Value of carcinoembryonic antigen and cytokeratins for the detection of recurrent disease following curative resection of colorectal cancer

AIM： To evaluate the efficacy of postoperative serial assay of carcinoembryonic antigen （CEA） and cytokeratins for the detection of recurrent disease in patients with colorectal adenocarcinoma after radical surgery. METHoDs： Between 1993 and 2000, 120 patients with colorectal adenocarcinoma underwent radical surgery in the Department of surgical Gastroenterology, Federal university of sao paulo-Escola Paulista de Medicina, sao paulo, Brazil. Periodic postoperative evaluation was performed by assaying markers in peripheral serum, colonoscopy and imaging examination. Presence of CEA was detected using the Delfia^R method with 5 μg/L threshold, and cytokeratins using the LIA-mat TPA-M Prolifigen^R method with 72 U/L threshold. REsULTs： In the first postoperative year, patients without recurrent disease had normal levels of CEA （1.5 ＋ 0.9μg/L） and monoclonal tissue polypeptide antigen-M （TPA-M, 64.4 ± 47.8 U/L）, while patients with recurrences had high levels of CEA （6.9± 9.8 ;μg/L, P 〈 0.01） and TPA-M （192.2 ±328.8 U/L, P 〈 0.05）. During the second postoperative year, patients without tumor recurrence had normal levels of CEA （2.0 ± 1.8μg/L） and TPA-M （50.8±38.4 U/L）, while patients with recurrence had high levels of CEA （66.3 ±130.8 μg/L, P 〈 0.01） and TPA-M （442.7 ± 652.8 U/L, P 〈 0.05）. The mean follow-up time was 22.3 mo. There was recurrence in 23 cases. Five reoperations were performed without achieving radical excision. Rises in tumor marker levels preceded identification of recurrences： CEA in seven （30%） and TPA-M in eleven individuals （48%）. CoNCLUsIoN： Intensive follow-up by serial assay of CEA and cytokeratins allows early detection of colorectal neoplasm recurrence.

A variety of clinical manifestations are associated directly or indirectly with tuberculosis. Among them, haematological abnormalities can be found in both the pulmonary and extrapulmonary forms of the disease. We report a case of immune thrombocytopenic purpura(ITP) associated with intestinal tuberculosis in a liver transplant recipient. The initial management of thrombocytopenia, with steroids and intravenous immunoglobulin, was not successful, and the lack oftuberculosis symptoms hampered a proper diagnostic evaluation. After the diagnosis of intestinal tuberculosis and the initiation of specific treatment, a progressive increase in the platelet count was observed. The mechanism of ITP associated with tuberculosis has not yet been well elucidated, but this condition should be considered in cases of ITP that are unresponsive to steroids and intravenous immunoglobulin, especially in immunocompromised patients and those from endemic areas.

This paper presents an analysis of 64 lightning‐induced voltage waveforms recorded in an experimental system implemented in the campus of the university of são paulo,*** voltages were measured on a 10 m high,2.7 km long un‐energized overhead line consisting of two conductors installed on 6 m *** of the conductors had surge arresters,while the other one was left *** speaking,all the recorded voltages on the unprotected conductor were bipolar,but in many cases only one semi‐cycle could be considered important for practical *** criteria are discussed for the determination of the waveshape parameters,which may differ signifi-cantly from those of the standard lightning impulse voltage(1.2/50μs).Based on the proposed criterion for classifying the induced voltages,unipolar waveforms accounted for 67%of the total and,except for one case,all of them had positive *** median values of the front time and time‐to‐half‐value were,respectively,5.2μs and 15.8μ*** waveforms were 33%of the total and in approximately 62%of the cases the first semi‐cycle was of positive *** 91%of the cases the voltage peak occurred either in the first or the second semi‐cycle.

Grapes and sweet potatoes are two different plant species that have distinct bioactive composition and the use of both in a non-alcoholic mixed beverage may be an interesting source of *** and sensory characteristics,profile of phenolic compounds and biogenic amines of mixed juices of grapes (‘BRs Violeta’,‘BRs Cora’ and ‘Bordô’) and purple-fleshed sweet potato (‘JNRX12’) (0,10 and 30%) were *** beverage with sweet potato had high antioxidant activity attributed to the phytochemical compounds (***,gallic acid,catechin,tryptophan,melatonin,serotonin and dopamine).Low levels of unwanted amines (histamine,tyramine,putrescine and cadaverine) were detected,showing that their consumption is ***-fleshed sweet potato had high levels of desirable amines,mainly melatonin,making the addition of sweet potato attractive for enriching *** color,texture and sensory aspects were similar in whole grape juice and in mixed *** from ‘BRs Violeta’,with or without sweet potato showed high levels of bioactive compounds but were the least *** overall appearance and aroma of juices elaborated from ‘BRs Cora’ or ‘Bordô' grapes had similar acceptance,regardless of the addition of sweet *** from ‘BRs Cora’ and 30% purple-fleshed sweet potato are the most interesting mixture due to tryptophan and melatonin *** juices of grapes and sweet potatoes (10% and 30%) are viable options for the use of tubers outside the commercial standard,as they add interesting bioactive compounds to beverages and please the consumer.

The diversity and distribution of Pronoctiluca, a marine dinoflagellate of enigmatic systematic position, was studied in the vicinity of the Kuroshio and oyashio Currents, the Philippines, Celebes, sulu and south China seas, western and central equatorial and southeast Pacific ocean. The abundance of Pronoctiluca was high- er, with a wide vertical distribution, in eutrophic temperate regions, whereas it was nearly absent in surface waters of oligotrophic tropical regions. Most of the specimens corresponded to P. spinifera. Pronoctiluca pelagica, covered by hyaline layers and with no flagella, is considered as an encysted form. The bipartition was only observed in P. acuta-P, spinifera forms, that together with the occurrence of intermediate forms between P. spinifera and P. pelagica suggest that they may correspond to developmental stages of a single species. Pronoctiluca is essential to understanding the evolutionary history of the alveolates.

Inductive Model Generation for Text Classification Using a Bipartite Heterogeneous Network

Algorithms for numeric data classification have been applied for text classification. Usually the vector space model is used to represent text collections. The characteristics of this representation such as sparsity and high dimensionality sometimes impair the quality of general-purpose classifiers. Networks can be used to represent text collections, avoiding the high sparsity and allowing to model relationships among different objects that compose a text collection. such network- based representations can improve the quality of the classification results. one of the simplest ways to represent textual collections by a network is through a bipartite heterogeneous network, which is composed of objects that represent the documents connected to objects that represent the terms. Heterogeneous bipartite networks do not require computation of similarities or relations among the objects and can be used to model any type of text collection. Due to the advantages of representing text collections through bipartite heterogeneous networks, in this article we present a text classifier which builds a classification model using the structure of a bipartite heterogeneous network. such an algorithm, referred to as IMBHN （Inductive Model Based on Bipartite Heterogeneous Network）, induces a classification model assigning weights to objects that represent the terms for each class of the text collection. An empirical evaluation using a large amount of text collections from different domains shows that the proposed IMBHN algorithm produces significantly better results than k-NN, C4.5, sVM, and Naive Bayes algorithms.

AIM： To evaluate the potential of s-nitroso-N-acetylcysteine （sNAC） in inhibition of lipid peroxidation and the effect of oral sNAC administration in the prevention of nonalcoholic fatty liver disease （NAFLD） in an animal model.METHoDs： NAFLD was induced in Wistar male rats by choline-deficient diet for 4 wk. sNAC-treated animals （n=6）（1.4 mg/kg/day of sNAC, orally） were compared to 2 control groups： one （n=6） received PBs solution and the other （n=6） received NAC solution （7 mg/kg/d）. Histological variables were semiquantitated with respect to macro and microvacuolar fat changes, its zonal distribution, foci of necrosis, portal and perivenular fibrosis, and inflammatory infiltrate with zonal distribution. LooHs from samples of liver homogenates were quantified by HPLC. Nitrate levels in plasma of portal vein were assessed by chemiluminescence. Aqueous low-density lipoprotein （LDL） suspensions （200 pg protein/mL） were incubated with CuCl2 （300 μmol/L） in the absence and presence of sNAC （300 μmol/L） for 15 h at 37 ℃ Extent of LDL oxidation was assessed by fluorimetry. Linoleic acid （LA）（18.8 μmol/L） oxidation was induced by soybean lipoxygenase （sLo）（0.056 μmol/L） at 37 ℃ in the presence and absence of N-acetylcysteine （NAC） and sNAC （56 and 560 pmol/L） and monitored at 234 nm. REsULTs： Animals in the control group developed moderate macro and microvesicular fatty changes in periportal area. sNAC-treated animals displayed only discrete histological alterations with absence of fatty changes and did not develop liver steatosis. The absence of NAFLD in the sNAC-treated group was positively correlated with a decrease in the concentration of LooH in liver homogenate, compared to the control group （0.7±0.2 nmol/mg vs 3.2±0.4 nmol/mg protein, respectively, P〈0.05）, while serum levels of aminotransferases were unaltered. The ability of sNAC in preventing lipid peroxidation was confirmed in in vitro experiments using LA and LDL as model substrates. CoNCLUsIoN： oral
