Background:Hepatocellular carcinoma(HCC)is the most common cause of cancer-related death in Saudi *** study aimed to investigate the patterns of HCC and the effect of TNM staging,Alfa-fetoprotein(AFP),and Child-Turcotte Pugh(CTP)on patients’overall survival(OS).Methods:A retrospective analysis was conducted on 43 HCC patients at a single oncology center in Saudi Arabia from 2015 to *** patients had to fulfill one of the following criteria:(a)a liver lesion reported as definitive HCC on dynamic imaging and/or(b)a biopsy-confirmed ***:The mean patient age of all HCC cases was 66.8 with a male-to-female ratio of 3.3:*** patients were stratified into two groups:viral HCC(n=22,51%)and non-viral HCC(n=21,49%).Among viral-HCC patients,55%were due to HBV and 45%due to *** was diagnosed in 79%of *** and sex did not significantly statistically differ in OS among viral and non-viral HCC patients(p-value>0.05).About 65%of patients had tumor size>5 cm during the diagnosis,with a significant statistical difference in OS(p-value=0.027).AFP was>400 ng/ml in 45%of the *** was a statistically significant difference in the OS in terms of AFP levels(p-value=0.021).A statistically significant difference was also observed between the CTP score and OS(p-value=0.02).CTP class B had the longest *** was the most common treatment provided to HCC patients followed by sorafenib *** was a significant statistical difference in OS among viral and non-viral HCC patients(p-value=0.008).Conclusions:The most common predictors for OS were the underlying cause of HCC,AFP,and tumor *** having non-viral etiology,a tumor size>5 cm,an AFP>400 ng/mL,and a CTP score class C were all negatively associated with OS.

Effect of Combined Therapy on Colistin Resistant <i>Pseudomonas aeruginosa</i>: An <i>in Vitro</i>Study

Pseudomonas aeruginosa is known for its antibiotic resistance to the clinicians. The infections caused by this pathogen are hard to treat because of its highly versatile property to mutate and acquire drug resistance. Pseudomonas aeruginosa also possesses intrinsic property of resistance to certain antibiotics like tetracyclines. However;in a practice to overcome the problem of multi drug resistance, clinicians restored the use of some antibiotics that were previously been used to treat the Pseudomonal infections;but they were discontinued because of its toxic effects. Colistin is an example of one such antibiotic. Use of Colistin was barred for its neurotoxicity. However in recent clinical trials, Colistin was reintroduced to fight with this superbug. Sadly in recent years, Pseudomonas aeruginosa developed resistance to Colistin as well. Therefore combined therapy is an alternate and suitable treatment to overcome the infections caused by multidrug-resistant Pseudomonas aeruginosa. The present study is an in vitro study;in which we tested synergy between two antibiotics namely streptomycin and Colistin on 29 clinical isolates of P. aeruginosa collected from hospitals in Jazan city KSA. The combination of two drugs showed synergistic activity on 55.1% of tested strains, while 20.6% strains had partial synergy, whereas indifferent synergy was observed in 13.8% strains and the 6.8% of strains had additive synergy. In addition to this, the drugs when combined also showed antagonism on one strain (3.44%). The present study showed synergistic action on Colistin-resistant Pseudomonas aeruginosa to greater extent (55.1%) by the two tested drugs. Hence Colistin and streptomycin can be used as a suitable combination therapy (in vivo) to treat multidrug resistant P. aeruginosa infections.

Sickle Cell Anemia(SCA)is a prevalent genetic condition in Saudi Arabia,particularly in the Jazan *** study’s purpose was to assess the prevalence of de-pression among individuals with SCA in this area and to identify the factors associated with this *** study involved 391 adult participants,all patients in the hematology center of prince mohammed bin nasser hospital(PMNH).Data was collected through an anonymous interview questionnaire–the Arabic version of the Patient Health Questionnaire-9(PHQ-9)–between March–May *** questionnaire covered socio-demographic information and variables related to the participant’s SCA history,such as pain levels,frequency of pain,hospitalization duration,and frequency of blood *** variables underwent Chi-square and One-way ANOVA testing,followed by multivariate *** study found a 42%prevalence of depression among the SCA *** significantly linked with heightened depression levels included being widowed or divorced,experiencing frequent and severe pain,and longer duration of hospital *** educational level of at least a bachelor’s degree was also a significant *** study reveals a high prevalence of depression among SCA patients in the Jazan *** underscores the need for policymakers to educate both physicians and patients about the importance of mental health in SCA *** study also highlights the need for more research into the specific causes and effects of depressive symptoms in SCA patients to inform the creation of effective management plans.

Exploring the Self-Perceived Needs for Family Members Having Adult Critically Ill Loved Person: Descriptive Study

The aim of this study was to explore the self-perceived needs for adult family members having critically ill patient, measure to how extent the needs are met and identify who meet the needs. The study took place in critical care units of the largest governmental hospital in Jordan. The study adopted a descriptive cross-sectional design. The Arabic translation of Critical Care Family Need Inventory was used to investigate the needs of convenient samples of 60 adult family members having critically ill patients. Findings revealed that 16 out of 45 need items on the CCFNI were rated as very important in >90% of the sample. The participants ranked need for assurance, information and proximity as the highest;need for support and comfort as the lowest. Families in this study viewed nurses as the most important source to meet their needs. A holistic approach of nursing management requires that nurses assess family needs using valid and reliable tools and develop intervention strategies to meet the identified needs.

BACKGROUND Incidence and severity variations of inflammatory bowel disease(IBD)have been reported from Western populations between continents and regions of the same ***,no data were available from other *** To investigate the regional differences in the IBD profiles of pediatric patients from the Kingdom of Saudi *** Data from a national multicenter IBD study were *** incidence,time trend,and clinical presentation of Crohn’s disease(CD)and ulcerative colitis(UC)in the Central region(CR),Western region(WR),and Eastern region(ER)were analyzed and *** analysis included Poisson regression analysis for incidence variation and Chi-square test for demographic and clinical parameters.A P<0.05 was considered *** The prevalence of positive family history was lower in children with CD from the ER than the CR or the *** rate was higher in children with CD and UC from the CR and the ER,*** incidences and time trends of CD and UC were not significantly different between *** the ER,a significantly higher percentage of children with CD presented with abdominal pain(P<0.001),blood in stools(P=0.048),stricturing or penetrating disease(P=0.029),higher erythrocyte sedimentation rate(P<0.001),higher C-reactive protein(P<0.001),higher anemia(P=0.017),and lower albumin level(P=0.014).For children with UC from the ER,a significantly higher percentage presented with anemia(P=0.006)and a lower percentage with pancolitis(P<0.001).CONCLUSION The most important finding is the identification of significantly more severe presentation of CD in the ER of the Kingdom of Saudi *** studies are needed to explain such variations.

Background: The need to establish a strong culture around radiation safety is derived from the assertion that medical practitioners, patients, and third parties should not be exposed to unnecessary radiation. Authorities have endeavored to enact policies to protect all employers and patients in radiology departments. Objectives: To assess the impact of radiation safety practices and regulations on the ongoing improvements in radiation safety culture and practices in radiology departments. This will be achieved through a subjective assessment of national and international rules and regulations by healthcare professionals. Materials and Methods: We conducted a questionnaire survey in the radiology departments of three JCI-accredited hospitals in the Riyadh region to identify and assess the impact of national radiation regulations and the accompanying processes on the improvement of radiation safety culture and practices in radiology departments. Results: There were statistically significant differences in the grading system results among various groups of respondents, based on their educational level. Also, there are statistically significant differences between the assessments of safety level results in the answers provided by various groups of respondents according to education level in favor of the master’s degree. Conclusion: The study concludes that technicians with a diploma degree require stricter regulation. Furthermore, the results of this study suggest that an exposure tracking system and a regulatory action supporting it may be useful in the ongoing task of improving patients’ radiation safety.

AIM: To assess the prevalence of nutritional disorders in children with inflammatory bowel disease(IBD) in Saudi ***: The data from a national cohort of children newly diagnosed with IBD between 2003 and 2012 were analyzed. The diagnosis of IBD and the differentiation between Crohn's disease(CD) and ulcerative colitis(UC) were confirmed by gastroenterologists according to the standard criteria. The body mass index(BMI) of each child [weight(kg)/height^2(m)] was calculated at the time of diagnosis. The World Health Organization standards and references were used and the BMI for age > +1 and <-2 standard deviation score were used to define overweight and thinness, respectively. Age stratification analysis was performed to investigate any age-related variation in the prevalence of nutritional status between children < 10 years of age and ***: There were 374 children from 0.33 to 17 years of age, including 119(32%) children with UC and 255(68%) with CD. All of the children were Saudi nationals, and 68(57%) of the UC and 150(59%) of the CD children were males. A positive history of anorexia at the time of diagnosis was found in 30(25%) patients with UC and 99(39%) patients with CD. The prevalence of thinness was 31%, 35% and 24% in children with IBD, CD and UC, respectively, with a significantly higher prevalence of thinness in children with CD than in children with UC(P = 0.037) only in the age group of 10-17 years(P = 0.030). The prevalence of overweight was 16 %, 15% and 20 % in the children with IBD, CD and UC, respectively, indicating a higher prevalence in UC that was statistically significant only in the age group of 10-17 years(P = 0.020). CONCLUSION: A high proportion of children with IBD presented with overweight instead of the classical underweight. Awareness of this finding is important for patient care.

AIM To assess the association of inter-ethnic vs intra-ethnic marriage with severity of coronary artery disease(CAD) in men undergoing *** We conducted a prospective multicenter,multi-ethnic,cross sectional observational study at five hospitals in Saudi Arabia and the United Arab Emirates,in which we used logistic regression analysis with and without adjustment for baseline differences. RESULTS Data were collected for 1068 enrolled patients undergoing coronary angiography for clinical indications during the period of April 1^(st),2013 to March 30^(th),2014. Ethnicities of spouses were available only for male patients. Of those enrolled,687 were married men and constituted the cohort for the present analysis. Intra-ethnic marriages were reported in 70% and inter-ethnic marriages in 30%. After adjusting for baseline differences,interethnic marriage was associated with lower odds of having significant CAD [adjusted odds ratio 0.52(95%CI:0.33,0.81)] or multi-vessel disease(MVD) [adjusted odds ratio 0.57(95%CI:0.37,0.86)]. The adjusted association with left main disease showed a similar trend,but was not statistically significant [adjusted odds ratio 0.74(95%CI:0.41,1.32)]. The association between interethnic marriage and the presence of significant CAD and MVD was not modified by number of concurrent wives(P interaction > 0.05 for both).CONCLUSION Among married men undergoing coronary angiography,inter-ethnic,as compared to intra-ethnic,marriage is associated with lower odds of significant CAD and MVD.
