Patient survival and limb salvage after shark attack with major vascular injury:A case report

Shark attacks are rare unique pathological *** of them represent devastating injuries with a high morbidity and significant *** published articles are *** increased human interaction within the environment of sharks is the cause of rising incidence of such *** study reported a case of level 4 shark injuries(shark-induced trauma scale)in a 33-year-old male patient,who presented with an extensive injury of the right lower limb with the characteristic features of shark *** admission the patient was in a state of shock with profuse bleeding that was controlled by *** patient was resuscitated according to the advanced trauma life *** examination showed hard signs of vascular injury with absent pedal pulse,associated neurological deficits and severance at the knee *** vascular intervention after resuscitation was performed to manage the major vascular injuries,together with proper washout and debridement of all the necrotic tissues under strong antibiotic coverage to prevent *** that,the patient underwent sequenced plastic,orthopedic,and neurological *** follow-up was conducted,which showed that the patient was saved and achieved a functioning *** study aims to highlight the management of level 4 shark injuries,which are considered serious and challenging with a high fatality rate and a great risk of amputation due to the associated major vascular *** well organized management plan is *** resuscitation and surgical intervention by a highly-skilled medical team are required to improve the chance of patient survival and limb salvage.

Management and Outcome of Hyperemesis Gravidarum at Tertiary Obstetric Facility, Khartoum-Sudan

Introduction: Hyperemesis gravidarum (HG) is vomiting and/or nausea in pregnancy causes dehydration, electrolytes imbalance, weight loss and further serious organs dysfunction. Methods: This was a descriptive, prospective, cross-sectional hospital based total coverage analytic study, which was conducted from November 2013 to May 2014 at Omdurman New hospital (ONH) for Obstetrics & Gynecology, Khartoum-Sudan. Results: HG prevalence is 13% of complicated pregnancy of ONH admission. Acetonurea is reported in all cases with significant association between acetonurea and smoking (P value = 0.005). A significant association between Hemoglobulin level and the readmission the (P value = 0.01) was reported. One maternal death is reported from severe hypoglycemia and liver impairment. Conclusion: Hyperemesis gravidarum has serious maternal morbidity with social negative impacts and significant financial burden on the health services.

Pattern of Potential Laboratory Markers for COVID-19 in Eastern Sudan

Background: Coronavirus disease 2019 (COVID-19) is a recent global health crisis. One of the major issues of COVID-19 is its unpredictable manifestations and serious outcomes. Many hematological parameters are thought to change dramatically during the course of the disease. These include white blood cells, red blood cells, and platelets. This study aimed at evaluating certain laboratory results;peripheral blood lymphopenia, relative neutrophilia, high neutrophil-lymphocyte ratio, and elevated C-reactive protein as potential laboratory markers of COVID-19 in Eastern Sudanese patients. Methods: We, retrospectively, aimed at the evaluation of peripheral blood leucocytes count, neutrophil-lymphocyte ratio NLR and C-reactive protein (CRP) levels in confirmed COVID-19 eastern Sudanese patients during the course of the disease. Results: The mean total leucocytes count, % neutrophils count, absolute neutrophils count and C-reactive protein (CRP) were significantly higher (P. value = 0.000) in COVID-19 patients than in the control group while the mean % lymphocytes count and % mixed cells count were found to be significantly lower in COVID-19 patients than in the control group (P. value 0.000). Conclusion: Peripheral blood leucocyte alterations (simultaneous presence of lymphopenia, relative neutrophilia and high neutrophil lymphocyte ratio (NLR) along with elevated CRP levels may be valuable biomarkers associated with COVID-19 in Port Sudan city, red sea state, Sudan. These markers might be important in prediction, inspection of disease progression and prognosis.
