Unveiling the silent crisis:global burden of suicide‑related deaths among children aged 10–14 years

Background The rise in suicides among children aged 10–14 years demands urgent attention globally.This study aims to assess the global burden of suicide-related deaths in this age group from 1990 to 2019,considering factors such as sex,geography,and sociodemographics,to inform prevention strategies and interventions.Methods The data from Global Burden of Disease 2019,encompassing 204 countries and territories,were analyzed to investigate deaths and years of life lost(YLLs)due to suicide among children aged 10–14 years.Statistical analyses,including mortality rates,YLLs,and the sociodemographic index(SDI),were conducted using standardized tools.Results In 2019,a total of 8327[95%uncertainty interval(UI)=7073–9685]children aged 10–14 years died globally due to suicide,with a mortality rate of 1.30(95%UI=1.10–1.51)per 100,000.The rates varied across countries/territories ranging between 0.05(95%UI=0.02–0.10)in South Africa and 7.49(95%UI=5.13–10.57)in Greenland.The contribution of suicide-related deaths to all-cause mortality ranged from 0.07%(95%UI=0.04%–0.15%)in South Africa to 33.02%(95%UI=24.36%–41.53%)in Greenland.Worldwide,there were approximately 636,196(95%UI=540,383–740,009)YLLs due to suicide,with a rate of 99.07(95%UI=84.15–115.23)per 100,000.The association between SDI and suicide-related deaths was evident,with higher contributions observed in countries with higher SDI.Conclusions This study reveals a concerning global burden of suicide-related deaths among children aged 10–14 years.Despite progress in reducing mortality rates,suicide remains a significant issue.While overall rates have declined,the percentage of deaths caused by suicide in this age group is increasing.

Rare Case of Large Anterior Meningocele Associated with Uterine Didelphys

We report a 27-year-old female who presented with abdominal distension, saddle anesthesia, and lower back pain. CT and MRI of the abdomen/spine were performed which showed a large anterior sacral meningocele occupying most of the pelvic and abdominal cavity and displacement of their respective contents. Anterior approach was chosen, uterine didelphys was identified and mobilized, meningocele identified and gentle drainage with subsequent resection of the cyst wall and watertight closure was achieved. Subsequent MRI demonstrated resolution of most the sacral meningocele. Patient symptoms improved and are doing well.

Variations in the dynein axonemal heavy chain gene,dynein axonemal heavy chain 6(DNAH6),lead to multiple morphological abnormalities of the flagella.Recent studies have reported that these deficiencies may result in sperm head deformation.However,whether DNAH6 is also involved in human acrosome biogenesis remains unknown.The purpose of this study was to investigate DNAH6 gene variants and their potential functions in the formation of defective sperm heads and flagella.Whole-exome sequencing was performed on a cohort of 375 patients with asthenoteratozoospermia from the First Affiliated Hospital of Anhui Medical University(Hefei,China).Hematoxylin and eosin staining,scanning electron microscopy,and transmission electron microscopy were performed to analyze the sperm morphology and ultrastructure.Immunofluorescence staining and Western blot analysis were conducted to examine the effects of genetic variants.We identified three novel deleterious variants in DNAH6 among three unrelated families.The absence of inner dynein arms and radial spokes was observed in the sperm of patients with DNAH6 variants.Additionally,deficiencies in the acrosome,abnormal chromatin compaction,and vacuole-containing sperm heads were observed in these patients with DNAH6 variants.The decreased levels of the component proteins in these defective structures were further confirmed in sperm from patients with DNAH6 variants using Western blot.After intracytoplasmic sperm injection(ICSl)treatment,the partner of one patient with a DNAH6 variant achieved successful pregnancy.Overall,novel variants in DNAH6 genes that contribute to defects in the sperm head and flagella were identified,and the findings indicated Icsl as an effective clinical treatment for such patients.

Whole-person care and holistic care approach has been proposed for complementary and integrative health care for type 2 diabetes mellitus.However,some doubts still exist on the feasibility of replicating processes followed in clinical trials and observational studies in real-world settings.This narrative literature review summarized and assessed existing clinical evidence(clinical trials,observational studies,and case reports)describing holistic and integrated care approach in adult and adolescent individuals with type 2 diabetes mellitus in clinical practice.The goal was to highlight existing evidence for implementation and outcomes of whole-medical systems and holistic integrated care approach for type 2 diabetes mellitus.A nonsystematic literature search was performed on Google Scholar,PubMed,Web of Science,ProQuest and ScienceDirect to identify clinical evidence from different parts of the world,evaluating the use of whole-medical systems and/or holistic care interventions in clinical practice for management of type 2 diabetes mellitus.Relevant keywords were used in the search.Data were analyzed using content analysis and simple descriptive statistics(percentages).Most of the studies(64%)were mainly conducted in Eastern countries(India,China and Israel)while 36%of the studies were conducted in the Western countries(USA,Netherlands,Canada and Mexico).Lifestyle medicine and integrated naturopathy were shown to be the commonly used whole-medical systems for type 2 diabetes mellitus management.Significant improvements in type 2 diabetes parameters,medication use,other symptoms,and overall feeling of wellness were observed in all studies.This review study revealed limited utilization and/or documentation of whole-medical systems or holistic care treatments for type 2 diabetes mellitus in regions of the world other than eastern countries.Lifestyle medicine,naturopathy,yoga,Ayurveda and traditional Chinese medicine were shown to be effective for type 2 diabetes mellitus,either as an alternative or as a complementary

BACKGROUND Low grade serous carcinoma of the ovary(LGSOC)is a rare type of epithelial ovarian cancer with a low incidence rate.The origin of ovarian cancer has always been a hot topic in gynecological oncology research,and some scholars believe that the origin of ovarian malignant tumors is the fallopian tubes.Primary fallopian tube cancer is the lowest incidence of malignant tumors in the female reproductive system.There are only a few reports in the literature,but the mortality rate is very high.But in clinical practice,fallopian tube cancer is very common,but in most cases,it is classified as ovarian cancer.CASE SUMMARY We report a 54 years old postmenopausal woman who was hospitalized with a lower abdominal mass and underwent surgical treatment.The final pathological confirmation was low-grade serous carcinoma of the right ovary and low-grade serous carcinoma of the left fallopian tube.No special treatment was performed after the surgery,and the patient was instructed to undergo regular follow-up without any signs of disease progression.CONCLUSION The prognosis of LGSOC is relatively good,over 80%of patients still experience disease recurrence.

Background: Infertility is a complex disorder with significant psycho-social and economic consequences. It globally affects 10% - 15% of couples. In Cameroon, little is known about what women do to overcome the psychosocial aspects of the disease. Objectives: This study aimed to identify the support systems and coping strategies of infertile women attending the outpatient consultation unit of the Gynaecological Endoscopic Surgery and Reproductive Teaching Hospital (CHRACERH), Yaoundé, Cameroon. Methods: A hospital-based cross-sectional study was conducted from the 14th of March to the 6th of April 2023 at CHRACERH Yaoundé. A total of 190 participants were recruited using a convenience sampling method. Data regarding socio-demographic characteristics, support systems and coping strategies were collected using a pretested questionnaire. Descriptive and analytic statistics were conducted using SPSS version 25. Results: The mean age of participants was 39.52 ± 7.64 years. The majority 78.9% of participants were workers (public, private sector, or traders) and were Christians 95.8%. The most common source of psychological support was from family 76.8 and husbands 72.63%. Most of the participants 89.5% resorted to prayer and getting busy 48.4% as a coping strategy. There was no statistically significant relationship between coping strategies and psychological disorders p > 0.05. Conclusion: The main support system of participants was family, husband, and friends. Prayer, getting busy and adoption were the most common coping strategies. There is a need for the Ministry of Public Health and other stakeholders to put in place other support systems and coping strategies (FELICIA) used elsewhere and provide adequate health education and infection control to prevent infertility in Cameroon.

To editor:Balanced chromosomal rearrangements(BCRs),predominantly reciprocal translocations and inversions represent disease-related structural variations(SVs)that directly disrupt gene structure and function.Moreover,BCRs predispose offspring to unbalanced rearrangements,resulting in adverse pregnancy outcomes.1 The timely and accurate identification of carriers is crucial for minimizing the risk of associated adverse pregnancy outcomes,assessing recurrent risk,and planning appropriate management.Conventional cytogenetic karyotyping serves as the standard first-tier approach for BCR detection,yielding a rate of 2%–4%in the recurrent miscarriage population.2 However,its limited resolution(typically around 5–10 Mb)poses challenges in detecting cryptic BCRs with smaller or similarly banded segments,as well as complex SVs involving multiple chromosomes.Optical genome mapping(OGM),an emerging technology from Bionano Genomics,has a high resolution for detecting nearly all types of genomic SVs and has been recognized as a revolution in next-generation cytogenomic analysis.3,4 A growing number of studies have recently demonstrated the efficiency and superiority of OGM in various clinical settings,including both prenatal and postnatal congenital conditions,as well as various hematological and other malignancies.5–7 Here,we explore the value of OGM in identifying cryptic BCRs using a case series from our clinical practice.This study was approved by the ethics committee of the Nanjing Drum Tower Hospital affiliated with Nanjing University Medical School(no.2021-464-02).

Background: An Intrauterine device (IUD) is a long-acting reversible contraceptive commonly used in clinical practice. Its insertion in the uterus is simple and safe. But sometimes, complications can occur. Case Report: Herein, we report a case of successful laparoscopic removal of a missing IUD Copper-T, mis-inserted in a 32-year-old woman, 4 months after she delivered by caesarian section, and presented persistent lower abdominal pain lasting 6 months. Uterine ultrasound was unremarkable, but a plain abdominopelvic X-ray confirmed the presence of the Copper-T inside the abdominal cavity. Conclusion: Insertion of IUD is simple and safe. When the diagnosis of uterine perforation following its insertion is clinically suspected and radiologically confirmed, laparoscopy, when available, remains one of the best options for removal.

Objective:To compare the effectiveness of an evidence-based bundle in minimizing cesarean section(CS)-related surgical site infections(SSIs)with that of standard routine care.Methods:This was a single-center,parallel,and randomized clinical trial conducted at KAHER’s Dr.Prabhakar Kore Charitable Hospital,Belagavi,India,from January 2021 to December 31,2021.Three hundred twenty women undergoing CS delivery were selected and allocated into two groups(1:1)using simple randomization with fixed allocation.An evidence-based surgical bundle(a combination of more than three perioperative interventions)was used for the intervention group,whereas the control group received routine standard perioperative care.The primary outcome was the incidence of SSI(superficial,deep,and organ or space SSIs)within 6 weeks postpartum in both groups.Categorical and continuous data were analyzed using appropriate statistical tests with a significance threshold set at Ppractice.Implementing the use of this bundle in routine perioperative care will reduce the cost of CS and improve health outcomes in CS.
