Carbon Monoxide and Respiratory Disorders in Professional Female Fish Smokers at 2 Artisanal Sites in Benin

Introduction: The use of biomass for cooking is a very common practice in Africa and Benin, and is associated with exposure to organic pollutants. The aim of this study was to assess carbon monoxide exposure and respiratory disorders in women at artisanal fish-smoking sites. Methods: This was a cross-sectional study of women fish smokers at two artisanal fish-smoking sites in Benin. Recruitment was exhaustive. An interview using a respiratory questionnaire inspired by the Union questionnaire, spirometry and carbon monoxide (CO) measurement were carried out. Descriptive analysis and comparison of means using Student’s t-test with a threshold of p = 0.05 were performed. Results: The median age of the 81 people surveyed was 40, with an interquartile range (IQT) of [25 to 75]. The median duration of daily exposure to smoke was 6 h IQT [4, 7]. Of those surveyed, 64 (79.01%) were illiterate and 39 (48.24%) had been working for 20 years. CO levels at the start of activity varied from 89 to 145 ppm in one case and from 40 to 89 ppm in the other. Respiratory symptoms were present in 19 (23.46%);18 (22.22%) had mild airway impairment on spirometry. There was a statistically significant decrease between the mean values of peak expiratory flow (PEF), mean expiratory volume in seconds (FEV1) and forced vital capacity (FVC) before and after exposure to smoke. Conclusion: CO levels are high on fish smoking sites and respiratory problems are common. Improved working conditions are important for these women.

Results of the Evaluation of the Respiratory Condition of Permanent Workers in an Oil Mill in Benin

Introduction: The process of extracting oil from cotton seeds can create dusty work atmospheres that can cause respiratory problems. The main objective of this study was to determine the prevalence of respiratory problems among permanent workers in an oil mill in Benin. Methods: This cross-sectional study of 52 workers in an oil mill took place in January 2017 as part of the annual medical check-ups of workers. A questionnaire was administered and spirometry using Spirobank II and chest radiography were performed. The spirometry results were interpreted by an occupational physician and a pulmonologist. Data were entered and analyzed using Epidata software. Results: The mean age was 42.7 ± 6.4 years, and 43 of the 52 workers were men. Of these, 58% were in technical production positions and 42% in administrative positions. Most of them had more than 10 years of seniority. The prevalence of respiratory symptoms among production workers was 4 (13%) versus 2 (9%) among administrative workers. A total of 8 (15.4%) abnormal spirometry was identified with 4 obstructive syndrome, 3 restrictive syndrome, 1 a mixed pattern. There were 5 (16.6%) workers in production versus 3 (13.6%) in administration who had abnormal spirometry. The means 25/75 forced expiratory flow (FEF) value of production workers was significantly lower than that of administration workers. Abnormal chest radiographs were 5 (17%) in production workers compared to 3 (14%) in administration workers. Conclusion: Oil mill workers had few respiratory symptoms. However, production workers had more ventilatory disorders than administrative workers. A spirometric follow-up of this group of workers is therefore necessary.

Introduction: In the general population as well as in the workplace, hypertension is one of the main causes of morbidity and mortality in the world. The particularities of hypertension in the world of work remain little studied. The objective of this study was to determine the prevalence and factors associated with hypertension among workers in a town hall in Benin in 2019. Methods: This was a descriptive, analytical cross-sectional study conducted from December 2018 to January 2019. A comprehensive recruitment was conducted. Data were collected during a face-to-face interview, supplemented by measurement of anthropometric parameters and blood pressure as well as a work stress assessment. Frequencies were compared using the chi-square test and Fisher’s test. The significance level was 5%. Results: A total of 86 workers were included. Their mean age was 41.55 ± 9.82 years with a sex ratio of 6.1. The prevalence of hypertension was estimated to be 18.6% [95% CI: 11.96% - 29.75%]. Hypertension was higher in workers under work stress (41.18%) as opposed to non-stressed workers (14.49%) (p = 0.032);in those who were overweight (30.77) as opposed to non-overweight (2.94%) (p Conclusion: Work stress and obesity are modifiable factors associated with hypertension in workers at the Bohicon city hall. These factors deserve to be corrected by occupational health promotion and prevention actions.

Prevalence and Associated Factors of occupational Stress in the Mining Sector: The Case Study of the Youga Mine at Zabre in Burkina Faso

Introduction: occupational stress (OS) is a public health issue;it affects all sectors of activity in Burkina Faso. General objective: Assess the situation of professional stress and its associated factors among workers at the Youga mine in the municipality of Zabré. Material and Methods: This was a descriptive and analytical cross-sectional study. Data collection took place August 15, 2020 to October 10, 2020 from 324 mine workers who met the inclusion criteria using an anonymous questionnaire. Data analysis was done with EpiDATA3.1 software. The Fisher and Chi-squared tests were used to compare the proportions. A binary logistic regression was performed by the stepwise top-down method. The significance level chosen for the tests was p ≤ 0.05. Results: In our study, 324 workers were included. The average age was 35.84 ± 6.56 years with extremes of 23 years and 58 years. The sex ratio was 8. The prevalence of OS was 26.23% and that of iso strain and lack of rewards were 24.38% and 45.06%, respectively. Factors associated with OS were marital status p = 0.010;higher education level p = 0.032, lack of recognition p = 0.001, intention to change jobs p = 0.009, job security p = 0.035;job satisfaction p = 0.046 and the stress felt by the worker at his job p = 0.003. Conclusion: occupational stress is a reality in the mining sector in Burkina Faso, our study consisted of evaluating the extent and determining the factors of OS in order to propose preventive measures for the good health of workers.

Hearing Loss among Grain Millers in the Informal Sector in Benin

Introduction: Noise-induced hearing loss is a preventable health problem worldwide. However, it continues to affect workers especially in the informal sector, due to the lack of medical and environmental monitoring. In Benin, millers are highly exposed. The objective of the study was to assess the hearing health situation of grain millers in the Dantokpa market in 2020. Methods: This was a cross-sectional study that included by exhaustive recruitment 57 millers. The data were collected using a standardized questionnaire followed by blood pressure measurements, noise levels and the performance of audiometries. Descriptive and univariate analysis was performed. Results: The sample consisted only of men. The median age was 25 years with extremes of 18 and 50 years. Noise levels at the workstation ≥ 85 dB (A) were obtained for 94.74% of the workers. No worker was wearing hearing protection equipment. The prevalence of hearing loss was 87.72% (95% CI = [76.32%;94.92%]) and that of occupational deafness was 29.82% (95% CI = [18.43% - 43.40%]) which was associated with age over 28 and high blood pressure. An awareness campaign with donation of hearing pads was carried out. Conclusion: Hearing loss is important among millers who do not benefit from any supervision because they are in the informal sector. A restructuring of this sector is necessary for the hearing health of workers.

Introduction: One form of child labor is early learning, which is a less worrying phenomenon in our communities in Benin. The objective of this study was to assess the practice of early learning for children in rural areas. Methods: This was a cross-sectional study combined with a qualitative component conducted in the Kissamey district of Benin with four targets: child apprentices (52), master craftsmen (41), parents and guardians (34), local authorities (9). The collection tools were a questionnaire and an interview guide. Results: The frequency of early learning among children was 32.07% with difficult socioeconomic conditions: polygamy (75%), strong siblings (79%), out of school (33%), unmet food needs (96%). The reasons for early learning according to parents were: refusal of the child to go to school (44%), financial difficulties (31%), school failure (22%), but 38% of these children did not know the reason for their learning. The actors had little knowledge of the regulatory texts. Conclusion: Early learning remains a societal problem related to out-of-school and difficult socioeconomic conditions.

Epidemiological Aspect of Burnout among Careful Staff of A Zone Hospital in Cotonou in 2019

Introduction: Burnout syndrome is one of the many forms of suffering at work that affects healthcare professionals. It is still little diagnosed in Benin. Objectives: the objective was to assess the prevalence of burnout among the nursing staff of a Zone Hospital (HZ) in Cotonou. Study Methods: This was a descriptive and analytical cross-sectional study, which took place from September to October 2019 and included all personnel assigned to care. This has been submitted to the Malasch Burnout Inventory (MBI). The data collected was analyzed using STATA software version 15. A logistic regression made it possible to identify those associated with burnout. The significance level was 5%. Result: A total of 173 caregivers were included, including 118 women, for a sex ratio of 0.47. The mean age was 39 ± 10.1 years. The study population consisted of 33.5% nursing assistants, 24.3% nurses, 24.2% physicians, 7.5% midwives, and 8.6% other health professionals. The prevalence of burnout was 30.6% of which 2.3% were severe cases. As a result of burnout, 33% of officers and 19.08% experienced depersonalization and 10.4% experienced a sense of low personal achievement. The associated factors identified were female sex (p Conclusion: the prevalence of burnout among caregivers is high. Improving the professional climate and family relations are avenues for prevention.

Characteristics of soil microbial community functional and structure diversity with coverage of Solidago Canadensis L

The relationship between Solidago canadensis L. invasion and soil microbial community diversity including functional and structure diversities was studied across the invasive gradients varying from 0 to 40%, 80%, and 100% coverage of Solidago canadensis L. using sole carbon source utilization profiles analyses, principle component analysis （PCA） and phospholipid fatty acids （PLFA） profiles analyses. The results show the characteristics of soil microbial community functional and structure diversity in invaded soils strongly changed by Solidago canadensis L. invasion. Solidago canadensis L. invasion tended to result in higher substrate richness, and functional diversity. As compared to the native and ecotones, average utilization of specific substrate guilds of soil microbe was the highest in Solidago canadensis L. monoculture. Soil microbial functional diversity in Solidago canadensis L. monoculture was distinctly separated from the native area and the ecotones. Aerobic bacteria, fungi and actinomycetes population significantly increased but anaerobic bacteria decreased in the soil with Solidago canadensis L. monoculture. The ratio of cyl9：0 to 18：1 co7 gradually declined but mono/sat and fung/bact PLFAs increased when Solidago canadensis L. became more dominant. The microbial community composition clearly separated the native soil from the invaded soils by PCA analysis, especially 18： lco7c, 16： lco7t, 16： lco5c and 18：2co6, 9 were present in higher concentrations for exotic soil. In conclusion, Solidago canadensis L. invasion could create better soil conditions by improving soil microbial community structure and functional diversity, which in turn was more conducive to the growth ofSolidago canadensis L.

Effect of the new maternity insurance scheme on medical expenditures for caesarean delivery in Wuxi, China： a retrospective pre/post-reform case study

Aiming to control rising medical expenditures and help improve China＇s healthcare systems, this study examined whether a cap-based medical insurance scheme with shared financial interest between the insurance and healthcare providers is effective in containing hospitals＇ C-section medical expenditures. We used 6547 caesarean delivery case records from a teaching tertiary-level general public hospital located in Wuxi, China （2004-2013）, and used the Chow test to investigate the possibility of significant variation in mean medical expenditures for caesarean deliveries pre- and post-reform. We also used paired sample t-tests and linear regression models to compare the mean medical expenditures between insured and uninsured women undergoing caesarean delivery during the post-reform period. After the scheme＇s implementation, medical expenditures for caesarean deliveries declined and the medical expenditures of women covered by the scheme were significantly lower than those of uninsured patients. These findings indicated the scheme＇s effectiveness in minimizing caesarean delivery expenditures. The cap-based medical insurance scheme with shared financial interest between insurance and healthcare providers would likely steer healtbcare providers＇ behaviors in a more cost-effective direction.
