The Role of Cosmetic Gynecology Treatments in Women in Perimenopausal Period

The conditions in which we live nowadays contribute to exposure of our bodies to harmful factors such as UV radiation, pollution, smoking, poor eating habits, low physical activity, stress, which thereafter lead to the acceleration of skin aging process. In women, the process is additionally dependent on the menopause, as a result of the disappearance of hormones, the process is faster. Physiological changes that occur in women’s body during perimenopausal and menopausal period affect their sexual function: pain during an intercourse, decreased libido, lack of agitation. Deficiency of hormones, besides skin changes, leads to vaginal dryness, accompanied by inflammation, often correlated with discharge and burning. As a result of the loss of hyaluronic acid labia become slack and less moisturized. In addition, there has been observed: a gradual loss of pigment and hair, involution of the clitoris, and involution and sticking of labia minora, lipoatrophy of labia majora with subsequent reduction in their volume and thinning the lining around the vaginal opening. This article presents processes of skin aging. It describes mechanisms of intrinsic and extrinsic aging. It also presents the mechanisms of accelerated aging in women in perimenopausal period—accelerated skin aging dependent on hormonal factors associated with the loss of ovarian function. The research presents how the hormonal loss influences woman’s body, her genitals—its functionality and aesthetic look. It also shows how using filler treatments in gynecology can restore the aesthetic appearance of female genital and improve their functionality.

aesthetic medicine is not a new area of medicine. It has been on the world’s market for several dozen years. aesthetic and anti-aging medicine is a separate field of medicine combined with other areas such as: neurology, plastic surgery, cosmetic surgery, general surgery, orthopaedics, psychology, psychiatry, dermatology, gynaecology, andrology, endocrinology and so on. aesthetic medicine is an area of medicine, dealing with human’s health in terms of: external appearance, image, aesthetics, well-being, visible skin changes and discomfort in the quality of life felt by a patient. The purpose of aesthetic medicine is to achieve patients’ satisfaction, as to their requirements related to the appearance, elimination of the complexes and the restoration of self-esteem. The following work shows, based on the overall impact of aesthetic medicine on the lives of patients and on the basis of the characteristics of some basic treatments in aesthetic medicine (fillers, botulinum toxin type A, intralipotherapy, needle mesotherapy, chemical peels), how this area of medicine is linked with other specialties and why it should be treated as a separate field of medicine.

Regulation of the intestinal immune system by flavonoids and its utility in chronic inflammatory bowel disease

Flavonoids are phytochemicals which can regulate the activity of the intestinal immune system. In patients with chronic inflammatory bowel disease(IBD) there is an overexpression and imbalance of the components of the inflammatory immune reactions which are chronically activated. Suppression of inflammation can be achieved by anti-inflammatory drugs which are used in clinical medicine but these can cause serious side effects. Flavonoids can have natural immunosuppressive properties and inhibit the activation of immune cells and its effectors(chemokines, TNF-, cytokines). Phytochemicals such as flavonoids bind to the nuclear Ah(aryl hydrocarbon)-receptor thereby stimulating protective enzyme activities. As shown by clinical evidence in patients and by experimental work some flavonoids(apigenin, epigallocatechin gallate) were effective in the inhibition of inflammation. Instead of or additionally to anti-inflammatory drugs flavonoids can be used in IBD patients to treat the over-reactive immunologic system. This is accomplished by upregulation of the Ah-receptor. Flavonoids interact with toll-like receptors expressing on the surface of immune cells, then they were internalized to the cytosol and transferred into the nucleus, where they were attached to the Ah-receptor. The Ah-receptor binds to the Ah-R nuclear translocator and via Ah response element beneficial protective enzymes and cytokines are induced, leading to upregulation of the anti-inflammatory system.

A Recognized Value System among Nurses Working in Poland

Aim of this work was to present a study indicating the recognized system of values of nursing units in healthcare entities in Poland. Professionally active nurses (249) employed in medicinal entities were subjects in the study. A random selection of individuals was applied in the study, and the participation in the research was declared by respondents consent to participate in it. A survey was used as the main form of measurement, and it was divided into direct and indirect survey. A standardized Rokeach’s scale of values [1] [2] consisting of two collections, each of which included 18 single values, was used. They were ranked by tested individuals according to the hierarchy of their values which allowed for recognizing which are the most important for them. Kruskal-Wallis’s test was applied in the study. Results: The social position is the most important value for nurses working in the district medical entities. For those working in the regional medical units, the most appreciated value is being ambitious, whereas in the clinical self-esteem. The socio-demographic data such as age, length of service, levels of education differentiate assessments of each recognized values in the group of nurses. The women between 31 and 40 consider being ambitious as the most important value, between 41 and 50 responsibility and between 20 and 30 self-esteem;responsibility is the most appreciated value for people with 21 - 25 years of experience because the older and more experienced the nurse is, the stronger her sense of responsibility becomes. A social position was appreciated the most by the nurses holding a nursing degree. Conclusions: There have been significant differences in terms of accepted values between nurses working in the district, provincial and clinical medical entities. Socio-demographic data such as age, length of service, education levels differentiate assessment of each recognized value in the group of nurses.

Emerging role of bioflavonoids in gastroenterology:Especially their effects on intestinal neoplasia

Flavonoids,secondary plant products which could be essential for normal physiology in humans and animals,may be the vitamins of the next *** belong to the polyphenols and possess antioxidative,anti-inflammatory,antimutagenic and anticarcinogenic *** the various flavonoid species,tea flavonoids such as apigenin (from camomile) and epigallocatechin gallate (EGCG from green tea) can be used for the prevention of intestinal neoplasia,especially for adenoma and cancer prevention in the gastrointestinal *** experimental studies with molecular and biological end points support the therapeutic efficacy of *** studies with cohorts and case-control trials suggest that flavonoids are effective in tertiary bioprevention but,as yet,there are no controlled randomized clinical *** can inhibit inflammatory pathways and could be useful for chronic inflammatory bowel *** deficiency syndromes could be therapeutic targets in the future.

Chronic Kidney Disease (CKD) poses a significant healthcare challenge in Saudi Arabia, necessitating a structured approach to management and treatment. Recent consensus guidelines in Saudi Arabia emphasize a comprehensive strategy for CKD management, tailored to local needs and healthcare resources. The guidelines advocate for early detection and stratified care to mitigate the progression of CKD and improve patient outcomes. Key recommendations include the routine assessment of albuminuria and estimated glomerular filtration rate (eGFR) for accurate diagnosis and staging of CKD. The consensus underscores the importance of utilizing updated tools like the CKD-EPI 2021 equation for eGFR calculation and incorporating cystatin C measurements for enhanced diagnostic accuracy. Management strategies focus on controlling underlying conditions such as diabetes and hypertension, which are prevalent risk factors in the Saudi population. Personalized treatment plans are recommended, incorporating lifestyle modifications, pharmacological interventions including SGLT-2 inhibitors and GLP 1 RA, and regular monitoring to manage CKD complications effectively. The guidelines also highlight the need for specialized care and referral pathways for advanced CKD stages, including the role of nephrologists in the comprehensive management of CKD patients. In addition, the consensus addresses the need for improving public awareness and education about CKD, emphasizing early screening and preventive measures, particularly in high-risk groups. The implementation of these guidelines aims to optimize CKD management across Saudi Arabia, reduce the burden of kidney disease, and enhance overall patient care in the region.

Clinical variants of pityriasis rosea

Pityriasis rosea(PR) is a common erythemato-squamous dermatosis which almost always, is easily diagnosed. Mostly the disease presents in its classical form. However, clinical dermatology is all about variations and PR is not an exception. Variants of the disease in some cases may be troublesome to diagnose and confuse clinicians. Prompt diagnosis and treatment of the condition becomes necessary to avoid unnecessary investigations. We hereby review and illustrate atypical presentations of the disease, including diverse forms of location and morphology of the lesions, the course of the eruption, and its differential diagnoses.

Microfocused Ultrasound for Nonablative Skin and Subdermal Tightening to the Periorbitum and Body Sites: Preliminary Report on Eighty-Two Patients

Lax crepey skin is a major unsightly finding throughout the face and body that occurs from chronological aging and environmental photodamage. Surgical lifting procedures are unable to induce sufficient neocollagenesis and also associated with visible scars, risks and recovery time. In this preliminary report, non-invasive delivery of microfocused ultrasonic thermal coagulation points at two levels of the dermis outside the orbital rims was effective in reduction of crepey, wrinkled and sagging skin for about 1 ? years in nineteen treated patients. Matched Orientation Mirror Imaging produced an average brow height elevation of 1 - 2 mm. Microfocused thermal coagulation points at multiple tissue levels also induced tissue tightening and reduction of crepey wrinkled skin in the décolletage, brachium, periumbilicus, inner thigh and knees. Investigator and Subject Global aesthetic Improvement Scale graded responses that correlated to the severity of the degree of crepiness and laxity. Pain management during the procedure included the use of infiltration of local anesthesia. No permanent adverse events were observed. Further innovations in the current technology are needed for more effective and safer delivery of energy to improve the appearance of crepey and lax tissue in the head, neck and body.

Vasectomy by epithelial curettage without suture or cautery： a pilot study in humans

Curettage of the epithelium of the vas deferens might be a safe and effective method of male sterilization. We conducted a pilot study of vasectomy by epithelial curettage with a novel microcurette called the Vas-X in 12 normal men requesting elective sterilization. Seminal fluid analysis was obtained monthly after the procedure for 6 months. Pain was assessed by questionnaire. Three months after the procedure, all men attained sperm concentrations of less than 0.2 million sperm per mL, and seven were azoospermic. Post-procedural pain was minimal. Nine men ultimately achieved and maintained azoospermia; however, 4 to 6 months after the procedure, sperm concentrations increased in three of the 12 subjects, necessitating repeat vasectomy. Microscopic examination of the vas deferens from these failures revealed re-canalization. Vasectomy by epithelial curettage can result in effective sterilization; however, 1/4 of the subjects were not effectively sterilized by the procedure due to re-canalization of the vas deferens. Epithelial curettage will require further refinement to determine if it is a viable form of vasectomy.
