AIM:To investigate all patients referred to our center with non-responsive celiac disease (NRCD),to establish a cause for their continued ***:We assessed all patients referred to our center with non-responsive celiac disease over an 18-mo *** individuals were investigated to establish the eitiology of their continued *** patients were first seen in clinic where a thorough history and examination were performed with routine blood work including tissue transglutaminase antibody *** were also referred to a specialist gastroenterology dietician to try to identift any lapses in the diet and sources of hidden gluten ingestion.A repeat small intestinal biopsy was also performed and compared to biopsies from the referring hospital where ***,lactulose hydrogen breath testing,pancreolauryl testing and computed tomography scan of the abdomen were undertaken if the symptoms *** clinical progress was followed over a minimum of 2 ***:One hundred and twelve consecutive patients were referred with *** were found not to have celiac disease (CD).Of the remaining 100 patients,45% were not adequately adhering to a strict gluten-free diet,with 24 (53%) found to be inadvertently ingesting gluten,and 21 (47%) admitting *** colitis was diagnosed in 12% and small bowel bacterial overgrowth in 9%.Refractory CD was diagnosed in 9%.Three of these were diagnosed with intestinal *** 2 years,78 patients remained well,eight had continuing symptoms,and four had ***:In individuals with NRCD,a remediable cause can be found in 90%:with continued gluten ingestion as the leading *** propose an algorithm for investigation.

The left atrium(LA) acts as a modulator of left ventricular(LV) filling. Although there is considerable evidence to support the use of LA maximum and minimum volumes for disease prediction,theoretical considerations and a growing body of literature suggest to focus on the quantification of the three basic LA functions:(1) Reservoir function:collection of pulmonary venous return during LV systole;(2) Conduit function:passage of blood to the left ventricle during early LV diastole; and(3) Contractile booster pump function(augmentation of ventricular filling during late LV diastole. Tremendous advances in our ability to non-invasively characterize all three elements of atrial function include speckle tracking echocardiography(STE),and more recently cardiovascular magnetic resonance myocardial feature tracking(CMR-FT). Corresponding imaging biomarkers are increasingly recognized to have incremental roles in determining prognosis and risk stratification in cardiac dysfunction of different origins. The current editorial introduces the role of STE and CMR-FT for the functional assessment of LA deformation as determined by strain and strain rate imaging and provides an outlook of how this exciting field may develop in the future.

Inflammatory bowel diseases are characterised by inflammation that compromises the integrity of the epithelial barrier. The intestinal epithelium is not only a static barrier but has evolved complex mechanisms to control and regulate bacterial interactions with the mucosal surface. Apical tight junction proteins are critical in the maintenance of epithelial barrier function and control of paracellular permeability. The characterisation of alterations in tight junction proteins as key players in epithelial barrier function in inflammatory bowel diseases is rapidly enhancing our understanding of critical mechanisms in disease pathogenesis as well as novel therapeutic opportunities. Here we give an overview of recent literature focusing on the role of tight junction proteins, in particular claudins, in inflammatory bowel diseases and inflammatory bowel disease associated colorectal cancer.

Background Vascular smooth muscle cells （VSMCs） can express heme-oxygenase （HO）, a rate-limiting enzyme in the degradation of heme to bilirubin, ferritin and carbon monoxide （ CO）. VSMC-derived CO can suppress VSMC proliferation and may serve as an antiproliferation factor. The promoter region of HO-1 shows a polymorphism with different （GT）n repeats that has been reported to differently induce gene expression. The objective of this study was to examine the effect of this variation on the occurrence of restenosis after in-stent treatment in patients with coronary artery disease. Methods Candidates who underwent coronary stent implantation were genotyped for the HO-1 promoter polymorphism using polymerase chain reaction （PCR） and automated DNA capillary sequencer. Serum levels of IL-6 and C-reactive protein （CRP） were obtained at baseline, 24 hours and 48 hours after stenting. The primary end point for the study was angiographic evidence of in-stent restenosis at 6 months. All parameters for evaluation of restenosis were analysed by quantitatve computer-assisted angiographic analysis （QCA）. Results One hundred and eighty-seven patients who underwent coronary stent implantation were studied of whom 27.8% showed 〉150% restenosis after 6 months. The distribution of （GT）n repeats of all patients in the promoter region of HO-1 genotype ranged from 22 to 42, with （GT） 25 and （GT） 32 being the two most common alleles. The allelic repeats were divided into the short class （S） with 29 （ GT）., the middle class （M） with 30- 37 （GT）n and the long class （L） with 38 （GT）n. There was no significant difference in the restenosis between the genotype groups or between post operation levels of inflammation markers, but carriers of the S allele （ n = 120） had 33.3% lower baseline IL-6 compared with non-S carriers （n =67, P =0. 0008）.
