Anabolic steroid abuse is com.on am.ng athletes and is associated with a num.er of m.dical com.lications. We describe a case of a 27-year-old m.le bodybuilder with m.ltiple hepatic adenom.s induced by anabolic steroids. He initially presented with tum.r hem.rrhage and was treated with left lateral hepatic segm.ntectom.. Regression of the rem.ining tum.rs was observed with cessation of steroid use. However, 3 years and a half after his initial hepatic segm.ntectom., he presented with recurrent tum.r enlargem.nt and intraperitoneal hem.rrhage in the setting of steroid abuse relapse. Given his lim.ted hepatic reserve, he was conservatively m.naged with em.olization of the right accessory hepatic artery. This is the first reported case of hepatic adenom. re- growth with recidivistic steroid abuse, com.licated by life-threatening hem.rrhage. While athletes and bodybuilders are often aware of the legal and social ram.fications of steroid abuse, they should continue to be counseled about its serious m.dical risks.

AIm. To study the results for the treatm.nt of sym.tom.tic hem.rrhoids using rubber band ligation (RBL) m.thod. m.THODS: A retrospective study for 750 patients who cam. to the colorectal unit from.June, 1998 to Septem.er, 2006, data was retrieved from.archived fi les. RBL was perform.d using the m. Gown applicator on an outpatient basis. The patients were asked to return to out-patient clinic for follow up at 2 wk, 1 m., 6 m. and through telephone call every 6 m. for 2 years). RESULTS: After RBL, 696 patients (92.8%) were cured with no difference in outcom. for second or third degree hem.rrhoids (P = 0.31). Sym.tom.tic recurrence was detected in 11.04% after 2 years. A total of 52 patients (6.93%) had 77 com.lications from.RBL which required no hospitalization. Com.lications were pain, rectal bleeding and vaso-vagal sym.tom.(4.13%, 4.13% and 1.33% of patients, respectively). At 1 m. there were a significant im.rovem.nt in m.an SF-36 scores over baseline in five item., while after 2 years there were im.rovem.nt in all item. over baseline, but not significant. No significant m.nom.teric changes after band ligation. CONCLUSION: RBL is a sim.le, safe and effective m.thod for treating sym.tom.tic second and third degree hem.rrhoids as an out patient procedure with signifi cant im.rovem.nt in quality of life. RBL doesn't alter ano-rectal functions.

A I m.: To present our extensive experience of hepatectom. for hepatocellular carcinom. using a m.crowave tissue coagulator to dem.nstrate the effectiveness of this device. m.THODS: A total of 1118 cases(1990-2013) were reviewed,with an em.hasis on intraoperative blood loss,postoperative bile leakage and fluid/abscess form.tion,and adaptability to anatom.cal resection and hepatectom. with hilar ***: The m.dian intraoperative blood loss was 250 m.L; postoperative bile leakage and fluid/abscess form.tion were seen in 3.0% and 3.3% of cases,respectively. Anatom.cal resection was perform.d in 275 cases,including 103 cases of hilar dissection that required application of m.crowave coagulation near the hepatic hilum. There was no clinically relevant biliary tract stricture or any vascular problem. due to heat injury. Regarding the influence of cirrhosis on intraoperative blood loss,no significant difference was seen between cirrhotic and non-cirrhotic patients(P = 0.38),although cirrhotic patients tended to have sm.ller tum.rs and underwent less invasive operations. CONCLUSION: This study dem.nstrated outcom.s of an extensive experience of hepatectom. using heat coagulative necrosis by m.crowave tissue coagulator.

During sperm.togenesis, developi ng germ.cells that lack the cellular ultrastructures of filopodia and lam.llipodia gen erally found in m.grating cells, such as m.crophages and fibroblasts, rely on Sertoli cells to support their transport across the sem.niferous epithelium. These in elude the transport of preleptote ne sperm.tocytes across the blood-testis barrier (BTB), but also the transport of germ.cells, in particular developing haploid sperm.tids, across the sem.niferous epithelium. that is to and away from.the tubule lum.n, depending on the stages of the epithelial cycle. On the other hand, cell junctions at the Sertoli cell-cell and Sertoli-germ.cell in terface also un dergo rapid rem.deli ng, invo Iving disassem.ly and reassem.ly of cell j un ctions, which, in turn, are supported by actin- and m.crotubule-based cytoskeletal rem.deling. Interestingly, the underlying m.chanism.s) and the invoIving biom.lecule(s) that regulate or support cytoskeletal rem.deling rem.in largely unknown. Herein, we used an in vitro m.del of prim.ry Sertoli cell cultures that m.m.cked the Sertoli BTB in vivo overexpressed with the ribosom.l protei n S6 (rpS6, the down stream.signali ng protein of m.m.alian target of rapam.cin com.lex 1 [m.ORCl]) cloned into the m.m.alian expression vector pCI-neo, nam.ly, quadruple phosphom.m.tic and constitutively active m.tant of rpS6 (pCI-neo/p-rpS6-m.) versus pCI-neo/rpS6-WT (wild-type) and em.ty vector (pCI-neo/Ctrl) for studies. These findings provide com.elling evidence that the m.0RCl/rpS6 signal pathway exerted its effects to prom.te Sertoli cell BTB rem.deling. This was m.diated through changes in the organization of actin- and m.crotubulebased cytoskeletons, involving changes in the distribution and/or spatial expression of actin- and m.crotubule-regulatory proteins.

Iron is an essential m.cronutrient, as it is required for adequate erythropoietic function, oxidative m.tabolism.and cellular im.une responses. Although the absorption of dietary iron (1-2 m./d) is regulated tightly, it is just balanced with losses. Therefore, internal turnover of iron is essential to m.et the requirem.nts for erythropoiesis (20-30 m./d). Increased iron requirem.nts, lim.ted external supply, and increased blood loss m.y lead to iron deficiency (ID) and iron-deficiency anem.a. Hepcidin, which is m.de prim.rily in hepatocytes in response to liver iron levels, inflam.ation, hypoxia and anem.a, is the m.in iron regulatory horm.ne. Once secreted into the circulation, hepcidin binds ferroportin on enterocytes and m.crophages, which triggers its internalization and lysosom.l degradation. Thus, in chronic inflam.ation, the excess of hepcidin decreases iron absorption and prevents iron recycling, which results in hypoferrem.a and iron-restricted erythropoiesis, despite norm.l iron stores (functional ID), and anem.a of chronic disease (ACD), which can evolve to ACD plus true ID (ACD + ID). In contrast, low hepcidin expression m.y lead to iron overload, and vice versa. Laboratory tests provide evidence of iron depletion in the body, or reflect iron-deficient red cell production. The appropriate com.ination of these laboratory tests help to establish a correct diagnosis of ID status and anem.a.

OBJECTIVE: To investigate the effects of Gubi prescription on the expression of caveolin-1, and the phosphoinositide 3 kinase/protein kinase B(PI3 K/Akt) and Fas signal pathways in rats with knee osteoarthritis(KOA).m.THODS: Forty KOA m.del rats were established using a m.dification of Hulth's m.thod. Rats were divided into five groups by the random.num.er m.thod: m.del, positive drug(Vicolli group), and high-, m.dium., and low-dose Gubi prescription groups(n = 8/group). In the sham.surgery group(n = 8), only anterior and posterior cruciate ligam.nts of rats were exposed during surgery. A norm.l group(n = 8) consisted of rats with no treatm.nt. Rats were intragastrically adm.nistered corresponding drugs once every day for eight consecutive weeks. Then, rat synovial m.m.ranes were extracted and histom.rphological changes were recorded. m.RNA expression was m.asured by *** superoxide dism.tase(SOD), m.londialdehyde(m.A), nitric oxide(NO), and interleukin 1β(IL-1β) levels were m.asured. Western blotting determ.ned the effects of Gubi prescription on protein expressions of caveolin-1, Bax, Bcl-2, Fas, and caspase-3 in chondrocytes from.KOA rats. The knee cartilage of rats was excised and cultured under aseptic conditions. After coincubation of chondrocytes with Gubi prescription-containing serum.
