Modulation of graft vascular inflow guided by flowmetry and manometry in liver transplantation

BACKGROUND:Survival of the partial graft after living donor liver transplantation owes much to its tremendous regenerative *** excellent venous outflow capacity,a graft within a wide range of graft-to-standard-liver-volume ratios can cope with portal hypertension that is common in liver transplant ***,when the ratio range is exceeded,modulation of graft vascular inflow becomes necessary for graft *** interplay between graft-to-standard-liver-volume ratio and portal pressure,in the presence of portosystemic shunt or otherwise,requires individualized modulation of graft portal and arterial *** of portal inflow by shunt ligation can be guided by transonic flowmetry,whereas muting of portal inflow by splenic artery ligation can be monitored by portal electronic ***:We describe four cases to illustrate the ***:One patient had hepatic artery thrombosis resulting from splenic artery steal syndrome which was the sequela of small-for-size *** splenic artery ligation and re-anastomosis of the hepatic artery successfully muted the portal inflow and boosted the hepatic arterial *** patient with portal vein thrombosis underwent *** inflow was boosted with ligation of portosystemic shunt,which is often present in these patients with portal *** coexistence of splenic aneurysm and splenorenal shunt required ligation of both in the third *** fourth patient,with portal pressure and flow monitoring,avoided ligation of a coronary vein which became a main portal inflow after portal ***:Management of graft inflow modulation guided selectively by transonic flowmetry or portal manometry was described.

The impact of family history of hepatocellular carcinoma on its patients' survival

BACKGROUND:Family history of hepatocellular carcinoma(HCC) has been identified as a risk factor for the development of the *** aim of this study is to evaluate the impact of such a history on HCC patients’ ***:Data of all HCC patients(n=4532) managed at our center from 1989 to 2008 were prospectively *** patients were quizzed on their various characteristics including family HCC ***:Totally 475(10.48%) patients had a family history of *** presented the disease at a significantly earlier age(median 53 vs 59 years,P<0.0001) and at an earlier stage(the United Network for Organ Sharing staging system).They had significantly better liver function in terms of ChildPugh classification and serum albumin and bilirubin *** more of them presented the disease without symptoms(44.0% vs 29.4%,P<0.0001).They also had significantly better overall survival under these specifications:patients in the whole study cohort,patients who had minor hepatectomy,patients with stage I disease,patients with stage II disease,and patients with stage III ***:Contrary to what is generally believed,we found in this study cohort that patients with a family history of HCC had better overall survival than those without such a *** believe this was in part due to earlier diagnosis of the disease and better liver function in this group of ***,the effects of genetic factors on the risk of HCC cannot be overlooked and are yet to be identified.

Outcomes of side-to-side conversion hepaticojejunostomy for biliary anastomotic stricture after right-liver living donor liver transplantation

BACKGROUND:Conversion hepaticojejunostomy is considered the salvage intervention for biliary anastomotic stricture,a common complication of right-liver living donor liver transplantation with duct-to-duct anastomosis,after failed endoscopic *** aim of this study is to compare the outcomes of side-to-side hepaticojejunostomy with those of endto-side ***:Prospectively collected data of 402 adult patients who had undergone right-liver living donor liver transplantation with duct-to-duct anastomosis were *** of biliary anastomotic stricture was made based on clinical,biochemical,histological and radiological *** treatment was the first-line treatment of biliary anastomotic ***:Interventional radiological or endoscopic treatment failed to correct the biliary anastomotic stricture in 13 patients,so they underwent conversion *** of them received end-to-side hepaticojejunostomy and three received side-to-side *** the end-to-side group,two patients sustained hepatic artery injury requiring repeated microvascular anastomosis,two developed restenosis requiring further percutaneous transhepatic biliary drainage and balloon dilatation,and two required revision *** the side-to-side group,one patient developed re-stenosis requiring further endoscopic retrograde cholangiography and balloon *** re-operation was needed in this ***,outcomes in the two groups were similar in terms of liver function and graft ***:Despite the similar outcomes,side-to-side hepaticojejunostomy may be a better option for bile duct reconstruction after failed interventional radiological or endoscopic treatment because it can decrease the chance of hepatic artery injury and allows future endoscopic treatment if re-stricture ***,more large-scale studies are warranted to validate the results.

Indoleamine 2, 3-dioxygenase (IDO) is a rate-limiting enzyme for the tryptophan catabolism. In human and murine cells, IDO inhibits antigen-specific T cell proliferation in vitro and suppresses T cell responses to fetal alloantigens during murine pregnancy. In mice, IDO expression is an inducible feature of specific subsets of dendritic cells (DCs), and is important for T cell regulatory properties. However, the effect of IDO and tryptophan deprivation on DC func- tions remains unknown. We report here that when tryptophan utilization was prevented by a pharmacological inhibitor of IDO, 1-methyl tryptophan (1MT), DC activation induced by pathogenic stimulus lipopolysaccharide (LPS) or inflam- matory cytokine TNF-α was inhibited both phenotypically and functionally. Such an effect was less remarkable when DC was stimulated by a physiological stimulus, CD40 ligand. Tryptophan deprivation during DC activation also regu- lated the expression of CCR5 and CXCR4, as well as DC responsiveness to chemokines. These results suggest that tryptophan usage in the microenvironment is essential for DC maturation, and may also play a role in the regulation of DC migratory behaviors.

Ultrafast laser-scanning time-stretch imaging at visible wavelengths

Optical time-stretch imaging enables the continuous capture of non-repetitive events in real time at a line-scan rate of tens of MHz—a distinct advantage for the ultrafast dynamics monitoring and high-throughput screening that are widely needed in biological ***,its potential is limited by the technical challenge of achieving significant pulse stretching(that is,high temporal dispersion)and low optical loss,which are the critical factors influencing imaging quality,in the visible spectrum demanded in many of these *** present a new pulse-stretching technique,termed free-space angular-chirpenhanced delay(FACED),with three distinguishing features absent in the prevailing dispersive-fiber-based implementations:(1)it generates substantial,reconfigurable temporal dispersion in free space(41 ns nm^(−1))with low intrinsic loss(o6 dB)at visible wavelengths;(2)its wavelength-invariant pulse-stretching operation introduces a new paradigm in time-stretch imaging,which can now be implemented both with and without spectral encoding;and(3)pulse stretching in FACED inherently provides an ultrafast all-optical laser-beam scanning mechanism at a line-scan rate of tens of *** FACED,we demonstrate not only ultrafast laser-scanning time-stretch imaging with superior bright-field image quality compared with previous work but also,for the first time,MHz fluorescence and colorized time-stretch *** results show that this technique could enable a wider scope of applications in high-speed and high-throughput biological microscopy that were once out of reach.

On the Light Curve and Spectra of X-Rays and Gamma-Rays from the Crab Pulsar

We use a three-dimensional pulsar magnetosphere model to study the light curve and spectra of x-rays and gamma-rays from the Crab pulsar. In this model, the vertical size of the outer gap is first determined by a self-consistent model in which the outer gap is limited by pair production from collisions of thermal photons produced by polar cap heating of backflow outer gap current and curvature photons emitted by gap accelerated charged particles. The transverse size of the outer gap is determined by local pair production conditions. In principle, there are two topologically disconnected outer gaps present in the magnetosphere of a pulsar, and both incoming and outgoing particle flows are allowed. However, double-peak light curves with strong bridges are most common, Making use of the three-dimensional structure of the outer gap and its local properties, we compare the results of our model with the light curve and phase-resolved spectra of the Crab pulsar.

Synthesis of Donor-Acceptorπ-Conjugated Macrocycles by Post-Functionalization

Main observation and conclusion In this paper,two novel donor-acceptor(D-A)macrocyclic compounds 5 and 7 were successfully synthesized by post-functionalization of a planar dimer macrocycle(1)and a highly twisted trimer macrocycle(2),respectively,via controllable oxidation of methoxy groups and condensation between diketones and phenylenediamine in *** with unembellished 9,10-dimethoxyphenanthrene,the resultant dibenzo[a,c]phenazine motif is electron-deficient rather than electron-rich,yielding two D-Aπ-conjugated macrocycles with more contractive energy *** functional theory(DFT)calculations further support the unequivocal evidence of the D-A properties of macrocycles 5 and *** difference of their photophysical properties was investigated by the aid of ultraviolet-visible absorption and fluorescence ***,the stacking patterns of 7 and its contrastπ-conjugated macrocycle trimer(2)were compared via the analysis of X-ray single crystal,in which the electron-deficient dibenzo[a,c]phenazine unit provides extra interaction sites,resulting in orderly arrangement of 7 in the solid state.

In the low-frequency fast multipole algorithm(LF-FMA)[19,20],scalar addition theorem has been used to factorize the scalar Green’s *** of this traditional factorization of the scalar Green’s function by using scalar addition theorem,we adopt the vector addition theorem for the factorization of the dyadic Green’s function to realize memory *** are to validate this factorization and use it to develop a low-frequency vector fast multipole algorithm(LF-VFMA)for lowfrequency *** the calculation of non-near neighbor interactions,the storage of translators in the method is larger than that in the LF-FMA with scalar addition *** it is independent of the number of ***,the storage of radiation and receiving patterns is linearly dependent on the number of *** it is worthwhile for large scale problems to reduce the storage of this *** this method,the storage of radiation and receiving patterns can be reduced by 25 percent compared with the LF-FMA.

Circular neighbor-balanced designs universally optimal for total effects

In many experiments, the performance of a subject may be affected by some previous treatments applied to it apart from the current treatment. This motivates the studies of the residual effects of the treatments in a block design. This paper shows that a circular block design neighbor-balanced at distances up toγ≤k - 1, where k is the block size, is universally optimal for total effects under the linear models containing the neighbor effects at distances up toγamong the class of all circular binary block designs. Some combinatorial approaches to constructing these circular block designs neighbor-balanced at distances up to k - 1 are provided.
