This article explores from an intermodal perspective how visual and verbal historical documents contribute to the construction of value positions regarding the Mapuche people during colonial and early republican times in the latest official 5 thgrade primary chilean History *** study analyzes,in particular,the visual and verbal historical secondary sources that build in combination evaluative prosodies that in turn contribute to construct determinate ideological perspectives regarding Mapuche *** discourse analysis takes a social and ideological intermodal approach(Oteíza&Pinuer 2016,2019)focusing on an appraisal analysis(Martin&White 2005;Martin2014;Hood 2010;Oteíza&Pinuer 2012,2019)informed by socio-semiotic and Systemic Functional Linguistics *** cumulative intersemiotic reading of symbolic images and written documents privileges evaluative prosodies of negative integrity and of high conflict,in which Mapuche people are represented with more agency than Hispanics,emphasizing the stereotype of indigenous people as foreign,exotic and brave warriors,but also as violent,primitive and savage people that have had resisted Hispanic desires of slavering and cultural *** secondary sources offer alternative historical interpretations,although mainly constructed by evoked evaluations that could be less evident to children.

This report presents our experience with T therapy in a cohort of T-deficient men on active surveillance （AS） for Gleason 3 ＋ 3 and Gleason 3 ＋ 4 prostate cancer （PCa）. A retrospective chart review identified 28 men with T deficiency who underwent T therapy （T group） for at least 6 months while on AS for PCa. A comparison group of 96 men on AS for PCa with untreated T deficiency （no-T group） was identified at the same institution. The AS protocol followed a modified Epstein criteria and allowed inclusion of men with a single core of low-volume Gleason 3 ＋ 4 PCa. Mean age was 59.5 and 61.3 years, and mean follow-up was 38.9 and 42.4 months for the T and no-T groups, respectively. Of all 28 men in the T group, 3 （10.7%） men developed an increase in Gleason score while on AS. Of 22 men in the T group with Gleason 3 ＋ 3 disease, 7 （31.8%） men developed biopsy progression including 3 men （13.6%） who developed Gleason 3 ＋ 4 PCa. Of 6 men with Gleason 3 ＋ 4 disease at baseline, 2 （33.3%） men developed an increase in tumor volume, and none developed upgrading beyond Gleason 3 ＋ 4. All 96 men in the no-T group had Gleason 3 ＋ 3 disease at baseline and, 43 （44.7%） developed biopsy progression, including 9 men （9.38%） with upgrading to Gleason 7 （3 ＋ 4）. Biopsy progression rates were similar for both groups and historical controls. Biopsy progression in men on AS appears unaffected by T therapy over 3 years. Prospective placebo-controlled trials of T therapy in T-deficient men on AS should be considered given the symptomatic benefits experienced by treated men.

Iron formations are valuable archives of sedimentary conditions and post-depositional ***,geochemical proxies commonly used to determine genetic characteristics can be variably modified during metamorphism and deformation,hampering their use as records of regional geological *** work focuses on strongly reworked magnetite-quartz-rich rocks from the São Josédo Campestre Massif,one of the oldest fragments of preserved crust in South *** genetic classification of these magnetite-quartz-rich rocks is not straightforward because primary assemblages and textures were variably modified by granulite facies metamorphism during a regional Paleoproterozoic migmatization *** address genetic ambiguities,we analyzed their magnetite and pyroxene chemistry,wholerock geochemistry,and Sm-Nd *** chemistry indicates that pyroxene-poor iron formations(Type B)are low in trace elements such as Ti,Al,V,and Mn,suggesting a chemical similarity to iron formations *** contrast,magnetites from pyroxene-enriched Type A iron formations are rich in trace elements and more akin to magnetite crystallized from higher temperature systems,such as skarn and ***^(147)Sm/^(144)Nd of these rocks show substantial variation even at the outcrop scale,indicating a locally-controlled,highly heterogeneous mixture of Archean,Paleoproterozoic,and Neoproterozoic ***,our geochemical tools point out to heterogenous signatures of these magnetitequartz rocks and proxies compatible with both low and high-temperature conditions and age of deposition spanning sources from the Archean to the *** interpret that the studied São Josédo Campestre magnetite-quartz rocks represent Archean iron formations with original magnetite chemistry and isotopic signatures variably modified by metamorphism and by at least one deformation-related hydrothermal *** results contrast with similar examples from China and Greenland where iron formation

The purpose of this article is to analyze disaster risk governance through assemblage theory, identifying how-during the altered political context of a military regime with a centralized disaster risk management as in the case of chile in 1985-new actors emerge during the disaster response phase as a de/reterritorialization effect that is influenced by their agencies and relationships, disfiguring the edges of the assemblage. Based on this conceptualization, it is possible to investigate the interactions between the different actors, their power relations, and their reconfigurations in the governance exercise. For this purpose, we reviewed the response phase of the 1985 San Antonio earthquake that affected the central zone of chile, where strategic functions, institutions, and forms of power are *** describe and visualize the actors during the response phase in the disaster risk governance framework, a map of actors was developed that identifies the existing relationships and their different weights. The central scale proved to be dominant and occupied a political space that was transfigured by its overrepresentation-enforced by allies such as the banking system and business associations-enhancing a neoliberal agenda. The leaps in scale from the central scale to the local scale cancel agency of the last, destabilizing its capacity to deal with the effects of the earthquake and isolating it from the decision-making processes. Consequently,delays in providing aid demonstrate that authoritarian governments do not provide better management in the disaster response phase.

Introduction: Activated partial thromboplastin time (aPTT) is one of the most used coagulation tests in preoperative evaluation. Incidental detection of a prolonged aPTT is a problem in primary care, in which the general pediatrician should be able to attend its initial management. Objective: To describe final diagnosis of patients with prolonged aPTT in preoperative study. Materials and Methods: This is a descriptive study of patients referred from otorhinolaryngology. Results: Totally, 508 adenoidectomies and/or tonsillectomies were performed in our center, 38 of which referred patients (7.5%) with prolonged aPTT, and 30 of which met inclusion criteria. The median age was 4 years. 56.6% of patients were males. 76.6% of patients normalized aPTT at the second follow-up. Among these, 73.9% showed a normal study, 4.3% ha2d lupus anticoagulant and in 21.7% Von Willebrand disease was detected. Among patients that persisted with prolonged aPTT, 42.8% had coagulant factors deficiency, 28.5% had lupus anticoagulant and in 28.5% of patients a diagnosis could not be achieved with the tests used in the present study. Multivariate analysis did not show correlation between final diagnosis and the variables measured. Conclusion: The presence of a prolonged aPTT in children under preoperative study is due to a pre-analytic factor in the majority of cases or to the presence of lupus anticoagulant, normalizing values on follow-up. We suggest that a new aPTT be performed on these patients, and only those that persist altered or present a symptoms and family history of coagulation disorders be referred to hematology.

Natural History of Symptomatic Meniscal Tears: Arthroscopic Evaluation of Chondral Damage at 4-Year Minimum

Background: The natural history of meniscal tears is unclear. Studies have tried to clarify many aspects. Association with chondral injuries are of main relevance to the long-term status of the knee but evidence is limited. Objective: The aim of this study is to describe the intraarticular state of the knee focused on chondral injury in patients with persistent symptomatic meniscal tears that were untreated for a minimum period of 4 years. Methods: A total of 47 patients with symptomatic meniscal tears in whom surgery was indicated but was delayed mainly for administrative reasons, were recruited between January 1st, 2004 and April 30th, 2010 in a regional hospital. Follow up ended until circumstances allowed surgical resolution through arthroscopy. Only patients with meniscal tear in which diagnosis was confirmed and remained symptomatic were included. Patients with initial grade IV Kellgren-Lawrence osteo-arthritis were excluded. Statistical chi-square and logistic regression analysis were used. Results: Mean follow up period was 5.8 years (range: 4.3 - 10.2 years). 57 meniscal injuries were found in 47 patients. 25 were lateral and 32 medial meniscal tears. Overall, 29 patients had articular cartilage damage (51%). Chondral injuries were found in 15 of 25 lateral meniscal lesions (60%) and 14 of 32 medial lesions (44%). Patients with lateral compartment meniscal injury had a relative risk (RR) of 2.5 of developing chondral injury (p-value 0.05). Compared to 51% of patients with associated chondral and meniscal damage (n = 29), only 12% of patients with healthy meniscus (n = 7) had chondral injury (p-value 0.05). Conclusion: Persistent symptomatic meniscal tears after medium to long term (4 to 10 years) are associated with chondral damage in 51% of patients. Chondral damage associated with meniscal tears is usually deep and involves femur and tibial sides. Background: Knee joint effusion is a common adverse event after arthroscopic procedures and its prevention is crucial to achieve better clinical outcomes, such as pain relief, improved range of movement and global satisfaction. Objective: The aim of this study is to compare joint effusion after partial arthroscopic meniscectomy between a common postoperative management and a short period in deep flexion. Methods: A prospective study randomizes 62 patients who underwent arthroscopic partial meniscectomy into 2 groups of postoperative care: compression elastic bandage with elevation for 24 hours (compression group) versus immediate postoperative deep flexion for 2 hours (flexion group). Clinical evaluation of effusion was made at 24 hours, 7 days and 1-month post-surgery. Results: Overall at 24 hours patients showed 53% absent effusion, 35% minimum, 11% had mild or more severe effusion. Effusion rates in the compression group were 13%, 10% and 0%;in the flexion group 10%, 6.4% and 0%, at 24 hours, 7 days and 1 month, respectively. There was no statistically significant difference between them (p > 0.05). Conclusion: In the studied population, position of the knee in deep flexion had similar influence in effusion outcomes than a standard widely used treatment, offering an alternative in the postoperative care of the patients.

Background There are different definitions to identify/classify children with medical complexity(CMC).We aimed to investigate and describe the definitions used to classify CMC in epidemiological *** PubMed,SciELO,LILACS,and EMBASE were searched from 2015 to 2020(last updated September 15th,2020)for original studies that presented the definition used to classify/identify CMC in the scientific research *** applied the Preferred Reporting Items for Systematic Reviews and Meta-Analyses *** the included studies,the following were identified:first author,year of publication,design,population,study period,the definition of CMC used,limitations,and *** Nine hundred and sixty-seven records were identified in the searched databases,and 42 met the inclusion *** the 42 studies included,the four most frequent definitions used in the articles included in this review were classification of CMC into nine diagnostic categories based on the International Classification of Diseases,Ninth Revision(ICD-9)(35.7%,15 articles);update of the previous classification for ICD-10 codes with the inclusion of other conditions in the definition(21.4%,nine articles);definition based on a medical complexity algorithm for classification(16.7%,seven articles);and a risk rating system(7.1%,three articles).Conclusions CMC definitions using diagnostic codes were more ***,several limitations were found in its *** research highlighted the need to improve health information systems to accurately characterize the CMC population and promote the provision of comprehensive care.
