Optimism, Social Support, and Caregiving Burden among the Long-Term Caregivers: The Mediating Effect of psychological Resilience

Background As the elderly population grows, the demand for long-term care services is increasing. Despite significant investments in care quality and workforce training, long-term care workers often face challenges such as work fatigue, heavy workloads, and inadequate support. These issues can impact job satisfaction, mental health, and care quality, leading to staff turnover. This study examines how optimism, social support, and psychological resilience relate to caregiving burden, aiming to understand their effects on caregivers’ well-being and performance to enhance the quality of long-term care services. Methods The participants were 542 long-term care workers. Descriptive statistics, t -tests, one-way ANOVA, and hierarchical regression were used for data analysis. Results (1) Optimism and social support were significantly and positively correlated with psychological resilience and significantly and negatively associated with caregiving burden. (2) Regarding differences in optimism, social support, psychological resilience, and caregiving burden among long-term care workers, females scored significantly higher than males in “social support;” married workers scored significantly higher than unmarried workers in “optimism,” “social support,” and “psychological resilience”; workers aged 45–65 scored significantly higher than those aged 25–45 in “optimism”; workers aged 25–45 scored significantly higher than those aged 45–65 in “caregiving burden”; social workers scored significantly higher than nursing staff in “optimism.” (3) psychological resilience partially mediated the relationship between social support and caregiving burden concerning explanatory and predictive power. Conclusions These findings suggest that optimism, social support, and psychological resilience are essential factors in reducing the caregiving burden among long-term care workers. Emerging research suggests a potential association of progression of Alzheimer's disease(AD)with alterations in synaptic currents and mitochondrial ***,the specific associations between these pathological changes remain *** this study,we utilized Aβ42-induced AD rats and primary neural cells as in vivo and in vitro *** investigations included behavioural tests,brain magnetic resonance imaging(MRI),liquid chromatography-tandem mass spectrometry(UPLC-MS/MS)analysis,Nissl staining,thioflavin-S staining,enzyme-linked immunosorbent assay,Golgi-Cox staining,transmission electron microscopy(TEM),immunofluorescence staining,proteomics,adenosine triphosphate(ATP)detection,mitochondrial membrane potential(MMP)and reactive oxygen species(ROS)assessment,mitochondrial morphology analysis,electrophysiological studies,Western blotting,and molecular *** results revealed changes in synaptic currents,mitophagy,and mitochondrial dynamics in the AD ***,intervention with Dengzhan Shengmai(DZSM)capsules emerged as a pivotal element in this investigation.Aβ42-induced synaptic dysfunction was significantly mitigated by DZSM intervention,which notably amplified the frequency and amplitude of synaptic *** cognitive impairment observed in AD rats was ameliorated and accompanied by robust protection against structural damage in key brain regions,including the hippocampal CA3,primary cingular cortex,prelimbic system,and dysgranular insular *** intervention led to increased IDE levels,augmented long-term potential(LTP)amplitude,and enhanced dendritic spine density and ***,DZSM intervention led to favourable changes in mitochondrial parameters,including ROS expression,MMP and ATP contents,and mitochondrial *** conclusion,our findings delved into the realm of altered synaptic currents,mitophagy,and mitochondrial dynamics in AD,concurrently highlighting the therapeutic potential of DZSM intervention.

Computerized Cognitive Training in Healthy Older Adults:Baseline Cognitive Level and Subjective Cognitive Concerns Predict Training Outcome

Computerized cognitive training (CCT) can improve cognition in older adults. However, specific programs for this target group have rarely been developed, and predictors of training success are largely unclear. In a randomized controlled pilot trial, we examined effects of a six-week CCT, tailored to the cognitive profile of healthy older adults (EG, n = 21) as compared to a nonspecific CCT (CG, n = 18) matched in frequency and intensity. No interaction effects between Group and Time were found. However, within-group analysis showed significant gains in verbal and non-verbal memory, executive and visuospatial functions and subjective cognitive concerns (SCC) in the EG, while the CG only benefitted in non-verbal memory and set-shifting. Low cognitive performance and lower SCC at baseline were the most consistent predictors of cognitive gains in the EG. Thus, CCTs specifically tailored to older adults seem advantageous compared to non-specific CCT. Further, we conclude that SCC may be related to reduced cognitive plasticity.

AIM To investigate the effects of depression and anxiety on health-related quality of life(QoL)in gastroesophageal reflux disease(GERD)patients and those suffering from cardiac(CCP)and noncardiac(NCCP)chest pain in Wuhan,*** In this cross-sectional study,a total of 358 consecutive patients with GERD were enrolled in Wuhan,China,of which 176 subjects had complaints of chest *** with chest pain underwent coronary angiography and were divided into a CCP group(52 cases)and NCCP group(124 cases).Validated GERD questionnaires were completed,and the 36-item Short-Form Health Survey and Hospital Anxiety/Depression Scale were used for evaluation of Qo L and psychological symptoms,*** There were similar ratios and levels of depression and anxiety in GERD with NCCP and ***,the QoL was obviously lower in GERD with CCP than NCCP(48.34±17.68 vs 60.21±20.27,Ppsychological impact for improving QoL in GERD-NCCP patients.

Relationship of Cognition, Depression and Anxiety to Glycemic Control in Older Adults with Diabetes

Objective: This study aimed to assess the relationship of cognition, depression and anxiety to glycemic control in elders with diabetes. DM is a chronic medical condition. Its control depends on adherence to medical therapy and making decisions related to lifestyle changes. This decision making capacity is affected by many factors including cognition and psychological status. Design: It was a case control study. Setting: It was done in Ain Shams University Hospital inpatients and DM outpatient clinic, Cairo, Egypt. Participants: Of the one hundred diabetic patients aged ≥ 60 years, 50 had Hemoglobin A1c (HbA1c) ≥ 7.5 (cases) and 50 had Hb A1c 7.5 (controls). Measurements: Cognition was assessed using minimental status examination (MMSE) test, Mattis Organic Mental Syndrome Screening Examination (MOMSSE) and Cambridge Cognitive Examination (CAMCOG) test. Geriatric depression scale-15 (GDS-15) was performed for depression assessment, while anxiety was assessed by DSM IV criteria. Laboratory investigations included: fasting blood sugar (FBS), post-prandial blood sugar (PPBS), glycated haemoglobin (Hb A1c), low density lipoprotein (LDL), high-density lipoprotein (HDL), total cholesterol, and triglycerides (TG). Results: Significant difference was found between the two groups regarding scores of cognitive tests: MMSE score (p = 0.004);below average (p = 0.02) and average scores (p = 0.05) of MOMSSE;CAMCOG score (p = 0.015);and CAMCOG divided items score including orientation (p = 0.003), comprehension (p = 0.005), expression (p = 0.020), attention (p = 0.002), and abstraction (p = 0.008) as well as depression screening scores (P = 0.002). Using Receiver Operating Characteristic, CAMCOG had better sensitivity and MOMSSE had better specificity. Conclusion: Cognitive impairment was associated with poor glycemic control, and impairment in attention and abstraction, related to executive function, functions were found to be associated with poor glycemic control. To understand the real experiences of patients with cancer-related fatigue and to promote quality of life,the researchers used computer searches of PubMed,EMbase,CINAHL,PsycINFO,Web of Science,and China National Knowledge Infrastructure,China Biology Medicine disc,Wanfang Data,China Science and Technology Journal Database for relevant qualitative studies from the inception to November 1,*** results showed that a total of 16 papers were included,and 57 outcomes were distilled into 9 categories,which were pooled into four integrated outcomes,such as somatic,psychological and social support and so *** is concluded that patients with cancer-related fatigue are not only in the midst of multidimensional,unspeakable somatic experiences,and complex psychological experiences,they are also hindered in coping with fatigue symptom and social ***,healthcare professionals should provide health interventions and psychological support to those patients whenever possible.

Causal relationships between survival rates, dietary and lifestyle habits, socioeconomic status and physical, mental and social health in elderly urban dwellers in Japan: A chronological study

Life expectancy was well known to associate with lifestyle habits, socioeconomic condition, and three health-related dimensions (physical, mental and social health status). However, the causal effect relationship among these variables remains unclear. The purpose of this study was to determine the causal relationships among health and life conditions, socioeconomic status, dietary and lifestyle habits and three health-related dimensions in elderly urban dwellers in Tokyo, Japan. Of 16,462 eligible elderly individuals aged 65 years and older, 13,195 participants responded to the questionnaire survey conducted in September 2001 (a response rate of 80.2%). A follow-up survey was conducted in September 2004 and a total of 8162 survivors were followed until the end of August 2007 through the municipal resident's registry. Exploratory factor analysis defined five latent variables based on the 13 observed variables. From a cross-lagged effects variation model using structural equation modeling, causal relationships were analyzed using longitudinal data obtained at the 2001 and 2004 survey and the number of survival days between 2004 and 2007. After estimating a best-fit model, we discovered that health and life conditions were not determined by current dietary and lifestyle habits, which many studies showed. However, the conditions were more directly affected by three health-related dimensions three years earlier, and indirectly affected by educational attainment and previous annual income as well. The current model suggests that it might be of great importance for elderly individuals to emphasize the maintenance of psychological well being, physical activity, social communication and participation, as well as income, rather than focusing on improvements in diet and health-related lifestyles per se.
