Association between vitamin D receptor polymorphisms and multiple sclerosis： systematic review and meta-analysis of case-control studies

Vitamin D receptor （VDR） polymorphisms have been studied as potential contributors to multiple sclerosis （MS）. However, published studies differ with respect to study design and the significance of the effects detected. The aim of this study was to quantify the magnitude of the risk associated with the Taql, Bsml, Apal and Fold VDR polymorphisms in MS using a meta-analysis approach. Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, we conducted a systematic search and meta-analysis of the literature. Subgroup analyses were performed to detect potential sources of heterogeneity from the selected study characteristics. The stability of the summary risk was evaluated using sensitivity analyses. The meta-analysis included a total of 3300 cases and 3194 controls from 13 case- control studies. There were no significant associations found between Taql and Bsml polymorphisms and MS risk. The association between the Apal polymorphism and MS risk was significant in the homozygous and codominant models （P=0.013 and P=0.031, respectively）, suggesting that the AA Apal genotype might be a significant MS risk factor. Publication year and age significantly affected the association between Taql polymorphisms and MS （P=0.014 and P=0.010, respectively）, which indicates a protective effect of the major T allele. The AA Apal and FF Fold genotypes are significant risk factors for MS. The association between the Taql polymorphism and MS risk is significantly affected by study characteristics.

Continuous respiratory monitoring for sleep apnea screening by ambulatory hemodynamic monitor

AIM:To validate the sleep-disordered breathing components of a portable electrocardiography and hemodynamic monitor to be used for sleep apnea ***:Sleep-disordered breathing(SDB) is associated with cardiovascular *** with existing cardiovascular disease may have unrecognized SDB or may develop SDB while under the care of a cardiologist.A screening device for SDB,easy to use and appealing to cardiologists,would assist in referral of appropriate patients for full polysomnography(PSG).A cardiac and respiratory monitor(CPAM) was attached to patients undergoing PSG and an apnea/hypopnea index(AHI) *** CPAM device produced respiration rate,snoring rate,individual apnea/hypopnea events and an SDB severity score(SDBSS).In addition to AHI,an expert over-reader annotated individual breaths,snores and SDB breathing events to which the automated algorithms were ***:The test set consisted of data from 85 patients(age:50.5 ± 12.4 years).Of these,57 had a positive PSG defined as AHI ≥ 5.0(mean:30.0 ± 29.8,negative group mean:1.5 ± 1.2).The sensitivity and specificity of the SDBSS compared to AHI was 57.9% and 89.3%,*** correlation of snoring rate by CPAM compared to the expert overreader was r = 0.58(mean error:1.52 snores/min),while the automated respiration rate had a correlation of r = 0.90(mean error:0.70 breaths/min).CONCLUSION:This performance assessment shows that CPAM can be a useful portable monitor for screening and follow-up of subjects for SDB.

Patent foram ovale(PFO)is the most common anatomical cause of an interarterial *** is usually asymptomatic but may cause paradoxical embolism and is a risk factor for non-lacunar cryptogenic cerebral ischemia in young *** the first clinical trials did not show a significant superiority of PFO closure in the secondary prevention of cerebral ischemia as compared with standard antithrombotic treatment,six subsequent randomized clinical trials(CLOSURE I,PC Trial,RESPECT,CLOSE,REDUCE,and DEFENSE-PFO)performed in a sample of cryptogenic stroke in patients aged 60 years or younger provided evidence of a significant reduction of recurrent cerebral ischemia after percutaneous PFO ***,the use of percutaneous PFO closure cannot be generalized to the entire population of patients with cerebral ischemia and PFO,but it is indicated in highly selected patients with non-lacunar cryptogenic cerebral infarction with a large right-to-left shunt,an atrial septal aneurysm and no evidence of atrial fibrillation,as well as in association with antithrombotic treatment for an optimal secondary prevention of cerebral ischemia.

The objective of this report is to give an overall view of the epidemiological, clinical, diagnostic and therapeutic features of Cytomegalovirus （CMV） infection in inflammaton/ bowel disease （IBD）. A review of published reports on this topic was carried out, with particular attention paid to the selection of patients included in studies and the diagnostic methods employed. CMV is frequently associated with IBD. In some cases, CMV infection is as- sociated with a poor outcome but it is not clear which patients are more likely to be affected and in which stage of the disease. The use of anti-viral therapy in IBD is controversial and an empirical study with controls is needed. The natural history of CMV infection related to the development and treatment of IBD has not been clarified but it is important to take it in consideration because of the possibility of viral persistence in the immunocompromised host and viral interaction with the immune system.

The association of gastroesophageal reflux disease(GERD) and esophageal cancer is well *** carcinogenic properties of the gastroduodenal contents may also lead to cancer in target organs for GERD especially considering that they do not have intrinsic protective mechanisms as found in the *** review focuses on the putative relation between GERD and non-esophageal *** of the papers reviewed are far from ideal to prove the relationship of extraesophageal cancer and GERD since a small number ofpatients is presented,most do not control cases based on tobacco usage and obesity,and the diagnosis of GERD is variable,not always from an objective measurement such as p H monitoring but relying on symptoms in most ***,head and neck and lung cancer have a growing incidence parallel to GERD and a shift towards non-smoking,female gender and adenocarcinoma(compared to squamous cell carcinoma) is arising,similar to the example of esophageal cancer with the exception of the female gender.

Background:The most common causes of early graft loss in pancreas transplantation are insufficient blood supply and leakage of the intestinal ***,it is critical to monitor graft perfusion and oxygenation during the early post-transplant *** goal of our pilot study was to evaluate the utility of hyperspectral imaging(HSI)in monitoring the microcirculation of the graft and adequate perfusion of the intestinal anastomosis during pancreatic ***:We imaged pancreatic grafts and intestinal anastomosis in real-time in three consecutive,simultaneous pancreas-kidney transplantations using the TIVITA®HSI ***,the intraoperative oxygen saturation(StO_(2)),tissue perfusion(near-infrared perfusion index,NIR),organ hemoglobin index(OHI),and tissue water index(TWI)were measured 15 minutes after reperfusion by ***:All pancreas grafts showed a high and homogeneous StO_(2)(92.6%±10.45%).Intraoperative HSI analysis of the intestinal anastomosis displayed significant differences of StO_(2)(graft duodenum 67.46%±5.60%*** jejunum:75.93%±4.71%,P<0.001)and TWI{graft duodenum:0.63±0.09[I(Index)]*** jejunum:0.72±0.09[I],P<0.001}.NIR and OHI did not display remarkable differences{NIR duodenum:0.68±0.06[I]*** jejunum:0.69±0.04[I],P=0.747;OHI duodenum:0.70±0.12[I]*** jejunum:0.68±0.13[I],P=0.449}.All 3 patients had an uneventful postoperative course with one displaying a Banff 1a rejection which was responsive to steroid ***:Our study shows that contact-free HSI has potential utility as a novel tool for real-time monitoring of human pancreatic grafts after reperfusion,which could improve the outcome of pancreas *** investigations are required to determine the predictive value of intraoperative HSI imaging.

Left atrial physiology and pathophysiology:Role of deformation imaging

The left atrium(LA) acts as a modulator of left ventricular(LV) filling. Although there is considerable evidence to support the use of LA maximum and minimum volumes for disease prediction,theoretical considerations and a growing body of literature suggest to focus on the quantification of the three basic LA functions:(1) Reservoir function:collection of pulmonary venous return during LV systole;(2) Conduit function:passage of blood to the left ventricle during early LV diastole; and(3) Contractile booster pump function(augmentation of ventricular filling during late LV diastole. Tremendous advances in our ability to non-invasively characterize all three elements of atrial function include speckle tracking echocardiography(STE),and more recently cardiovascular magnetic resonance myocardial feature tracking(CMR-FT). Corresponding imaging biomarkers are increasingly recognized to have incremental roles in determining prognosis and risk stratification in cardiac dysfunction of different origins. The current editorial introduces the role of STE and CMR-FT for the functional assessment of LA deformation as determined by strain and strain rate imaging and provides an outlook of how this exciting field may develop in the future.
