Impact of time between meniscal injury and isolated meniscus repair on post-operative outcomes:A systematic review

BACKGROUND Meniscal tears are one of the most common knee *** the diagnosis of a meniscal tear has been made,there are several factors physicians use to guide clinical *** influence of time between injury and isolated meniscus repair on patient outcomes is not well *** this relationship is important as it may influence clinical decision-making and can add to the preoperative patient education *** hypothesized that increasing the time from injury to meniscus surgery would worsen postoperative *** To investigate the current literature for data on the relationship between time between meniscus injury and repair on patient *** PubMed,Academic Search Complete,MEDLINE,CINAHL,and SPORTDiscus were searched for studies published between January 1,1995 and July 13,2023 on isolated meniscus *** criteria included concomitant ligament surgery,incomplete outcomes or time to surgery data,and *** demographics,time to injury,and postoperative outcomes from each study were abstracted and *** Five studies met all inclusion and exclusion *** were 204(121 male,83 female)patients *** of five(60%)studies determined that time between injury and surgery was not statistically significant for postoperative Lysholm scores(P=0.62),Tegner scores(P=0.46),failure rate(P=0.45,P=0.86),and International Knee Documentation Committee scores(P=0.65).Two of five(40%)studies found a statistically significant increase in Lysholm scores with shorter time to surgery(P=0.03)and a statistically significant association between progression of medial meniscus extrusion ratio(P=0.01)and increasing time to *** Our results do not support the hypothesis that increased time from injury to isolated meniscus surgery worsens postoperative ***-making primarily based on injury interval is thus not recommended.

Buttress plate fixation of coronoid process fractures via a medial approach

Purpose:To assess the clinical and radiographic outcomes of coronoid process fractures surgically managed with buttress plate fixation via a medial ***:A retrospective review of all coronoid fractures surgically fixed in our institution using a buttress plate technique via a medial approach between June 2012 and April 2015 by the senior author was *** fractures were all sizeable fractures contributing to persistent elbow instability in terrible triad or varus posteromedial rotatory instability injury patterns.A prospective telephone questionnaire was conducted to assess patient outcomes using the disabilities of the arm,shoulder and hand(DASH)score and Mayo hlbow performance score(MEPS).Results:Twelve patients were included in the study,comprising 10 males and 2 females with an average age of 39 years(range,19-72 years).Mean follow-up was 16 months(range.4-18 months).The average time to radiographic union was 4 months(range.3-7 months).Range of motion measurements at final follow-up were obtained in 11 out of 12 patients,with one patient defaulting *** 11 patients displayed a functional elbow range of motion of at least 30°-130°.with an average arc of motion of 130°(range.110°-140°).mean elbow flexion of 134°(range.110°-140°)and mean flexion contracture of 3°(range.0°-20°).The mean DASH score was 16(range.2.5-43.8)and the mean MEPS was 75(range.65-100).Complications observed included one patient with a superfic ial wound infection which resolved with a course of oral antibiotics and one patient with radiographic evidence of heterotopic ossification which was conservatively *** residual elbow instability was observed and no reoperations were ***:Buttress plate fixation via a medial approach of coronoid process fractures that contribute to persistent elbow instability represents a reliable method of treatment that produces satisfactory and predictable outcomes.

Tracking the in vivo spatio-temporal patterns of neovascularization via NIR-II fluorescence imaging

The in vivo spatio-temporal patterns of neovascularization are still poorly understood because it is limited to multi-scale techniques from the cellular level to living animal *** to deep tissue-penetration and zero autofluorescence background,the second near-infrared(NIR-II,1,000–1,700 nm)fluorescence imaging recently shows promise in breaking through this dilemma by dynamically tracking the pathophysiological process of neovascularization in ***,NIR-II fluorescence imaging was recruited for monitoring blood vessels in order to visualize the vascular injury and quantitively assess neovascularization in mouse models of acute skeleton muscle contusion and hindlimb *** temporal analysis of real-time NIR-II fluorescence intensity demonstrated that the blood flow perfusion of ischemia area was able to rapidly restore to 96%of pre-ischemic state within one ***,the spatial analysis revealed that the lower and outer quadrants of ischemia area in the mouse model of hindlimb ischemia always had relatively high blood flow perfusion compared with other quadrants during three weeks post-ischemia,and even exceeded pre-ischemic quantity at 21 days *** conclusion,this in vivo imaging technique has significant potential utility for studying the spatio-temporal patterns of neovascularization in vivo.

Comminuted olecranon fracture fixation with pre-contoured plate:Comparison of composite and cadaver bones

AIM: To determine whether use of a precontoured olecranon plate provides adequate fixation to withstand supraphysiologic force in a comminuted olecranon fracture ***: Five samples of fourth generation composite bones and five samples of fresh frozen human cadaveric left ulnae were utilized for this study. The cadaveric specimens underwent dual-energy X-ray absorptiometry(DEXA) scanning to quantify the bone quality. The composite and cadaveric bones were prepared by creating a comminuted olecranon fracture and fixed with a pre-contoured olecranon plate with locking screws. Construct stiffness and failure load were measured by subjecting specimens to cantilever bending moments until failure. Fracture site motion was measured with differential variable resistance transducer spanning the fracture. Statistical analysis was performed with two-tailed Mann-Whitney-U test with Monte Carlo Exact ***: There was a significant difference in fixation stiffness and strength between the composite bones and human cadaver bones. Failure modes differed in cadaveric and composite specimens. The load to failure for the composite bones(n = 5) and human cadaver bones(n = 5) specimens were 10.67 nm(range 9.40-11.91 nm) and 13.05 nm(range 12.59-15.38 nm) respectively. This difference was statistically significant(P ? 0.007, 97% power). Median stiffness for composite bones and human cadaver bones specimens were 5.69 nm/mm(range 4.69-6.80 nm/mm) and 7.55 nm/mm(range 6.31-7.72 nm/mm). There was a significant difference for stiffness(P ? 0.033, 79% power) between composite bones and cadaveric bones. No correlation was found between the DEXA results and stiffness. All cadaveric specimens withstood the physiologic load anticipated postoperatively. Catastrophic failure occurred in all composite specimens. Digital templating in total hip arthroplasty:Additional anteroposterior hip view increases the accuracy

AIM To analyze planning total hip arthroplasty(THA) with an additional anteroposterior hip view may increases the accuracy of preoperative planning in *** We conducted prospective digital planning in 100 consecutive patients: 50 of these procedures were planned using pelvic overview only(first group), and the other 50 procedures were planned using pelvic overview plus antero-posterior(a.p.) hip view(second group). The planning and the procedure of each patient were performed exclusively by the senior surgeon. Fifty procedures with retrospective analogues planning were used as the control group(group zero). After the procedure, the planning was compared with the eventually implanted components(cup and stem). For statistic analysis the x^2 test was used for nominal variables and the t test was used for a comparison of continuous *** Preoperative planning with an additional a.p. hip view(second group) significantly increased the exact component correlation when compared to pelvic overview only(first group) for both the acetabular cup and the femoral stem(76% cup and 66% stem vs 54% cup and 32% stem). When considering planning ± 1 size, the accuracy in the second group was 96%(48 of 50 patients) for the cup and 94% for the stem(47 of 50patients). In the analogue control group(group zero), an exact correlation was observed in only 1/3 of the *** Digital THA planning performed by the operating surgeon and based on additional a.p. hip view significantly increases the correlation between preoperative planning and eventual implant sizes.

Multi-factorial sustainability approach is necessary to preserve knee function following osteoarthritis diagnosis

Knee function preservation following a diagnosis of osteoarthritis may benefit from healthy patient lifestyles, exercise or activity habits, and daily living routines. Underlying societal issues and social roles may contribute further to both ecological and knee function preservation concerns. Based on sustainability theory and social ecology concepts we propose that factors such as health history, genetic predisposition, socioenvironmental factors and local-regional-global physiological system viability contribute to knee function preservation. Addressing only some of these factors or any one factor in isolation can lead the treating physician, surgeon and rehabilitation clinician to less than optimal treatment effectiveness. An example is presented of a 57-year-old man with medial tibiofemoral osteoarthritic. In the intervention decision-making process several factors are important. Patients who would benefit from early knee arthroplasty tend to place osteoarthritis knee pain elimination at the top of their list of treatment expectations. They also have minimal or no desire to continue impact sport, recreational or vocational activities. In contrast, patients who are good candidates for a knee function preservation treatment approach tend to have greater expectations to be able to continue impact sport, recreational or vocational activities, are willing and better able to implement significant behavioral changes and develop the support systems needed for their maintenance, are willing to tolerate and live with minor-to-moderate intermittent knee pain, and learn to become more pain tolerant.

Effects of soccer training on health-related physical fitness measures in male adolescents

Purpose: The aims of this study were to(1) investigate the health-related physical fitness profile of untrained adolescent boys in comparison to adolescent soccer players,(2) determine the intensity and enjoyment of 6 v 6 and 4 v 4 small-sided games, and(3) evaluate the health-related effects of a short-period of soccer training in the untrained ***: Forty-one adolescent boys(untrained, n = 24: age = 15.9 ± 0.6 years; trained, n = 17: age = 15.7 ± 0.7 years) were recruited. For Purpose 1,the players(n = 17) and the untrained(n = 24) boys were tested for speed, jumping power, postural balance, flexibility, and aerobic capacity. After baseline testing, Purposes 2 and 3 were addressed by randomly assigning the untrained boys to either a soccer-training group(small-sided games,2 sessions per week for 8 weeks) or to a control group, followed by identical ***: At baseline, physical fitness was higher(p < 0.001) in trained players than in untrained for aerobic fitness, sprinting, jumping power, and postural balance. Small-sided games using 6 v 6 or 4 v 4 elicited similar heart rate(HR)(mean: ~ 85% peak heart rate, HR_(peak)), rate of perceived exertion, and enjoyment responses. Over 8 weeks, the between-group analysis revealed that soccer training had a large beneficial effect on postural balance(45%) when compared with control group with unclear effects on other fitness ***: Adolescent soccer players had markedly higher physical fitness compared with untrained adolescents. Small-sided soccer games practiced by untrained adolescents elicited high exercise intensity. While 8 weeks of twice-weekly soccer training sessions induced significant improvement in postural balance, the short duration of the study was not sufficient to result in between-group differences in sprint and jump performance or aerobic fitness.

Postoperative deep shoulder infections following rotator cuff repair

Rotator cuff repair(RCR) is one of the most commonly performed surgical procedures in orthopaedic surgery. The reported incidence of deep soft-tissue infections after RCR ranges between 0.3% and 1.9%. Deep shoulder infection after RCR appears uncommon, but the actual incidence may be higher as many cases may go unreported. Clinical presentation may include increasing shoulder pain and stiffness, high temperature, local erythema, swelling, warmth, and fibrinous exudate. Generalized fatigue and signs of sepsis may be present in severe cases. Varying clinical presentation coupled with a low index of suspicion may result in delayed diagnosis. Laboratory findings include high erythrocyte sedimentation rate and C-reactive protein level, and, rarely, abnormal peripheral blood leucocyte count. Aspiration of glenohumeral joint synovial fluid with analysis of cell count, gram staining and culture should be performed in all patients suspected with deep shoulder infection after RCR. The most commonly isolated pathogens are Propionibacterium acnes, Staphylococcus epidermidis, and Staphylococcus aureus. Management of a deep soft-tissue infection of the shoulder after RCR involves surgical debridement with lavage and long-term intravenous antibiotic treatment based on the pathogen identified. Although deep shoulder infection after RCR is usually successfully treated, complications of this condition can be devastating. Prolonged course of intravenous antibiotic treatment, extensive soft-tissue destruction and adhesions may result in substantially diminished functional outcomes.
