Evolutionary relationship of 5′-untranslated regions among Thai dengue-3 viruses,Bangkok isolates,during 24 year-evolution

Objective:To study evolutionary relationship of the 5'untranslated regions(5'UTRs) in low passage dengue3 viruses(DEN3) isolated from hospitalized children with different clinical manifestations in Bangkok during 24 year-evolution(1977-2000) comparing to the DEN3prototype(H87).Methods:The 5'UTRs of these Thai DEN3 and the H87 prototype were amplified by RT-PCR and *** multiple sequence alignments were done by Codon Code Aligner v 4.0.4 software and their RNA secondary structures were predicted by MFOLD *** of five Thai DEN3 candidates comparing to the 1187 prototype were done in human(HepG2) and the mosquito(C6/36) cell ***:Among these Thai DEN3,the completely identical sequences of their first 89 nucleotides,their high-order secondary structure of 5'UTRs and three SNPs including the predominant C90 T,and two minor SNPs including A109 G and A112 G were *** C90 T of Thai *** isolates was shown predominantly before *** Thai DEN3 candidates with the predominant C90 T were shown to replicate in human(HepG2) and the mosquito(C6/36) cell lines better than the H87 ***,their highly conserved sequences as well as SNPs of the 5'UTR did not appear to correlate with their disease severity in ***:Our findings highlighted evolutionary relationship of the completely identical 89 nucleotide sequence,the high-order secondary structure and the predominant C90 T of the 5'UTR of these Thai DEN3 during 24 year-evolution further suggesting to be their genetic markers and magic targets for future research on antiviral therapy as well as vaccine approaches of Thai DEN3.

Outcomes of Transcatheter Closure in Outlet-Type Ventricular Septal Defect after 1 Year

Background:Ventricular septal defect(VSD)is the most common congenital heart *** VSD closure is an effective treatment for patients with muscular and perimembranous ***,there is a limit data for outlet VSD,especially impact to the aortic valve leaflet after transcatheter *** study aims to assess the outcomes of transcatheter closure of the outlet-type ventricular septal defect(OVSD)after 1 postoperative ***:A retrospective study was performed including 50 patients who underwent transcatheter(n=25)and surgical(n=25)OVSD closure during the exact time frame at two medical ***:The median age and body weight of patients in the transcatheter group were significantly higher than those of patients in the surgical group(7.0 vs.2.8 years;27.0 vs.11.4 kg;p<0.01).The defect size in the surgical group was significantly larger than that in the transcatheter group(5.0 vs.3.0 mm;p<0.01).All OVSD patients have successful transcatheter closure(100%)as effective as surgical *** than small residual shunt was present 20%and 8%immediately after the procedure in the transcatheter and surgical groups(p=0.50),which decreased to 12%and 4%at the 1-year follow-up(p=0.61),*** incidence of complete atrioventricular block and other complications was observed in both groups,and no significant differences were noted in the new onset or worsening of the aortic regurgitation in both groups(p=1.0).Conclusions:Transcatheter treatment could be effectively and safely achieved for OVSD closure at 1-year follow-up.

Field Evaluation of a Transport Medium and Enrichment Broth for Isolation of Campylobacter Species from Human Diarrheal Stool Samples

Campylobacter continues to be a major cause of bacteriamediated diarrheal diseases, both for Thai citizens and travelers to Thailand. For field epidemiological studies, appropriate methods for storage, intralaboratory transport of patients specimens and use of enrichment culture to isolate this organism is critical. Study A, represents patient stool specimens collected in Bangkok and processed for Campylobacter culture within three hours after collection. Study B, represents stool specimens collected from patients in northeast and Southern regions of Thailand in modified CaryBlair transport medium. These specimens were transported and processed for Campylobacter in Bangkok at varying intervals ranging from 1 to 7 days. Of 900 diarrheal samples examined in study A, a total of 158 were Campylobacter positive through culture. Of these, 145 and 141 isolates were cultured by direct plating and enrichment plating respectively (P = 0.5839). From 1,168 diarrheal stool samples examined in study B, 184 were positive for Campylobacter. Direct and enrichment plating resulted in 139 and 168 culture isolates;respectively (P = 0.0003). Samples from study B delayed in processing for 1 to 3 days, resulted in 46 and 50 isolated by direct and enrichment plating;respectively (P = 0.4545). However, among samples delayed in processing for 4 to 7 days, a total of 128 Campylobacter isolates were cultured, having cultured 93 and 118 isolates through direct and enrichment plating;respectively (P = 0.0003). At present these studies demonstrate that enrichment culture has no benefit when stool specimen collection and immediate processing occur and when there is a processing delay period of 1 - 3 days. However, enrichment culture was beneficial in instances where transport and processing was delayed 4 - 7 days.

Background:Results-Based Financing(RBF)has proliferated in health sectors of low and middle income countries,especially fragile and conflict-affected ones,and has been presented as a way of reforming and strengthening strategic ***,few studies have empirically examined how RBF impacts on health care purchasing in these *** article examines the effects of several RBF programmes on health care purchasing functions in three fragile and post-conflict settings:Uganda,Zimbabwe and the Democratic Republic of Congo(DRC)over the past ***:The article is based on a documentary review,including 110 documents from 2004 to 2018,and 98 key informant(KI)interviews conducted with international,national and district level stakeholders in early 2018 in the selected districts of the three *** and analysis followed an adapted framework for strategic purchasing,which was also used to compare across the case ***:Across the cases,at the government level,we find little change to the accountability of purchasers,but RBF does mobilise additional resources to support *** relation to the population,RBF appears to bring in improvements in specifying and informing about entitlements for some ***,the engagement and consultation with the population on their needs was found to be *** relation to providers,RBF did not impact in any major way on provider accreditation and selection,or on treatment ***,it did introduce a more contractual relationship for some providers and bring about(at least partial)improvements in provider payment systems,data quality,increased financial autonomy for primary providers and enforcing equitable *** generally,RBF has been a source of much-needed revenue at primary care level in under-funded health *** context-particularly the degree of stability and authority of government-,the design of the RBF programme and the potential for effective int

Parkinson's disease(PD)is the most common movement disorder,with resting tremor,rigidity,bradykinesia and postural instability being major symptoms(1).Neuropathologically,it is characterised by the presence of abundant filamentous inclusions ofα-synuclein in the form of Lewy bodies and Lewy neurites in some brain cells,including dopaminergic nerve cells of the substantia nigra(2).PD is increasingly recognised as a multisystem disorder,with cognitive decline being one of its most common non-motor *** patients with PD develop dementia more than 10 years after diagnosis(3).PD dementia(PDD)is clinically and neuropathologically similar to dementia with Lewy bodies(DLB),which is diagnosed when cognitive impairment precedes parkinsonian motor signs or begins within one year from their onset(4).In PDD,cognitive impairment develops in the setting of well-established *** PD and DLB,multiple system atrophy(MSA)is the third major synucleinopathy(5).It is characterised by the presence of abundant filamentousα-synuclein inclusions in brain cells,especially oligodendrocytes(Papp�Lantos bodies).We previously reported the electron cryo-microscopy(cryo-EM)structures of two types ofα-synuclein filaments extracted from the brains of individuals with MSA(6).Each filament type is made of two different *** we report that the cryo-EM structures ofα-synuclein filaments from the brains of individuals with PD,PDD and DLB are made of a single protofilament(Lewy fold)that is markedly different from the protofilaments of *** findings establish the existence of distinct molecular conformers of assembledα-synuclein in neurodegenerative disease.

新型可逆性男用避孕药的研发进展缓慢。虽然研究已充分表明激素类药物具备有效和可逆的避孕作用,但仍然没有此类药物应用于临床。目前,正在研发一种睾酮和孕激素的自用凝胶Nestorone@,而以往研究更多采用的是注射给药方法。具有雄激素和孕激素活性的新型长效类固醇制剂,也正在进行早期临床试验。非激素疗法作用于精子发生的潜在位点,或干扰附睾或输精管中的精子成熟,具有某些潜在的优势,但仍处于临床前试验阶段。调查表明男性及其伴侣更愿意尝试新的男性避孕方法,只是没有相关产品上市。
