diameter, thickness of the arterial wall, and cross-sectional area. Castration decreased nNOS and α-actin expression and increased collagen expression, p38 （Thr180/ryr182） phosphorylation, as well as caspase 3 cleavage. In conclusion, androgen deficiency is associated with impairment of IPA reactivity and structure and increased apoptosis signaling markers. Our findings suggest that androgen deficiency-induced vascular dysfunction is an event involving hypotrophic vascular remodeling of IPAs.

Background:In Brazil,people still fall ill and die from tuberculosis(TB),and this can be explained by the significant impasse in the equity of distribution of therapeutic resources to the population as a whole.The aim was to identify geographical areas which have shown progress in terms of equity(of income,schooling and urban occupancy)and test its effect on mortality from TB in a municipality of southeast Brazil.Methods:It is an ecological study considering TB as the basic cause for deaths registered between 2006 and 2013 on the Mortality Information System and other variables obtained through the Demographic Census of the Brazilian Institute of Geography and Statistics(2010).The geographical area for analysis comprised the areas of coverage of the health services.Social indicators have been constructed through the Principal Component Analysis(PCA).The cases were geocoded and the annual mortality rate from TB was calculated with smoothing using the local empirical Bayesian method.Multiple linear regression was then performed.There was confirmation of the existence of spatial dependence of residue through the application of the Global Moran I test,and application of the Models with Global Spatial Effects,to identify the best standard of spatial regression.Results:The mortality rates ranged from 0.00 to 2.8 deaths per 100,000 people,per year.In the PCA,three indicators were constructed,and designated as indicators of income,social inequality,and social equity.In multiple linear regression,the indicator of social equity was statistically significant(P<0.0001)but had a negative association,an adjusted R^(2)of 28.36%and with spatial dependence(Moran I=0.21,P=0.003455).The best model to deal with existing spatial dependence was the Spatial Lag Model.Conclusions:The better social conditions have shown progress in reducing mortality from TB,thereby reinforcing the achievement of Sustainable Development Goals.In addition,cartography was also applied,which can be replicated in other scenarios throughout the world,using a

OBJECTIVE:To evaluate the sternocleidomastoid and the descending portion of the trapezius muscles in terms of electromyographic activity and pressure pain threshold in women with chronic cervical pain after acupuncture treatment.METHODS:Twenty-five women,aged 18-50 years[(31±3)years],with chronic neck and temporomandibular disorder participated in the study.The electromyographic activity of the sternocleidomastoid muscle and the descending portion of the trapezius muscle were recorded at rest,during shoulder elevation,(left and right)rotation of the neck and crucifix position.Maximum voluntary contraction was used to normalize the electromyographic activity data.The points used for acupuncture were Jiache(ST 6),Xiaguan(ST 7),Quanliao(SI 18),Tinggong(SI 19),Yifeng(TE-17),Fengchi(GB 20),Renzhong(GV 26),Yangbai(GB-14),and points of the lower and upper limbs:Tai chong(LV 3),Zusanli(ST 36),Sanyinjiao(SP 6),Kunlun(BL 60),He Gu(LI4)and Yanglingquan(GB 34).The treatment course was 10 sessions,30 min long per session,and twice a week.The final electromyographic activity and pressure pain threshold data collection were performed 15 d after treatment completion.RESULTS:The normalized electromyographic activity and pressure pain threshold data were tabulated and submitted to statistical analysis using t-test(P<0.05).Significant difference was observed during shoulder elevation for the left trapezius muscle,during neck rotation from the right side to the left,and pressure pain threshold for the trapezius and sternocleidomastoid muscles,bilaterally,after the acupuncture treatment.CONCLUSION:The acupuncture treatment improved the neck muscles in terms of electromyographic activity and increased the tolerance to chronic pain in women.

Glioblastoma(GBM) is one of the deadliest tumors and has a median survival of 3 months if left untreated. Despite advances in rationally targeted pharmacological approaches, the clinical care of GBM remains palliative in intent. Since the majority of altered signaling cascades involved in cancer establishment and progression eventually affect cell cycle progression, an alternative approach for cancer therapy is to develop innovative compounds that block the activity of crucial molecules needed by tumor cells to complete cell division. In this context, we review promising ongoing and future strategies for GBM therapeutics aimed towards G2/M inhibition such as anti-microtubule agents and targeted therapy against G2/M regulators like cyclin-dependent kinases,Aurora inhibitors, PLK1, BUB, 1, and BUBR1, and survivin. Moreover, we also include investigational agents in the preclinical and early clinical settings. Although several drugs were shown to be gliotoxic, most of them have not yet entered therapeutic trials. The use of either single exposure or a combination with novel compounds may lead to treatment alternatives for GBM patients in the near future.

Drinking Water as Iron Carrier for the Prevention of Iron Deficiency Anemia:The Brazilian Experience

On a global base, estimated 2 billion people are iron deficient and/or anemic with small children and child bearing age most likely to be the ones affected. These high levels of world anemia prevalence have been maintained and even growing in spite of the global and national food fortification programmes carried out in several places. These have been based on the iron fortification of local foods. In Brazil a wheat flour iron fortification program has been going on for several years and the anemia prevalence has been going on as a still large public health problem. We started since the nineties and in several under five day care centers, in various part of Brazil a new program using drinking water as the iron carrier for the prevention of iron deficiency anemia. It was shown through several studies in different places in Brazil that drinking water being available everywhere, daily consumed by everyone, children, adults and old people have shown to be effective for the prevention of iron anemia. Iron salts are low-priced, water soluble, effective, easily and locally prepared, should be known and accepted as a rational, practical and effective locally community preventive solution for iron anemia, still a great problem of our developing and underdeveloped countries of the world.

Ochoa Syndrome—A Twins Case Report

Urofacial syndrome, known as Ochoa syndrome, is a very rare autosomal recessive disorder described as a clinical setting in which there is an association of a lower urinary tract and bowel dysfunction with a typical facial expression: the patient seems to be grimacing or crying when attempting to smile. If the diagnosis and treatment are delayed, the patients might have a poor outcome with continuous upper urinary tract deterioration, which makes the early diagnosis of this condition of vital importance. We report a case of two females identical twins, who are, nowadays, 19 years old, which had the Ochoa Syndrome diagnosis made when they were 10 years old, but discontinued medical follow up during treatment. They have developed chronic renal failure with hemodialysis need and are in a waiting list for kidney transplantation. In this rare disorder, even more unusual in twins and with high risks on progressing with renal failure, a simple attempt to smile may save lives if the condition is known by doctors such as pediatricians and pediatric urologists. Conclusion: We believe that unknowing this condition is the main explanation for this fact. Early diagnosis and close follow up are the key points to a successful outcome.

Low-level laser therapy enhances the number of osteocytes in calvaria bone defects of ovariectomized rats

Background : Osteoporosis can make bone repair difficult. Low-level laser therapy( LLLT) has been shown to be a promising tool for bone neoformation. This study aimed to analyze the effect of LLLT on calvaria bone defects of ovariectomized rats using stereology. Methods : Fifty-four Wistar rats were subjected to bilateral ovariectomy, and bone defects were created in calvaria after 150 days. The animals were divided into nine groups(n = ?6 per group), and 24 hours after the bone defects were created they received three, six or 12 sessions of LLLT at 0, 20 or 30 J/cm 2, using a 780-nm low-intensity GaAlAs laser. One-way ANOVA followed by Tukey ' s post hoc test was used for data processing. A difference of P < 0.05 was considered statistically significant. The parameters evaluated were osteocyte density( Nv_(ost)), total osteocyte number( Nto ost), trabecular surface density( Sv_t), and trabecular surface area( Sa_t). Results : Data obtained showed that Nto ost, Sv t, and Sa t in group G2 rats were significantly different from G1(0 J/cm^2)( P < 0.05). Compared to group G4, G5 presented higher values for the parameters Sv t and Sa t, and G6 presented significantly higher values for almost all the analyzed parameters( Nv _(ost), Nto_(ost), Sv_t, and Sa t)( P < 0.05). Compared to group G7, G8 showed a higher value only for the parameter Sa t, and G9 showed significantly higher values for parameters Nv ost, Nto ost, Sv_t, and Sa_t. Conclusion : We conclude that LLLT stimulated bone neoformation and contributed to an increase in the total number of osteocytes, especially with a laser energy density of 30 J/cm^2 given for six and 12 sessions.

Because the aetiology of chronic pelvic pain is complex, studies of the condition involve extensive investigation but provide few conclusions. Numerous studies have addressed the experiences of women with chronic pelvic pain, as well as the interaction between those women and their health care providers. Our objective was to investigate how physicians at a specialised clinic perceive the medical care provided to such women. This was a qualitative study employing semi-structured interviews and content analysis. We interviewed seven physicians at the Chronic Pelvic Pain Outpatient Clinic of the University Hospital, Faculty of medicine of Ribeir?o preto, University of S?o Paulo, Brazil. Medical training and practice constituted the central theme of the study, which was subdivided into categories addressing the influence that the current medical training has on the type of medical care provided to women with chronic pelvic pain. Medical practice has been characterised by a reductionist approach to health and illness, as well as by the fragmentation of health care. These characteristics are, to a certain extent, the result of the biomedical model of education, which has been predominant, ignoring social, cultural, psychological and emotional aspects. There is a need to shift the medical paradigms toward a humanistic model of health care. We hope that we have provided a critical view of current medical training and practice, as well as of their effects in various health care settings, particularly in the provision of care to women with chronic pelvic pain.

Adjuvant therapy for gastric cancer:What have we learned since INT0116?

Gastric cancer is one of the main cancer-related causes of death worldwide. The curative treatment of gastric cancer consists of tumor resection and lymphadenectomy. However, surgical treatment alone is associated with high recurrence rates. Adjuvant treatment strategies have been studied over the last decades, but there have been controversial results from the initial studies. The pivotal INT0116 study demonstrated that the use of adjuvant chemoradiotherapy with 5-fluorouracil increases relapse-free and overall survival, and it has been adopted across the Western world. The high toxicity of radiochemotherapy and suboptimal surgical treatment employed, with fewer than 10% of the patients submitted to D2 lymphadenectomy, were the main study limitations. Since its publication, other adjuvant treatment modalities have been studied, and radiochemotherapy is being refined to improve its efficacy and safety. A multimodal approach has been demonstrated to significantly increase relapsefree and overall survival, and it can be offered in the form of perioperative chemotherapy, adjuvant chemoradiotherapy or adjuvant chemotherapy, regardless of the extent of lymphadenectomy. The objective of the present review is to report the major advances obtained in the last decades in the adjuvant treatment of gastric cancer as well as the perspectives of treatment based on recent knowledge of the molecular biology of the disease.
