Topographic factors associated with anterior chamber angle narrowing in patients with keratoconus

AIM:To identify topographic determinants of the anterior chamber angle(ACA)in patients with keratoconus(KCN).METHODS:Four hundred and ten eyes of 294 patients with KCN were recruited for this ***,complete ocular examinations were performed for all patients,including visual acuity measurement,refraction,and slit-lamp ***,all participants underwent corneal imaging by the Oculus Pentacam ***:The mean age of the participants was 32.40±8.52y(15-60y)and 69.5%of them were *** mean ACA was 38.47°±5.75°(range:14.40°to 56.50°)in the whole sample,38.24°±6.00°in males,and 38.98°±5.11°in females(P=0.447).The mean ACA was significantly different among different groups of cone morphology,as patients with nipple cones showed the lowest mean ***,there were statistically significant differences in the mean ACA among different groups of cone locations,with patients having central cones exhibiting the lowest mean ACA(P<0.001).Anterior and posterior Q values were significantly,directly correlated with ACA(anterior Q:r=0.122,P=0.014,posterior Q:r=0.192,P<0.001).CONCLUSION:This study provides critical insights into the risk factors for ACA narrowing in KCN patients,which is essential for planning intraocular *** with nipple and central cones exhibited the most significant ACA ***,more negative Q-values are associated with increased ACA narrowing,highlighting the need for targeted diagnostic and therapeutic strategies.

Biomechanical study of the visual system by ocular response analyzer investigates the inter-structural biological relationships,mechanics,and function of the visual *** review aimed to investigate the changes in corneal biomechanical parameters with age and *** articles published in PubMed between 2000 and 2021 were investigated and critiqued,and valid scientific evidence was collected,reviewed and concluded according to the inclusion and exclusion *** studies showed that corneal biomechanical changes occur infrequently in children up to the age of 20y,and with increasing age and wider age range,there was a significant decrease in corneal biomechanical indices,especially corneal *** children and adults,most studies have shown that these biomechanical indicators,especially corneal resistance factor,were higher in *** hormonal changes may contribute to this finding,the role of axial length and other biometric indicators should not be *** axial length,the intraocular pressure,and the corneal thickness are other factors associated with biomechanical parameters that should be taken into account in clinical diagnosis and management especially for patients undergoing refractive surgery as well as keratoconus patients.

Objective:To explore the perspectives of high-risk pregnant women regarding the home care program for high-risk ***:This qualitative study was conducted in a university hospital and 5 comprehensive health centers in Ahvaz,Iran,from February 2023 to July *** sampling method used was purposive sampling considering the maximum possible diversity,which continued until data saturation.11 in-depth and semistructured interviews were conducted with high-risk pregnant *** were simultaneously analysed using Graneheim and Lundman content analysis by MAXQDA version 20 ***:Data analysis extracted 3 themes,12 categories,and 26 *** themes and categories were“high-risk pregnancy and related issues(pursuing treatment and paying attention to the high-risk situation by pregnant women,psychological aspect of high-risk pregnancy on pregnant women and caregivers,emotional and physical aspects of high-risk pregnancy,the consequences of high-risk pregnancy on the people around them and empowerment of high-risk pregnant women)”,“support dimension(family and community support for high-risk pregnant women)”,and“framework of program(acculturalization,means and equipment,security,manpower and the necessary arrangements for the home care)”.Conclusions:High-risk pregnancy affects various aspects of the lives of high-risk pregnant women and their *** the necessary conditions for providing care at home plays a vital role in supporting high-risk pregnant women and removing related obstacles to receiving prenatal care in *** will be especially beneficial for high-risk pregnant women in low-income areas.

An investigation into the physical assessment abilities of critical care nurses in an internationally recognized hospital

Objective: Physical assessment is an indispensable and pivotal skill that nurses must aptly monitor, evaluate, and deliver timely care, particularly in the context of critically ill patients. However, studies have revealed instances where nurses demonstrate inaccurate practices. This study is aimed to measure the physical assessment skills of critical care nurses and the differences between levels of nursing education. Methods: A descriptive cross-sectional study was conducted involving a simple random sample of 101 nurses employed in critical care units. Results: Of the 101 participants, 56.4% were identified as female and 43.6% were male. In the sample, 71.0% held a diploma in nursing, while 29.0% held a Bachelor of Science in nursing (BSN). Vital signs, orientation, and skin assessments were performed correctly in ≥90% of cases, whereas the majority (≥60%) did not correctly assess neck veins, Homan’s signs, breath sounds, heart sounds, and bowel sounds. Only ≤10% of participants accurately executed these assessments. Significant associations were found between sex and pain (P=0.034), as well as between the type of education and closure of the health assessment (P=0.041). Further, female nurses scored 10.0 ± 2.3, and male nurses 8.5 ± 2.5, diploma holders scored 9.4 ± 1.8, and BSN holders scored 9.3 ± 2.7 at the global rating. Conclusions: The presence of an existing gap in the accurate assessment of patients poses a potential risk to patient care. It is essential to investigate the factors contributing to this gap and to develop strategies to address it, thereby enhancing the quality of physical assessment and ultimately improving patient care.

Objective:The purpose of this study was to develop and psychometrically analyze the characteristics of the good nurse from the perspective of service ***:The current research was conducted namely in 2 qualitative and psychometric *** the first stage,the items of the questionnaire were designed by interviews with nursing service recipients,and in the next stage,construct validity,content validity,face validity,reliability,and internal consistency were all ***:Based on the results of the content analysis of the interview with nursing service recipients and related literature review,an 80-item questionnaire was *** analyzing its content validity,40 items were validated and 40 were eliminated,which were prepared by using exploratory factor analysis,6 factors of attentive and communicative,patron and companionable,complete caregiver,safer and skillful,advocate and *** reliability of the tool was assessed by applying Cronbach’s alpha method of ***:“The good nurse characteristics tool”could be used to assess the service receivers’point of view and provide useful information regarding nurses’performance to managers and nurses and will provide the basis for improving the quality of nursing services.

Introduction:Alzheimer 's disease(AD) is a common neurodegenerative disorder and the primary cause of dementia. Considerable evidence supports the “amyloid hypothesis,” stating that the pathogenesis of AD is primarily caused by the deposition of amyloid-β(Aβ), which drives tau phosphorylation, neuroinflammation, and neurodegeneration in the brain. The amyloid hypothesis is strengthened by the significant and moderate benefit of lecanemab, a humanized antibody through an anti-amyloid mechanism,showing slowed clinical decline(van Dyck et al.,2023). The recent positive results of anti-amyloid trials have brought back focus on the amyloid hypothesis through biochemical, genetic, and pharmacological approaches(Zhang, 2023).

Practice-based research networks (PBRN) seek to improve healthcare through the use of research, quality improvement, and collaborative learning. When used by nontraditional models of care such as the nurse managed healthcare center (NMHC), PBRNs can be incorporated into successful quality improvement (QI) programs. UT Health Services is a NMHC utilizing a PBRN as one component of a comprehensive QI program in an effort to deliver high quality healthcare.
