The incidence of in-hospital cardiac arrest (IHCA) has increased over the past decade,with more than half occurring in intensive care units (ICUs).^([1])ICU cardiac arrest (ICU-CA)presents unique challenges,with worse outcomes than those in monitored wards,highlighting the need for early detection and intervention.^([2])Up to 80%of patients exhibit signs of deterioration hours before IHCA.^([3])Although early warning scores based on vital signs are useful,their eff ectiveness in ICUs is limited due to abnormal physiological parameters.^([4])laboratory markers,such as sodium,potassium,and lactate,are predictive of poor outcomes,^([5])but static measurements may not capture the patient’s *** in laboratory indicators,such as variability and extremes,may offer better predictive value.^([6])This study aimed to evaluate ICU-CA predictive factors,with a focus on vital signs and trends of laboratory indicators.

During cardiac arrest (CA),severe ischemia and hypoxia occur in tissues and organs of the entire body,inflammatory cytokines are released,and ischemiareperfusion injury occurs after the return of spontaneous circulation (ROSC),leading to multiple organ dysfunction in the body;this condition is called post-CA syndrome(PCAS).^([1])According to the BASeline Investigation of Out-of-Hospital Cardiac Arrest (BASIC-OHCA) study,the crude incidence of emergency medical service (EMS)-assessed OHCA was 95.7 per 100,000 individuals,and only1.2%of those individuals survived to hospital discharge or30 d after being resuscitated by the EMS in China.

Pinelliae Rhizoma(PR), known as Banxia in Chinese, Hange in Japanese, and Banha in Korean,is a renowned herbal medicine in East Asia derived from the dry tuber of Pinellia ternata(Thunb.) Breit.(PT). It is extensively utilized in dispensing granules, classical prescriptions,and herbal formulas to treat various conditions, including cough, infection, phlegm, nausea,asthma, and inflammation. Despite numerous studies on PR and its classical prescriptions over recent decades, a comprehensive synthesis of available evidence regarding its multifunctional roles and therapeutic potential is lacking. This review aims to address this gap by examining emerging evidence from metabonomics, preclinical studies, and clinical trials, while exploring potential trends and prospects for future research. A systematic literature search was conducted across six electronic databases, including Pub Med, Web of Science, Scopus,Science Direct, Wanfang, and China National Knowledge Infrastructure, to identify relevant articles on PR published until March 2023. PR contains 107 compounds with diverse pharmacological activities, including anti-inflammatory, immune regulatory, anti-viral, anti-cancer,anti-asthma, antitussive and expectorant, antioxidant, anti-obesity, anti-atherosclerosis, antimicrobial, emetic and anti-emetic, anti-convulsant and anti-epileptic, sedative and hypnotic,learning and memory enhancement, and anti-depressant effects. Metabonomic studies suggest that raw PR may exhibit cardiotoxicity and pregnancy toxicity while showing no apparent hepatorenal toxicity. However, limited pharmacokinetic investigations on PR constrain its clinical translation. Furthermore, clinical safety data on PR is scarce, with only four clinical trials assessing its positive effects in pediatric epilepsy, nausea and vomiting, soft tissue injury, and chronic sinus tract. We rethink the segment anything model(SAM) and propose a novel multiprompt network called COMPrompter for camouflaged object detection(COD). SAM has zero-shot generalization ability beyond other models and can provide an ideal framework for COD. Our network aims to enhance the single prompt strategy in SAM to a multiprompt strategy. To achieve this, we propose an edge gradient extraction module, which generates a mask containing gradient information regarding the boundaries of camouflaged objects. This gradient mask is then used as a novel boundary prompt, enhancing the segmentation process. Thereafter, we design a box-boundary mutual guidance module, which fosters more precise and comprehensive feature extraction via mutual guidance between a boundary prompt and a box prompt. This collaboration enhances the model's ability to accurately detect camouflaged objects. Moreover, we employ the discrete wavelet transform to extract high-frequency features from image embeddings. The high-frequency features serve as a supplementary component to the multiprompt ***, our COMPrompter guides the network to achieve enhanced segmentation results, thereby advancing the development of SAM in terms of COD. Experimental results across COD benchmarks demonstrate that COMPrompter achieves a cutting-edge performance, surpassing the current leading model by an average positive metric of 2.2% in COD10K. In the specific application of COD, the experimental results in polyp segmentation show that our model is superior to top-tier methods as well. The CRISPR RNA(crRNA)consists of a conserved repeat RNA(rRNA)and an alterable spacer RNA(sRNA),which can guide the Cas12a effector to recognize and target DNA molecules of interest in both full-length and split *** herein demonstrated the split crRNA can be repurposed for RNA detection through serving the sRNA as an RNA *** by this phenomenon,we developed a Cas12a-based direct RNA detection method,known as split crRNA-motivated amplification-free RNA testing(SMART).We adopted SMART to detect both short-stranded and long-stranded RNA target using two Cas12a orthologs(LbCas12a and FnCas12a),and it showed more prominent ability in detecting short-stranded RNA than long-stranded *** potential mechanism revealed that RNA overhangs impede the RNA strand and dsDNA activator from accessing the catalytic site in the RuvC domain of the Cas12a effector,compromising the stability of the quaternary complex,and thus reducing the efficiency of ***,by simply introducing a short DNA activator,SMART could detect attomolar miRNA targets and femtomolar long-stranded RNA target without the need for additional preamplification or reverse transcription *** addition,SMART showed wonderful discrimination ability toward single-nucleotide ***,the collaboration of SMART with the CRISPR–Cas13a system enabled simultaneous detection of multiplex ***,SMART is a simple,yet potent tool that can be flexibly applied to various short-stranded RNA detection,and holds great potential to be extended to other Cas12 orthologs.

Stem-leaf saponins from Panax notoginseng(SLSP) comprise numerous PPD-type saponins with diverse pharmacological properties; however, their role in Parkinson's disease(PD),characterized by microglia-mediated neuroinflammation, remains unclear. This study evaluated the effects of SLSP on suppressing microglia-driven neuroinflammation in experimental PD models, including the 1-methyl-4-phenylpyridinium(MPTP)-induced mouse model and lipopolysaccharide(LPS)-stimulated BV-2 microglia. Our findings revealed that SLSP mitigated behavioral impairments and excessive microglial activation in models of PD, including MPTP-treated mice. Additionally, SLSP inhibited the upregulation of inducible nitric oxide synthase(i NOS) and cyclooxygenase-2(COX2) and attenuated the phosphorylation of PI3K,protein kinase B(AKT), nuclear factor-κB(NFκB), and inhibitor of NFκB protein α(IκBα) both in vivo and in vitro. Moreover, SLSP suppressed the production of inflammatory markers such as interleukin(IL)-1β, IL-6, and tumor necrosis factor alpha(TNF-α) in LPS-stimulated BV-2cells. Notably, the P2Y2R agonist partially reversed the inhibitory effects of SLSP in LPStreated BV-2 cells. These results suggest that SLSP inhibit microglia-mediated neuroinflammation in experimental PD models, likely through the P2Y2R/PI3K/AKT/NFκB signaling pathway. These novel findings indicate that SLSP may offer therapeutic potential for PD by attenuating microglia-mediated neuroinflammation.

The activation of the sirtuin1(SIRT1)/nuclear factor erythroid 2-related factor 2(Nrf2)/heme oxygenase 1(HO-1) pathway has been shown to mitigate oxidative stress-induced apoptosis and mitochondrial damage by reducing reactive oxygen species(ROS) levels. Clinical trials have demonstrated that Zhongfeng Xingnao Liquid(ZFXN) ameliorates post-stroke cognitive impairment(PSCI). However, the underlying mechanism, particularly whether it involves protecting mitochondria and inhibiting apoptosis through the SIRT1/Nrf2/HO-1 pathway, remains unclear. This study employed an oxygen-glucose deprivation(OGD) cell model using SHSY5Y cells and induced PSCI in rats through modified bilateral carotid artery ligation(2VO).The effects of ZFXN on learning and memory, neuroprotective activity, mitochondrial function, oxidative stress, and the SIRT1/Nrf2/HO-1 pathway were evaluated both in vivo and in vitro. Results indicated that ZFXN significantly increased the B-cell lymphoma 2(Bcl2)/Bcl2-associated X(Bax) ratio, reduced terminal deoxynucleotidyl transferase-mediated d UTP nickend-labeling(TUNEL)+ cells, and markedly improved cognition, synaptic plasticity, and neuronal function in the hippocampus and cortex. Furthermore, ZFXN exhibited potent antioxidant activity, evidenced by decreased ROS and malondialdehyde(MDA) content and increased superoxide dismutase(SOD), catalase(CAT), and glutathione(GSH) levels. ZFXN also demonstrated considerable enhancement of mitochondrial membrane potential(MMP), Tom 20 fluorescence intensity, adenosine triphosphate(ATP) and energy charge(EC) levels, and mitochondrial complex Ⅰ and Ⅲ activity, thereby inhibiting mitochondrial damage. Additionally,ZFXN significantly increased SIRT1 activity and elevated SIRT1, nuclear Nrf2, and HO-1 ***, these effects were substantially counteracted when SIRT1 was suppressed by the inhibitor EX-527 in vitro.
