Liver transplantation(LT) is the only potentially curative treatment for selected patients with cirrhosis and hepatocellular carcinoma(HCC) who are not candidates for resection. When the Milan criteria are strictly applied, 75% to85%of 3-to 4-year actuarial survival rates are achieved, but up to 20% of the patients experience HCC recurrence after transplantation. The Milan criteria are based on the preoperative tumor macromorphology, tumor size and number on computed tomography or magnetic resonance imaging that neither correlate well with posttransplant histological study of the liver explant nor accurately predict HCC recurrence after LT, since they do not include objective measures of tumor biology. Preoperative biological markers, including alpha-fetoprotein, desgamma-carboxiprothrombin or neutrophil-to-lymphocyte ratio and platelet-tolymphocyte ratio, can predict the risk for HCC recurrence after *** biomarkers have been proposed as surrogate markers of tumor differentiation and vascular invasion, with varied risk magnitudes depending on the defined cutoffs. Different studies have shown that the combination of one or several biomarkers integrated into prognostic models predict the risk of HCC recurrence after LT more accurately than Milan criteria alone. Stress urinary incontinence （SUI） and end-stage erectile dysfunction （ED） after radical prostatectomy （RP） can decrease a patient＇s quality of life （QoL）. We describe a surgical technique involving scrotal incision for simultaneous dual implantation of an artificial urinary sphincter （AUS） and an inflatable penile prosthesis （IPP）. Patients with moderate to severe SUI （〉3 pads per day） and end-stage ED following RP were selected for dual implantation. An upper transverse scrotal incision was made, followed by bulbar urethra dissection and AUS cuff placement. Through the same incision, the corpora cavernosa was exposed, and an IPP positioned. Followed by extraperitoneal reservoirs placement and pumps introduced in the scrotum. Short-term, intra- and post-operative complications; continence status and erectile function; and patient satisfaction and QoL were recorded. A total of 32 patients underwent dual implantation. Early AUS-related complications were： AUS reservoir migration and urethral erosion. One case of distal corporal extrusion occurred. No prosthetic infection was reported. Over 96% of patients were socially the continent （≤1 pad per day） and 〉95% had sufficient erections for intercourse. Limitations of the study were the small number of patients, the lack of the control group using a perineal approach for AUS placement and only a 12 months follow-up. IPP and AUS dual implantation using a single scrotal incision technique is a safe and effective option in patients with SUI and ED after RP. Further studies on larger numbers of patients are warranted.

Objective: This article is a critical review of the literature concerning thyroid cartilage chondrosarcoma and is particularly focused on the management and prognosis of this rare entity. Study design: A review of all the cases of thyroid cartilage chondrosarcoma reported in the literature up to January 2013. Methods: The search was carried out through the introduction of the MeSH terms: Chondrosarcoma, Laryngeal Cartilages, Thyroid Cartilage, Therapeutics. All the studies related to thyroid cartilage chondrosarcoma were selected together with a newly presented case. Results: A total of 47 cases have been described in the literature, and 35 of these include sufficient data for statistical analysis. The age of patients ranged from 40 to 77 years, with a male predilection (88.6%). The main symptoms were neck mass and hoarseness. Grade I and II tumors were most frequent. A total of 13 cases described an extension of the tumor beyond the thyroid cartilage. The most common treatment was partial laryngectomy (50%). All patients who died were male, with grade II or II-III tumor or associated sarcoma, had metastases and received radiotherapy in addition to total laryngectomy. Conclusion: Patient age and tumor size does not influence the prognosis. Tumor grade I and I-II have good prognoses, whereas the prognosis is unpredictable in tumors between grade II and III. Patients with associated sarcoma, or metastasis, have a worse prognosis. Partial laryngectomy is a good therapeutic option, although total laryngectomy is sometimes required in order to ensure complete resection.
