Predictors for attending annual eye screening for diabetic retinopathy amongst patients with diabetes in an urban community of Beijing

AIM： To gain a better understanding of possible factors that may influence the decision of diabetes persons to participate in annual eye screening in an urban community setting of ***： A structured interview including questions on attendance of eye screening, knowledge and awareness of diabetic retinopathy was conducted. The presence and degree of retinopathy were assessed using two field nonmydriatic retinal photography. RESULTS： Totally 720 diabetes persons were recruited and 519 were enrolled in this cross-sectional study. In this urban setting of Beijing, among diabetes patients of average of 10 y duration, 77% confirmed having undergone at least one eye examination and 61% reported having at least one eye examination with dilated pupil. As for the last 12 mo, the number decreased to 210（47%） and 131（30%） separately. Most of the participants（95%） were aware that diabetes could affect their vision and that regular eye examination was necessary. Very few of them（12%） however were aware that the early stages of diabetic retinopathy presented without symptoms of vision loss. Having attended patient education on diabetes was effective in building awareness about diabetic eye disease and was a significant positive predictor for attending eye screening [education in a year, Adj. OR=0.47（0.29-0.74）, P〈0.001, education years ago, Adj. OR=0.56（0.33-0.96）, P=0.036]. The duration of disease also increasedthe likelihood of having undergone eye screening（Adj. OR=0.96, P〈0.05）. CONCLUSION： Being exposed to education about the complications of diabetes increases the probability of attending diabetic eye screening. An appropriate patient knowledge building strategy should be made available to patients from the time of diagnosis.

We measure and calculate the aerial image modulation （AIM） of human retina for visible wavelengths based on the individual eye model. By employing the optical design software ZEMAX, we obtain the modulation transfer function （MTF） of human eye in visible wavelengths. Using CSV-1000 and VAF-1000, the contrast sensitivity function （CSF） and the visual acuity （VA） for the same eye are measured. Then the AIM of human retina could be acquired by the relations between MTF and CSF and between MTF and VA. The AIM of human retina is independent of MTF, and the values of AIM for normal eyes （without retina disease） are similar, so the assembly average for large numbers of normal eyes can be a standard AIM curve, which is helpful for the diagnosis of diseases in the retina system.

Central serous chorioretinopathy （CSC） is characterized by a localized accumulation of subretinal fluid and an idiopathic focal leakage from choroidal vessels. The exact pathogenesis of CSC, however, still remains obscure. In this paper, we hypothesized that CSC may result from a response of choroidal vessels to an acute increase in the environmental light intensity leading to a focal leakage from the choroidal vessels. High levels of glucocorticoids, in our proposed model, may cause persistence rather than initiation of the focal leakage, probably by suppressing the synthesis of collagen and extracellular matrix components and inhibiting fibroblastic activity.

Comparison of complications after Ahmed versus Baerveldt implant in glaucoma patients:one year follow-up

AIM:To compare surgical results of the Ahmed and Baerveldt implant procedures in glaucoma patients at 1 y follow-up at Jakarta eye center(JEC)eye ***:This cohort retrospective study was conducted on glaucoma patients aged≥18 y who had undergone Ahmed and Baerveldt implant *** pressure(IOP),visual acuity,glaucoma medication,success rate,early and late postoperative complications,and the number of resurgeries were ***:A total of 351 eyes in the Ahmed group and 94 eyes in the Baerveldt group were included in this *** 1 y follow-up,the mean IOP was found to be significantly lower in the Baerveldt group(13±4.47 mm Hg)compared to the Ahmed group(15.02±5.73 mm Hg;P=0.025).Glaucoma medication was required in both the Ahmed and Baerveldt groups(58.92%vs 71.67%).Comparable success rate was found in both *** Ahmed group revealed a complete and qualified success of 86.82%,and failure of 13.17%.Similarly,the Baerveldt group showed complete and qualified success in 87.75%and failure in 12.25%*** the Ahmed group,11.97%early complications,26.06%late complications and 9.97%resurgeries were *** comparison,in the Baerveldt group,23.40%early complications,30.95%late complications and 11.70%resurgeries were ***:Both groups of glaucoma implants show significant IOP reduction,however,the Baerveldt implant group demonstrates greater IOP reduction with more failure rates and complications than the Ahmed implant group.

AIM:To evaluate the diagnostic ability of topographic and tomographic indices with Pentacam and Sirius as well as biomechanical parameters with Corvis ST for the detection of clinical and subclinical forms of keratoconus(KCN).METHODS:In this prospective diagnostic test study,70 patients with clinical KCN,79 patients with abnormal findings in topography and tomography maps with no evidence on clinical examination(subclinical KCN),and 68 normal control subjects were *** accuracy of topographic,tomographic,and biomechanical parameters was evaluated using the area under the receiver operating characteristic curve(AUC)and cross-validation *** Delong method was used for comparing ***:In distinguishing KCN from normal,all parameters showed statistically significant differences between the two groups(P0.05).CONCLUSION:In clinical KCN,all topographic,tomographic,and biomechanical indices have acceptable outcomes in terms of sensitivity and ***,in differentiating subclinical forms of KCN from normal corneas,curvature-based parameters(SIb and IS value)followed by integrated indices(PRFI and TBI)are the most powerful tools for early detection of KCN.

Millions of cataract surgeries with intraocular lens(IOL) implantation are performed worldwide. Although cataract surgery brings many benefits to the patients, the risk of various complications is still a concern. One of the infrequent adverse events but potentially affecting on patients’ visual acuity and contrast sensitivity is losing the transparency of IOL. IOL opacification may lead to IOL removal or exchange, which is unpleasant to both the patient and the surgeon. Several reports of acute IOL clouding are available in the literature describing various etiologies of this phenomenon, however, the exact mechanism remained unclear in some cases. Herein, we aimed to review the causes and outcomes of intraoperative and early postoperative IOL opacification.
