Advanced trauma life support training: How useful it is?

We have tried in a recently published systematic review(World J of Surg 2014; 38: 322-329) to study the educational value of advanced trauma life support(ATLS) courses and whether they improve survival of multiple trauma patients. This Frontier article summarizes whatwe have learned and reflects on future perspectives in this important area. Our recently published systematic review has shown that ATLS training is very useful from an educational point view. It significantly increased knowledge, and improved practical skills and the critical decision making process in managing multiple trauma patients. These positive changes were evident in a wide range of learners including undergraduate medical students and postgraduate residents from different subspecialties. In contrast, clear evidence that ATLS training reduces trauma death is lacking. It is obvious that it is almost impossible to perform randomized controlled trials to study the effect of ATLS courses on trauma mortality. Studying factors predicting trauma mortality is a very complex issue. Accordingly, trauma mortality does not depend solely on ATLS training but on other important factors, like presence of well-developed trauma systems including advanced pre-hospital care. We think that the way to answer whether ATLS training improves survival is to perform large prospective cohort studies of high quality data and use advanced statistical modelling.

Multiple myeloma(MM) is still an incurable hematologic malignancy, which is eagerly to the discovery of novel therapeutic targets and methods. N-acetyltransferase 10(NAT10) is the first reported regulator of mRNA acetylation that is activated in many cancers. However, the function of NAT10 in MM remains unclear. We found significant upregulation of NAT10 in MM patients compared to normal plasma cells, which was also highly correlated with MM poor outcome. Further enforced NAT10 expression promoted MM growth in vitro and in vivo, while knockdown of NAT10 reversed those effects. The correlation analysis of acetylated RNA immunoprecipitation sequencing(ac RIP-seq) and ribosome profiling sequencing(Ribo-seq) combined with RIP-PCR tests identified centrosomal protein 170(CEP170) as an important downstream target of NAT10. Interfering CEP170 expression in NAT10-OE cells attenuated the acceleration of cellular growth caused by elevated NAT10. Moreover,CEP170 overexpression promoted cellular proliferation and chromosomal instability(CIN) in ***, remodelin, a selective NAT10 inhibitor, suppressed MM cellular growth, induced cellular apoptosis in vitro and prolonged the survival of 5TMM3VT mice in vivo. Collectively, our data indicate that NAT10 acetylates CEP170 mRNA to enhance CEP170 translation efficiency, which suggests that NAT10 may serve as a promising therapeutic target in MM.

Inland water systems are regarded as a pathway and sink of plastic pollutants from the terrestrial *** ecosystems are globally contaminated with microplastics,but the spatiotemporal occurrence and density of microplastics in freshwater ecosystems remain poorly *** present study seasonally assessed differences in microplastic density in the sediments from two South African recreational reservoirs associated with low human activities(macadamia orchards)and high human activities(communal areas).Microplastics were recovered from all of the reservoirs assessed,indicating their extensive occurrence and *** numbers were significantly higher in reservoirs associated with high anthropogenic activities during the hot-dry season(140.6 particles kg^(-1)dwt)and lower in reservoirs associated with low anthropogenic activities during the hot-wet and cool-dry seasons,i.e.,22.60 particles kg^(-1)dwt and 16.13 particles kg^(-1)dwt,***,polypropylene(31%)and polystyrene(30%)were identified as the dominant types of microplastic polymer in both reservoir ***,no correlative relationships were observed for environmental parameters and microplastic densities across reservoirs and seasons,suggesting a widespread and largely context-independent pollution *** results show that microplastics are not evenly distributed between waters associated with low human activities and high human *** work should seek greater sample sizes and centre around observing microplastic contamination in the region by examining their sources,transport,and impacts to freshwater environments,whilst informing management strategies.

Objective Our objective was to describe the postgraduate education trajectories of family and community physicians in Brazil,where neither primary healthcare nor family and community medicine is recognised as a knowledge area for the purpose of research and postgraduate education(master’s and PhD degrees).Design An observational,exploratory study,using administrative data.A nationwide list of family and community physicians as of late November 2018 was compiled from multiple *** on the mode of specialisation was obtained from the same sources and were correlated with data on master’s and PhD degrees,obtained from the curricula vitae on the Lattes *** This study was set in *** 6238 family and community physicians(58.3%female),of whom 2795 had earned a specialist certificate(identified from the list of physicians certified by Sociedade Brasileira de Medicina de Família e Comunidade)and 3957 had completed medical residency(identified from SisCNRM,the national information system for medical residency).Results A master’s degree was held by 747(12.0%)family and community physicians,and a PhD by 170(2.7%);most degrees were in collective health(47.0%and 42%,respectively).Men were more likely than women to hold a master’s degree(adjusted odds ratio(aOR)1.24,95%uncertainty interval(UI)1.07-1.45)and even more likely to a hold PhD(aOR 1.86,95%UI 1.35-2.59).Family and community physicians were also less likely to hold a PhD degree if their master’s degree was professional(oriented towards jobs outside academia)instead of academic(aOR 0.15,95%UI 0.05-0.39)or in some area other than collective health or medicine(aOR 0.41,95%UI 0.21-0.78,compared with a master’s degree in collective health).The postgraduate degree was more likely to precede specialisation for family and community physicians specialising through certification(master’s degree 39.9%,PhD 33%)than through medical residency(master’s degree 9.1%,PhD 6%).Conclusion Family and community

热处理是金属材料热机械加工的常用手段.随着热处理温度的升高而湮灭的缺陷通常导致材料的塑性提升而强度降低.本研究中,我们通过提高热处理温度促进相溶解而协同提升了TiZrNbTa高熵合金的强度和塑性.当热处理温度从800提升至1250°C,合金的拉伸屈服强度提高了40%,达到1003±16 MPa.同时,合金的伸长率增加了近一倍,达到16.79%±1.03%.热处理温度提升引起的相溶解加剧了晶格畸变,从而增强了晶格摩擦应力并提升了屈服强度.相溶解也降低了界面失配并缓解了应力集中.此外,1250°C热处理合金中的局部化学有序结构促进了位错共平面滑移和位错增殖.两种机制共同提升了合金的塑性.该研究不仅扩展了关于金属材料中热处理和相溶解的理解,而且也为合金的强韧化设计提供了思路.

Background Major infectious disease has become a serious threat to people’s health *** the world’s largest healthcare workforce and the core forces fighting against the epidemic,nurses are on the frontline of this battle.A number of ethical issues have given rise to numerous concerns that have largely affected nurses in different ways as they respond to the *** addition,excessive expectations from people can exert undue pressure,which can easily lead to burnout in *** In this consensus,the expert panel method was used to develop and reach a *** members involved in the formation of the consensus included an expert discussion panel and a consensus writing expert group,a methodologist,and four *** 16 rounds of online expert consultation and two rounds of expert panel meetings,the writing team analyzed and reviewed the 78 amendments suggested by the experts to develop a consensus on nursing ethics for prevention and control of major infectious disease outbreaks based on the ethical vision of life *** This expert consensus focuses on five essential domains:the responsibilities and rights of nurses,the nurse-patient relationship,the doctor-nurse relationship,and the relationship between society and nurses throughout the *** We hope this consensus can help nurses better understand and respond to the ethical issues and challenges in public health emergencies,and raise reasonable public expectations of the roles and responsibilities of nurses in these situations.
