A study evaluating the effect of Educational Needs Assessment Tool (ENAT) focused patient education, on health outcomes in patients with Rheumatoid Arthritis: Protocol of an ongoing study

Aims and objectives: To evaluate the effectiveness of a patient education tool (the ENAT) on self efficacy, patient knowledge, health outcomes and its usability in practice. Background: The effectiveness and value of patient education in rheumatoid arthritis have been questioned. The ENAT was developed to help the nurses to provide more focused, individualized and relevant education. The psychometric properties of the ENAT have been assessed. Design: This study is a single blind randomized controlled trial using both quantitative and qualitative methodology. The primary outcome is patient self efficacy. Secondary outcomes are physical function, psychological status, pain levels, social interaction and patient knowledge of disease. Methods: 130 RA patients will be randomized into an experimental (EG) or control group (CG). The EG patients will complete the ENAT prior to seeing a Clinical nurse Specialist (CNS) at weeks 0, 16 and 32. The CNS will use the ENAT as a template to meet patients perceived educational needs in addition to the usual care. The CG will receive their usual care from the CNS without the aid of the ENAT. To determine the ENAT’s usability, semi-structured quailtative interviews will take place with the practitioners and a sample of patients once they have completed the quantitative arm of the study. Results: The study commenced in April 2011 and the results are expected to be available after April 2013. Conclusions: It is hypothesized that nurses’ use of the ENAT will lead to increased patient self efficacy, and improved patient knowledge and health outcomes among the patients who used it. Furthermore, it is hypothesized that the ENAT will be easy for staff and patients to use. Relevance to clinical practice: The ENAT was developed to assess the perceived educational needs of patients with rheumatoid arthritis and thus help the nurses to provide more focused, individualised and relevant education.

Context: Most older adults with dementia will be cared for by primary care physicians, but the primary care practice environment presents important challenges to providing quality care. Objective: To test the effectiveness of a collaborative care model to improve the quality of care for patients with Alzheimer disease. Design, Setting, and Patients: Controlled clinical trial of 153 older adults with Alzheimer disease and their caregivers who were randomized by physician to receive collaborative care management (n=84) or augmented usual care (n=69)-at primary care practices within 2 US university-affiliated health care systems from January 2002 through August 2004. Eligible patients (identified via screening or medical record) met diagnostic criteria for Alzheimer disease and had a self-identified caregiver. Intervention: Intervention patients received 1 year of care management by an interdisciplinary team led by an advanced practice nurse working with the patient’s family caregiver and integrated within primary care. The team used standard protocols to initiate treatment and identify, monitor, and treat behavioral and psychological symptoms of dementia, stressing nonpharmacological management. Main Outcome Measures: Neuropsychiatric Inventory (NPI)-administered at baseline and at 6, 12, and 18 months. Secondary outcomes included the Cornell Scale for Depression in Dementia (CSDD), cognition, activities of daily living, resource use, and caregiver’s depression severity. Results: Initiated by caregivers’reports, 89%of intervention patients triggered at least 1 protocol for behavioral and psychological symptoms of dementia with a mean of 4 per patient from a total of 8 possible protocols. Intervention patients were more likely to receive cholinesterase inhibitors (79.8%vs 55.1%; P=.002) and antidepressants (45.2%vs 27.5%; P= .03). 目的了解重症监护专科护士对体验式专科培训的需求,为后续研究构建重症监护专科护士体验式培训方案提供参考依据。方法采用便利抽样法,应用自行编制的ICU专科护士体验式培训需求调查问卷,对湖南省内2所二级综合医院和6所三级综合医院各类ICU病房的360名护士进行问卷调查。结果63.6%的ICU护士未接受过专科培训,接受过培训的53.0%护士对培训效果满意。关于培训模式,48.1%护士对半脱产培训表示赞成,36.1%的护士认为9~12周的培训周期更为合理,并根据体验式教学的四个阶段选择不同的体验式教学方法。ICU护士对体验式培训的需求总体处于较高水平,为(4.41±0.48)分,其中对“重症监护技术”章节的体验式培训需求水平最高。体验式培训需求得分较高的5个分章节依次为专科操作技术(4.67±0.53)分、危重疾病护理(4.66±0.55)分、危重症患者抢救流程(4.65±0.56)分、监测指标评估(4.63±0.56)分和相关仪器使用(4.61±0.57)分。结论因此,亟待以需求为导向不断更新培训方法,构建系统、全面、多层次的ICU专科护士体验式培训方案,从而进一步提升培训效果。

Background: Trichinellosis is a parasitic disease of humans caused by nematodes of the genus Trichinella, and cause morbidity and mortality of human globally. Deciphering processes that drive species diversity and adaptation are key to understanding parasitism and developing control strategies. The aim of the present study was to analyse adaptation evolution of Trichinella spiralis genes by comparative genomic analysis with Brugia malayi, Trichuris suis, Ancylostoma aceylanicum and Caenorhabditis elegans and may serve as a foundation for future investigations to understand parasite anaerobic environment adaptation at molecular levels. Method: CODEML program from the PAML package was used to infer the most likely d N/d S ratio for each pair of sequences, the ones with d N/d S ratio > 1 were considered as positive selected genes. Context. Patient safety has been recognized as an important problem in health care. However, knowledge about adverse events and preventable adverse events in children is relatively limited. Objective. To describe the incidence and types of adverse events and preventable adverse events in children. Design. Analysis of pediatric hospitalizations in the Colorado and Utah Medical Practice Study, which involved a retrospective, 2-level (nurse and physician) medical record review of a populationbased, representative sample of all pediatric hospital discharges. Main Measures. Adverse events were defined as an injury caused by medical management rather than disease processes that resulted in either prolonged hospitalization or disability at discharge. A preventable adverse event was defined as an avoidable adverse event based on currently available knowledge and accepted practices. Patients. 3719 discharged hospital patients, 0-20 years old, and 7528 nonelderly (21-65 years old) discharged adult patients in Colorado and Utah. Setting. All hospitals in Colorado and Utah. Results. Adverse events occurred in 1%of pediatric hospitalizations in Colorado and Utah; 0.6%were preventable. Preventable adverse events rates were 0.53%in neonates and infants (0-0.99 years), 0.22%in children 1-12 years of age, and 0.95%in adolescents 13-20 years of age, compared with a rate of 1.50%in nonelderly adults. Of preventable adverse event types, birth related (32.2%) and diagnostic related (30.4%) events were the most common and were significantly more common than surgically related preventable adverse events (3.5%). Conclusions. These data suggest that ～70 000 children hospitalized in the United States experience an adverse event each year; 60%of these events may be preventable. The epidemiology of adverse events and preventable adverse events in children is different than in adults. Summary:nurses'work-related fatigue has been recognized as a threat to nurse health and patient *** aim of this study was to assess the prevalence of fatigue among first-line nurses combating with COVID-19 in Wuhan,China,and to analyze its influencing factors on fatigue.A multi-center,descriptive,cross-sectional design with a convenience sample was *** statistical population consisted of the first-line nurses in 7 tertiary general hospitals from March 3,2020 to March 10,2020 in Wuhan of China.A total of 2667 samples from 2768 contacted participants completed the investgation,with a response rate of 96.35%.Social-demographic questionnaire,work-related questionnaire,Fatigue Scale-14,Generalized Anxiety Disorder-7,Patient Health Questionnaire-9,and Chinese Perceived Stress Scale were used to conduct online *** descriptive statistic of nurses'social-demographic characteristics was conducted,and the related variables of work,anxiety,depression,perceived stress and fatigue were analyzed by Z-tests,nonparametric test and Pearson's correlation *** significant factors which resulted in nurses'fatigue were further analyzed by multiple linear regression *** median score for the first-line nurses'fatigue in Wuhan was 4(2,8).The median score of physical and mental fatigue of them was 3(1,6)and 1(0,3)*** to the scoring criteria,35.06%nurses(w=935)of all participants were in the fatigue status,their median score of fatigue was 10(8,11),and the median score of physical and mental fatigue of them was 7(5,8)and 3(2,4)*** linear regression analysis revealed the participants in the risk groups of anxiety,depression and perceived stress had higher scores on physical and mental fatigue and the statistically significant positive correlation was observed between the variables and nurses'fatigue,the frequency of exercise and nurses'fatigue had a statistically significant negative correlation,and average daily working

Due to globalization, there is a need for nurses with skills and competence in providing safe, competent and culturally appropriate care. The aim of the study was to investigate whether International Study Experiences (ISE) in other countries during basic nursing education had an impact on newly graduated nurses as regards to self-reported competence. Moreover, a second aim was to explore what background factors that facilitated or constituted a hindrance for nursing students to choose to conduct part of their basic nursing education abroad. At 11 Universities/University Colleges (henceforth called Higher Education Institutions [HEIs]) in Sweden, 565 nursing students responded to the nurse Professional Competence (NPC) Scale. Students with ISE rated their competence significantly higher on three NPC competence areas—“Legislation in nursing and safety planning”, “Leadership and development of nursing” and “Education and supervision of staff/students”. Background factors that significantly seemed to enhance ISE were living alone, not having children or other commitments in relation to family, international focus at the HEI and previous international experience. Lack of financial means was reported to prevent students from choosing ISE. The study implies that several background factors are of importance whether students choose ISE or not. ISE during basic nursing education might result in better self-reported competence in leading and developing nursing care, including education of future nurses, and in providing safe care.

目的探索影响男护士心理幸福感的环境因素和个人因素,以及护理实践环境影响男护士工作投入的心理机制,从而为临床护理管理者制订措施提供一定的参考。方法采用护理实践环境指数量表(Nursing Work Index-Practice Environment Scale,NWI-PES)、心理资本问卷(Psychological Capital Questionnaire,PCQ-24)以及工作投入量表(Utrecht Work Engagement Scale,UWES)对中国长沙市3所三级甲等医院的161名男护士进行调查。结果男护士护理实践环境得分为(2.88±0.31)分、心理资本得分为(4.42±0.62)分,工作投入得分为(3.17±1.39)分,均处于中等以上水平。其中,3个量表中得分最低的维度分别为:护理人力资源充足(2.72±0.48)分、希望(4.33±0.72)分以及奉献(2.96±1.61)分。护理实践环境和心理资本都能够正向预测男护士的工作投入,男护士心理资本在护理实践环境和工作投入之间起完全中介作用。结论改善护理实践环境能够通过增加男护士的心理资本来增加其工作投入。护理管理者可以通过提供合理的工作量,和男护士一起制订工作目标等途径来增加他们的希望水平。
