Analysis of electrocautery smoke released from the tissues frequently cut in orthopedic surgeries

BACKGROUND electrosurgical smoke could be different by the device of cutting or the type of tissue that is being *** To analyze the electrocautery smoke released from the tissues that are frequently cut in orthopedic *** The released smoke from electrocautery of five different tissue types(meniscus,ligament,adipose,muscle,and synovium)of five patients who underwent total knee arthroplasty were collected and analyzed for volatile organic compounds(VOCs)and 27 candidate polycyclic aromatic hydrocarbons(n=25).Surgical smoke was produced with an electrocautery device for 4 *** None of the 27 evaluated polycyclic aromatic hydrocarbons compounds were detectable in electrocautery smoke collected from the surgical cutting of the different *** number and identity of detected VOCs were similar between the patients but not between tissue *** number of detected VOCs was the highest in synovial tissue(n=21)and the lowest in the meniscus and adipose tissue(n=12).The number of toxic and/or carcinogenic VOCs were the most in the muscle and meniscus tissues(Toluene,ethylbenzene,and Styrene).No toxic and/or carcinogenic VOCs were identified in the ligament and adipose *** Meniscus and muscle tissue are associated with the highest number of toxic and/or carcinogenic ***,we recommend that surgeons avoiding the electrocautery of these tissues.

Spontaneous wound dehiscence after penetrating keratoplasty

Spontaneous wound separation may be developed even months after suture removal especially in the context of long-term corticosteroid therapy. A 68-yearold Caucasian woman presented to our cornea clinic with spontaneous wound dehiscence after her third penetrating keratoplasty(PKP) which was performed three years ago. An Ahmed glaucoma valve(New World Medical, Ranchos Cucamonga, CA) was inserted ten months after the third PKP, which successfully controlled intraocular pressure(IOP). At the examination, the last sutures were removed eight months ago and she was using flourometholone 0.1%(Sina Darou, Tehran, Iran)with a dose of once a day. There was one quadrant of wound dehiscence from 8 to 11 o`clock associated with anterior wound gape and severe corneal *** was performed for the patient. At the one month examination, the corneal edema was resolved and best corrected visual acuity was 20/200 mainly due to previous glaucomatous optic neuropathy. Caution about the prolonged use of corticosteroids is necessary. Topical immunosuppressives could be a promising choice in this field.

Ocular higher-order aberrations and mesopic pupil size in individuals screened for refractive surgery

AIM: To study the distribution of ocular higher-order aberrations(HOAs) and mesopic pupil size in individuals screened for refractive surgery. · MeTHODS: Ocular HOAs and mesopic pupil size were studied in 2 458 eyes of 1 240 patients with myopia, myopic astigmatism and compound myopic astigmatism and 215 eyes of 110 patients with hyperopia, hyperopic astigmatism and compound hyperopic astigmatism using the Zywave aberrometer (Busch& Lomb). All patients had correctable refractive errors without a history of refractive surgery or underlying diseases. Root-mean-square values of HOAs, total spherical aberration, total coma and mesopic pupil size were analyzed. Ocular HOAs were measured across a ≥ 6.0 mm pupil, and pupil size measurements were performed under the mesopic condition. · ReSULTS: The mean values of HOAs, total spherical aberration and total coma in the myopic group were 0.369 μm, ±0.233, 0.133±0.112μm and 0.330±0.188μm, respectively. In the hyperopic group the mean values of HOAs, total spherical aberration and total coma were 0.418μm±0.214, 0.202±0.209μm and 0.343±0.201μm, respectively. Hyperopes showed greater total HOAs (P erical aberration (P ed to myopes. In age-matched analysis, only the amount of total spherical aberration was higher in the hyperopic group (P =0.05). Mesopic pupil size in the myopic group was larger (P ≤0.05). · CONCLUSION: The results suggested that significant levels of HOAs were found in both groups which are important for planning refractive surgeries on Iranians. There were significantly higher levels of total spherical aberration in hyperopes compared to myopes. Mesopic pupil size was larger in myopic group.

Selective laser trabeculoplasty in patients with pseudoexfoliative glaucoma vs primary open angle glaucoma： a one-year comparative study

AIM：To compare the efficacy of single-session 360-degree selective laser trabeculoplasty（SLT） for reduction of intraocular pressure（IOP） in patients with pseudoexfoliative glaucoma（PXFG） and primary open angle glaucoma（POAG）.· MeTHODS：This is a single-center,prospective,nonrandomized comparative *** older than 18 years of age with uncontrolled PXFG or POAG eyes requiring additional therapy while on maximally tolerated IOP-lowering medications were *** primary outcome measure changed in IOP from *** was defined as IOP reduction ≥20%from baseline without any additional IOP-lowering *** patients were examined at 1d,1wk,1,3,6,9,12 mo after SLT.· ReSULTS：Nineteen patients（20 eyes） with PXFG and27 patients（28 eyes） with POAG were included in the *** the visual fields mean deviation was-2.88（±1.67）in the POAG and-3.1（±1.69） in the PXFG groups（P=0.3）.The mean（±SD） IOP was 22.9（±3.7） mm Hg in the POAG group and 25.7（±4.4） mm Hg in the PXFG group at baseline and decreased to 18.4（±3.2） and 18.0（±3.9） mm Hg in the POAG group（P〈0.001 and P=0.02）,and to 17.9（±4.0） and 21.0（±6.6） mm Hg in the PXFG group（P〈0.001 and P=0.47） at 6 and 12 mo,*** number of medications was 2.6（±0.8） in the POAG group and 2.5（±0.8） in the PXFG group at baseline,and did not change at all follow-up visits in both groups（P =0.16 in POAG and 0.57 in PXFG）.Based on Kaplan-Meier survival analysis,the success rate was 75%in the POAG group compared to 94.1%in the PXFG group（P=0.08;Log-rank test） at 6mo,and 29.1%and 25.0%at 12 mo,respectively（P=0.9;Log-rank）.· CONCLUSION：The 360-degree SLT is an effective and well-tolerated therapeutic modality in patients with POAG and PXFG by reducing IOP without any change in number of *** response was more pronounced early in the postoperative period in patients with PXFG whereas there was no statistically significant difference at 12-month follow-up. 〈/tr〉

How reliable is the lacrimal scintigraphy report? An interobserver agreement and reliability study

AIM: To assess the inter-observer agreement and reliability as well as intra-observer repeatability for lacrimal scintigraphy(LS) reports with and without considering the irrigation test ***: A prospective, observational, cross sectional study. Two masked clinicians(lacrimal surgeon and nuclear medicine specialist) independently reported 100 LS images(50 patients of >6 years of age with unilateral anophthalmic socket) in a university hospital. The lacrimal surgeon performed a diagnostic irrigation test and repeated the report of the same LS images 2 y after the first report(intra-observer agreement). A weighted Kappa analysis was performed to determine inter-observer agreement and reliability as well as intra-observer repeatability for the type(normal, partial and complete obstruction) and location(presac, preduct, and intraduct) of the obstruction. Subgroup analysis was also performed with consideration of irrigation test results. ReSULTS: A significantly moderate agreement was found between lacrimal surgeon and nuclear medicine specialist for both the type(Kappa=0.55) and location(Kappa=0.48) of obstruction. Agreement values were higher for the type(Kappa=0.61 vs 0.41) and location(Kappa=0.56 vs 0.31) of obstruction in cases with normal than abnormal irrigation test. Strong and significant intraobserver(lacrimal surgeon) repeatability was found for both the type(Kappa=0.66) and location(Kappa=0.69) of obstruction. LS showed no to slight reliability based on irrigation ***: A moderate agreement is found between lacrimal surgeon and nuclear medicine specialist regarding the interpretation of LS suggesting the importance ofconsensus groups among nuclear medicine specialists and lacrimal surgeons to create a common language for interpretation of LS. Intra-observer repeatability is strong for the lacrimal surgeon.

Comparison of anterior segment measurements using rotating Scheimpflug imaging and partial coherence interferometry

AIM:To compare central corneal thickness(CCT) and anterior chamber depth(ACD) measurements using rotating Scheimpflug imaging and partial coherence interferometry.·MeTHODS:As part of the first phase of Shahroud eye Cohort Study with 5 190 subjects of 40 to 64 years of age,CCT and ACD were measured using Scheimpflug imaging with the Pentacam(Oculus,Inc.,Lynnwood,WA,USA) and partial coherence interferometry with the Allegro BioGraph(Wavelight,erlangen,Germany).·ReSULTS:After applying exclusion criteria,we had data of 4 387 subjects with a mean age of 50.7±6.2 *** CCT with Pentacam and BioGraph were 528.6 ±33.2μm and 525.6 ±32μm respectively;the difference was statistically significant(P e correlation was high(R=0.920).Mean ACD measurements using Pentacam and BioGraph were 2.68 ±0.35mm and 2.62 ±0.33mm respectively;the inter-device difference was significant(P elation(R=0.944).The 95% limits of agreements between devices were-22.65μm to28.61μm and-0.16mm to 0.29mm for CCT and ACD measurements,respectively.·CONCLUSION:For both CCT and ACD,the BioGraph gave significantly lower values than the Pentacam(P espite the high inter-device correlation,the 95%limits of agreements were wide,and this may limit their interchangeability in measuring the CCT and ACD.

Overexpression of low-density lipoprotein receptor prevents neurotoxic polarization of astrocytes via inhibiting NLRP3 inflammasome activation in experimental ischemic stroke

Neurotoxic astrocytes are a promising therapeutic target for the attenuation of cerebral ischemia/reperfusion ***-density lipoprotein receptor,a classic cholesterol regulatory receptor,has been found to inhibit NLR family pyrin domain containing protein 3(NLRP3)inflammasome activation in neurons following ischemic stroke and to suppress the activation of microglia and astrocytes in individuals with Alzheimer’s ***,little is known about the effects of low-density lipoprotein receptor on astrocytic activation in ischemic *** address this issue in the present study,we examined the mechanisms by which low-density lipoprotein receptor regulates astrocytic polarization in ischemic stroke ***,we examined low-density lipoprotein receptor expression in astrocytes via immunofluorescence staining and western blotting *** observed significant downregulation of low-density lipoprotein receptor following middle cerebral artery occlusion reperfusion and oxygen-glucose deprivation/***,we induced the astrocyte-specific overexpression of low-density lipoprotein receptor using astrocyte-specific adeno-associated ***-density lipoprotein receptor overexpression in astrocytes improved neurological outcomes in middle cerebral artery occlusion mice and reversed neurotoxic astrocytes to create a neuroprotective ***,we found that the overexpression of low-density lipoprotein receptor inhibited NLRP3 inflammasome activation in oxygen-glucose deprivation/reoxygenation injured astrocytes and that the addition of nigericin,an NLRP3 agonist,restored the neurotoxic astrocyte *** findings suggest that low-density lipoprotein receptor could inhibit the NLRP3-meidiated neurotoxic polarization of astrocytes and that increasing low-density lipoprotein receptor in astrocytes might represent a novel strategy for treating cerebral ischemic stroke.
