Association between Nonalcoholic Fatty Liver Disease and Carotid Artery Disease in a Community-Based chinese Population： A Cross-Sectional Study

Background： Nonalcoholic fatty liver disease （NAFLD） is one of the most common chronic liver diseases with a high prevalence in the general population. The association between NAFLD and cardiovascular disease has been well addressed in previous studies. However, whether NAFLD is associated with carotid artery disease in a community-based chinese population remained unclear. The aim of this study was to investigate the association between NAFLD and carotid artery disease. Methods： A total of 2612 participants （1091 men and 1521 women） aged 40 years and older from Jidong of Tangshan city （China） were selected for this study. NAFLD was diagnosed by abdominal ultrasonography. The presence of carotid stenosis or plaque was evaluated by carotid artery ultrasonography. Logistic regression was used to analyze the association between NAFLD and carotid artery disease. Results： Participants with NAFLD have a higher prevalence of carotid stenosis （12.9% vs. 4.6%） and carotid plaque （21.9% vs. 15.0%） than those without NAFLD. After adjusting for age, gender, smoking status, income, physical activity, diabetes, hypertension, triglyceride, waist-hip ratio, and high-density lipoprotein, NAFLD is significantly associated with carotid stenosis （odds ratio [OR]： 2.06, 95% confidence interval [CI]： 1.45-2.91）, but the association between NAFLD and carotid plaque is not statistically significant （OR： 1. 10, 95% CI： 0.86-1.40）. Conclusion： A significant association between NAFLD and carotid stenosis is found in a chinese population.

A Scenario-Based Evaluation of COVID-19-Related Essential Clinical Resource Demands in China

The coronavirus disease 2019(COVID-19)pandemic is a global crisis,and medical systems in many countries are overwhelmed with supply shortages and increasing demands to treat patients due to the surge in cases and severe illnesses.This study aimed to assess COVID-19-related essential clinical resource demands in China,based on different scenarios involving COVID-19 spreads and interventions.We used a susceptible–exposed–infectious–hospitalized/isolated–removed(SEIHR)transmission dynamics model to estimate the number of COVID-19 infections and hospitalizations with corresponding essential healthcare resources needed.We found that,under strict non-pharmaceutical interventions(NPIs)or mass vaccination of the population,China would be able to contain community transmission and local outbreaks rapidly.However,under scenarios involving a low intensity of implemented NPIs and a small proportion of the population vaccinated,the use of a peacetime–wartime transition model would be needed for medical source stockpiles and preparations to ensure a normal functioning healthcare system.The implementation of COVID-19 vaccines and NPIs in different periods can influence the transmission of COVID-19 and subsequently affect the demand for clinical diagnosis and treatment.An increased proportion of asymptomatic infections in simulations will not reduce the demand for medical resources;however,attention must be paid to the increasing difficulty in containing COVID-19 transmission due to asymptomatic cases.This study provides evidence for emergency preparations and the adjustment of prevention and control strategies during the COVID-19 pandemic.It also provides guidance for essential healthcare investment and resource allocation.

To the Editor:Esophageal cancer,one of the most common cancer types in China,with an estimated 346,633 new cases and 323,600 deaths in 2022,is becoming an increasingly serious clinical and public health problem.^([1])The successful promotion of the self-management strategy has indicated that lifestyle modifications can be valuable in the primary prevention of cancer development.Adopting a healthy lifestyle has become a novel strategy for primary prevention and risk reduction in high-risk areas.Previous epidemiological studies have identified several lifestyle-related risk factors for esophageal cancer,including smoking and diet.^([2])Each factor can typically explain a modest proportion of cancer risk.However,when combined,these known risk factors may substantially affect the risk of esophageal cancer.Nevertheless,some risk factors for esophageal cancer are non-modifiable,including age,low socioeconomic status,and family history.Whether and how these non-modifiable risk factors affect primary cancer prevention by intervening with modifiable risk factors remain unclear.

Research misconduct in hospitals is spreading:A bibliometric analysis of retracted papers from chinese university-affiliated hospitals

Purpose: The number of retracted papers from chinese university-affiliated hospitals is increasing, which has raised much concern. The aim of this study is to analyze the retracted papers from university-affiliated hospitals in China’s mainland from 2000 to 2021. Design/methodology/approach: Data for 1,031 retracted papers were identified from the Web of science Core collection database. The information of the hospitals involved was obtained from their official websites. We analyzed the chronological changes, journal distribution, discipline distribution and retraction reasons for the retracted papers. The grade and geographic locations of the hospitals involved were explored as well.Findings: We found a rapid increase in the number of retracted papers, while the retraction time interval is decreasing. The main reasons for retraction are plagiarism/self-plagiarism(n=255), invalid data/images/conclusions(n=212), fake peer review(n=175) and honesty error(n=163). The disciplines are mainly distributed in oncology(n=320), pharmacology & pharmacy(n=198) and research & experimental medicine(n=166). About 43.8% of the retracted papers were from hospitals affiliated with prestigious universities. Research limitations: This study fails to differentiate between retractions due to honest error and retractions due to research misconduct. We believe that there is a fundamental difference between honest error retractions and misconduct retractions. Another limitation is that authors of the retracted papers have not been analyzed in this study.Practical implications: This study provides a reference for addressing research misconduct in chinese university-affiliated hospitals. It is our recommendation that universities and hospitals should educate all their staff about the basic norms of research integrity, punish authors of scientific misconduct retracted papers, and reform the unreasonable evaluation system.Originality/value: Based on the analysis of retracted papers, this study further analyzes the characteristics of institut

目的旨在分析护士志愿参与照护失能老人活动的意愿及影响因素,并构建结构方程模型,明确行为态度、主观规范、知觉行为控制对行为意向的影响程度,为后期建立为失能老人提供服务的志愿者团队奠定基础。方法采用横断面调查法,选取2020年8-11月苏州市30所不同等级医院的护士进行问卷调查。采用自行编制的护士志愿照护失能老年人行为意向问卷进行调查。问卷包括行为态度(7个条目)、主观规范(8个条目)、知觉行为控制(8个条目)、行为意向(3个条目)4个维度,共26个条目。采用logistic回归分析,并通过Smart PLS 3.0软件进行结构方程模型构建。结果共1998名护士参与了调查,其中1191名护士(59.6%)愿意参与志愿照护失能老人活动。问卷各维度得分:行为态度(26.31±5.94)分、主观规范(30.93±6.62)分、知觉行为控制(27.58±6.70)分及行为意向(10.78±2.50)分。Logistic回归分析结果显示,城市户口、在科室中担任职务、得到过其他志愿者无偿帮助及因为志愿活动受到医院或组织奖励的护士参与意愿更强(P<0.05)。偏最小二乘法结果分析显示,行为态度对行为意向存在正向影响(β=0.456,P<0.001);主观规范对行为意向存在正向影响(β=0.167,P<0.01);知觉行为控制对行为意向存在正向影响(β=0.123,P<0.01)。护士的态度越积极,感知到来自他人的支持越多,障碍越少,参与志愿活动的意愿越强。结论未来在我国调动护士参与志愿活动具有可行性。政策制定者及领导者在此过程中需完善相关法律法规来保障志愿护理活动的安全性,减少志愿活动存在的外部阻碍因素,完善激励措施,以此来提高护士的参与意愿并转化为实际行动。

Body Mass Index and Hypertension Hemodynamic Subtypes in Yi Farmers and Migrants

Objective To examine the relationship between overweight or obesity and the risk of the various hypertension hemodynamic subtypes in Yi farmers and migrants. Methods A cross-sectional study of 2 358 Yi farmers and 2 392 Yi migrants was carried out in the Liangshan Yi autonomous prefecture, Sichuan, China in 2007. Results The standardized prevalence of overweight in female Yi farmers （6.22%） was higher than in males （3.25%）, whereas in Yi migrants 32.56% of males and 28.78% of females were overweight. The standardized prevalence of obesity was 0 and 0.62% in male and female Yi farmers, compared to 3.91% and 5.57% in male and female Yi migrants, respectively. For both genders the standardized prevalence of ISH, IDH, and SDH was higher in Yi migrants than Yi farmers. Overweight and obese Yi men and women had a higher risk for IDH and SDH （P〈O.002） than non-overweight/obese individuals. However, an association of overweight or obesity with ISH was observed only in men. Conclusion Yi migrants have substantially higher proportion of overweight and obese individuals, as well as individuals affected by ISH, IDH, and SDH, than do Yi farmers. Overweight and obesity are significant risk factors for the development of hypertension in Yi people.

The development of chinese medicine and Western medicine andrology is based on different social background and academic systems,either chinese medicine or Western medicine andrology has their limitations,therefore,integration of chinese and Western medicine(ICWM)andrology is in a great need.After more than 30 years of development,andrology has made great achievements in the construction of specialized academic association,holding academic conferences and publication of academic monographs,and the research progress on this field is mainly in the combination of disease and syndrome,microdifferentiation of symptoms and signs and basic research development.However,the comprehensive theoretic system of ICWM andrology has not yet established,and the related studies are still on the primary stage.In the future studies,great efforts still need to be made to expand the methods for the investigation of ICWM,and make innovations in the field of andrology.

Adverse childhood experiences from family and society contribute to increased risk of depressive symptoms and cognitive impairment:a cross-sectional study

Background Family environments can shape children’s personalities and social networks,rendering distinguishing adverse childhood experiences(ACEs)from family and society essential,but related evidence remains limited.Aims This cross-sectional study aimed to investigate the correlations between intrafamilial and social ACEs,their associations with depressive symptoms and cognitive impairment and the(education-moderated)mediating role of social ACEs.Methods Data for this cross-sectional study were from the China Health and Retirement Longitudinal Study.Nine intrafamilial(0,1,2,3,and 4 or more)and three social(0,1,and 2 or more)ACEs were identified.Depressive symptoms were assessed using the 10-item Center for Epidemiological Studies Depression Scale.Global cognition,including episodic memory and mental intactness,was calculated as z scores.Binary and ordered logistic regressions,generalised linear models with Gaussian family and identity link,and mediation analysis were used.Results 13435 participants aged 59.0(51.0–66.0)were included.Compared with participants with no intrafamilial ACEs,those with 1,2,3,and 4 or more intrafamilial ACEs tended to develop more social ACEs,with odds ratios(ORs)of 1.55(95%confidence interval(CI):1.36 to 1.76),2.36(95%CI:2.08 to 2.68),3.46(95%CI:3.02 to 3.96)and 6.10(95%CI:5.30 to 7.02),respectively.Both intrafamilial and social ACEs were associated with depressive symptoms(OR>3 for four or more intrafamilial ACEs and two or more social ACEs)and global cognition(β=−0.26 for four or more intrafamilial ACEs andβ=−0.29 for two or more social ACEs).social ACEs mediated the associations of intrafamilial ACEs with depressive symptoms and global cognition by 12.3%and 13.1%,respectively.Furthermore,as education levels increased,the impact of intrafamilial ACEs on depressive symptoms was increasingly mediated through social ACEs,while the mediating role of social ACEs between intrafamilial ACEs and cognitive impairment gradually diminished.Conclusions Improving children’s social environments an

Effect modifications of BMI transition and trajectory in the associations of adverse childhood experiences with new-onset dementia and its subtypes in older US adults

Background Adverse childhood experiences(ACEs)and dementia are associated and comorbid with obesity.However,according to emerging research,the role of obesity in the association between ACEs and dementia seems controversial.Aims This analysis aimed to explore the associations between ACEs and different dementia subtypes and the effect modification of long-term body mass index(BMI).Methods Data were obtained from the US Health and Retirement Study.Six ACEs were categorised as 0,1 and 2 or more.All-cause dementia,Alzheimer’s disease(AD)and other dementias were defined by self-reported or proxy-reported physician diagnosis.Cox proportional hazards regression was used to explore the associations of ACEs with new-onset all-cause dementia,AD and other dementias from 2010 to 2020.Effect modification of BMI in 2010 and BMI transition and trajectory(fitted by group-based trajectory modelling)from 2004 to 2010 were assessed.Results 15282 participants with a mean age of 67.0 years(58.0–75.0)were included in the 2010 data analysis.Significant interactions of ACEs with baseline BMI,BMI transition and BMI trajectory in their associations with new-onset all-cause dementia and AD were observed(all p<0.05).For instance,positive associations of two or more ACEs(vs none)with all-cause dementia and AD were found in those with a BMI trajectory of maintaining≥30 kg/m2(maintain obesity)rather than a decline to or maintaining<25 kg/m2(decline to or maintain normal weight),with hazard ratios(HRs)of 1.87(95%confidence interval(CI):1.45 to 2.42)and 1.85(95%CI:1.22 to 2.80),respectively.Conclusions ACEs were associated with dementia and AD in US adults with long-term abnormally elevated BMI but not with long-term normal or decreasing BMI.Integrated weight management throughout life could prevent dementia among those with childhood adversity.

Out-of-pocket payments and economic consequences from tuberculosis care in eastern China:income inequality

Background:Despite the availability of free tuberculosis(TB)diagnosis and treatment,TB care still generates substantial costs that push people into poverty.We investigated out-of-pocket(OOP)payments for TB care and assessed the resulting economic burden and economic consequences for those with varying levels of household income in eastern China.Methods:A cross-sectional study was conducted among TB patients in the national TB programme networks in eastern China.TB-related direct OOP costs,time loss,and coping strategies were investigated across households in different economic strata.Analysis of Variance was used to examine the differences in various costs,and Kruskal-Wallis tests were used to compare the difference in total costs as a percentage of annual household income.Results:Among 435 patients,the mean OOP total costs of TB care were USD 2389.5.In the lower-income quartile,OOP payments were lower,but costs as a percentage of reported annual household income were higher.medical costs and costs prior to treatment accounted for 66.4 and 48.9%of the total costs,respectively.The lower the household income was,the higher the proportion of medical costs to total costs before TB treatment,but the lower the proportion of medical costs patients spent in the intensive phase.TB care caused 25.8%of TB-affected households to fall below the poverty line and caused the poverty gap(PG)to increase by United States Dollar(USD)145.6.Patients in the poorest households had the highest poverty headcount ratio(70.2%)and PG(USD 236.1),but those in moderately poor households had the largest increase in the poverty headcount ratio(36.2%)and PG(USD 177.8)due to TB care.Patients from poor households were more likely to borrow money to cope with the costs of TB care;however,there were fewer social consequences,except for food insecurity,in poor households.Conclusions:medical and pretreatment costs lead to high costs of TB care,especially among patients from the poorest households.It is necessary to train health system staff in general h
