Objectives: To describe the natural history of pyelectasis from its detection in the second trimester to delivery, its capability to predict renal pathology and whether prenatal development of pelvic dilatation is correlated to its postnatal evolution. Study design: A retrospective analysis involving 375 fetuses with a complete urological follow- up. Prenatal ultrasound was correlated with the results of postnatal investigation and frequency of postnatal surgery was established. Results: Two- hundred and eighty cases underwent at least two examinations before birth. 73.1% were male fetuses. 57.4% had a bilateral pyelectasis. Prenatal evolution of pelvic dilatation was the following:18.6% of the cases normalized, in 34.6% of the cases the dilatation reduced but did not disappear, in 30.7% it remained unchanged, while it worsened in 16.4% . One case from the first group, three cases from the second, seven cases from the third and 11 cases from the fourth needed surgical treatment. 1.9, 7.2, 18.6, 23.9% of cases respectively worsened after birth in the four groups (trend: P=0.001). Conclusions: Prenatal diagnosis of pyelectasis improves the outcome of these children due to a surgical approach that avoids renal damage. There is a good correlation between prenatal evolution and postnatal outcome, although a postnatal follow- up is opportune in those cases that normalized before birth.

Transient Global Amnesia (TGA) is a common condition of unknown aetiology char acterised by the abrupt onset of severe anterograde amnesia, which lasts less th an 24 hours. Some authors have suggested that subclinical impairment of memory f unctions may persist for much longer, but neuropsychological assessment lasting years after TGA attack has not been performed so far. The aim of this study was to evaluate long term cognitive functions in patients with a previous TGA episo de. Fifty five patients underwent a standardised neuropsychological assessment after at least one year from the TGA attack, and were compared with 80 age mat ched controls. TGA patients showed worse performances on tests evaluating verbal and nonverbal long term memory and attention, with comparable global cognitive functions. By applying current criteria for amnestic Mild Cognitive Impairment (MCI a) on TGA subjects, a group consisting of 18/55 (32.7%) MCI a subjects w as identified. There was no association between the presence of MCI a and demog raphic variables, vascular risk factors, years since the TGA episode, or ApoE ge notype. This study demonstrates that TGA appears to be a relatively benign syndr ome although objective memory deficits ffulilling MCI a criteria persist over t ime, as detected by multidimensional neuropsychological tasks performed at long term follow up.

Objectives: To evaluate and correlate the effects of long-term antihypertensive treatment on left ventricular(LV) mass and carotid structural changes in a large group of essential hypertensive patients, participating in the European Lacidipine Study on Atherosclerosis(ELSA). Design: In four(Brescia, Glasgow, Naples and Pisa) of 23 centres participating in the ELSA study, an echocardiographic examination was performed at baseline and repeated, until the end of the 4-year study, in essential hypertensive patients, followed-up for carotid quantitative ultrasound examination of intima-media thickness(IMT), after random allocation to treatment with either lacidipine or atenolol(and added hydrochlorothiazide, as required for control of blood pressure). Methods: M-mode, two-dimensional guided echocardiography was used to measure left ventricular(LV) wall thickness and dimensions, from which LV mass was calculated, using an anatomically validated formula(Penn Convention)and indexed to body surface area(left ventricular mass index, LVMI). The echocardiographic tracings were blindly evaluated in a single reading centre(Brescia). Bilateral IMT was measured at the site of common carotid and bifurcation far walls(CBMmax). Results: At baseline, cardiac and carotid ultrasound scans were available in 278 patients(mean age 54±7 years, 57%males, 22%obese). A significant correlation was observed between baseline LVMI and CBMmax(r=0.22, P1), independent of age. In multivariate analysis, CBMmax and mean 24-h pulse pressure were most strongly associated with baseline LVMI. A significant reduction in LVMI was observed both during lacidipine(n=96)(-12.5%reduction) and atenolol(n=78)(-13.9%reduction) treatments(up to 4 years)(P1 for both, without significant differences between treatments). AIM: To study that inflammatory fibroid polyps(IFPs) in children are extremely uncommon tumors that may occur throughout the gastrointestinal tract. METHODS: A systematic review of the pediatric literature and a report of a new case of IFP is also pres-ented. The Pub Med database was searched for original studies on pediatric IFPs since 1960, according to "Preferred reporting items for systematic reviews and meta-analyses" guidelines for systematic reviews. RESULTS: Five studies were finally enclosed, encompassing 6 children with IFPs(mean age 64 mo). Tumors were located in the stomach(2 patients), in the small bowel(2 patients), in the rectum(1 patient) and in the colon(1 patient). Open surgery was performed in all patients and complete excision of the mass was achieved in all cases. All patients are alive and free of symptom. Authors described a further case of a 3-year-old boy with a large duodenal IFP, in whom the tumor was removed by "en block resection". The presence of IFP throughout the gastrointestinal tract and its variable clinical appearances make it difficult to diagnose. An accurate pre-operative assessment is fundamental in order to differentiate IFP from other more aggressive gastrointestinal tumor, enabling unnecessary demolitive surgery. CONCLUSION: When complete resection of the IFP is achieved, the prognosis is excellent.

Preoperative tracheobronchoscopy(TBS) in the diag-nostic assessment of newborns affected by esophageal atresia(EA) was described in 1981. Nevertheless, the value of the procedure is actually much debated; only a few studies have clearly explored the advantages of TBS and this procedure is not yet routinely included inthe diagnostic and therapeutic assessment in many in-ternational pediatric surgery settings. Routine preoper-ative TBS is a safe procedure that enables the accurate examination of the tracheobronchial tree, the visualiza-tion of tracheoesophageal fistula and the diagnosis of tracheomalacia or associated respiratory anomalies. When a distal fistula is found, its occlusion with a Fog-arty balloon catheter improves mechanical ventilation and facilitates surgical repair. This review provides a detailed overview on the use of TBS in newborns with EA, focusing on technical aspects, anesthesiological management, indications and limits. The benefits and risks of the procedure are also compared with alterna-tive diagnostic tools, such as an esophageal contrast study, computed tomography scan and ultrasound.
