Evaluation of Helicobacter pylori Infection in Patients with Chronic Hepatic Disease

Background： The ^13C urea breath test （^13C-UBT） is the gold standard for detecting Helicobacterpylori infection. H. pylori pathogenesis in patients with hepatitis B virus （HBV） and related diseases remains obscure. We used ^13C-UBT to detect H. *** infection in patients with chronic HBV infection, HBV-related cirrhosis, HBV-related hepatic carcinoma, and other chronic hepatic diseases. Methods： A total of 131 patients with chronic hepatitis B （HB）, 179 with HBV-related cirrhosis, 103 with HBV-related hepatic carcinoma, 45 with HBV-negative hepatic carcinoma, and 150 controls were tested for H. pylori infection using ^13C-UBT. We compared H. pylori infection rate, liver function, complications of chronic hepatic disease, serum HBV-DNA, serum alpha-fetoprotein （AFP）, and portal hypertensive gastropathy （PHG） incidence among groups. Results： HBV-related cirrhosis was associated with the highest H. pylori infection rate （79.3%）. H. pylori infection rate in chronic HB was significantly higher than in the HBV-negative hepatic carcinoma and control groups （P 〈 0.001 ）. 11. pylori infection rate in patients with HBV-DNA ≥10^3 copies/ml was significantly higher than in those with HBV-DNA 〈103 copies/ml （76.8% vs. 52.4%, P 〈 0.001）. Prothrombin time （21.3 ± 3.5 s vs. 18.8 ±4.3 s）, total bilirubin （47.3±12.3 μmol/L vs. 26.6±7.9 μmol/L）, aspartate aminotransferase （ 184.5 ±37.6 U/L vs. 98.4 ± 23.5 U/L）, blood ammonia （93.4 ± 43.6 μmol/L vs. 35.5 ± 11.7 μmol/L）, and AFP （203.4±62.6 μg/L vs. 113.2± 45.8 μg/L） in the ^13C-UBT-positive group were significantly higher than in the ^13C-UBT-negative group （P 〈 0.01）. The incidence rates of esophageal fundus variceal bleeding （25.4% vs. 16.0%）, ascites （28.9% vs. 17.8%）, and hepatic encephalopathy （24.8% vs. 13.4%） in the ^13C-UBT-positive group were significantly higher than in the 13C-UBT-negative group （P 〈 0.01 ）. AIM:To describe the learning curves of hand-assisted laparoscopic D2 radical gastrectomy(HALG) for the treatment of gastric ***:The HALG surgical procedure consists of three stages:surgery under direct vision via the port for hand assistance,hand-assisted laparoscopicsurgery,and gastrointestinal tract *** to the order of the date of surgery,patients were divided into 6 groups(A-F) with 20 cases in each *** surgeries were performed by the same group of *** performed a comprehensive and indepth retrospective comparative analysis of the clinical data of all patients,with the clinical data including general patient information and intraoperative and postoperative observation ***:There were no differences in the basic information among the patient groups(P > 0.05).The operative time of the hand-assisted surgery stage in group A was 8-10 min longer than the other groups,with the difference being statistically significant(P = 0.01).There were no differences in total operative time between the groups(P = 0.30).Postoperative intestinal function recovery time in group A was longer than that of other groups(P = 0.02).Lengths of hospital stay and surgical quality indicators(such as intraoperative blood loss,numbers of detected lymph nodes,intraoperative side injury,postoperative complications,reoperation rate,and readmission rate 30 d after surgery) were not significantly different among the ***:HALG is a surgical procedure that can be easily mastered,with a learning curve closely related to the operative time of the hand-assisted laparoscopic surgery stage.

Hepatic encephalopathy is an often devastating complication of chronic liver disease,associated with high mortality and increased burden on patients and healthcare *** agents(such as nonabsorbable disaccharides and oral antibiotics)are often only partially effective and associated with unpleasant side *** our improved understanding of the pathophysiology of hepatic encephalopathy,multiple treatment modalities have emerged with promising results when used alone or as an adjunct to standard *** mechanisms of these agents vary greatly,and include the manipulation of gut microbial composition,reduction of oxidative stress,inhibition of inflammatory mediators,protection of endothelial integrity,modulation of neurotransmitter release and function,and other novel methods to reduce blood ammonia and *** their promising results,the studies assessing these treatment modalities are often limited by study design,sample size,outcome assessment heterogeneity,and paucity of data regarding their safety *** this article,we discuss these novel agents in depth and provide the best evidence supporting their use,along with a critical look at their limitations and future directions.

BACKGROUND: Glycogen synthase kinase (GSK)-3β/β-catenin signaling regulates ischemia-reperfusion (I/R)-induced apoptosis and proliferation, and inhibition of GSK-3β has beneficial effects on I/R injury in the heart and the central nervous system. However, the role of this signaling in hepatic I/R injury remains unclear. The present study aimed to investigate the effects and mechanism of GSK-3β/β-catenin signaling in hepatic I/R injury. METHODS: Male C57BL/6 mice (weighing 22-25 g) were pretreated with either SB216763, an inhibitor of GSK-3β, or vehicle. These mice were subjected to partial hepatic I/R. blood was collected for test of alanine aminotransferase (ALT), and liver specimen for assays of phosphorylation at the Ser9 residue of GSK-3β, GSK-3β activity, axin 2 and the anti- apoptotic factors Bcl-2 and survivin, as well as the proliferative factors cyclin D1 and proliferating cell nuclear antigen, and apoptotic index (TUNEL). Real-time PCR, Western blotting and immunohistochemical staining were used. RESULTS: SB216763 increased phospho-GSK-3β levels and suppressed GSK-3β activity (1880±229 vs 3280±272 cpm, P<0.01). ALT peaked at 6 hours after reperfusion. Compared with control, SB216763 decreased ALT after 6 hours of reperfusion (4451±424 vs 7868±845 IU/L, P<0.01), and alleviated hepatocyte necrosis and vacuolization. GSK-3β inhibition led to the accumulation of β-catenin in the cytosol (0.40±0.05 vs 1.31±0.11, P<0.05) and nucleus (0.62±0.14 vs 1.73±0.12, P<0.05), β-catenin further upregulated the expression of axin 2. Upregulation of GSK-3β/β-catenin signaling increased Bcl-2, survivin and cyclin D1. Serological and histological analyses showed thatSB216763 alleviated hepatic I/R-induced injury by reducing apoptosis (1.4±0.2% vs 3.6±0.4%, P<0.05) and enhanced liver proliferation (56±8% vs 19±4%, P<0.05). CONCLUSION: Inhibition of GSK-3β ameliorates hepatic I/R injury through the GSK-3β/β-catenin signaling pathway.

We report two cases of pulmonary sequestration infected with nontuberculous mycobacteria(NTM): Mycobacterium avium and Mycobacterium *** computed tomography showed pneumonic consolidation in the right lower lobe,which received a systemic blood supply from the descending aorta in both ***-assisted thoracoscopic surgeries were successfully performed and pathologica]examinations revealed multiple caseating granulomas.A review of the literature revealed only seven previous case reports of pulmonary sequestration infected with NTM,and no case with Mycobacterium abscessus has been reported.

To treat respiratory distress syndrome,surfactant is currently delivered via less invasive surfactant administration（LISA） or INtubation SURfactant Extubation（INSURE）.The aim of this study was to compare the effect of the two delivery methods of surfactant on cerebral *** infrared spectroscopy monitoring was carried out to detect cerebral oxygen saturation（Sc O2）,and the mean arterial blood pressure（MABP） was simultaneously *** 44 preterm infants included,the surfactant was administrated to 22 via LISA and 22 via *** clinical characteristics,treatments and outcomes of the infants showed no significant differences between the two *** correlation coefficient of Sc O2 and MABP（rSc O2-MABP） 5 min before administration was similar in the two *** surfactant administration,rSc O2-MABP increased in both groups（0.44±0.10 to 0.54±0.12 in LISA,0.45±0.11 to 0.69±0.09 in INSURE）.In the first and second 5 min after instillation,rSc O2-MABP was not significantly different from baseline in the LISA group,but increased in the first 5 min after instillation（0.59±0.13,P=0.000 compared with the baseline in the same group） and recovered in the second 5 min after instillation（0.48±0.10,P=0.321） in the INSURE *** were significant differences in the change rates of rSc O2-MABP between the two groups during and after surfactant *** results suggest that cerebral autoregulation may be affected transiently by surfactant *** effect duration of LISA is shorter than that of INSURE（〈5 min in LISA vs.5–10 min in INSURE）.

The optimal ventilation method for patients suffering from cardiac arrest and receiving cardiopulmonary resuscitation(CPR)remains unclear,and the recent guidelines do not provide detailed information.[1-3]During CPR,changing thoracic pressures due to chest compressions and ventilation pressure infl uence venous return and cardiac output,respectively.[4-6]Novel mechanical ventilation modes that are synchronized to chest compressions may improve blood flow and oxygenation.[4]In this porcine trial,we investigated the feasibility of a specially designed chest compression synchronized ventilation(CCSV)mode with peak pressures limited to 40 mbar(1 mbar=0.1 kPa)and an experimental synchronized ventilation(SV)limited to 20 mbar,respectively,intended to achieve a more lung-protective ventilation pattern during *** evaluated whether the diff erent synchronized pressure levels ameliorate hemodynamics,gas exchange and pulmonary function and compared it to intermittent positive pressure ventilation(IPPV).
