Lax crepey skin is a major unsightly finding throughout the face and body that occurs from chronological aging and environmental photodamage. Surgical lifting procedures are unable to induce sufficient neocollagenesis and also associated with visible scars, risks and recovery time. In this preliminary report, non-invasive delivery of microfocused ultrasonic thermal coagulation points at two levels of the dermis outside the orbital rims was effective in reduction of crepey, wrinkled and sagging skin for about 1 ? years in nineteen treated patients. Matched Orientation Mirror Imaging produced an average brow height elevation of 1 - 2 mm. Microfocused thermal coagulation points at multiple tissue levels also induced tissue tightening and reduction of crepey wrinkled skin in the décolletage, brachium, periumbilicus, inner thigh and knees. Investigator and Subject Global Aesthetic Improvement Scale graded responses that correlated to the severity of the degree of crepiness and laxity. Pain management during the procedure included the use of infiltration of local anesthesia. No permanent adverse events were observed. Further innovations in the current technology are needed for more effective and safer delivery of energy to improve the appearance of crepey and lax tissue in the head, neck and body.

Aim: Over the last 15 years, hyaluronic acid (HA) fillers have become the most popular injectable biomaterial for soft tissue correction. With the increasing number of available HA fillers and the multiplication of facial treatments all over the world, there has been a need from physicians to better understand the HA fillers science. There is especially a growing interest in the science-based evaluation of rheological characteristics which represents an essential tool to guide physicians in the selection of the most appropriate HA fillers, administration techniques and depths of injection for their clinical applications. Methods: Four key rheological parameters (viscosity η, elasticity G', normal force FN and elasticity E') are measured and discussed on five HA fillers. Results: These four key rheological parameters are demonstrated to play a pivotal role, in combination with the cohesivity, for better predicting the clinical behavior of HA fillers at different stages of their lifetime. Conclusion: This article discusses the importance of four key rheological parameters during the main steps of the clinical HA fillers' lifetime, from the product injection to the loss of clinical effects. A better knowledge of these HA fillers' rheological parameters can help the physicians to optimize their aesthetic outcomes, safety and patient satisfaction.

Magnetic resonance(MR) imaging of the liver is an important tool for the detection and characterization of focal liver lesions and for assessment of diffuse liver disease,having several intrinsic characteristics,represented by high soft tissue contrast,avoidance of ionizing radiation or iodinated contrast media,and more recently,by application of several functional imaging techniques(i.e.,diffusion-weighted sequences,hepatobiliary contrast agents,perfusion imaging,magnetic resonance(MR)-elastography,and radiomics analysis). MR functional imaging techniques are extensively used both in routine practice and in the field of clinical and pre-clinical research because,through a qualitative rather than quantitative approach,they can offer valuable information about tumor tissue and tissue architecture,cellular biomarkers related to the hepatocellular functions,or tissue vascularization profiles related to tumor and tissue biology. This kind of approach offers in vivo physiological parameters,capable of evaluating physiological and pathological modifications of tissues,by the analysis of quantitative data that could be used in tumor detection,characterization,treatmentselection,and follow-up,in addition to those obtained from standard morphological imaging. In this review we provide an overview of recent advanced techniques in MR for the diagnosis and staging of hepatocellular carcinoma,and their role in the assessment of response treatment evaluation.

The Prostate Imaging Reporting and Data System(PI-RADS)has good ability to identify the nature of lesions on prostate magnetic resonance imaging(MRI).However,some lesions are still reported as PI-RADS 4 and 5 but are biopsy-proven ***,we aimed to summarize the reasons for the negative prostate biopsy of patients who were assessed as PI-RADS 4 and 5 by biparameter *** retrospectively sorted out the prostate MRI,treatment,and follow-up results of patients who underwent a biparameter MRI examination of the prostate in The First Affiliated Hospital of Nanjing Medical University(Nanjing,China)from August 2019 to June 2021 with PI-RADS 4 and 5 but a negative *** focused on reviewing the MRI characteristics.A total of 467 patients underwent transperineal prostate *** them,biopsy pathology of 93 cases were *** follow-up,90 patients were ruled out of prostate *** the 90 cases,40 were considered to be overestimated PI-RADS after review.A total of 22 cases were transition zone(TZ)lesions with regular appearance and clear boundaries,and 3 cases were symmetrical *** 15 cases,the TZ nodules penetrated the peripheral zone(PZ)and were mistaken for the origin of PZ.A total of 17 cases of lesions were difficult to distinguish from prostate *** them,5 cases were granulomatous inflammation(1 case of prostate tuberculosis).A total of 33 cases were ambiguous lesions,whose performance was between PI-RADS 3 and *** summary,the reasons for“false-positive MRI diagnosis”included PI-RADS overestimation,ambiguous images giving higher PI-RADS,diseases that were really difficult to distinguish,and missed lesion in the initial biopsy;and the first two accounted for the most.

Introduction: The growing demand for non-surgical, non-invasive procedures for fat and circumference reduction and for cellulite treatments has led to the development of various energy-based technologies. Bi-polar radiofrequency (RF) technology combined with additional technologies, such as vacuum, massage or infrared (IR) light is widely used in various body contouring treatments in the medical aesthetic market. The current study is based on a retrospective efficacy analysis of an RF device, combining two RF modalities, coupled with suction. Methods: Representative treatment outcomes, documented in photographs taken at baseline and post-treatment were gathered from several clinics. Photos of the various treatment areas were evaluated by independent evaluators for cellulite and contour improvement. Results: Before and after photographs were gathered from 31 patients treated in different clinics. Analysis was carried out on different body areas such as thighs, abdomen, back, buttocks and arms and analyzed by two independent evaluators. Scoring was performed according to the Global Aesthetic Improvement Scale (GAIS) scale and indicated an overall improvement in the contour of all treated body areas. In cellulite cases, improved appearance according to the cellulite grading scale was noted. Conclusion: This retrospective analysis supports the device’s efficacy for fat and circumferences reduction and cellulite improvement. The device’s unique specifications contribute to treatment safety and efficacy which is highly tolerable by the patients.

Reducing Paediatric Outpatient Waitlists—A Proposed Allied Health Multidisciplinary Approach

Extremely long Australian paediatric outpatient waiting lists have resulted in large numbers of children waiting to see a paediatrician. Analyses of the patient referrals suggest that a large proportion of children may benefit from allied health input, in addition to being seen by the paediatrician. This paper provides an organisational strategy that involves streamlining Allied Health clinical services in an effort to assist with bringing down a Paediatric Out-patient waitlist. The paper describes principles to guide the formation of a Paediatric Allied Health Multi-Disciplinary team that proposes assessment and brief interventions. The service would focus on supporting the paediatricians through the use of structured procedures, telephone intakes, multidisciplinary triage, shared and standardised assessments, collaborative formulations and reports, brief interventions, clinical-community linkages, and allied health student support. A variety of additional important organisational principles are proposed to facilitate patient flow from the perspective of streamlining administrative processes, having adequate administrative support, shared responsibilities, teamwork, flexibility, carrying out intake assessments, determining which referrals were appropriate for allied health, and working closely with medical staff.

BACKGROUND Insulin resistance(IR)was reported in most polycystic ovarian syndrome(PCOS)***,a biguanide drug,successfully reduced *** Model Assessment for IR(HOMA-IR)and Doppler parameters assessed metformin's *** To verify whether the area under the curve of the internal carotid artery(AUC-ICA)Doppler wave can be a useful marker for assessing IR among PCOS cases who presented with menstrual irregularity and were treated with metformin over 6 *** An observational,cross-sectional study recruited 54 eligible PCOS women;the anthropometrics were as follows:age,body mass index(BMI),menstrual cycle days,biochemical serum cholesterol,low and high-density lipoprotein,sex hormone-binding globulin,fasting blood glucose,and HOMA-IR,hormonal testosterone,luteinizing hormone over follicle-stimulating hormone ratio,and ultrasonic pulsatility index(PI)and resistance index(RI),carotid artery intima-media thickness(CIMT)and(AUC-ICA)parameters were initially recorded and repeated 3 mo and 6 mo later with metformin tab 500 mg;three times/day for 6 *** addition,AUC-ICA was assessed by taking repeated systolic and diastolic wave height *** Metformin caused a progressive reduction in BMI,menstrual cycle days,biochemical hormonal,and Doppler parameters(CIMT,PI,RI,and AUC-ICA).AUC-ICA correlated strongly to all PCOS ***-ICA correlated inversely with treatment time(r=-0.98,Ppractice.
