Background Noncommunicable diseases(NCDs)are the main reasons of mortality *** of every two person is dying due to NCDs in *** policy actors,mainly the World health organization(WHO),published several reports and declarations on controlling and preventing *** study aimed to provide a situation for governance of NCDs in Afghanistan and proper solutions for identified *** We conducted qualitative research utilizing interpretive phenomenology.A self-developed questionnaire was developed to conduct the semi-structured interviews with 39 experts from *** results were analyzed using a deductive framework *** building block framework of health system developed by the WHO was used as predefined framework for this *** The governance building block of health system consists of five subthemes including policy making,planning,organizing,stewardship,and *** identified main strengths,weaknesses,opportunities,and challenges for these *** experts also provided key recommendations to address the *** Management of NCDs is a neglected part of the health system in *** evidence-based policy making with technical and indigenous planning,establishing responsive units with adequate financial and human resources within different ministries to address“health in all policies”concept,passing and implementing national laws and regulations to support national strategies for prevention and control of NCDs,and establishing decentralized monitoring systems to control the implementation of these strategies are the main recommendations of this *** government and international policy actors should invest and support the development of a multisectoral coordination system at national level for Afghanistan.

How neonatal diet affects the long-term development of rumination behavior, rumen fermentation and feed digestion in dairy calves fed a high milk level?

This study was to investigate growth performance, rumination development, rumen fermentation and feed digestion in young calves provided high volumes(about 20% of calf birth weight) of milk with or without forage inclusion and how these parameters correlate with each other. Immediately after birth,160 newborn Holstein female calves(41.6 ± 4.2 kg of initial BW) were randomly divided into 2 treatments: 1) starter(CON, only starter) and 2) starter and hay(HAY, both starter and hay). The calves were fed their respective experimental diets from d 4 to 84, after which they were all introduced to similar diets until the end of the experiment on d 196. Treatment had no effect on growth and structural measurements throughout the experimental period. However, treatment had an effect on the other parameters, mainly during the post-weaning period. Forage supplementation tended to reduce starter dry matter intake(P = 0.05), while increasing the forage intake(P < 0.01) and the feed-to-gain ratio(P < 0.01). HAY calves had increased neutral detergent fiber(NDF) and physically effective NDF(peNDF)intakes(P < 0.05) and tended to lower(P < 0.01) starch intake compared to CON calves. The HAY calves had a higher rumination time(P < 0.01), ruminal pH(P < 0.01), and acetate-to-propionate ratio(P = 0.05)compared to the CON calves. Spearman correlation analysis showed that rumination time was positively related to the ruminal p H at d 84(P = 0.01) and 196(P = 0.02). The HAY calves had similar apparent total-tract digestibility of dry matter(DM), NDF and ether extract(EE), but lower digestibility of organic matter(OM, P = 0.03), crude protein(CP, P < 0.01) and starch(P < 0.01) compared to those of the CON calves at week 12. Furthermore, there were no positive relationships between rumination time and nutrient digestibility or between rumination time per kilogram DM and nutrient digestibility. Background Tuberculosis (TB) remains a major public health problem in Nepal, high in settings marked by prevalent gender and social inequities. Various social stratifiers intersect, either privileging or oppressing individuals based on their characteristics and contexts, thereby increasing risks, vulnerabilities and marganilisation associated with TB. This study aimed to assess the inclusiveness of gender and other social stratifiers in key health related national policies and the health Management Information System (HMIS) of National Tuberculosis Programme (NTP) by conducting an intersectional analysis of TB cases recorded via *** A desk review of key policies and the NTP’s HMIS was conducted. Retrospective intersectional analysis utilized two secondary data sources: annual NTP report (2017–2021) and records of 628 TB cases via HMIS 6.5 from two TB centres (2017/18–2018/19). Chi-square test and multi-variate analysis was used to assess the association between social stratifers and types of TB, registration category and treatment *** Gender, social inclusion and concept of intersectionality are incorporated into various health policies and strategies but lack effective implementation. NTP has initiated the collection of age, sex, ethnicity and location data since 2014/15 through the HMIS. However, only age and sex disaggregated data are routinely reported, leaving recorded social stratifiers of TB patients static without analysis and dissemination. Furthermore, findings from the intersectional analysis using TB secondary data, showed that male more than 25 years exhibited higher odds [adjusted odds ratio (aOR) = 4.95, 95% confidence interval (CI): 1.60–19.06,P = 0.01)] of successful outcome compared to male TB patients less than 25 years. Similarly, sex was significantly associated with types of TB (P < 0.05) whereas both age (P < 0.05) and sex (P < 0.05) were significantly associated with patient registration category (old/new cases).Conclusio

Introduction: On the 5th of June 2022, an incident of a mass attack following multiple gunshots and explosions occurred in a community in Ondo State Nigeria. This study aims to assess the mental health status of victims of the mass attack to guide further interventions among them. Methods: A cross-sectional study was conducted among victims of a mass attack in Owo community, Ondo State. A total of 209 affected victims were interviewed on socio-demographic characteristics, symptoms of anxiety (AD) and post-traumatic stress disorder (PTSD), threat experienced, and mental health support received. A 7-item Generalized Anxiety Disorder (GAD-7) and 9-item Post Traumatic Stress Disorder (PTSD) scale were used to assess the mental health status of the victims. A point was assigned to respondents who reported the symptoms of GAD, with a maximum score of 7 attained. For GAD, scores were categorized as follows: 1 - 2 as mild, 2 - 3 as minimal, 4 - 5 as moderate and 6 - 7 as severe. The PTSD symptoms were rated using a 5-point Likert scale response, and assigned the following points;4 = extremely, 3 = quite a bit, 2 = moderate, 1 = a little bit and 0 = not at all. From a maximum score of 36, participants with scores 18 and above were categorized as those with provisional PTSD. The independent samples t-test and correlational analysis were used to determine the association between PTSD score and other independent variables, with an alpha level of significance set at 0.05. Results: Generally, 38 (18.2%) of the respondents had severe AD. About half (89;42.6%) were categorized as those with provisional PTSD. The mean level of both AD (3.40 ± 2.26) and PTSD (16.51 ± 7.63) score is higher among those who were married compared to those not married (anxiety disorder;2.52 ± 2.20, P = 0.005 and PTSD;13.20 ± 8.86, P = 0.004). A multi-faceted Case Area Targeted Intervention (CATI) approach emphasizing the integration of Water, Sanitation and Hygiene (WASH) interventions and Oral Cholera Vaccine (OCV) campaign was employed to respond to the outbreak of cholera in Garissa County. Drinking water sources in areas heavily impacted by cholera were systematically mapped and tested for microbiological quality. The quality assessment was carried out in April 2023 during an ongoing cholera outbreak in the county. A total of 109 samples were collected and tested for thermotolerant coliforms and other in situ parameters. The finding revealed that more than 87% of the samples did not meet the World health organization (WHO) standard for thermotolerant coliforms;and 30% had turbidity values above the recommended threshold values. None of the 109 samples had any traceable residual chlorine. Following these findings, the county government implemented the targeted interventions which resulted in a positive impact in the fight against cholera. The WHO supported key interventions which included capacity building in water quality monitoring and prepositioning of critical WASH commodities to the cholera affected areas.
