Discussions on real-world acupuncture treatments for chroni clow-back pain in older adults

Chronic low-back pain(CLBP) is one of the most common pain conditions. Current clinical guidelines for low-back pain recommend acupuncture for CLBP. However, there are very few high-quality acupuncture studies on CLBP in older adults. Clinical acupuncture experts in the American Traditional Chinese Medicine Association(ATCMA) were interested in the recent grant on CLBP research announced by the National Center for Complementary and Integrative health. The ATCMA experts held an online discussion on the subject of real-world acupuncture treatments for CLBP in older adults. Seven participants, each with more than 20 years of acupuncture practice, discussed their own unique clinical experience while another participant talked about the potential mechanism of acupuncture in pain management. As a result of the discussion, a picture of a similar treatment strategy emerged across the participants for CLBP in older adults. This discussion shows that acupuncture may have complicated mechanisms in pain management, yet it is effective for the treatment of chronic pain involving maladaptive neuroplasticity;therefore, it should be effective for CLBP in older adults.

Introduction: Although many cardiopulmonary patients require home care services, the 6-minute walk test (6MWT) is unusable in most dwellings for objectively evaluating exercise capacity because it requires a 20 - 30 meter hallway. To meet this need, we developed a 2-minute step test (2MST): stepping up and down an 8 inch step for 2 minutes (1 step = bilateral step up + step down). Purpose: Evaluate the statistical validity, reproducibility, and sensitivity of the 2MST in assessing exercise capacity. Method: We compared the heart rate, oxygen saturation and perceived exertion obtained during performance of 2MST with those obtained during the 6MWT. Results: Comparing 2MST and 6MWT in 158 subjects for validity, r = 0.925 (P Conclusion: The 2MST is valid, reproducible, sensitive, safe, well-tolerated, and is a suitable substitute for the 6MWT.

Osteoporosis has become one of the most prevalent bone diseases in developed countries. As the world’s population ages, this complex skeletal disorder is predicted to become an even more widespread and serious health condition, posing greater societal economic burden. Current therapeutic agents are limited by clinically significant side effects. In this report, we present a case that suggests intra-articular platelet-rich plasma (PRP) injectate may improve bone mineral density (BMD). This observation raises a question with exciting implications—can PRP serve as a viable and relatively safe, targeted treatment option to improve bone density?

4 节段性神经肌肉疗法(SNMT)治疗的目标是要逆转节段性敏感,所以必须同时治疗周围的疾病或损伤过程(周围敏化)以及由它所引起的中枢神经系统的改变(中枢敏化).这一治疗方法将从根本上解除令患者感到沮丧难忍的痛觉过敏和触物痛感.没有降低SSS的治疗通常不能消除周围疼痛发生器,而只注重周围疼痛发生器的治疗可能只会暂时缓解疼痛而不能达到长期缓解的目的 ,疼痛症状和功能障碍还会复发.
