AIM To examine the influence of night duty(ND) on endoscopic therapy for biliary duct *** The subjects consisted of 133 patients who received initial endoscopic therapy for biliary duct stones performed by eight endoscopists after they had been on(ND group, n = 34 patients) or not [day duty(DD) group, n = 99 patients]. Patient characteristics(age, gender, history of abdominal surgery, transverse diameter of the largest stone, number of stones), years of experience of the endoscopists, endoscopic procedures [sphincterotomy, papillary balloon dilation(EPBD), papillary large balloon dilation(EPLBD)], and outcomes of initial endoscopy(procedure time; rate of stone removal by the first endoscopist; proceduresuccess rate by the first endoscopist: removal of stones or endoscopic retrograde biliary drainage; rate of final stone removal; final procedure success rate; complications; hospitalization after the procedure) were compared retrospectively between the two groups. History of abdominal surgery and treatment outcomes were also compared between the groups for each of the four endoscopists who performed most of the procedures in the ND *** There were no significant differences regarding the number of treatments performed by each endoscopist or the years of experience between the ND and DD groups. The frequency of endoscopic retrograde cholangiopancreatography procedures did not differ significantly between the groups. There were also no significant differences regarding patient characteristics: age, gender, history of abdominal surgery(ND 7: Billroth II 4, R-Y 3; DD 18: double tract reconstruction 1, Billroth I 3, Billroth II 6, R-Y 7, duodenoduodenostomy for annular pancreas 1), transverse diameter of largest stone, and number of stones between the two groups. Primary gout in Shantou: a clinical and epidemiological study

Objective To evaluate the prevalence of primary gout in the Shantou area, China, and to understand its clinical features. Methods Samples from three surveys of the Chenghai across ten years were studied. Clinical, laboratory and radiology data of 419 cases of primary gout were collected and analyzed. Results Prevalence of primary gout in the three surveys were 0.17%, 0.15% and 0.26%, respectively. Among the clinical patients, the ratio of male to female was 21∶1; the average age of onset of women was significantly higher than that of men. The number of cases in the last ten years has been increasing. In the first wave of cases, the first metatarsophalangeal joint was involved in 82.1% of all cases; no fourth metatarsophalangeal joint involvement had been observed. Hyperlipemia was the most common accompanying abnormal laboratory test (57.1%). Radiographic bone erosion was found in 49.4% patients, most of them with disease duration more than 5 years. Conclusions The prevalence of primary gout in Shantou area has been increasing in the last ten years. Changes in diet and lifestyle may be responsible for this rapid increase.

Objective： To evaluate the effect of a 10-day course of triptolide （TP） on rat cytochrome （CY） P3A4 activity, and on the pharmacokinetics of cyclophosphamide （CPA）. Methods： In the pharmacokinetics experiment, rats were orally given 0.9% NaCI solution （n=5） and TP [1.2 （mg/kg.d）] for 10 days and a single dose of CPA was administered intravenously （100 mg/kg） to rats on day 11. Blood samples were collected up to 4 h at predetermined time intervals, the plasma concentration of CPA was determined by high performance liquid chromatography （HPLC） and pharmacokinetic parameters were determined. In the in vitro CYP3A4 activity inhibition research, rat blank liver microsomes were divided into 3 groups： a control group, a TS （5μL, 200 μmol/L） with TP （5 μL, 12.5 μmol/L） group, a TS with ketoconazole （5 μL, 1 μmol/L） group. Concentration of 613 -hydroxylated testosterone （6β -OHT） in liver microsomes was measured by HPLC and the activity of CYP 3A4 was calculated through the following formula： Einhlbltot//Eoontrol×100%=Cinhlbltor/Ccontrol×100%. Results： Compared with the control group, the area under the plasma concentration-time curve （AUC0.∞） of CPA was significantly increased by 229.05% pretreated with TP （P〈0.01）. Peak plasma concentrations （Cmax of CPA was significantly increased and plasma half-life was correspondingly extended. The CYP3A4 activity was significantly inhibited by ketoconazole 93.5%± 0.2% and TP 84.6% ± 0.3% compared with the control group （P〈0.01 and P〈0.05, respectively）. Conclusion： Our results strongly suggested that long-term oral intake of TP can distinctly inhibit the CYP3A4 activity and this inhibition evidently decrease the formation of toxic metabolites of CPA.

The patient was treated with systemic corticosteroids, spironolactone, hydroxychloroquine(HCQ), mycophenolate mofetil (MMF), and intravenous immunoglobulin. After 4 wkof hospitalization, the patient was discharged. Indocyanine green angiography showed no leakagefrom choroidal vessels, and ocular coherence tomography detected low amounts of subretinalfluid right before discharge. The patient was prescribed oral methylprednisolone, HCQ, and *** months after the first visit, ophthalmological examination revealed a visual acuity of 20/20bilaterally, and SLE disease activity was well controlled;her symptoms disappeared *** we presented a case of lupus choroidopathy, successfully treated with systemic corticosteroids,and discussed previously reported cases, focusing on differential diagnosis with a centralserous chorioretinopathy.
