What is already known on this topic?The morbidity and mortality of chronic obstructive pulmonary disease(COPD)is associated with adverse weather and air pollution.However,COPD patients are not able to be alerted in advance of high risk environments.What is added by this report?This prospective controlled trial conducted in pudong new area of Shanghai from October 2019 to April 2020 provided evidence of COPD risk forecasting service on the reductions in visits and costs of COPD patients in outpatient and emergency departments in China for the first time.What are the implications for public health practice?This study suggests that COPD risk forecasting service could be integrated into existing COPD management in public health to improve the health and economic outcomes.

Objective Aeromonas has recently been recognized as an emerging human pathogen.Aeromonasassociated diarrhea is a phenomenon occurring worldwide.This study was designed to determine the prevalence,genetic diversity,antibiotic resistance,and pathogenicity of Aeromonas strains isolated from food products in Shanghai.Methods Aeromonas isolates(n=79)collected from food samples were analyzed using concatenated gyrB-cpn60 sequencing.The antibiotic resistance of these isolates was determined using antimicrobial susceptibility testing.Pathogenicity was assessed usingβ-hemolytic,extracellular protease,virulence gene detection,C.elegans liquid toxicity(LT),and cytotoxicity assays.Results Eight different species were identified among the 79 isolates.The most prevalent Aeromonas species were A.veronii[62(78.5%)],A.caviae[6(7.6%)],A.dhakensis[3(3.8%)],and A.salmonicida[3(3.8%)].The Aeromonas isolates were divided into 73 sequence types(STs),of which 65 were novel.The isolates were hemolytic(45.6%)and protease-positive(81.0%).The most prevalent virulence genes were act(73.4%),fla(69.6%),aexT(36.7%),and ascV(30.4%).The results of C.elegans LT and cytotoxicity assays revealed that A.dhakensis and A.hydrophila were more virulent than A.veronii,A.caviae,and A.bivalvium.Antibiotic resistance genes[tetE,blaTEM,tetA,qnrS,aac(6)-Ib,mcr-1,and mcr-3]were detected in the isolates.The multidrug-resistance rate of the Aeromonas isolates was 11.4%,and 93.7%of the Aeromonas isolates were resistant to cefazolin.Conclusion The taxonomy,antibiotic resistance,and pathogenicity of different Aeromonas species varied.The Aeromonas isolates A.dhakensis and A.hydrophila were highly pathogenic,indicating that food-derived Aeromonas isolates are potential risks for public health and food safety.The monitoring of food quality and safety will result in better prevention and treatment strategies to control diarrhea illnesses in China.

China has accumulated multiple practices and experiences in building and enhancing malaria surveillance and response system. As China’s engagement into global health has gathered stronger momentum than ever, China together with the Swiss Tropical and Public Health institute and WHO has organised five sessions of the International Forum on Surveillance-Response System Leading to Tropical diseases Elimination during 2012–2020, in which malaria elimination has always been one of the hottest topics. In this study, the roles of international network on the surveillance and response system were explored to achieve a global malaria-free goal. China’s approach to malaria elimination has demonstrated significance of global collaboration on taking joint prevention and control, and building a worldwide institutional-based network.

Background Chemotherapy and hematopoietic stem cell transplantation(HSCT)can damage the immune system,and may result in a loss of protection from infectious diseases.This study aimed to evaluate the impact of these treatments on the decrease in antibody titers of the measles,mumps,and rubella(MMR)vaccine and seroconversion post-revaccination of MMR.Methods After completion of treatment for primary diseases,participants received an MMR revaccination.Antibody titers for MMR before revaccination were analyzed for all 110 children.After revaccination,68 participants received a follow-up evaluation of antibody titer and adverse reaction.Results Multivariable analysis showed that therapeutic schedules were the only factor correlated with lack of antibody titers for measles after completing treatment(P=0.008),while for mumps and rubella,no statistically significant difference was observed.Importantly,our study clearly demonstrated positive seroconversion rates for measles(97.5%),mumps(81.0%),and rubella(93.2%),with antibody levels rising across the board and peaking at around 6 months following revaccination.However,6 months after revaccination,a downtrend of antibody titer levels was observed,which is comparatively earlier than the waning immunity observed in healthy children.Furthermore,we found MMR revaccination to be safe,with only a single adverse reaction(local pain at the injection site)reported.Conclusions MMR revaccination is immunogenic for the population.We suggest periodic monitoring of antibody titers,in addition to a booster vaccination,although the optimal timing of booster vaccination remains to be investigated further.

Dear Editor,Coronavirus disease 2019(covid-19),caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),has led to 460 million cases confirmed and 6.1 million deaths across almost 200 countries until March 2022.In addition to the vaccine-based protection against viral infection,it is urgently needed to accurately diagnose covid-19 patients as early as possible after infection.

Background:Colorectal cancer(CRC)still ranks the top in morbidity and mortality of cancers worldwide,posing a huge threat and burden to the society.We aimed to determine the age-standardized incidence,mortality,and disability-adjusted life years(DALYs)of CRC and explore potential changes in the temporal trends of the CRC burden in Shanghai during 2002 to 2016.Methods:The cancer statistics and demographics were obtained from the Cancer Registry and the Statistics Bureau of pudong new area,respectively.Data from 2002 to 2016 were included and analyzed retrospectively.DALYs were calculated using DisMod and the age-standardized rates(ASRs)were obtained according to Segi world standard population.Joinpoint regression was used to measure the trends in CRC incidence and to estimate the annual percent change.Results:The increasing trend of CRC ASR incidence halted after 2014,coinciding with the introduction of the Shanghai CRC screening program.The ASRs of mortality and DALYs increased,at 0.42%(Pdisease burden of CRC in Shanghai remains serious,especially among men,and individuals aged>74 years.The benefits of the screening program have been partially proven by the ASRs of CRC incidence,providing important insights into better and wider application of screening programs.

Summary What is already known about this topic?Since the launch of China Healthy Lifestyle for All(CHLA),each action area has been evaluated at the local level and effective results have been achieved in most areas.What is added by this report?Based on the Reach,Effectiveness,Adoption,Implementation,and Maintenance(RE-AIM)model,the study found that there is an imbalance in the development of CHLA,and some of the action goals and indicators are not satisfactory.What are the implications for public health practices?A national action evaluation index system should be built to ensure the sustainability and scientific nature of this strategy.In addition,the government needs to attach great importance to CHLA to effectively help residents take health actions.

Objectives The purpose of this study was to assess the association between triglycerides(TG),total cholesterol(TC)at baseline,and type 2 diabetes mellitus(T2DM)incidence in a general Chinese population.Further,it aimed to evaluate the ability of TG and TC to predict T2DM incidence.Methods Qingdao Diabetes prevention Program participants recruited between 2006 and 2009 were followed up in 2012–2015.TG,TC,and T2DM status were measured.Cox proportional hazards models were used to estimate the association between TG,TC,and T2DM incidence.The receiver operating characteristic(ROC)curve was used to evaluate the ability of TG and TC to identify T2DM participants.Results The incidence of T2DM significantly increased with TG in women and TC in both men and women(Ptrend1.15 and>1.23 mmol/L in men and women,respectively.For TC,they were>5.17 and>5.77 mmol/L in men and women,respectively.The area under the ROCs of TG and TC were 0.54(0.51–0.57)and 0.55(0.52–0.58),respectively,in men,and 0.60(0.58–0.62)and 0.59(0.56–0.61),respectively,in women.Conclusion Elevated TG and TC were risk factors for T2DM incidence.However,no predictive capacity was found for both factors to identify T2DM incidence in Chinese men and women.Hence,TG and TC levels in both Chinese men and women might be used for decreasing the incidence of T2DM but no clinical predictive capacity for T2DM.

Background:The Shanghai growth standards are higher than World Health Organization (WHO) growth standards,which may influence the feeding practices of the caregivers and increase the risk of overweight in these infants.This study aimed to compare the effects of different growth standards on childhood obesity in Shanghai metropolitan area. Methods:This was a cluster-randomized controlled trial conducted in 2 downtown areas with 19 community health service centers in Shanghai from November 2013 to December 2015.Randomization was done at the level of the community.Infants (health newborns) were assessed and monitored by the Shanghai growth standards (S-group) and the 2006 WHO growth standards (W-group),respectively.Measurements were taken at 1.0,2.0,4.0,6.0,9.0 and 12.0 months of age during follow-up period.Based on the values of length and weight measurements,according to the group’s growth standards,doctors provided the caregivers with corresponding clinical consultation.Changes in weight-for-age z-score (WAZ),length-for-age z-score (LAZ),and weight-for length z-score (WLZ) between 2 groups were assessed using mixed regression models.Overweight was compared between 2 group at all follow-up measurements. Results:A total of 6509 infants (52.1% were boys) were in the W-group,and 8510 infants (51.4% were boys) were in the S-group.The overweight ratios between two groups were distinct at 9 months of age (3.4% in W-group and 4.3% in S-group) and 12 months of age (2.2% in W-group and 3.8% in S-group),and the differences were statistically significant (P= 0.020 and Parea up to 1 ye

Changsha was one of the most affected areas during the 2009 A(H1N1)influenza pandemic in China.Here,we analyze the spatial–temporal dynamics of the 2009 pandemic across Changsha municipal districts,evaluate the relationship between case incidence and the local urban spatial structure and predict high-risk areas of influenza A(H1N1).We obtained epidemiological data on all cases of influenza A(H1N1)reported across municipal districts in Changsha during period May 2009–December 2010 and data on population density and basic geographic characteristics for 239 primary schools,97 middle schools,347 universities,96 malls and markets,674 business districts and 121 hospitals.Spatial–temporal K functions,proximity models and logistic regression were used to analyze the spatial distribution pattern of influenza A(H1N1)incidence and the association between influenza A(H1N1)cases and spatial risk factors and predict the infection risks.We found that the 2009 influenza A(H1N1)was driven by a transmission wave from the center of the study area to surrounding areas and reported cases increased significantly after September 2009.We also found that the distribution of influenza A(H1N1)cases was associated with population density and the presence of nearest public places,especially universities(OR=10.166).The final predictive risk map based on the multivariate logistic analysis showed high-risk areas concentrated in the center areas of the study area associated with high population density.Our findings support the identification of spatial risk factors and highrisk areas to guide the prioritization of preventive and mitigation efforts against future influenza pandemics.
