surgery and radiation h.ve both.been sh.wn to increase th. longterm diseasespecific survival rate for men with.din icaUy localized prostate cancer. Alth.ugh.both.modalities h.ve demonstrated favor able effects on cancer control, questions regarding quality of life （QoL） and func tional outcomes remain incompletely answered. To date, no randomized prospective trials h.ve been performed comparing th. two treatment modalities and so indirect compar isons of longterm functional outcomes h.ve served as a substitute to aid in patient coun seling and decisionmaking. As th.re is a pau city of longterm data comparing functional outcomes after radical prostatectomy and external beam radiation th.rapy, a recent art icle by Resnick et al.,1 h.s attempted to pro vide additional information about th.s topic in terms of continence, erectile function and bowel function. Utilizing th. Prostate Cancer Outcomes Study coh.rt, a populationbased coh.rt of men diagnosed with.prostate cancer in th. pro statespecific antigen （PSA） era, th. auth.rs compared rates of urinary incontinence, erect ile dysfunction, and bowel urgency at 2, 5 and 15 years after primary th.rapy. Th.y sh.wed th.t men undergoing prostatectomy h.d h.gh.r rates of incontinence and erectile dys function at 2 and 5 years, but th.se rates were similar to th.se in th. radioth.rapy group at 15 years. More specifically, men were approxi mately five times more likely to h.ve urinary continence issues if th.y underwent prostatec tomv versus radioth.rapy and almost th.ee anda h.lf times more likely to develop erectile dys function in th. sh.rt to intermediateterm following primary treatment. As expected, rates of bowel urgency were h.gh.r in th. radio th.rapy group at 2 and 5 years, but not sig nificantly different from th. surgery group at 15 years. Th. tumor microenvironment(TME) is complex and constantly evolving. Th.s is due, in part, to th. crosstalk between tumor cells and th. multiple cell types th.t comprise th. TME, wh.ch.results in a h.terogeneous population of tumor cells and TME cells. Th.s review will focus on two stromal cell types, th. cancerassociated adipocyte(CAA) and th. cancer-associated fibroblast(CAF). In th. clinic, th. presence of CAAs and CAFs in th. TME translates to poor prognosis in multiple tumor types. CAAs and CAFs h.ve an activated ph.notype and produce growth.factors, inflammatory factors, cytokines, ch.mokines, extracellular matrix components, and proteases in an accelerated and aberrant fash.on. Th.ough.th.s activated state, CAAs and CAFs remodel th. TME, th.reby driving all aspects of tumor progression, including tumor growth.and survival, ch.moresistance, tumor vascularization, tumor invasion, and tumor cell metastasis. Similarities in th. tumorpromoting functions of CAAs and CAFs suggest th.t a multipronged th.rapeutic approach.may be necessary to ach.eve maximal impact on disease. Wh.le CAAs and CAFs are th.ugh. to arise from tissues adjacent to th. tumor, multiple alternative origins for CAAs and CAFs h.ve recently been identified. Recent studies from our lab and oth.rs suggest th.t th. h.matopoietic stem cell, th.ough.th. myeloid lineage, may serve as a progenitor for CAAs and CAFs. We h.poth.size th.t th. multiple origins of CAAs and CAFs may contribute to th. h.terogeneity seen in th. TME. Th.s, a better understanding of th. origin of CAAs and CAFs, h.w th.s origin impacts th.ir functions in th. TME, and th.temporal participation of uniquely originating TME cells may lead to novel or improved anti-tumor th.rapeutics.

Patients and ph.sicians understand th. importance of self-care following spinal cord injury (SCI), yet many individuals with.SCI do not adh.re to recommended self-care activities despite logistical supports. Neurobeh.vioral determinants of SCI self-care beh.vior, such.as impulsivity, are not widely studied, yet understanding th.m could inform efforts to improve SCI self-care. We explored associations between impulsivity and self-care in an observational study of 35 US adults age 18 - 50 wh. h.d traumatic SCI with.paraplegia at least six month. before assessment. Th. primary outcome measure was self-reported self-care. In LASSO regression models th.t included all neurobeh.vioral measures and demograph.cs as predictors of self-care, dispositional measures of greater impulsivity (negative urgency, lack of premeditation, lack of perseverance), and reduced mindfulness were associated with.reduced self-care. Outcome (magnitude) sensitivity, a latent decision-making parameter derived from computationally modeling successive ch.ices in a gambling task, was also associated with.self-care beh.vior. Th.se results are preliminary;more research.is needed to demonstrate th. utility of th.se findings in clinical settings. Information about associations between impulsivity and poor self-care in people with.SCI could guide th. development of interventions to improve SCI self-care and h.lp patients with.elevated risks related to self-care and secondary h.alth.conditions.
