Prevalence of depression and anxiety among children with type 1 and type 2 diabetes:a systematic review and meta-analysis

Background Evaluation of psychiatric disorders in children is essential in timely *** individual studies,there is no information on the exact status of psychiatric disorders in *** present study was conducted to determine the prevalence of depression among children with type 1 and type 2 *** This meta-analysis is registered with PROSPERO(CRD42021231491).Several databases(Web of Science,Scopus,and PubMed)were searched from January 1,2000 to December 15,*** key words included:depression,anxiety,children,and *** steps of search,comprising screening,risk of bias,and extraction of study data,were performed separately by two *** It was found that a total of 109 studies had been conducted involving 52,493 children with type 1 and type 2 *** studies were included in the present review *** prevalence of depression,anxiety among children with type 1 diabetes were 22.2%[95%confidence interval(CI)19.2–25.2],and 17.7%(95%CI 15.2–20.1),*** prevalence of depression was higher among girls(29.7%)than boys(19.7%).The prevalence of depression was higher in lower-middle-income countries at 29.3%(95%CI 18.6–40.0).The prevalence of depression among children with type 2 diabetes was 22.7%(95%CI 17.3–28.0).Conclusion The results of the present study indicate the importance of paying attention to extensive periodic screening and appropriate activities to reduce pediatric depression.

Total Esophagogastric Separation in Neurologically Impaired Children: Outcome and Efficacy

Background: Gastroesophageal reflux disease (GERD) is one of the most clinical disorders affecting the gastrointestinal tract of neurologically impaired children (NIC). The reported incidence of GERD in NIC ranges from 15% to 75%. For a long time Nissen fundoplication became the first choice for management with some complications. For this reason, and as an alternative to repeated fundoplications, total esophagogastric separation (TEGS) became popular in treating these cases. Even some studies consider TEGS as a primary approach for GERD in NIC. Purpose: The aim of the study was to assess the efficacy and outcome of total esophagogastric separation in cases of severe gastroesophageal reflux in neurologically impaired children. Materials and Methods: Medical records of 11 patients who underwent total esophagogastric separation (TEGS) with Roux en Y anastomosis for recurrent GERD in our institute in pediatric surgery unit, Medina children hospital, KSA, were reviewed retrospectively. Selective criteria included all cases of recurrent GERD in NIC after fundoplications. TEGS was not performed in any case as a primary procedure. Results: Our study included eleven cases, seven males and four females. Their ages ranged from 6 to 14 years. All of them were neurologically impaired, with difficulty to take oral feeds and all cases had GERD or its related complications preoperatively. Median operating time was 5 hours ranging from 3.5 to 7 hours with a median blood loss of 200 ml. Hospital stay ranged from 5 to 17 days with a median of 9 days and median ICU stay was 3.5 days. Complications occurred in four cases (36.3%) with no mortality. Three cases needed reoperations (27.2%). Two cases (18.2%) developed hiatus hernia and were operated between 8 and 10 months postoperatively. Two cases (18.2%) develop adhesive intestinal obstruction who underwent conservative management and passed smoothly. Organisms causing spontaneous bacterial peritonitis in children with liver disease and ascites in Southern Iran

AIM： To determine the causative agents of spontaneous bacterial peritonitis （SBP） in children with liver disease and ascites in our center. METHODS： During a 2.5 year period, from September 2003 to March 2006, 12 patients with 13 episodes of SBP were studied. In all cases at the time of admission serum albumin and glucose, urinalysis and urine culture was performed. Analysis [white blood cell （WBC） count with differential, albumin, glucose], gram stain, culture by BACTEC method and antibiogram was done on ascitic fluids. Abdominal paracentesis was repeated after 48 h of antibiotic therapy for bacteriologic assay. The patients were followed for at least three months in a gastroenterology clinic. RESULTS： There were 7 girls （58%） and 5 boys （42%） with a median age of 5.2 years （range, 6 mo to 16 years）. All cases had positive ascitic fluid culture. Gram stain was positive in 5 （38.5%） of them. The isolated organisms were S. pneumoniae in 5 （38.5%）, E. coli in 2 （15.3%）, S. viridans in 2 （15.3%）, and K. pneumoniae, H. influenza, Enterococci, and nontypable Streptococcus each in one （7.7%）. All of them except Enterococci were sensitive to ciprofloxacin and ceftriaxone. All ascitic fluid cultures were negative after 48 h of antibiotic therapy. CONCLUSION： S. pneumoniae is the most common cause of SBP in the pediatric age group and we recommend a third generation cephalosporine （e.g., Ceftriaxlone or Cefotaxime） for empirical therapy in children with SBP.

Tricky Presentation of Multisystem Inflammatory Syndrome in Children (MIS-C) with in pediatric

Introduction: In emergence a COVID patient with multisystem inflammatory syndrome in children (MIS-C), associated with SARS-CoV-2, has shown increasing number among pediatric. Even the same presentation of Kawasaki disease we have to keep in mind MIS-C, in order to reach a consensus on this new disease in the future. Three variations of the disease patterns were reported: a group of children with increase in inflammatory activity and persistent fever, without criteria for Kawasaki disease, a second group with Kawasaki disease criteria, and a third group with shock, coronary aneurysms, severe cardiac dysfunction, and gastrointestinal symptoms. Classic Kawasaki disease is self-limited vasculitis which affects medium-sized vessels, almost occurred in children under five years old. Here, we bring a Saudi case, present to emergency department in Prince Sultan Military Medical City. Case report: 10 years old female, medically free, presented with abdominal pain, fever and skin rashes. Patient had history of COVID-19 infection 1 month before presentation. Initial investigations showed acute kidney injury with elevated inflammatory markers. Conclusion: Further evidence of the increase in the incidence of pediatric MIS-C, temporarily is associated with SARS-CoV-2. Physician should give more attentions to this new diagnosis with more fatal outcomes than Kawasaki cases.

Surgical outcome after complete repair of tetralogy of Fallot with absent pulmonary valve: comparison between bovine jugular vein-valved conduit and monocusp-valve patch

Background The prognosis of tetralogy of Fallot with absent pulmonary valve (TOF/APV) without operation is poor. We evaluated the surgical outcome of TOF/APV in a single center. Methods Twenty-two TOF/APV patients underwent complete surgical correction in our hospital. Right ventricular outflow tract reconstruction was performed using bovine jugular vein (BJV)-valved conduit implantation (n=10), homograft-valved conduit implantation (n=2), or monocusp-valve patch (n=10). Health-related quality of life (QOL) was evaluated during follow-up. Results The overall survival at 5 and 10 years was 86.4±7.3% (confidence interval 69.4–97.2%). The survival rates were significantly different between patients with and without bronchial stenosis (40 and 100%, P=0.0003, log-rank test). The survival of patients aged>6 months was higher than those≤6 months (100 vs. 40%, P=0.0003, log-rank test). Patients with BJV-valved conduits had higher systolic gradients from the right ventricle to the pulmonary artery (RV–PA) compared to those with monocusp-valve patches. BJV-valved conduit implantation was a risk factor for post-operative pulmonary-valve stenosis. The QOL score for patients with BJV-valved conduits was lower than those with monocusp-valve patches (P<0.05). No reoperation was performed during follow-up. Conclusions Bronchial stenosis and lower age (≤6 months) were the main factors influencing post-operative survival. The use of a BJV-valved conduit was a main reason for RV–PA restenosis;thus, the use of a BJV-valved conduit may increase the need for repeat intervention and decrease the post-operative quality of life.

A Plasmonic Mass Spectrometry Approach for Detection of Small Nutrients and Toxins

Nutriology relies on advanced analytical tools to study the molecular compositions of food and provide key information on sample quality/safety. Small nutrients detection is challenging due to the high diversity and broad dynamic range of molecules in food samples, and a further issue is to track low abundance toxins. Herein, we developed a novel plasmonic matrix-assisted laser desorption/ionization mass spectrometry(MALDI MS)approach to detect small nutrients and toxins in complex biological emulsion samples. Silver nanoshells(SiO_2@-Ag) with optimized structures were used as matrices andachieved direct analysis of ~ 6 n L of human breast milk without any enrichment or separation. We performed identification and quantitation of small nutrients and toxins with limit-of-detection down to 0.4 pmol(for melamine) and reaction time shortened to minutes, which is superior to the conventional biochemical method currently in use. The developed approach contributes to the near-future application of MALDI MS in a broad field and personalized design of plasmonic materials for real-case bio-analysis.
