卫生经济学评价是对备选行动方案的成本和结果的比较分析。2013年发表的《卫生经济学评价报告标准共识》(CHEERS)提出了卫生经济学评价的特点,确保结果正确阐释,以支持决策制订。2013版CHEERS旨在指导研究报告撰写者准确报告卫生干预和对照措施、背景、过程、结果等评价细节,使审稿人理解文章内容,帮助读者使用研究结果。本版共识将取代2013版共识,其更加适用于各类卫生经济学评价,结合学科发展和方法更新的需要,更注重患者、公众等相关利益方的参与。本共识适用多种个体或人群健康干预措施(简单或复杂)在不同政策场景的应用分析,包括医疗服务、公共卫生、教育、社会服务等。本文概要介绍了CHEERS 2022共识中包含的28个检查项及每一个检查项的相关建议。CHEERS 2022共识主要用于指导研究人员撰写拟投递同行评议学术期刊的卫生经济学评价文章,亦可用于期刊编辑和审稿专家对待发表文章的评价。熟悉了解本共识要求,还有助于研究人员规划评价研究。随着决策透明度要求提高,本共识可支持卫生技术评估机构建立评价报告标准。

Background:This study aimed to clarify the effects of a Graded Brisk Walking Test(GBWT)program on blood pressure(BP)control in elderly patients with essential hypertension and to determine its appropriate degree of intensity for elderly ***:In total,421 out of 620 participants were randomly sampled from a community health center in Shanghai,*** them,200 and 221 patients were assigned to the intervened and control groups,*** were all treated with the GBWT program for 8 weeks,16 weeks,and 24 ***:Based on ANOVA,there was no significant time main effect,condition main effect,or time by condition interaction both in heart rate and body mass index indicators(P>0.05).There were significant condition main effects,time by condition interaction,or time main effects in blood pressure(F=21.875,33.457,65.342,respectively;All Pexercise),no significant differences existed both in SBP and DBP groups compared with those before the intervention(P=0.07).Conclusion:GBWT is an effective exercise prescription to mitigate the essential hypertension in elderly Chinese patients,and the intensity,walking distance,and target number of steps can be adjusted according to the age of the patients.

The results of the present study indicate that glandless cottonseed meal (CSM) can be incorporated in extruded corn flour snacks at a 10% content level, which increases snack protein content to 12.8% and reduce fat content to 6.2%. To improve snacks’ nutritional quality, CSM and corn flour were extruded using a simple screw extruder. An expansion index (EI) ranging of 1.2 - 4.7 was obtained. Penetration force (PF) was 7 - 9 times harder than other extruded products. High extrusion temperature and high CSM concentrations decreased (p p p< 0.05) extrudates’ water solubility index. Extrusion conditions used showed a 68.5% starch gelatinization, and a starch availability of more than 97%, which explains the high expansion index obtained.

Coronavirus Disease 2019(COVID-19)has caused a global outbreak and become a major public health *** January 20,2020,the announcement of National Health Commission(No.1 of 2020)included COVID-19 as a category B infectious disease in Law of the People's Republic of China on prevention and control of Infectious Diseases,and took preventing and controlling measures like those of category A infectious diseases^([1]).Tumor patients are more susceptible to infection because of their vulnerable nutritional state and systemic immunosuppressive state caused by antitumor treatments,such as radiotherapy,chemotherapy and surgery^([2]).Cancer patients had an estimated 2-fold increased risk of COVID-19 than the general population according to the Report of the WHO-China Joint Mission on COVID-19^([3]);The incidence rate of COVID-19 in cancer patients in Wuhan is 0.79%,while the general population is 0.37%in the same period^([4]).And the COVID-19 cancer patients were with higher risk to become severe condition and worse prognosis,especially in those patients who received chemotherapy or surgery recently,than those without cancer^([5-6]).In addition,the inconvenience for patients going out to see doctors also leads to the delay of diagnosis and *** a result,under the current severe situation,scientific and effective management measures and reasonable responsive strategies are urgently needed to minimize the impact of the epidemic on tumor patients^([7-9]).Especially recently,the outbreak continues to spread in many countries around the *** Health Organization(WHO)believes the COVID-19 epidemic has the characteristics of a pandemic,calling on all countries to take urgent and active *** such circumstances,experts organized by Classical Traditional Chinese Medicine Prescription for Cancer Treatment and Research Committee of World Federation of Chinese Medicine Societies,discussed and formed"Expert Consensus on Integrated Traditional and Western Medicine P

Background:Anomalous aortic origin of a coronary artery(AAOCA)is a congenital heart disease with a 0.3%−0.5%*** is challenging due to nonspecific clinical *** stratification and treatment are currently based on expert consensus and single-center case ***:Demographical and clinical data of AAOCA patients from 17 tertiary-care centers were *** imaging studies(Bidimensional echocardiography,coronary computed tomography angiography[CCTA]were *** correlations with anomalous coronary course and origin were ***:Data from 239 patients(42%males,mean age 15 y)affected by AAOCA were collected;154 had AAOCA involving the right coronary artery(AAORCA),62 the left(AAOLCA),23 other anomalies.211(88%)presented with an inter-arterial *** electrocardiogram(ECG)was abnormal in 37(16%).AAOCA was detected by transthoracic echocardiography and CCTA in 53%and 92%of patients,*** of the patients reported cardiac symptoms(119/239;50%),mostly during exercise in 121/178(68%).An ischemic response was demonstrated in 37/106(35%)and 16/31(52%)of patients undergoing ECG stress test and stress-rest single positron emission cardiac *** with AAORCA,patients with AAOLCA presented more frequently with syncope(18%vs.5%,P=0.002),in particular when associated with inter-arterial course(22%vs.5%,P<0.001).Conclusion:Diagnosis of AAOCA is a clinical challenge due to nonspecific clinical presentations and low sensitivity of first-line cardiac screening *** seems to be strictly correlated to AAOLCA with inter-arterial course.

Endothelium,acting as a barrier,protects tissues against factors that provoke insulin resistance and type 2 diabetes and itself responds to the insult of insulin resistance inducers with altered *** insulin resistance and vascular dysfunction occur early in the evolution of insulin resistance-related disease,can co-exist with and even contribute to the development of metabolic insulin resistance,and promote vascular complications in those *** impact of endothelial insulin resistance and vascular dysfunction varies depending on the blood vessel size and location,resulting in decreased arterial plasticity,increased atherosclerosis and vascular resistance,and decreased tissue *** with insulin resistance and diabetes are disproportionately impacted by cardiovascular disease,likely related to differential sex-hormone endothelium ***,reducing endothelial insulin resistance and improving endothelial function in the conduit arteries may reduce atherosclerotic complications,in the resistance arteries lead to better blood pressure control,and in the microvasculature lead to less microvascular complications and more effective tissue *** diabetes therapeutic modalities,including medications and exercise training,improve endothelial insulin action and vascular *** action may delay the onset of type 2 diabetes and/or its complications,making the vascular endothelium an attractive therapeutic target for type 2 diabetes and potentially type 1 diabetes.

Objective: To determine the effects of an educational program of exercise and nutrition in overweight or obese binge eating adults on body composition, physical fitness and biochemical parameters. Being educational, the program also aimed to promote learning and self-control in the practice of physical activity and in the food choices of this population. Methodology: 40 patients overweight or obese binge eating adults aged between 30 and 60 years participated in the study. Participants were assigned to an experimental group (EG)(N = 23), to which the program was applied or a control group (CG)(N = 17). The study lasted eight months, where an initial and final evaluation was carried out, in order to verify the benefits obtained through this program. The program consisted of 47 one-hour, twice-a-week group practice sessions, one weekly self-help session, and three nutritional monitoring sessions throughout the program. Results: There were significant improvements in the experimental group in the body composition, namely in the percentage of fat mass, fat free mass, fat mass in the trunk, visceral index and metabolic age. In physical fitness, improvements were also seen in the experimental group, specifically in flexibility and handgrip strength. Conclusion: A multidisciplinary program involving regular physical exercise and nutritional educationled to achieve improvements in physical fitness and body composition in overweight and obese binge-eating disorder patients.

In Japan and many other industrialized countries, zinc is readily available as a nutritional supplement, for cosmetic purposes and for the treatment of atopic dermatitis. The potential risks associated with its use are not, however, fully recognized. As a reciprocal relationship exists between copper and zinc, excessive zinc can produce hypocupraemia,which can cause anaemia and neutropenia. We report on a male infant who presented with anaemia and neutropenia and showed signs of developmental delay after dietary restriction for food allergy and eating difficulties and zinc therapy administered for the treatment of atopic dermatitis at a dose nine times the daily dietary allowance for his age group. After 1 mo of zinc withdrawal, copper and ceruloplasmin concentrations had increased, and the blood cell count had improved, activity was increasing but verbal development remained limited. As development improved after withdrawal of zinc, we cannot rule out a relation between developmental delay and hyperzincaemia and/or hypocupraemia. Conclusion: Caution must be exercised in administering zinc to children during their neurological development.

Objective. The aim of this study was to elucidate the predictive value of intra-operative enteroscopy (IOE) and the effect of enteral nutrition (EN) with regard to the postoperative recurrence of Crohn disease (CD). Material and methods. Forty patients requiring surgery for severe intestinal complications of CD were examined by IOE, and the severity of the remnant small intestine was determined. Patients were subclassified into either an EN group (> 1,200 kcal/day) or a non-EN group (< 1,200 kcal/day) according to the amount of daily EN intake after surgery. Contributions of IOE findings and EN to postoperative recurrence were analysed retrospectively. Results. IOE identified intestinal lesions in 39 patients and active intestinal lesions in 24 patients. The cumulative rate of postoperative recurrence was significantly higher in patients with cobblestone appearance confirmed by IOE (p = 0.006). However, other active intestinal lesions were not related to postoperative recurrence. EN reduced the cumulative rate of postoperative recurrence (p = 0.017), especially in patients with penetrating type (p = 0.005), in patients who did not have colitis (p = 0.051) and in patients who did not have active intestinal lesions confirmed by IOE (p = 0.02). Conclusions. EN is a prophylactic that prevents the postoperative recurrence of small intestinal CD. Patients with the penetrating type of CD, and those who do not have active lesions in the small intestine according to IOE, are candidates for EN after surgery.
